1.Inclusion complex of the volatile oil from Duhuo Jisheng Granule

AIM: To compare N-LOK with ?-CD on inclusion ability for the volatile oil from Duhuo Jisheng Granula (Radix Angelicae, Pubescentis, Herba Asari, Radix Saposhni-koviae, etc). and its process. METHODS: Colloidal mill and mixer were adopted, and analysis was on use of DSC and GC/MS. RESULTS: N-LOK inclusion complex included oil ratio was twice as large as the ?-CD, GC/MS analysis showed that the main components of volatile oil before and after being covered had no significant difference. CONCLUSION: N-LOK inclusion complex included oil ratio was far higher than the ?-CD. N-LOK inclusion complex could spray to dry directly, but ?-CD inclusion complex needed refrigeration and low temperature were necessary. The method of covered with N-LOK is suitable for mass production in the traditional Chinese patent medicine in the size of manufacture.
2.Study on Preparation Technology of Repaglinide and Metformin Hydrochloride Tablets

Objective:To design and prepare repaglinide and metformin hydrochloride tablets for pilot scale test and industrial pro-duction. Methods:Dissolution was applied to optimize the formula and preparation process. Results: The tablets were prepared ac-cording to the determined formula and process with poloxamer 188, meglumine, PVP K30, croscarmellose sodium, microcrystalline cellulose PH101, sorbitol, magnesium stearate and coating powder as the adjuvants. The dissolution behavior of the tablets in four media was similar to that of the reference. Conclusion:The preparation process of repaglinide and metformin hydrochloride tablets is simple and feasible, and the quality is controllable and stable.
3.Effect of body mass index on setup errors in intensity-modulated radiotherapy for cervical cancer

Objective:To investigate the effect of body mass index (BMI) on setup errors in intensity-modulated radiotherapy for cervical cancer and explore the optimal position for patients with different BMI without taking into account the rotation error and the changes in target area and adjacent organs.Methods:A total of 90 patients were divided into three groups according to their BMI: light weight group (BMI≤18.4 kg/m 2), normal weight group (18.5 kg/m 2≤BMI≤23.9 kg/m 2) and overweight group (BMI≥24 kg/m 2). Thirty patients were assigned into each group including15 patients in the supine position and 15 patients in the prone position. In total, 2 250 sets of CBCT scan data of 90 patients were obtained. The setup errors were recorded and analyzed in each group. The margins of the optimal position were calculated according to the formula of M PTV=2.5+ 0.7. Results:When BMI was not taken into account, there was no significant difference in the setup errors between the supine and prone positions in the x, y and z directions (all P>0.05). When BMI was considered, the setup error in the supine position were significantly smaller than those in the prone position in the x and y directions in the light weight group, whereas there was no significant difference in the setup errors between the supine and prone positions in the z direction ( P>0.05). The corresponding M PTV in the supine position was 4.76, 4.27 and 5.73 mm, respectively. In the normal weight group, there was no significant difference in the setup errors between the supine and prone positions in the x and y directions (both P>0.05), whereas the setup error in the prone position was smaller than that in the supine position in the z direction. The corresponding M PTV in the prone position were 6.42, 10.21 and 4.91 mm, respectively. In the overweight group, there was no significant difference in the setup errors between the supine and prone positions in the x and z directions (all P>0.05), whereas the setup error in the prone position was smaller than that in the supine position in the y direction. The corresponding M PTV in the prone position were 5.88, 5.26 and 5.32 mm, respectively. Conclusions:Without taking into account the rotation error and the changes in target area and adjacent organs, when the BMI≤18.4, the supine position is recommended. When the BMI≥18.5, it is better to choose the prone position.
4.Reflection on Opening of Section for Outpatients for Risk Assessment and Communication of Medication Use in Pregnancy by Pharmacists

Objective:To discuss the necessity of the opening of a pharmacist section for outpatients for risk assessment and com-munication of drugs in pregnancy by surveying the use situation and risk reasons of the drugs was in pregnancy. Methods:Referring to the risk classification of medicines in pregnancy formulated by FDA and integrating various factors in pregnant women, such as drug dosage, administration time, genetic factors, prenatal care and potential diseases et al, pharmacists established the section for outpa-tients for risk assessment and communication of drugs in pregnancy. Results:The visiting number of pregnant women was gradually in-creased after the establishment by reviewing the clinical data from the hospital information system. The new pharmacy service mode pro-vided by pharmacists for obstetrical patients was positively recognized by physicians and patients. Conclusion:The section for outpa-tients for risk assessment and communication of drugs in pregnancy should be established in order to promote the pharmaceutical knowl-edge in pregnancy and improve the medication safety.
5.Clinical analysis on 28 cases of head-neck malignant fibrous histiocytoma

Objective:To analyze the clinical feature and treatment of head-neck malignant fibrous histiocytoma (MFH).Method:A retrospective analysis on the clinical data of 28 cases of MFH were carried out, of which 9 were in the maxilla, 3 in the nasal cavity, 5 in the larynx, 3 in the parotid, 3 in the temporal bone, 1 in the hypothyroid, and 4 in the head and neck region. All of the cases underwent immune histochemistry: 4 cases of surgery alone, 23 cases of surgery followed by radiotherapy, and 1 case of radiotherapy alone.Result:Except 5 cases lost follow-up after 1 year, all the other cases were followed-up over 3 years, the survival rate for 1 or 3 years was respectively 96.4%(27/28) and 57.1%(16/28); 23 cases were followed up for 5 years, the survival rate was 26.1%(6/23). The recurrent rate in 3 years was 60.7%(17/28) ,with 1 to 7 times recurrence at a mean interval of 5.6 months. Twelve recurrent cases were adopted expanded resection of non-defined operation except 1 case with radiotherapy.Conclusion:The diagnosis of MFH depends on the technology of immune histochemistry. Early diagnosis, expanded resection, and integrated therapy could reduce the recurrence and increase the survival rate; the recurrence could adopt expanded resection of non-defined operation to prolong the life.

