1.Comparison Analysis of Stress Radiography for the Evaluation of Posterior Knee Laxity

Objective Stress radiography provides an objective tool to measure posterior knee instability.This study was conducted to evaluate the intraobserver and interobserver reliability of measurements using Telos device to quantify posterior knee instability,compared wim KT-1000 and PDT test for consistency analysis.Methods From October 2008 to June 2009,68 stress radiographs in 34 patients with posterior knee instability were taken using Telos device.The amount of posterior displacement on the radiographs was measured independently by 2 surgeons on 2 separate occasions.Changes in intraclass correlation coefficients(ICCs)were examined to assess the intraobserver and interobserver reliability of the measurement,and were compared with those from KT-1000 and PDT test for consistency analysis.Results Intraobserver ICC was 0.992,while interobserver ICC Was 0.991.There was no significant difierence between Telos and KT-1000 in pair-t test.The data from Telos device was consistent with KT-1000.The coincident ratio Of PDT test was 20% while the posterior displacement of the tibia calculated on stress radiography was 5-10 mm.The coincident ratio of the PDT was 71.4% while the posterior displacement of the tibia calculatcd from stress radiography was 10-15 mm.Conclusion Using Telos device for stress radiograph provides a reproducible method to quantify posterior knee instability,and the consistency between Telos divece and KT-1000 was reliable.The coincident ratio of the PDT test with stress radiography increased when the posterior displacement of the tibia from stress view became more severe.
3.Effect of Sodium Butyrate on Phosphorylation of Histone at ?-Globin Gene Promoter Regions in K562 Cells

Objective To explore the effect of sodium butyrate(NaB) on phosphorylation/ acetylation of histone H3(ph/acH3) at G?-globin gene and A?-globin gene promoter regions in K562 cells.Methods K562 cells were devided into 2 groups:K562 cells were grown in the presence or absence of 0.5 mmol?L-1NaB for 48 h [K562(NaB) group] and untreated K562 cells group(K562 group).Semi-quantitative RT-PCR was employed to measure the levels of G?-globin mRNA and A?-globin mRNA.The real time PCR-based chromatin immunoprecipitation(ChIP) was used to detect the levels of ph/acH3 at G?-globin gene and A?-globin gene promoter regions.Results Compared with the K562 group,there was a 1.4-fold(t=-149.022,P=0.000) and 1.2-fold(t=-13.363,P=0.000) increase in G?-globin mRNA and A?-globin mRNA,respectively,in K562(NaB) group.The level of ph/acH3 at G?-globin gene and A?-globin gene promoter region increased by 2.9-fold(t=-12.833,P=0.006) and 3.2-fold(t=-10.484,P=0.000),respectively,in K562(NaB) group,compared with the K562 group.The %Input value of G?-globin and A?-globin promoter fragment was 10.0-fold(P=0.000) and 9.5-fold(P=0.000) higher than that value of Necdin gene promoter fragment in the K562(NaB) group,while the %Input value of G?-globin and A?-globin promoter fragment was 3.2-fold(P=0.000) and 2.7-fold(P=0.000) higher than that value of necdin gene promoter fragment in K562 group.Conclusions NaB improves the phosphorylation and acetylation of H3 at ?-globin gene promoter regions,and this may be one of the mechanisms of expression of ?-globin genes induced by NaB.
4.Arthroscopic prevalence of Ramp lesion of anterior cruciate ligament injury

