1.Value of spatio-temporal image correlation in prenatal diagnosis of complex heart malformations

Objective To evaluate the value of spatio-temporal, image correlation (STIC) in prenatal diagnosis of complex heart malformations.Methods STIC was used to examine 25 fetuses with 20-30 weeks of gestation who were doubted with complex heart malformations prospectively by two-dimensional (2D) echocardiography.Dynamic three-dimensional imaging was achieved by postprocessing technique with STIC across the entire fetal heart and thorax.An offline analysis of the acquired volume was carried out to examine the fetal cardiac anatomy with multiplanar mode, surface rendering and minimum mode.The results of prenatal diagnosiswere compared with postnatal color Doppler and surgical findings.Results Successful STIC volume datasets were obtained in all 25 fetuses.The findings of all 25 fetus with complex heart malformations included double outlet of right ventricle in 1 case, truncus arteriosus in 1 case, complete transposition of the great arteries in 3 cases,corrected transposition of the great arteries in 1 case, complete endocardial cushion defect in 4 cases, hypoplastic right ventricle syndrome in 1 case, hypoplastic left heart syndrome in 2 cases, tricuspid atresia in 1 case,pulmonary atresia in 1 case, tetralogy of Fallot in 3 cases, single atrium in 1 case, single atrium and single ventrium in 4 cases,in 1 case, atrial septal defect and ventricular septal defect with coronary sinus dilatation in 1 case.The major diagnoses derived from STIC in 23 of 25 cases were consistent with the results of postnatal color Doppler and surgical findings.Conclusions STIC can provide more accurate information of structures of complex fetal heart malformations and has great value in pregnant prognosis.

3.Comparatively Analysis on Clinicopathological Characteristics and Prognosis of Small Cell Carcinoma of the Cervix(SCCC) and Non-SCCC-A Single Center of Clinical Research

Objective:To investigate the differences between patients with small cell carcinoma of the cervix and other type of cervical cancer on clinicopathological characteristics,treatment,prognosis through the retrospective analysis,also study the prognostic factors in patients with SCCC.Methods:The patients with SCCC(63 cases,research group) and patients with Non-SCCC(60 cases,control group) such as squamous cell carcinoma and adenocarcinoma of cervix were chosen during May 2003 and June 2015 in department of obstetrics and gynecology,west China second university hospital.The clinical pathological and survival data were collected,sort out,checked and analyzed.Chi-square test or Fisher's exact probability method was used to compare the differences of clinical features between the two groups.The 5-year disease-free survival(DFS) rate and 5-year overall survival(OS) rate were generated using the Kaplan-Meier method.Univariate and multivariate analysis were applied to research the prognostic factors.Results:①The median age of the patients in research group was 40 years old,younger than control group whose median age was 44 years old (P =0.001).The rate of lymph node metastasis,parametrial infiltration,vascular involvement and positive surgical margins in research group were significantly higher than those of control group (P < 0.05).There were no significantly differences between two groups in clinical symptoms,FIGO stage,therapeutic schedule.②In the survival analysis,the research group estimated 5-year overall survival (OS) rate (29.2%) and estimated 5-year disease-free survival rate (DFS)(26.6%) was significantly lower than those of the control group(82.6%,and 76.7%,respectively) P < 0.05.③In the COX multivariate regression analysis,the results showed that the lymph node metastasis was the independent factor influencing overall survival rate and disease-free survival rate(HR =4.784、3.067,P < 0.05).In addition,the tumor size was the independent factor influencing overall survival rate (HR =3.610,P < 0.05),and advanced FIGO staging was the independent factor influencing disease-free survival rate (HR =2.793,P < 0.05).Conclusions:The patients with SCCC were younger than other cervical cancer,and the clinical symptoms,cervical appearance,treatment between the two types are almost the same.However,The rate of lymph node metastasis,parametrial infiltration,vascular involvement and positive surgical margins in patients with SCCC were significantly higher than other cervical cancer,and the prognosis of the patients with SCCC was very poor.The main prognostic factors were lymph node metastasis,advanced FIGO stage and tumor size.
4.Research Progress,Bottlenecks and Strategies in the Plant Bioreactor

Past decade,plants as production systems for recombinant proteins has been actively investigated,and is one of the most active research fields in life sciences. Plants are attractive as protein factories because they with advantages of cost,scale,and safety,and emerge capability to produce pharmaceutical production with bioactivity proved by previously studies. The plant-made pharmaceuticals possess enormous viability for market and wide prospects for application. Based on those reasons,the rise of plant bioreactor was reviewed,and research advance on the expression systems and plant-made recombinant proteins was introduced. Bottleneck problems and adopted strategies using plant system to produce recombinant proteins and research hotspot in the near future were summarized,and prospect of plant bioreactor in future was described. Synchronously,some opinions including research progress of plant bioreactor,differences to foreign and strategies in the future were discussed.
5.Homology modeling of mosquitocidal Cry30Ca2 of Bacillus thuringiensis and its molecular docking with N-acetylgalactosamine.