7.Ultrasound imaging of acardia twin before and after blood spontaneously blocked and pump twin′s outcome analysis in twin reversed arterial perfusion sequence

Objective To discuss the ultrasound imaging of acardia twin before and after spontaneously blood blocked in twin reversed arterial perfusion sequence (TRAP), and to analyze the inlfuence factor for the prognosis of pump twin. Methods Seven TRAP with pump twins and acardia′s blood lfow blocked were diagnosed by US and autopsy in Hubei Women and Children′s Hospital between 2001 January to 2012 September. The ultrasound images, clinical data and pump twin’ outcome were analyzed. Results Ultrasound images showed skin edema, acardia, spine and lower limbs in seven acardia cases before blood blocked, among which 4 were acardius acephalus, 3 were acardius anceps. Single umbilical artery were detected in 7 cases with reversed umbilical artery perfusion toward fetus. Four cases had rare blood lfow (UA-PI ratio<0.7. Five acardia cases had slower growing rate than the pump twin. Ultrasound images showed no growing, no blood lfow in the acardia, the acardia twin became vagued in the second and third trimester, and ifnally developed into a paper fetus. A linear umbilical cord extend from umbilical region to placenta were found in 7 cases whose blood lfow were spontaneously blocked. Pump twin′s outcome:3 pump twins survived, 3 died in uterus, and 1 was induced labor due to hydrocephalus and cardiac failure. The most important factors that affected the prognosis of pump twin included:faster growing in acardia twin with less blood supply, abnormal brain and heart function in pump twin, chromosome abnormalities, abnormal amniotic fluid and cord entanglement. Conclusions Less blood flow and slow growth speed prompt in acardia twin suggested spontaneously acardia blood block. It is mandatory to monitor the pump twin after the spontaneous block of acardiac’s blood lfow.
8.Expressions and correlation of HPA, CK2beta and HIF-1alpha in nasopharyngeal carcinoma.

OBJECTIVE:
To investigate the expression of HPA, CK2beta and HIF-1alpha gene in nasopharyngeal carcinoma (NPC) tissues, and the correlation between their expression with the clinical characteristics of NPC and the relativity of HPA, CK2beta and HIF-1alpha gene in NPC tissues.
METHOD:
HPA, CK2beta and HIF-1alpha were detected with Super-Vision immunohistochemical method using antibody in 49 NPC specimens and 30 specimens with chronic nasopharyngitis tissue (CNT).
RESULT:
The expression of HPA, CK2beta and HIF-1alpha in NPC tissue were significantly higher than those in CNT tissue (P<0.05, separately). The expression of HPA, CK2beta and HIF-1alpha were significantly related to the TNM stage and whether recurrence or metastasis occur after treatment (P<0.05, separate ly), but there was no obvious correlation between its expression and the sex of NPC patient (P>0.05). The expression of HIF-1alpha was significantly related to the age of NPC patient (P<0.05), while HPA, CK2beta were not. The expression of HPA, CK2beta and HIF-1alpha in NPC tissue was positively correlated with each other (P<0.05, separately).
CONCLUSION
HPA, CK2beta and HIF-1alpha play synergetic role in development of NPC, which plays an important role in invasiveness,recurrence and metastasis of NPC. There could be a positive cooperation among HPA, CK2beta and HIF-1alpha in the carcinogenesis and development of NPC.

10.The value of galactomannan detection in bronchoalveolar lavage fluid in the diagnosis of invasive pulmonary aspergillosis in elderly patients with lung diseases

ObjectiveTo evaluate the role of galactomannan (GM) detection in bronchoalveolar lavage fluid (BALF) in the diagnosis of invasive pulmonary aspergillosis (IPA) in elderly patients with lung diseases.MethodsThe elderly patients with lung diseases and suspected of having IPA were enrolled. BALF for culture and GM detection, and serum samples for GM detection were obtained from all participants.According to European Organization for Research and Treatment of Cancer/Mycoses Study Group (EORTC/MSG) with host factors, clinical criteria, mycological evidence and histological or cytopathological evidence, all patients were categorized into the proven, probable,possible and no IPA patients. The IPA patients were considered as IPA group while no IPA was considered as control group. ResultsAmong 76 enrolled elderly patients, 18 cases were diagnosed with IPA (1 proven, 11 probable and 6 possible), the remained 58 patients were as control group.Based on a GM index cutoff value of ≥0. 5, the sensitivity, specificity, positive predictive value and negative predictive value for BALF were 83.3%, 82. 8%, 60.0% and 94.1%, respectively; these variables in serum were 55.60% , 91.4 %, 66.7 % and 86.9% , respectively. Using a GM index cutoff value of ≥1.0, the specificity and positive predictive value of both BALF GM and serum GM reached 100%, but the sensitivity and negative predictive value of BALF GM were higher than serum GM (66.7% vs. 22.2%, 90.60% vs. 80.60%, respectively). Receiver operating characteristic curve analysis yielded a sensitivity of 83.4% and a specificity of 96.6% at a BALF GM index cutoff value of ≥0.725. ConclusionsGM detection in BALF seems to be a useful tool in the diagnosis of IPA in elderly patients with lung diseases,and could be widely used in future.