ObjectiveTo retrospectively determine the prevalence of Ramp lesion (the peripheral attachment of the posterior horn of the medial meniscus) of the posterior horn of the medial meniscus with concurrent anterior cruciate ligament(ACL) ruptures and analyze the relevant risk factors.MethodsFrom April 2002 to October 2007,868 consecutive knees were diagnosed as ACL injury and received arthroscopic surgery for ACL reconstruction(ACLR).All the patients were verified for the tears of Ramp area under arthroscopy.The prevalence of Ramp lesion was evaluated respectively.Then all cases were divided into different groups depending on the time interval from ACL injury to ACLR (TFI) and other relevant risk factors as age and gender.ResultsAmong 868 knees underwent surgery for ACLR,144 knees were diagnosed as Ramp lesion.The average age was 24.7 years,113 males,31 females.The mean TFI to ACLR was 29.3months.The prevalence of Ramp lesion was 16.59%,which was analyzed as logarithmic correlation with TFI.Patients younger than 30-year old and male patients had significant higher prevalence of Ramp lesion.ConclusionRamp lesion is a common meniscus injury that can occur at the time of ACL rupture or as a result of knee laxity associated with ACL insufficiency.The prevalence of Ramp lesion is 16.59% which increases with time until 24 months after initial injury.Patients younger than 30-year old and male patients have higher prevalence of Ramp lesion.

Objective:To describe the clinical results of a one-stage operation for posterior cruciate ligament reconstruction and a posterolateral corner reconstruction of popliteus tendon reconstruction,com-pared with an open popliteofibular ligament reconstruction.Methods:Our study included 33 patients who had undergone posterior cruciate ligament reconstruction with use of an achilles tendon-bone allograft and posterolateral corner reconstruction with arthroscopic anatomical reconstruction of popliteus tendon or open reconstruction of popliteofibular ligament.The patients were assessed for knee instability with use of the dial test at 30°and 90°,together with posterior stress radiography and KT-1000 measurement.Re-sults:The mean time of follow-up was more than 2 years.At the final follow-up evaluation,the tibial posterior translation as measured by stress radiography at 90°of knee flexion,the anterior-posterior trans-lation as measured by KT-1000,and the external rotation of tibia as measured by dial test were reduced postoperatively (P<0.001 )in both groups.Between the two groups,the preoperative tibial posterior translation as measured by stress radiography and the preoperative mean anterior-posterior translation as measured by KT-1000 in the popliteofibular ligament group were a little more than those in the popliteus tendon group (P=0.014,P<0.001).But the other comparisons were not significantly different (P>0.05).The final flexion losses were 3.33°±4.88°and 3.06°±3.38°for the two groups.Conclusion:Combined with posterior cruciate ligament reconstruction,anatomical posterolateral corner reconstruc-tion of the popliteus tendon and open reconstruction of popliteofibular ligament showed similar out-comes.This study demonstrated that both posterolateral corner reconstruction techniques were a reliable alternative method in addressing posterolateral corner and posterior cruciate ligament insufficiency of the knee.
7.Anterior Cruciate Ligament Reconstruction Combined with Anterolateral Tenodesis Procedure Treating Patient with High Grade Pivot-shift after ACL Injuries

Objective To evaluate the early clinical outcomes of anterior cruciate ligament(ACL)reconstruction combined with anterolateral tenodesis procedure in patient with high grade pivot-shift after ACL injuries.Methods Among the 140 patients diagnosed as ACL injury and undergoing operations in our hospital between June and December 2015,18 presenting high grade pivot-shift 2+ or 3+ under anesthesia were included into this study.During their surgeries,the anterolateral tenodesis procedure using iliotibial tract ITB was applied together with ACL reconstruction.The anteroposterior stability of the knee was examined and pivot-shift test was conducted right after the operation as well as 6 and 12 months after that.The Lysholm score was used to evaluate the clinical function of their knees.Results The mean follow-up period of the 18 patients was 12.7 ± 2.4 months.There were 14 males and 4 females,with an average of 28.3 ± 7.1 years.No patients complained about unstable symptoms during the follow-up.The mean side-to-side difference of anteroposterior knee laxity assessed using KT1000 at 30 degree knee flexion was 2.3 ± 0.7 mm,which was significantly improved compared with preoperative assessment(9.2 ± 2.6 mm)(P<0.01).For the preoperative examination,the pivot-shift examination indicated 2+ in 13 patients and 3+ in 5 patients.However,all were negative right after the operation.Seventeen patients presented negative pivot-shift at the final follow-up,while 1 had 1+ rotational laxity.Significant differences were found in the Pivot-shift test before and after the operation.The average Lysholm score improved significantly from 67.7 ± 5.9(ranging from 55 to 78)before the operation to 89.0-± 4.6(ranging from 75 to 95)after that.Conclusion The high grade pivot-shift rotational instability could be effectively restored using the anterolateral tenodesis procedure,in addition to ACL reconstruction.The early stage clinical outcomes indicate significant improvement of the knee stability postoperatively.
8.Expression of plexinA1 and Ki-67 in tissue of patients with human glioma cells and their clinical significance