OBJECTIVETo investigate the theoretical model of the three-dimensional structure of mosquitocidal Cry30Ca2 and its molecular docking with N-acetylgalactosamine.
METHODSThe theoretical model of Cry30Ca2 was predicted by homology modeling on the structure of the Cry4Ba. Docking studies were performed to investigate the interaction of Cry30Ca2 with N-acetylgalactosamine on the putative receptor.
RESULTSCry30Ca2 toxin is a rather compact molecule composed of three distinct domains and has approximate overall dimensions of 95 by 75 by 60Å. Domain II is a helix bundle, Domain II consists of three antiparallel β-sheets, Domain III is composed of two β-sheets that adopt a β-sandwich fold. Residue 321Ile in loop1, residues 342Gln 343Thr and 345Gln in loop2, residue 393Tyr in loop3 of Cry30Ca2 are responsible for the interactions with GalNAc via 7 hydrogen bonds, 6 of them were related to the oxygen atoms of hydroxyls of the ligand, and one to the nitrogen of the ligand.
CONCLUSIONThe 3D structure of Cry30Ca2 resembles the previously reported Cry toxin structures but shows still some distinctions. Several residues in the loops of the apex of domain II are responsible for the interactions with N-acetylgalactosamine.

6.Survey and control of extensively drug-resistant Acinetobacter baumannii infection outbreak in a basic-level hospital

Objective To survey and control an extensively drug-resistant Acinetobacter baumannii (A.bauman-nii)(XDRAB)lower respiratory tract infection (LRTI)outbreak in intensive care unit (ICU)of a hospital. Methods From October 27,2013 to December 27,2013,5 cases of XDRAB LRTI occurred in ICU,epidemiological investigation was conducted,sources and transmission routes of infection were searched,intervention measures were performed.Results XDRAB were isolated from patients’sputum and environmental object surface,drug-resistant spectrums of isolated XDRAB were almost the same,suggesting the outbreak was due to the contamination of envi-ronment by the same pathogen.54 specimens were taken before disinfection,26 were positive,XDRAB were isola-ted from door handles,sheets and telephones.Investigation concluded that the outbreak of XDRAB infection was due to contamination of indoor environment and equipment,incomplete disinfection,and inadequate hand hygiene compliance of health care workers.After the implementation of a series of control measures,XDRAB was not found,and there was no new XDRAB infection cases.Conclusion Environment contamination is the main cause of this XDRAB healthcare-associated infection(HAI)outbreak,strict implementation of isolation,prevention and con-trol measures of MDROs can effectively control HAI outbreak caused by A.baumannii.

9.Study on corneal endothelial cells after phacoemulsification

AlM:To observe the changes of corneal endothelium after phacoemulsification cataract surgery in different types of cataract patients.
METHODS: Randomly selected age-related cataract, diabetic cataract and cataract of high myopia 30 eyes of 30 cases, respectively, in our hospital. All patients underwent phacoemulsification combined with intraocular lens implantation, corneal endothelial density and the percentage of hexagonal cells were measured by corneal endothelial cell instrument without touching before surgery and one week after surgery.
RESULTS: The difference of the preoperative corneal endothelial cell density and the percentage of hexagonal cells among three groups were not statistically significant (P>0. 05). One week after surgery, the cell density in three groups were respectively 2 496. 86 ± 298. 96/mm2 , 2 379. 51 ± 375. 13/mm2 , 2 425. 38 ± 312. 68/mm2 , the percentage of hexagonal cells were respectively ( 46. 20 ± 12. 03)%, (43. 44±13. 99)%, (44. 35±8. 13)%. Both the cell density and the percentage of hexagonal cells one week after surgery were lower than those before operation. There were significant difference in three groups ( P <0. 05). Both the measurements in diabetic cataract group and cataract of high myopia group after surgery were lower than those in age-related cataract group, the cell density and the percentage of hexagonal cells in diabetic cataract group were lower obviously compared with those in age- related cataract group and the difference was significant (P<0. 05).
CONCLUSlON:The tolerance of corneal endothelial cell to phacoemulsification cataract surgery is lower in cataract with diabetes and high myopia. Corneal endothelium should be assessed preoperatively and protected intraoperatively.
10.Construction expression and purification of C reactive protein

Objective To build a foundation for determination of C reaction protein,C reaction protein was expressed and purified,and the immune reactivity of the purified protein was identified.Methods The CRP cDNA was amplified by RT-PCR from human liver cDNA library and inserted into expression vector pCRTT/NT.The recombined plasmid CRP-pCRTT/NT which expressed the fusion protein of CRP was then transferred into lysogenic host strain E coli.BL21 (DE3).The target protein was identified using SDS- polyacrylamide gel electrophoresis (SDS-PAGE).Affinity chromatography was used for protein purification.The immune reactivity of purified CRP was identified by Western blot using anti-CRP specific antibody.Results Recombiant human CRP was expressed in inclusion bodies of E.coli with a six histamine tag.The purify of recombinant protein was detected by SDS-PAGE as a single band at 30 000 and was identified by Western blot.Conclusions A plasmid expressed CRP protein is constructed and the purification system of CRP protein is established.The immune reactivity of the purified protein is identified by Western blot,which makes a good base for the preparation of CRP test kit.
