1.The clinical and radiographi c characteristics of erosive hand osteoarthritis

Objective To evaluate the clinical and radiographic characteristics and function of erosive hand osteoarthritis (EOA) patients. Methods Data were obtained from 19 patients with EOA, including their social conditions, clinical conditions, radiographic scores and hand function evaluation. The number of hand osteoarthritis (HOA) patients was 312. The control group consisted of non-EOA patients with hand osteoarthritis with a ratio of 4:1 to EOA patients. A non-parameter test analysis was performed. All data were analyzed by SPSS 23.0 statistical analysis, t test, χ2 test, Fisher exact probility and Spearman's correlations analysis were used for statistical analysis. Results Totally data of 19 patients were collected. Eighteen were female. Onset age was (56±8). Average duration was 56 (12~120) months. FIHOA scores of all the EOA patients were at least 5. All the erosions of 39 joints were characteristically central and erosive changes in 7 joints (18%) showed up as gull-wing. Among 39 erosive joints, including 12 (31%) E and 27 (69%) R, 34 (87%) distal interphalangeal joints were involved. Data analysis found out that EOA patients had longer disease duration (Z=2.610, P=0.009), more severe K-L level (44 ±11 vs 26 ±7, t=7.134, P<0.01), higher AUSCAN total score (28±6 vs 21±7, t=3.781, P<0.01) and higher AUSCAN function score (18±6 vs 12±6, t=4.042, P<0.01). The differences of ESR and CRP were not significant between EOA and non-EOA patients. Conclusion Erosions seen in EOA patients are centrally located gull-wing in the DIP joints. EOA patients have longer duration, more severe radiographic damage and worse joint function.
2.Expression and its significance of Cyclin D1 in oral squamous cell carcinoma.

OBJECTIVETo investigate the expression and significance of Cyclin D1 in oral squamous cell ma (OSCC).
METHODSA immunohistochemistry method, Envosion, was employed to test the manifesting Cyclin D1 in pathological slices of 50 OSCC cases and 10 normal cases, and the results was treated with statistical lysis.
RESULTSIn 50 OSCC cases, Cyclin D1 mainly manifested in karyon, and a little in cytoplasm. manifesting rates of Cyclin D1 in the samples was 80.0%, which was significantly higher than the manifesting of 20.0% in normal oral mucous membrane (P < 0.01). The manifestation of Cyclin D1 was correlated with rent pathological grades, clinical phases and lymph node metastasis (P < 0.05).
CONCLUSIONThe abnormal tation of Cyclin D1 is closely related with the occurrence and development of OSCC. Therefore, it can subsidiary index for OSCC treatment and prognosis.

3.Purification of monoclonal antibody to clenbuterol and its biology identity.

OBJECTIVETo identify the self-preparation monoclonal antibody which target to clenbuterol, and set up the standard curve to clenbuterol (CL) detection.
METHODSThe affinity constants and activity of the monoclonal antibody which target to CL were determined by ELISA. ELISA was also used to confirm whether the monoclonal antibody had any across-reaction with BSA and CL analogues. The rat ascites which contains the monoclonal antibody target to CL was purified by (NH4)2SO4 salt-out method and further by affinity column. At last, the CL detection standard curve which based on indirect competition ELISA was established.
RESULTSThe ELISA experiment showed that the antibody titer was 10(6) and the monoclonal antibody affinity constants was 2.90 x 10(10) L/mol. The result of the indirect competition ELISA confirmed that the monoclonal antibody had no cross-reaction with BSA and a few kind of CL analogue. CL detection standard curve based on indirect competition ELISA was established, which R2 was 0.9812, and the lowest detectable limit was 1.0 ng/ml.
CONCLUSIONThe standard curve based on indirectly competitioning ELISA was established. The self-preparation monoclonal antibody which target to CL has high affinity and high specific to CL, which had established the foundation to the advanced development of the CL immune test paper and CL ELISA kit.

4.Assessment of early-stage physiological response to acute lung injury in canine models using balloon catheter system for measuring esophageal and gastric pressure.

OBJECTIVETo explore feasible and reliable methods for estnbolishment and of acute lung injury model in animal models.
METHODSTwenty-four healthy adult mongrel dogs with oleic acid-induced acute lung injury were evaluated for early-stage physiological response to acute lung injury using a balloon catheter system for measuring esophageal and gastric pressure.
RESULTSIn canine models of early-stage oleic acid-induced acute lung injury that sustained spontaneous breathing, in terms of respiratory mechanics, some parameters obviously increased including the respiratory rate (RR), minute ventilation (VE), peak inspiratory volume (Vinsp, peak), mean inspiratory volume (VT/Ti), inspiratory airway resistance (Raw, insp) (P<0.001 for all the parameters), with also significantly increased peak transdiaphragmatic pressure (Pdi, peak, P=0.0185). The tidal volume (VT) and dynamic lung compliance (CL,dyn), however, were significantly decreased (P<0.001), and significant variation occurred in the ratio of inspiratory time to duration of one breath (Ti/Ttot, P=0.163). In terms of gas exchange, the pH, PaO(2), SaO(2), PaO(2)/FiO(2), and end tidal partial pressure of carbon dioxide (PETCO(2)) all evidently declined (P<0.001), but PaCO(2) and ratio of alveolar dead space to tidal volume [VD(alv)/VT] increased significantly (P<0.001).
CONCLUSIONApplication of balloon catheter system for measuring esophageal and gastric pressures allows objective evaluation of the various physiological responses in early stage of acute lung injury.

