1.The effect of TMS on motor function in patients with acute brain infarction

ObjectiveWe discussed the effect of TMS on the motor conduction pathway and studied the function of TMS in the motor function of patients with stroke. Methods65 patients with acute brain infarction were divided into two groups, treatment group given TMS once a day and contral group. Motor nerve center was stimulated twice or three times each day. 14 days were one period of treatment. MEP, CMCT, force and Fugl-Meyer scores were recorded on the first and the last days. Results Before treatment, there was no difference between two groups on CMCT, force and Fugl-Meyer scores. After treatment, the patients in the treatment group got much better improvement in movement ability than the patients in the control group. But there was no difference between two groups on MEP neither before nor after treatment. Conclusions TMS can help patients with stroke to regain movement ability, and improve the recovery of force and fine movement, and also reduce the CMCT distinguishably. TMS is easy to use and no evidently uncomfortable influence on patients happens.
2.Study on Preparation and Quality Standard of Jiekang Lotion

OBJECTIVE: To prepare gynecological Jiekang lotion and to establish its quality standard. METHODS: The Caffeotannic acid in Flos Lonicerae, the Berberine in Cortex Phellodendri and the Ammothamnine in Radix Sophorae Flavescentis were identified by TLC; HPLC was adopted to determine the content of Osthole with Hypersil DOS C18 as chromatograpic column. The mobile phase consisted of acetonitrile-water (54∶46) with a flow rate of 1.1mL? min-1. The detection wavelength was set at 321nm. RESULTS: Both the chief components in Jiekang lotion and the corresponding reference substances showed the identical discrimination reactions. The linear range of Osthole was 13~130?g?mL-1. CONCLUSION: The method is sensitive in qualitative reaction, specific in TLC identification, accurate in HPLC quantitation and recurrent. The established standard is applicable for the quality control of Jeikang lotion.
3.Regulating Sub-health Along Meridians And Viscera

This study was aimed to observe the clinical effect of low frequency negative pressure meridian viscera regulating sub-health.According to the diagnostic criteria,34 cases of sub healthy subjects were randomly selected.The low frequency negative pressure meridian viscera regulating method was used.By analyzing the cupping spot color feedback conditioning information of the sub healthy subjects and conditioning before and after the synthesis of the four diagnostic methods of digital and quantitative,its effect was evaluated.After 4 weeks of continuous adjustment,34 cases of sub healthy subjects,compared with preconditioning,cupping spot color of the subjects was shallow or disappeared (P < 0.01);after adjusting,the proportion of light red tongue and thin white tongue coating increased obviously (P < 0.05),the proportion of ecchymosis tongue was decreased compared to precoditioning (P < 0.05);after adjustment,core symptoms such as back pain,neck pain,anxiety and irritability,were alleviated or disappeared;after adjustment,the pulse string tightness decreased significantly (P < 0.01).It was concluded that the low frequency negative pressure meridian viscera regulation can significantly improve discomfort symptoms of sub healthy subjects and promote therecovery of tongue,pulse and cupping spot color in sub healthy subjects.This method is worth popularizing and further exploring.
4.Effects of post length on horizontal load of post-core system.

OBJECTIVETo access the effects of post length on horizontal load of cast post and core crown.
METHODS120 simulated roots made of polymethylmethacrylate were divided into 15 groups according to different mechanical fulcrum and length of post, made the wax pattern of post-core, casted and cemented the post-core. After saline soak, loaded these specimens on the Instron-4302 by the speed of 1 mm/min and recorded the maximum force of load.
RESULTSThe means of fracture resistance Wp11 of 7.0 mm, 7.5 mm, 8.0 mm, 8.5 mm, 9.0 mm were (94.24 +/- 12.47) N, (129.76 +/- 19.89) N, (195.98 +/- 1 2.46) N, (208.88 +/- 24.37) N, (225.67 +/- 53.07) N and the means of dangerous stress (sigma degrees 11 were 65.12 N, 93.07 N, 145.84 N, 161.58 N, 181.49 N; the means of fair stress [sigma11] were 39.41 MPa, 56.34 MPa, 88.34 MPa, 97.86 MPa, 109.94 MPa. It showed that the horizontal load of post-core crown system increased gradually with the increasing of the post length. It appeared the linear regression between the length of the post and fracture resistance and physiologic load.
CONCLUSIONThe increasing of the post length can increase the horizontal load of post-core crown system. During the pulp preparation of post-core system, the post length should be added as much as possible.

6.Effects of post diameter on retention of post-core crown system.

OBJECTIVETo access the effects of post's diameter on the retention of post-core restorations.
METHODSThirty-three simulant roots with same length,taper and different diameter (1.305 mm, 1.830 mm, 2.175 mm and 2.700 mm respectively) were made of polymethylmethacrylate and were divided into 4 groups. The wax patterns of post-cores were manufactured and cast. The post-cores were cemented. After that, the adhesion, frictional force and restriction of the post-core restorations were measured.
RESULTSWhen the proportion of length and diameter was smaller than 4.372, the linear regression between the post's diameter and the retention of post-core crown system was F = 107.98 + 72.08D. The linear regression between the post's diameter and the adhesion was P = 9.60 + 2.22D. The linear regression between the post's diameter and the frictional force and restriction was Q = 98.37 + 69.87D. The statistical analysis of the results indicated significant difference (P < 0.05). When the proportion of length and diameter was lager than 4.372, the retention of post-core crown system increased although the post's diameter decreased.
CONCLUSIONWhen the proportion of length and diameter was smaller than 4.372, the retention of post-core crown system increased with the decrease of the post's diameter. While the proportion was lager than a certain value, this rule did not exist.

