1.Relationship between essential hypertension and the quantity of apolipoprotein CⅢ-455C gene

Objective To study the changes of T-C of apolipoprotein CⅢ (ApoCⅢ)-455C gene and to de-tect the relationship between essential hypertension (EH) and the quantity of ApoCⅢ-455C gene. Methods 606 subjects were divided into hypertension group(n=306) and non-hypertension group(n=300). And the two groups 606 were divided into subgroup A (n=90) who had hypertension companied with hypertriglyceridem, subgroup B (n=216)who had hypertension only, subgroup C (n=51) who had hypertriglyceridem only and subgroup D (n= 249) who had none of the two diseases, by the concentration of triglycerides (TG) (TG≥1.69 mmol/L). The quan-tity of ApoCⅢ-455 C was detected by Real Time PCR. TC, TG, HDL-C, LDL-C, ApeA, ApoB 100, ApoCⅢ, ApoE, blood glucose,plasma insulin and waist circumference were measured in all cases. Results There was no difference in the quantity of ApoCⅢ-455C gene between group B and the group D [(45.16±5.97) vs (46.21±6.13)] (P>0.05). The concentration of ApoCⅢ, ApoB100 and the quantity of ApoCⅢ-455C gene in group A were higher than those in group B [0.14±0.03]g/L, (95.00±15.69)g/L, (28.13±4.11) vs (0.09±0.03)g/L,(81.22± 11.87)g/L,(45.16±5.97)(P<0.01)]. It was similar in ApoE [(0.05±0.01) g/L vs (0.04±0.01)g/L] and LDL-C [(2.54±0.61)mmol/L vs (2.15±0.41)mmol/L] (P<0.05). Conclusions The quantity of ApoC Ⅲ-455C gene is not related to EH.

Objective To investigate the effects of low molecular heparin(LWMH)on cytokines(TNF-?,IL-1? and IL-10)in blood plasma and bronchoalveolar lavage fluid of invasive pulmonary aspergillosis(IPA)mice.Methods The neutropenic IPA mouse models were established by administration of cyclophosphamide for immunologic function inhibition and intranasally challenge with Aspergillus fumigatus conidia(1?106 conidia/mouse).One hundred and twenty mice were randomly divided into 4 groups:normal control,IPA model,normal saline+LWMH and IPA+LWMH group.Normal saline+LWMH group and IPA+LWMH group received LWMH(subcutaneous injection,1 000 IU/kg,qd?2 d).Normal control and IPA model group received normal saline instedad of LWMH.At 4,8,12,24 and 48 h after inoculation,six mice were randomly taken from each group to be sacrificed.ELISA method was used to determine the concentrations of TNF-?,IL-1? and IL-10 in blood plasma and BALF.Results TNF-?,IL-1? and IL-10 in blood plasma and BALF increased significantly several hours after inoculation of conidia in IPA model and IPA+LWMH group.There were significant higher concentrations of TNF-? and IL-1? in blood plasma and BALF in IPA+LWMH group than in IPA model group(P

Objective To investigate the clinical effect and safety of miconazole nitrate 1200 mg in treating vulvovaginal candidiasis(VVC).Methods An open,multicentre,non case control clinical trial was conducted in 568 patients suffering from VVC from Jul 1,2006 to Nov 30,2006.Routine examination,score of clinical symptoms and physical signs,mycetology test and safety evaluation were done in all patients before treatment,7-14 days after treatment and 30 days after treatment.Results Seven to fourteen days after treatment,563 patients could be followed and 323 patients(57.30k)were cured.The overall effective rate was 90.2%.The mycologic cure rate was 91.3%(514).Thirty days after treatment,480 patients could be followed and 411 patients(85.6%)were cured.The total effective rate was 96.0%.Mycologic cure rate was 92.3%(443/480).Adverse effect rate was 2.7%(15/563)and they were relieved without any treatment in one or two days.Conclusions Miconazole nitrate 1200 mg is effective in the treatment of WC,with good compliance and few adverse effects.Moreover,it can be accepted easily.

