2.Relationship between father parenting stress, marital satisfaction and father involvement

Objective To examine the effect of father parenting stress and paternal marital satisfaction on father involvement,and to explore the mediating role of maternal perceptions of father involvement in father involvement and maternal marital satisfaction.Methods 300 children's fathers and mothers from 5 kindergartens in Beijing city,Dezhou city,Wuhu city were surveyed with the Chinese version of the PSI/SF,the father involvement questionnaire and the marital satisfaction questionnaire.Results The score of paternal marital satisfaction was (3.98±0.75),(3.84±0.76) for maternal marital satisfaction,(2.26±0.50) for father parenting stress,(2.60±0.49) for father involvement,(2.50±0.50) for maternal perceptions of father involvement.Correlation analysis showed that father parenting stress was negatively correlated with father involvement(r=-0.36,P<0.01),paternal marital satisfaction was positively correlated with father involvement (r=0.33,P<0.01),father involvement was positively correlated with maternal marital satisfaction(r=0.36,P <0.01),father involvement was positively correlated with maternal perceptions of father involvement (r=0.48,P<0.01).A structure equation model consisting of father parenting stress,father involvement and marital satisfaction had a better degree of fitting:GFI =0.99,NFI =0.99,CFI =0.99,RMSEA<0.001.Maternal perceptions of father involvement mediated the effect of father involvement on maternal marital satisfaction (effect value:0.41,P<0.05).Conclusion Father parenting stress and paternal marital satisfaction could significantly predict father involvement,father involvement could significantly predict maternal marital satisfaction.These studies verified the spillover effect and the crossover effect between spousal subsystem and parental subsystem.

4.Expression of ADAMTS4 in the heart of myocardial infarction rat

Objective To detect the expression of a disintegrin and metalloproteinase with thrombospondin motifs 4 (ADAMTS4),Versican,and Aggrecan in hearts of myocardial infarction rats.Methods Male Wistar rats were chosen to make myocardial infarction model.Fifteen rats were randomly assigned into the myocardial infarction group and five rats into the sham-operation group.Rats were killed 3 d after surgery and the left ventricles were taken to produce frozen section.ADAMTS4,Versican,and Aggrecan protein expressions were detected by imnunohistochemistry method.Results ADAMTS4 expressed in myocardial cells at marginal zone of the infarcted area.ADAMTS4 expressed weakly while Versican expressed strongly in capillary endothelial cells of marginal zone of the infarcted area.Aggrecan showed no expression in the cardiac tissue and vascular endothelium of both myocardial infarction group and sham-operation group.Conclusions ADAMTS4 is involved in the inflammation after myocardial infarction.Degradation of ADAMTS4 to Versican exists in the vessels after myocardial infarction.Aggrecan is not involved in the pathophysiological process of myocardial infarction.
5.Laparoscopic anterior perineal plane for ultra-low anterior resection of the rectum for low rectal cancer

The introduction of stapling instruments and improved understanding of pathology has resulted in a greater proportion of low rectal cancer patients undergoing sphincter-preserving resection.A variety of alternative techniques have been proposed to avoid a permanent stoma,including abdominal pull through,abdominal trans-sphincteric resection and intersphincteric resection.However,these damages always inflicted on the anal sphincters with poor functional results.More recently,the anterior perineal plane for ultra-low anterior resection of the rectum (APPEAR) technique was developed which approaches the anurectum via an anterior transperineal approach and exploits an anatomic space within the pelvic floor musculature termed “rectal no-man's laud”.The ability to access this segment of distal rectum by the perineal approach may determine whether a sphincter-saving resection can be performed for a proportion of patients who would otherwise require a permanent stoma.We performed laparoscopic APPEAR for a 46-year old woman with low rectal cancer with satisfactory results.
6.Predictive value of procalcitonin for outcome of ventilator weaning in patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To investigate the possibility that procalcitonin (PCT) can predict the outcome of ventilator weaning in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Methods The AECOPD patients caused by bacteria who received mechanical ventilation in intensive care unit (ICU)of the General Hospital of China National Petroleum Corporation in Jilin from January 2010 to March 2014 were retrospectively selected,and their venous blood PCT levels were monitored daily. When they had passed the spontaneous breathing trial(SBT),their breathing machine and trachea cannula were removed. Afterwards they were closely observed for 48 hours and divided into two groups:success and failure groups according to the outcome of the first weaning,and then the analysis of the PCT difference between the two groups was made. Results In the 52 patients enrolled,there were 38 patients in the success group and 14 patients in the failure group. The baseline information as age,sex,acute physiology and chronic health evaluationⅡ(APACHEⅡ)score between the two groups showed no significant difference(all P>0.05). There was no statistical significant difference in PCT level(μg/L) between success and failure groups when they were admitted(5.33±1.35 and 5.37±1.34,respectively). When they were weaning,the PCT levels of both groups had reduced to some extent(1.37±0.39 and 1.74±0.42,respectively), and the success group was lower than that in the failure group(t=2.971,P=0.005). Conclusion PCT level can be used as a predictive index to guide the time for ventilator weaning in patients with AECOPD.
7.Partition-type spinal cord catheter combined with bone marrow stromal stem cells in the repair of spinal cord transection injury in rats