7.Changes of intracellular cholesterol metabolism in neuroendocrine differentiation of prostate cancer and their significance.

OBJECTIVETo explore the roles of intracellular cholesterol metabolism in neuroendocrine (NE) differentiation of prostate cancer based on an androgen-independent prostate cancer NE cell model induced by androgen deprivation.
METHODSLNCaP cells were cultured in androgen-depleted medium, and NE phenotypes were identified by observing the changes in cell morphology, molecular markers (SgIII, NSE and CgA) and cell proliferation. The expression and distribution of cholesterol and Sg III were determined by immunofluorescence staining. The expressions of the key genes LDL-R, SREBP-1 and SREBP-2 involved in cholesterol synthesis and uptake were detected by semi-quantitative RT-PCR.
RESULTSThe LNCaP cells showed shrinking bodies and extending axons after androgen deprivation, and all the molecular markers, such as Sg III, NSE and CgA, significantly increased in a time-dependent manner, while the cell proliferation was obviously inhibited (P < 0.05). The cholesterol distribution in the LNCaP cells after NE differentiation presented remarkable aggregation at the axon terminals. However, there were no significant differences in the expression of cholesterol between the two types of cells, nor in the changes of the expressions of key genes LDL-R, SREBP-1 and SREBP-2 involved in cholesterol synthesis and uptake (P > 0.05).
CONCLUSIONTransient androgen depletion could successfully induce NE differentiation of LNCaP cells, and the intracellular cholesterol could re-distribute into axon terminals to enhance the formation of neurosecretory granules.

9.Bacterial culture of bile from patients with obstructive jaundice and antibiotic sensitivity

Objective To investigate the characteristics of microbial isolates and the positive rate from bile cultures taken from obstructive jaundice patients, then compare the antimicrobial sensitivities to guide the rational choice and use of antibiotics. Methods Bile cultures from 322 patients from January 2012 to April 2016 were reviewed. Specimens were obtained from patients that were diagnosed obstructive jaundice. The bile specimens were examined for pathogenic respectively. At the same time, the empirical antibiotics of the 322 cases prior to operation were also analyzed. Results Bile culture was positive in 246 among322 cases, the positive culture rate was 76.40%. A total of 267 pathogens were isolated in bile culture: 208 Gram-negative bacteria, 48 Gram-positive bacteria and 11 fungus. The most common pathogens in all were Escherichia coli (208 strains, 53.18%), Enterococcus faecium (28 strains, 10.49%), Klebsiella pneumoniae (27 strains, 10.11%). The most sensitive antibiotics against Gram-negative bacteria were imipenem and meropenem (with susceptibility rate for 98.08%, respectively). The more sensitive antibiotics against Gram-negative bacteria were efoperazone/sulbactam, amikacin, piperacillin/tazobactam (susceptibility 92.31%, 88.46% and 85.58%). The lowest susceptibility rate of Gram-negative to twelve kings of antimicrobial agents were levofloxacin, andaztreonam (susceptibility 29.81%, 28.37%). The most sensitive antibiotics against Gram-positive bacteria were linezolid, teicoplanin and vancomycin(with susceptibility rate for 100.00%, respectively). The more sensitive antibiotics against Gram-positive bacteria was chloromycrtin (susceptibility 88.89%). The lower susceptibility rate of Gram-positive were levofloxacin (susceptibility 25.00%). 281 cases of patients before surgery empirical use of antimicrobial drugs. Monotherapy was used in 219 cases. The main drug was levofloxacin (86/219), the second was cefoperazone/sulbactam (70/219). The dual therapy was used in 62 cases, the mainly was cefoperazone/sulbactam plus ornidazole or metrornidazole(27/62), the second was levofloxacin plus ornidazole or metrornidazole (12/62). Conclusions Gram negative bacteria were the predominant bile pathogens found in patients with obstructive jaundice. And the most prominent Gram-negative pathogens were Escherichia coli. The sensitive rates of cefoperazone/sulbactam, amikacin and piperacillin/tazobactam were higher, which could be used as the preferred antimicrobial agents. Imipenem and meropenem should remain the last alternative when all other therapies fail or serious infection; The empirical antibiotics's pertinence was not very suitable, levofloxacin's resistance rates in obstructive jaundice was higher. It should be avoided to choose during empirical antimicrobial therapy.
10.Application of ROC curve for evaluation of the effect of prenatal screening during second trimester of pregnancy.

OBJECTIVETo develop a method for evaluating the feasibility of prenatal screening using local median value and determining the cut-off value.
METHODSWith receiver operating characteristic curve (ROC) analysis, results of second trimester prenatal screening calculated by a local median value in a new model and the built-in median value in 2T software were compared. The cut-off value was set by serial analysis of true and false positive rates and other relevant data.
RESULTSThe ROC curve has accurately estimated the difference in the screening efficacy between a local median value and that embedded in the 2T model, and established a reasonable cut-off value for the laboratory based on false positive rate and detection rate.
CONCLUSIONThe method of ROC curve can be used to evaluate the performance of local median value in prenatal screening and to test the rationality of cut-off value established in the laboratory. As the result, a better cut-off value may be derived.