Objective To evaluate the clinical and radiographic characteristics and function of erosive hand osteoarthritis (EOA) patients. Methods Data were obtained from 19 patients with EOA, including their social conditions, clinical conditions, radiographic scores and hand function evaluation. The number of hand osteoarthritis (HOA) patients was 312. The control group consisted of non-EOA patients with hand osteoarthritis with a ratio of 4:1 to EOA patients. A non-parameter test analysis was performed. All data were analyzed by SPSS 23.0 statistical analysis, t test, χ2 test, Fisher exact probility and Spearman's correlations analysis were used for statistical analysis. Results Totally data of 19 patients were collected. Eighteen were female. Onset age was (56±8). Average duration was 56 (12~120) months. FIHOA scores of all the EOA patients were at least 5. All the erosions of 39 joints were characteristically central and erosive changes in 7 joints (18%) showed up as gull-wing. Among 39 erosive joints, including 12 (31%) E and 27 (69%) R, 34 (87%) distal interphalangeal joints were involved. Data analysis found out that EOA patients had longer disease duration (Z=2.610, P=0.009), more severe K-L level (44 ±11 vs 26 ±7, t=7.134, P<0.01), higher AUSCAN total score (28±6 vs 21±7, t=3.781, P<0.01) and higher AUSCAN function score (18±6 vs 12±6, t=4.042, P<0.01). The differences of ESR and CRP were not significant between EOA and non-EOA patients. Conclusion Erosions seen in EOA patients are centrally located gull-wing in the DIP joints. EOA patients have longer duration, more severe radiographic damage and worse joint function.
6.Management of hemodynamically unstable pelvic fracture in pregnancy: a case report and review of literature.

We present an unusual case of an unstable pelvic fracture during pregnancy period, who suffered fetal death and splenic rupture simultaneously which developed massive delayed hemorrhage in abdomen. When considering potential causes of fetal death, direct trauma to the uterus, placenta, or fetus was not associated with a higher fetal mortality rate, compared with maternal hemorrhage. A cesarean section and splenectomy could rescue the maternal life from the hemorrhage situation. Successful treatment of these rare cases is possible with careful pre-, peri-, and post-operative evaluation of the mother and fetus by a multidisciplinary team.

7.Metabolites of R, S-1-(2-methoxyphenyl)-4-3-(naphtha-1-yl-oxy)-2-hydroxypropyl-piperazin in rat plasma.

AIMTo study the metabolites of R, S-1-(2-methoxypheyl)-4-[3-(naphthal-yl-oxy)-2-hydroxypropyl] -piperazine, (naftopidil, NAF), a novel antihypertensive drug in rat plasma.
METHODSThe rat plasma samples were analyzed by LC/MS after oral administration of NAF. According to MS relativity of metabolites and parent compound (NAF) and metabolic rule of compound with similar structure, the structure of potential metabolites were postulated. Phase I metabolites were identified by HPLC/MS and by comparison with authentic standards, phase II conjugates were indirectly identified with beta-D-glucuronidase in presence or absence of glucuronidase selective inhibitor D-saccharric acid beta-1,4-Lactone.
RESULTSPhase I metabolites desmethyl-naftopidil (DMN), (phenyl) hydroxynaftopidil (PHN), (naphthyl) -hydroxy-naftopidil (NHN) were separated and identified in rat plasma by comparison with reference substances, phase II conjugates, NAF and NHN glucuronide conjugates were separated and tentatively identified by hydrolysis with glucuronidase, the aglycones, NAF and NHN, were identified in rat plasma.
CONCLUSIONThe major metabolic pathway of NAF in rat plasma should be the hydroxylation of the phenyl or nephthyl moiety of NAF and demethylation of NAF. Therefore, (naphthyl) hydroxyl-metabolite and NAF followed by conjugation with beta-glueuronic acid.