BACKGROUND:There is a high morbidity after spinal cord injury, and the therapeutic strategy is limited to early surgical intervention, medication and post-treatment exercise that only can improve the motor function slightly. However, there is no effective cure method. OBJECTIVE:To study the effect of partition-type spinal cord catheter combined with bone marrow stromal stem cels on T8 spinal cord transection damage in rats. METHODS:Fifty rats were randomized into five groups (n=10 per group): group I, T8 spinal cord transection (5 mm) was made in rats with no treatment; group II, the partition-type tube was inserted into the injured site after modeling; group III, partition-type tube combined with bone marrow stromal stem cels was implanted into the injured site after modeling; group IV, partition-type tube combined with polyglycolic acid fibers was implanted into the injured site after modeling; group V, partition-type tube combined with bone marrow stromal stem cels and polyglycolic acid fibers was implanted into the injured site after modeling. RESULTS AND CONCLUSION:At 2 and 12 weeks postoperatively, Basso, Beattie and Bresnahan scores were significantly higher in the groups III and IV than the groups I, II, IV (P < 0.05). At 12 weeks postoperatively, the latency of motor evoked potential below the injury plane was significantly decreased in group V compared with groups I, II, III, IV (P < 0.05). Immunohistochemical results displayed that in the groups III and V, regenerated nerve fibers grew positively and arranged orderly among the tubes, and there was no obvious winding phenomenon. Under transmission electron microscopy, a certain number of myelinated nerve fibers were found as bridges among groups. These findings indicate that the partition-type chitosan tube combined with bone marrow stromal stem cels has a good connection with the injured spinal cord a good connection to restore part of electrophysiological properties, accelerate the axon regeneration, recover the motor function, thereby providing a new direction for the treatment of spinal cord injury. 8.Noninvasive differentiation of pulmonary arterial hypertension and pulmonary venous hypertension by echocardiography

Objective To determine whether pulmonary arterial hypertension(PAH) and pulmonary venous hypertension (PVH) can be differentiated noninvasively by echocardiography. Methods Fifty-six patients with pulmonary arterial systolic pressure(PASP) ≥40 mmHg by echocardiography were involved,and cardiac catheterization performed within 7 days of each other. Based on left ventricular end-diastolic pressure or pulmonary capillary wedge pressure(PCWP) ,30 patients were classified as PAH group and 26 patients as PVH group. The early(E) and late(A) diastolic mitral inflow velocities,E/A ratios,deceleration time(DT),early dastlic mitrial annular velocity(E') and E/E' ratios were measured by conventional and Doppler tissue imaging echocardiography in the two groups. Results Compared with PVH group,the PAH group had significantly higher A,DT,PASP and E',and significantly lower E,E/A ratio and E/E' ratio (P < 0. 01 or P <0. 001). E/E' and E/A ratio was optimal indexes for differentiation of PAH and PVH,the area under receiver operating characteristic curve was 97% and 91 %, respectively. Optimal cutoff for diagnosing PVH was E/E'>9.2 (sensitivity 95%, specificity 97%) and E/A> 1. 7 (sensitivity 75%,specificity 92%). Conclusions PAH and PVH imaging can be reliably differentiated by echocardiography.
9.Advanced bone graft combined with locking compression plate for the treatment of middle and distal tibia nonunion.

OBJECTIVETo explore methods of treating middle and distal tibia nonunion with the treatment of advanced bone graft combined with locking compression plate.
METHODSFrom January 2011 to December 2012, 12 patients with middle and distal tibia nonunion were treated with advanced bone graft combined with locking compression plate. Among patients, there were 8 males and 4 females aged from 20 to 69 with an average of 47 years old. The time from first injuries to bone nonunion was from 9 months to 5 years, avergaed 19 months. Four cases were treated with external fixation, 6 cases were treated with plate fixation, 2 cases of 12 patients occurred broken of plate and nail. Eleven patients were non-infective bone nonunion and 1 patient was infective bone nonunion. Preoperative X-ray and CT showed all patients had sequestration and formation of ossified bone with different degrees. Operative time, blood loss, wound healing were observed, fracture healing time was evaluated by postoperative X-ray. Johner-Wruhs scoring standards was used to evaluate ankle joint function after operation at 10 months.
RESULTSOperative time ranged from 90 to 185 min with an average of (125.00±20.15) min; blood loss ranged from 225 to 750 ml with an average of (415.00±120.00) ml. All patients were followed up from 10 months to 2.5 years with an average of 1.5 years. Postoperative X-ray showed bone union was formed around fracture after operation at 4 months in all patients, 3 cases obtained bone healing within 6 months after operation, 9 cases obtained from 8 to 12 months. No infection, injury of nerve and vessles, and broken of plate and nail were ocurred. According to Johner-Wruhs scoring at 10 months after operation, 10 cases obtained excellent results, 1 good and 1 moderate.
CONCLUSIONAdvanced bone graft combined with locking compression plate, which can build fracture multi-point supporting based on full compression of bone nonunion to get effective fixation, is an effective method in treating middle and distal tibia nonunion.
