To investigate the clinical risk factors of cardiac diastolic dysfunction in type 2 DM, Doppler echocardiographic examinations were performed on 63 male patients.With a routine 2 dimensional echocardiograph, mitral and pulmonary venous flow velocity were recorded. The patients were classified into DM, IGT and NGT based on well standardized criteria of WHO. All analyses were performed by the SPSS/PC or Windows Release 9.0 Statistical Package. The risk factors of impairment of diastolic function were analyzed by regression. Results showed that the levels of blood glucose and insulin were independent risk factors for diabetic cardiac diastolic dysfunction.

OBJECTIVE: To study the chemical constituents of the whole herb of Dichrocephala Benthamii C. B. Clarke. METHODS: The compounds were isolated by column chromatography on silica gel and Sephadex LH-20. Their structures were identified on the basis of spectral analysis and physicochemical characters and confirmed by comparison of their spectral data with those in the literature. RESULTS: Eighteen compounds were isolated and identified respectively as hexadecanoic acid(1), 1-stearin(2), ethyl-stearate(3), tricosanoic acid(4), tetracosanoic acid(5), octacosane(6), dotriacontanic acid(7), 3, 5-dihydroxy-6, 7, 3′, 4′-tetramethoxy flavone(8), chrysosplenetin B(9), oroxylin A(10), apigenin(11), piceol(12), p-hydroxy benzaldehyde(13), aloe-emod-in(14), chrysophanol(15), physcion(16), xanthoxylin(17), and dibutylphthalate(18). CONCLUSION: All the compounds were isolated for the first time from this genus except compound 18. Copyright 2012 by the Chinese Pharmaceutical Association.

In order to identify Peucedani Radix, Peucedani Decursivi Radix and their adulterants, the internal transcribed spacer 2 (ITS2) regions of Peucedani Radix, Peucedani Decursivi Radix and their adulterants were amplified and bidirectionally sequenced based on the Principles for Molecular Identification of Traditional Chinese Materia Medica Using DNA Barcoding, which has been promulgated by Chinese Pharmacopoeia Commission. Sequences were analyzed and assembled by Codon Code Aligner V3. 7.1. The relevant data were analyzed by MEGA 5. 0. Species identification analyses were performed by using the nearest distance methods and neighbor-joining (NJ) methods. The result showed that the ITS2 sequence lengths of Peucedani Radix were 229-230 bp and the average intra-specific genetic distances were 0.005. The ITS2 sequence lengths of Peucedani Decursivi Radix were 227 bp and the sequences contained no variation site. The average inter-specific K2P genetic distance of Peucedani Radix, Peucedani Decursivi Radix and their adulterants species were 0.044 and 0.065 respectively. The minimum inter-specific divergence is larger than the maximum intra-specific divergence of Peucedani Decursivi Radix. The nearest distance methods and NJ trees results indicated that Peucedani Radix, Peucedani Decursivi Radix and their adulterants species could be identification clearly. The ITS2 regions can stably and accurately distinguish Peucedani Radix, Peucedani Decursivi Radix and their adulterants.

Objective To study the effects of health management on cardiovascular events in the elderly patients with hypertension. Methods A total of 182 elderly patients with hypertension were randomly assigned to the control group (n = 61 ), pharmacologic therapy group (n = 61 ) or health management group ( n = 60). Serum biomarkers, brachial-ankle pulse wave velocity ( baPWV ), and blood pressure were tested at baseline and after intervention. Results There were no differences between the 2 groups in clinical characteristics at baseline. The average following-up period was (21 ± 7 ) months. The improvement of systolic blood pressure ( t = 3.915, P = 0. 000 ), pulse pressure ( t = 3. 966, P = 0. 000), and baPWV ( t = 3. 093, P = 0. 002) in the health management group was more significant than the control group;the systolic blood pressure ( t = 2. 008, P= 0. 046 ) was bitterly improved than the pharmacologic therapy group. The accumulative survival rate of the health management group (96. 7% ) was higher than the control group (83.6%; x2 =5. 921 ,P =0. 015) ,similar to the pharmacologic therapy group (93.3%; x2 =2. 821,P=0.091 ). Decreased systolic blood pressure, diastolic blood pressure, pulse pressure and baPWV were protective factors. After adjusted by age and gender,the improvement of systolic blood pressure was found to be an independent protective factors ( RR = 0. 75, P ＜ 0. 05 ). Conclusion Health management in elderly patients with hypertension could more significantly reduce the risk of cardiovascular diseases.

This paper presents some ideas and practices to realize the homogeneity quality management process within the Beijing Children Hospital Group,the largest specialized medical group in the country.This research aims at providing references and thoughts on homogeneity quality management for trans-regional medical unions in the country.

ObjectiveTo investigate the mechanism and prevention of invisible injury of recurrent laryngeal nerve (RLN) system in thyroid surgery, with the application of intraoperative neuromonitoring ( IONM ) system. MethodsThe type of invisible RLN injury and its protection with the application of IONM system were analyzed. ResultsThe causes of invisible RLN injury mainly included stretching of Berry ligament or the tumor,contusion, thermal injury, cutting of silk and suction injury. RLN invisible injury was recoverable through neurotrophic and symptomatic treatment. No permanent vocal cord paralysis occurred. ConclusionsWith the application of IONM system, some invisible type of RLN injuries can be found. The risk of RLN injury can be reduced if the surgical techniques are improved with the development of study on mechanism of IONM system.