5.Effect of mulberry leaves extracts on glucose uptake of insulin-resistant HepG2 cells and the mechanism.

The effect and mechanism of mulberry leaves extracts (MLE) on glucose uptake of insulin-resistant HepG2 cells in vitro was explored. The insulin resistant models of HepG2 were induced by high concentration of insulin for 24 h. The models were incubated in a buffer containing mulberry leaves extracts. The glucose consumption was detected by glucose assay kits and the AMP-activated protein kinase (AMPK), Akt activation was examined by Western blotting. Mulberry leaves polysaccharides, mulberry leaves flavonoids and mulberry leaves extracts advanced glucose uptake of insulin-resistant HepG2 cells; Mulberry leaves extracts enhance phosphorylation of AMPK. Mulberry leaves extracts do not change the phosphorylation status of Akt. The glucose consumptions of insulin resistant model of HepG2 were promoted by mulberry leaves extracts. MLE stimulates HepG2 cell AMPK activity acutely without changing the Akt activity.

6.Expression and significance of angiopoietins in oral squamous cell carcinoma.

OBJECTIVETo observe the expression of angiopoietins in oral squamous cell carcinoma and relationship between the expression of angiopoietins and pathologic classification.
METHODSThe expression of Ang-1 and Ang-2 protein in samples from 42 oral squamous cell carcinoma and 16 oral normal mucosa were detected by immunohistochemical technique.
RESULTSThe expression of Ang-1 and Ang-2 was found in both oral squamous cell carcinoma and normal control. It was found Ang-1 expression alike in oral squamous cell carcinoma and control (P > 0.05). Ang-2 expressed at low level in control while strongly positive in oral squamous cell caroinoma and the level of Ang-2 expression in oral squamous cell caroinoma was related to pathologic classification (P < 0.05).
CONCLUSIONAng-2 expression in oral squamous cell carcinoma is related to angiogenesis and pathological classification, which is probably involved in angiogenesis regulation, promotes the development and metastasis of oral squamous cell carcinoma.

7.Effects of different interventions for early-stage acute lung injury on respiratory response and work of breathing in canine model.

OBJECTIVETo evaluate the effects of different interventional strategies, namely controlled high-concentration oxygen therapy, continuous positive airway pressure (CPAP) and bi-level positive airway pressure (BiPAP) ventilation, on respiratory response and work of breathing (WOB) in canine models of early-stage acute lung injury (ALI).
METHODSAfter successful duplication of ALI models with oleic acid (diagnostic criteria: Pa(O2)/Fi(O2) RESULTSBiPAP resulted in the most significant effects in reducing the respiratory rate (RR) and f/V(T) (P<0.001), followed by CPAP and O2 interventions (P<0.001). None of the 3 treatments showed obvious effects on V(E) (P>0.05), which maintained the level of early ALI/ARDS stage. BiPAP greatly improved V(T) and V(T)/Ti, showing better effects than CPAP and O2. No significant differences were noted among the 3 groups in T(I)/T(tot) (P>0.05). BiPAP showed superior effect to CPAP in lowering the peak transdiaphragmatic pressure (Pdi). CPAP and BiPAP both effectively counteracted intrinsic positive end expiratory pressure (PEEPi) (P<0.01), while O2 produced no obvious such effects (P>0.05). BiPAP showed the most evident effects, followed by CPAP, in reducing WOB, but oxygen therapy produced no obvious effects. CPAP (P<0.01) and BiPAP (P>0.05) both effectively reduced the proportion of ingredients in WOB related to PEEPi. CONCLUSIONBiPAP and CPAP can produce favorable effects in relieving dyspnea, reducing WOB and improving respiratory response to control the deterioration of ARDS. BiPAP has more significant therapeutic effects than CPAP and oxygen therapy.