8.Application of tissue granule of oral mucous in plastic surgery.

OBJECTIVETo study the application of tissue granule of oral mucous in plastic surgery.
METHODSTissue granule of oral mucous was placed on glutin sponge and they were used for urethral (12 cases) and vaginal (14 cases) reconstruction in 26 cases.
RESULTSSatisfactory results were achieved in 24 cases. One case of urethral fistula and one case of urethral meatus stricture were happened. The two cases underwent secondary operation.
CONCLUSIONSTissue granule of oral mucous membrane is good supply for repairing mucous defect. It has the advantages of high growth and survival rate, and less shrinkage. It is useful for urethral or vaginal reconstruction which are covered with mucous membrane.

9.Reconstruction of orbital deformity after radiotherapy with transcranial orbitotomy advancement combining free flap.

OBJECTIVETo study clinic therapeutic effect about reconstruction of severe orbital and cul-de-sac deformity after the radiotherapy with transcranial orbitotomy advancement combining cascade free flap both dorsum pedis flap and anterior tibial fascial flap.
METHODSFive cases was subjected to orbital and cut-de-sac severe deformities after both operation and radiotherapy because of retinoblastoma. The technique included transcranial orbital advancement by anterior orbital osteotomy and rigid fixed with titanic plate by coronal incision, and meanwhile incising the cul-de-sac which would be extended circumference around the central incision separation, and then designing extent of cascade flap consisted of dorsum pedis flap and anterior tibial fascial flap according to the size of cul-de-sac defect and extent of temporal depression. Then, the aforementioned two parts of cascade flap were transplanted into cul-de-sac and temple respectively. There is either the superficial temporal artery and vein or facial artery and jugular vein to chose vascular anastomosis.
RESULTSAll flaps survived. After 3 to 6 months following up, the results showed satisfactory orbital contour and temporal depression improved significantly in all cases. After the conjunctival sac were fixiformed with prefabricated eye prosthesis mode about 3 months. 3 cases have good appearance with wearing eye prosthesis and the other 2 cases' appearance is poor. One of the poor appearance cases, with depressed eye socket, have orbital implant underlying conjunctival sac in secondary operation. The other one, with swallowed inferior fornix, is transplanted autogenous hard palatal mucosa into inferior fornix in secondary operation. In addition, delayed healing in donor site of dorsum pedis occurred in one of the 4 cases.
CONCLUSIONSIt is a reliable procedure about reconstruction of severe orbital and cul-de-sac deformity after both the operation and radiotherapy with transcranial orbitotomy advancement combining cascade free flap both dorsum pedis flap and anterior tibial fascial flap. All deformity was corrected by one staged procedure which lessen patient's suffering and shorten patient's hospital stay and spare patient's costs.

10.Quantitative determination of ginkgolides by liquid chromatography-electrospray mass spectrometry.

OBJECTIVEThis paper reports a method based on liquid chromatography coupled with electrospray mass spectrometry for the analysis of terpenoids in ginkgo laminae.
METHODThe analysis was performed on ZORBAX RX-C18 (2.1 mm x 150 mm) column with methanol-water(with gradient) as mobile phase at a flow rate of 0.25 mL x min(-1), and column temperature of 25 degrees C. Analyses were carried out in the selected ion monitoring (SIM) mode.
RESULTGinkgolides (GA, GB and GC) and bilobalide were quantitatively detected by external standardization with linear in the range of 4.04-1.012 x 10(2) ng, with coefficient and relative standard deviations being 0.993 7-0.999 8 and 2.50%-4.73%. LC-ESI-MS shows a greatly increased sensitivity compared with other methods. The detection limit of this method by SIM was 1.47 x 10(-3)-0.320 microg x mL(-1).
CONCLUSIONThis method is specific, reproducible, rapidly and permits quantitative analyses of ginkgolides and bilobalide in different samples with simple pre-purification steps.
