1.Essence of professor SHI Xin-de's experience in application of persicae ramulus-associated prescriptions.
China Journal of Chinese Materia Medica 2014;39(13):2569-2572
Persicae Ramulus decoction, as the first prescription in Treatise on febrile Diseases, has been recommended by physicians of successive generations. It is also the general prescription for harmonizing yingfen and weifen, yin and yang, qi and blood. Although it only consists of five herbal medicines, it has a wider range of application and more categorized formulas than other prescriptions. Though Persicae Ramulus decoction was originally formulated to treat taiyang apoplexy, it has functions beyond the treatment of exopathic diseases. This formula is also effective in treating internal diseases, surgical diseases, gynecologic diseases, paediatric diseases, etc. KE Yun-bo praised it as the No. 1 formula among ZHANG Zhong-jin's formulas as well as the general prescription for harmonizing Yin and Yang, yingfen and weifen, resolving fleshy exterior and inducing perspiration. Professor SHI Xin-de has been expert at treating intractable diseases by using Persicae Ramulus-associated prescriptions, such as Xiaojianzhong decoction and Baohe pill for children's chronic eczema, Persicae Ramulus and Puerariae Lobatae Radix decoction and Yupingfeng powder for chronic nephritis, and Persicae Ramulus and Longgu Muli decoction for insomnia. Instead of being restricted to Chinese or Western disease names, he prescribed appropriate formulas according to syndromes, thereby achieving a good efficacy.

Objective To investigate the surgical methods and experience of laparoscopic radical cystectomy and orthotopic ileal neobladder for invasive bladder cancer. Methods The clinical data of 14 patients with invasive bladder cancer underwent laparoscopic radical cystectomy and orthotopic ileal neobladder were collected retrospectively during March 2011 and October 2014. Results The 13 patients with invasive bladder cancer were successfully completed laparoscopic radical cystectomy and orthotopic ileal neobladder. 1 case was treated with laparotomy because of unsatisfactory surgery ifeld caused by excessive tumor bleeding. Twelve cases of the urethra-neobaldder anastomosis were completed through the abdominal incision, while for the other 2 cases, the anastomosis was done under the laparoscope, 2 cases were performed neovesicourethral anastomosis using single-needle running sutures through laparoscopy. The median operative time was 444 minutes, the mean intraoperative blood loss was 490 ml. Postoperative pathologic results conifrmed that 12 cases were bladder transitional cell carcinoma (1 case with partial squamous cell carcinoma) and 2 cases with bladder adenocarcinoma. No severe complication occurred except for 2 cases of urinary leakage and 1 case of urinary incontinence. Patients were followed up for 6-56 months, within which 3 patients were died of distant metastasis, 1 case was detected with intracranial metastasis, 1 case was found with urethra-vesical anastomotic stenosis while cured after urethrotomy. Ten cases were well recovered and the mean volume of the neobladder was 300 ml. Conclusions Laparoscopic radical cystectomy and orthotopic ileal neobladder have the advantage of better therapeutic effects, safety, minimal invasion and rapid recovery, which are the preferred therapeutic methods for invasive bladder cancer.

OBJECTIVETo evaluate the value of magnetic resonance imaging (MRI) in displaying the parotid gland segments of the facial nerve.
METHODSSixteen volunteers (9 males and 7 females) and 132 surgically confirmed patients with parotid tumors locating in the deep or shallow lobe of the parotid gland (including 89 with benign and 43 with malignant tumors) underwent MRI using T1WI and T2WI. The transverse images were obtained with the plane tilted 35 degrees to the foot, and the coronal images were acquired using conventional scanning.
RESULTSOn transverse T1WI, the parotid gland segments of the facial nerve displayed low signal with arc-shaped curve in the cross-section, showing a symmetrical dot-like low signal in the coronal plane. The facial nerve in 63% of the patients with parotid tumors in the cross-section could be displayed, but in the coronal plane the proportion reached 83%. MRI could accurately reveal the position of the parotid tumors in the deep or shallow lobe of the parotid gland.
CONCLUSIONMRI can show the major portion of the parotid gland segments of the facial nerve and has important value in locating the parotid tumors and displaying the relationship between the tumor and facial nerve.

OBJECTIVE:To provide evidence for strengthening education and training before clinical practice and employment guidance among pharmacy junior students of our university. METHODS:Anonymous questionnaire survey was conducted for our university. Questionnaire survey included student employment intention and internship units, relationship of internships with employ-ment,the tendency of employment after graduation, personal attitude about pharmaceutical professional development prospect,ex-pected monthly income in the first year of their career, etc. RESULTS:Totally 71 questionnaires were sent out,71 were effectively received. Among all respondents,74.3%(52 students)believed employment intention had great relationship with internship units;24.3%(17 students)thought employment intention had not great relationship with internship units;moreover, one student thought employment intention had no any relationship with internship units. Most students(94.3%,66 students)with good attitude on in-ternships believed it could accumulate work experience;32.9%(23 students) thought they could stay in the internship unit after practice;moreover,17.1%(12 students) believed practice had little effect on employment. Among them,94.0%(63 students) tended to be employed after graduation, and only 6.0% chose a graduate school to continue their studies. Among the students who chose employment after graduation,76.1%(51 students)tended to engage in pharmaceutical related work, while 17.9%(12 stu-dents) tended to be engaged in work nothing to do with pharmacy or self-employed. The majority (69.6%,48 students) believed that pharmaceutical major prospect was general and pharmaceutical major development varied from person to person;17.4%(12 students)thought pharmaceutical major had good prospect and was promising;13.0%(9 students)believed that pharmaceutical ma-jor had no good prospect and didn't know its prospect. Most of the students(78.2%,54 students)expected a monthly income of 2500-5000 yuan in the first year of their career;18.9%expected a monthly income more than 5000 yuan(13 students);the minor-ity expected a monthly income of 1500-2500 yuan or more than 8000 yuan. CONCLUSIONS:The view of students on internship and employment have a certain gap with the social situation. Related departments of colleges and universities need to adjust the thought of students and strengthen guidance.

BACKGROUND: At present, finite element analysis can be used to judge intertrochanteric fractures, but mostly limited in the distribution of stress. Finite element model of various intertrochanteric fractures has not been reported in detail.OBJECTIVE: To build various types of intertrochanteric fracture models with Hypermesh 14.0 and LS-DYNA software to simulate the falling-induced external force on proximal femur, and to evaluate the effect of models, and to analyze the biomechanical mechanism of intertrochanteric fractures. METHODS: Normal side CT image data of one case of elderly intertrochanteric fracture were collected and imported into Mimics software to establish the proximal femur geometric models, were then analyzed and operated by LZ-DYNA solver after imported into Geomagic studio 2013 and Hypermesh 14.0 for smoothing and meshing. Before analysis, the material parameters were set, the boundary conditions were confirmed, and given the loading parameters. The operating results were checked in Hyper View. RESULTS AND CONCLUSION: (1) The distribution of stress of proximal femur exactly matched to the previous study. EvansⅠtype intertrochanteric fracture model was obtained under continuous shear stresses, and six types of fractures were obtained by adjusting the load. (2) These results manifest that based on the Hypermesh 14.0 and LS-DYNA software, the finite element can well simulate the intertrochanteric fractures, and shear stress plays an important role in intertrochanteric fractures, which can provide experimental basis for the prevention and treatment of intertrochanteric fractures.