8.Evaluating application effect of medical messenger software on patients' rehabilitation guidance in Hand Surgery

Objective To evaluate application effect and significance of medical messenger on patients' rehabilitation guidance.Methods Two hundred and forty rehabilitation patients in hand and foot surgery from March 2011 to March 2012 were divided into the study group (n =120) and the control group (n =120).Patients in both groups were all receiving systematic functional rehabilitation training for 6 weeks (including passive and active m(o)tion exercise,strength training and occupational therapy for 45 main per day).Besides rehabilitation training,patients in the study group received rehabilitation-guidance messages sent by medical messenger on each Friday for 13 weeks.Function of the injured hand was assessed with Upper Extremity Functional Index (UEFI) and the active range of motion (AROM) at the 1,4,7,10,13 week,and patients'satisfaction was evaluated at the 4,7,13 week.The follow-up visit rate at the thirteenth week was analyzed in two groups.Results The thirteenth week follow-up results showed that UEFI of the control group improved from (0.75±0.14) to (2.99±0.13),and UEFI of the study group improved from (0.78 ±0.17) to (3.74 ±0.25),and the difference was statistically significant (t =3.50,P < 0.05).Increase of AROM was (30 ± 1.25) ° in the control group,and (49 ± 2.94) ° in the study group,and the difference was statistically significant (t =7.72,P < 0.05).Satisfactory rate of the control group was (70 ± 6.92),and was (94 ± 4.28) in the study group,and the difference was statistically significant (t =2.05,P < 0.05).Follow-up visit rate of the control group was 75%,of the study group was 98%.Conclusions The application of medical messenger software on patients' rehabilitation guidance can increase the level of active participation of patients,accelerate the progress of rehabilitation,improve the effectiveness of rehabilitation,and increase patients' self-confidence of returning to society.
9.Development of high throughput targeted phospholipidomics based on liquid chromatography-mass spectrometry

Objective To establish a high throughput targeted method to study phospholipids profding and to screen out and quantitate the potential biomarkers in urine samples.Methods The phospholipids in urine was extracted by modified MTBE(methyl tert-butyl ether)method.Semi -quantitative analysis of phospholipids in urine was realized by using high performance liquid chromatography-electrospray ionization-qua-drupoles/trap (HPLC-ESI-Q/Trap) technique.7 standards and 15 endogenous phospholipids were chosen to conduct method validation,including specificity,sensitivity,precision,matrix effect,carryover effect,stability and recovery.Principal component analysis (PCA) of quality control (QC) samples interspersed during the detection process was used to evaluate the reliability of the data obtained.Results The lower limit of quantitation of 7 phospholipids were phosphatidylcholine and lysophosphatidylcholine 0.25 ng· mL-1,phosphatidylethanolamine and phosphatidylserine 2.00 ng· mL-1,phosphatidylglycerol and phosphatidylinositol 1.00 ng · mL-1,sphingomyelin 625.00 pg · mL-1,respectively.During the continuous analysis,the relative standard deviation (RSD) of the retention time was 0.72%-3.44%,the peak area was 0.71%-10.53%.The recoveries of the 7 phospholipids were in the range of 54.05%-105.73%.All samples were stable after being stored 12 h at room temperature,being stored 24h after preparation,two freeze-thaw cycles and being cryopreserved 1 month at-80 ℃.QC samples in the first principal component diagram showed that the data was reliable.Conclusion The developed HPLC-ESI-Q/Trap method was simple,stable and sensitive,which can be applied to the subsequent study of large sample size of phospholipidomics research and quantitative analysis of potential urinary phospholipids biomarkers.
10.Changes of histopathology and molecular indicators in breast cancer after neoadjuvant chemotherapy

Purpose To observe the changes of histopathology and expression levels of ER, PR, HER-2 and Ki-67 in breast cancer after neoadjuvant chemotherapy (NTC), and to evaluate the relationship between the curative effect and clinico-pathological characteristics of breast cancer. Methods 93 ca-ses of invasive breast cancer with NTC were collected and retro-spectively analyzed. Pathologic evaluation of chemotherapeutic effect were evaluated by Miller-Payne (MP) grading system. Results Tumor cells, tumor stroma and lymph nodes status presented different histopathological changes after NTC. There were significant relationship between curative effect and patients age (Z=-1.993, P=0.046 ), histological grade (χ2=7.261, P=0.027), molecular subtypes (χ2=8.289, P=0.040), while it had no statistical relationship between curative effect and tumor size (Z=-1.091, P=0.275) and lymph node status (Z=-1.107, P = 0.268). Expression of ER, PR, HER-2 and Ki-67 showed different degrees of change before and after NTC. The concordance rates of ER, PR, HER-2 and Ki-67 were 81.0%, 72.2%, 83.5% and 55.7%, respective-ly. And there was no significant difference in expression of these four molecular indicators before and after NTC (χ2 =0.428, P=0.934). Conclusion The histomorphology of tumor cell, tumor stroma and lymph node status can be influenced by NTC. Objective evaluation of the changes of histopathology and molecular indicators after NTC may valuable in predicting clinical prognosis and guiding individual treatment of breast cancer.