Objective To explore expression of plexinA1 and Ki-67 in tissue of human brain glioma cells and their clinical significance.Methods 43 specimens from patients with brain glioma were collected.Immunohistochemical (IHC) staining was used for detecting the expression of tissue plexinA1 and Ki-67 in human glioma cells of 43 cases of patients with brain glioma.The positive expression rate of plexinA1 and Ki-67 among the different pathological grade tissues and their clinical significance were analyzed.So did correlation studies about plexinA1 and Ki-67.Results The positive expression rates of plexinA1 in Ⅰ-Ⅱ grade group (18 cases) and Ⅲ-Ⅳ grade group (25 cases) were 22.22 % (4/18) and 72.00 % (18/25) (P < 0.05).The positive expression rates of Ki-67 in Ⅰ-Ⅱ grade group and Ⅲ-Ⅳ grade group were 16.67 % (3/18) and 56.00 % (14/25),respectively (P < 0.05).PlxinA1 and Ki-67 expression in the tissue of human brain glioma were positively correlated (r =0.997,P < 0.05).Conclusions The positive expression rate of plexinA1 is higher in high malignancy human glioma group than that in low malignancy group which has an important reference value in the estimation of prognosis for human glioma.PlexinA1 and Ki-67 maybe synergism in occurrence and development of glioma.
9.Quantitative evaluation of residual bony impingement lesions after arthroscopic treatment for isolated pincer-type femoroacetabular impingement using three-dimensional CT

Objective To determine the clinical characteristics and its effect on the clinical efficacy of residual bony impingement lesion after hip arthroscopic osteoplasty for pincer-type femoroacetabular impingement.Methods There were 30 patients who underwent hip arthroscopic osteoplasty for isolated pincer-type femoroacetabular impingement from December 2010 to January 2012 and had a minimum follow-up period of 2 years,including 21 males and 9 females.The average age at surgery was 34.5 years (range from 28-42 years).All of the patients had no advanced hip arthritis and the T(o)nnis grade was 0-2.Radiological evaluation was performed by 3-dimensional CT.According to residual rate,these patients were divided into 3 groups:residual rate <10% group,residual rate of 10%-20% groups and residual rate >20% group.Results The average follow-up period was 26.3 months (range,22-34 months) and nineteen cases had residual bony impingement lesion after surgery.Among them,84.2% of the residual bony impingement lesion was posterior to the acetabular actual resection zone.The preoperative and postoperative bony impingement angle were 77.47°±21.31° and 12.94°±18.04°,respectively.The residual rate was 16.7%± 8.83%.The modified Harris hip score got significantly improved from 55.18±7.96 preoperatively to 94.71±4.39 postoperatively.The overall satisfactory rate was 76.7%,and the postoperative modified Harris hip score for these three groups was 95.86±1.71,95.23±1.99 and 85.52±6.41,respectively.The postoperative modified Harris hip score in residual rate >20% group was significantly worse than those in other two groups.The satisfactory rate was 92.9%,80.0% and 33.3%,respectively.The satisfactory rate in residual rate >20% group was significantly worse than those in other two groups.There was a significant negative linear relationship between the residual rate and postoperative modified Harris hip score (R2=0.516,P=0.001).Conclusion The incidence of residual bony impingement lesion after hip arthroscopic osteoplasty for pincer-type femoroacetabular impingement was 63.3%.The residual rate was 16.7%.The residual impingement lesion was mainly at the posterior part of the acetabulum.The patients with residual rate >20% got significantly worse clinical score and satisfactory rate.
