AIM:To observe and compare clinical effects of coaxial 1. 8mm microincision phacoemulsification and 3. 2mm small incision phacoemulsification.
●METHODS:A total of 117 eyes of 85 patients with age-related cataract in our hospital were divided randomly into two groups:43 patients (59 eyes) in the coaxial 1. 8 mm microincision cataract surgery group ( C - MlCS ) , 42 patients (58 eyes) in the coaxial 3. 2 mm traditional small incision cataract surgery group (C-SlCS). A total of 117 eyes were received phacoemulsification with intraocular lens implantation. Uncorrected visual acuity was recorded preoperatively and postoperatively at 1, 7, 30 and 90d. The effective phacoemulsification time and average ultrasound energy were recorded in surgery. Corneal endothelial cell and corneal topography were recorded preoperatively and postoperatively at 90 d.
●RESULTS:Uncorrected visual acuity ( logMAR) was no overall statistical significance difference between C-MlCS group and C-SlCS group (P>0. 05), but was significant statistical difference in different time-point within both groups(P<0. 05). Uncorrected visual acuity in different time-point had nothing to do with corneal wound size in cataract surgery(P>0. 05). On the 1 day after surgery, uncorrected visual acuity was 0. 16±0. 11 in C-MlCS group and 0. 22±0. 18 in C-SlCS group(P<0. 05). AVE was (7. 00± 2.72)% in C-MlCS group and (6. 16±3. 16)% in C-SlCS group (P>0. 05). EPT was (3. 09±1. 61)s in C-MlCS group and (3. 20±1. 92)s in C-SlCS group (P>0. 05). At 90 d after surgery, corneal endothelial cell loss percentage was (5. 81±2. 28)% in C-MlCS group and (5. 69±2. 38)% in C-SlCS group (P>0.05), SlA was (0.35±0.11) Din C-MlCS group and (0. 61±0. 13) D in C-SlCS group (P<0. 05).
● CONCLUSION: Compared with coaxial 3. 2mm traditional small incision cataract surgery, 1. 8mm coaxial microincision cataract surgery can get earlier visual rehabilitation and significantly reduce SlA. The coaxial 1. 8mm microincision cataract surgery is safe, effective and deserves further clinical applications.
5.The application of system epidemiology theory and methodology in occupational epidemiology

Abstract:
Occupational epidemiology aims to explore the effect of occupational hazards on the health of workers and understand
,
their mechanisms. It plays an important role in occupational health and occupational medicine.Currently occupational
, ,
exposures in the workplace are complex and diverse and multiple factors affect workers´ health at the same time. Therefore it
is important to elucidate the pathogenesis of occupational disease caused by occupational hazards and implement early
- -
intervention. System epidemiology collects data on multi level exposure and multi omics information to conduct network analysis
-
on the relationship amongrisk factors. and to study the mechanisms of exposures and health outcomes based on multi level data.
, , , ,
Using the study design of system epidemiology occupational environmental lifestyle and social factors are combined as a
,
system to evaluate the health of workers which can better evaluate the adverse health effects caused by occupational hazards.
- , ,
The studies base on multi omics design will explore the pathogenesis of occupational diseases at the molecular cellular and
tissue levels to evaluate the impact of occupational hazards on workers´ health and to explore interventions from multiple
-
perspectives to reduce the occurrence of occupational or work related diseases.

The Coat protein and Maturase gene of E.coli bacteriophage MS2 was amplified by PCR,then the gene was cloned into pET32a to construct the intermediate vector pET32aCP.The conservative sequence of FMDV internal ribosome entry site(IRES) was cloned into the downstream of pET32aCP bacteriophage gene to construct the prokaryotic expression vector pCPES.The recombinant plasmid pCPES transformed into E.coli strain BL21(DE3) was induced to express with 1mmol/L IPTG.The expression products were purified by sucrose density gradient centrifugation. The expression products observed by TEM were circular viruslike particles, and the diameter of these particles was about 26nm.The stability of viruslike particles was detected, and the viruslike particles was identified by RTPCR.The results showed that the viruslike particles contain the FMDV IRES RNA and have good stability. The viruslike particles have great prospect as the standard and quality control in the area of RNA virus detection.

Objective To study the operative strategy of congenital atlantoaxial dislocation(CAAD)-induced Chiari malformation and (or) syringomyelia. Methods The operation in reported 23cases of CAAD-induced Chiari malformation and (or) syringomyelia was composed with the transoral resection of odontoid process to achieve anterior decompression at first stage and occipito-cervical bone grafting fusion at second stage. Results MRI examination revealed the tonsils ascent and (or) syrinx reduction in 19 cases after first-stage operation. Compared with their preoperative manifestations, 14cases were obviously improved and 5 improved to some extent after operation, while 4 were unchanged. Conclusions CAAD is the main cause of tonsils descent and (or) syringomyelia in the series of patients. After anterior decompression by transoral resection of odontoid process, most patients will get recovered in tonsils descent and (or) syringomyelia reduction. The main aim of posterior operation is to reconstruct the stability of craniovertebral junction.

Objective To analyze the clinical efficacy of capecitabine monotherapy or in combination with oxaliplatin concurrent chemoradio-therapy on patients with advanced colorectal cancer. Methods Fifty-two patients with advanced colorectal cancer were randomly divided into treatment group and control group , 26 cases in each group. Two groups were all taken 6 mV X -ray irradiation, 5 times a week, totally five weeks. On the same time , control group was given capecitabine , twice a day, with the total dose of 1500-1700 mg· m-2 .Treatment group was given oxaliplatin total dose of 50 -75 mg · m -2 , once a week , and capecitabine, twice a day, with the total dose of 1200-1550 mg· m-2.And 35 d a course , all patients were given three courses treatment.Clini-cal efficacy , improvement in quality of life and adverse drug reactions of the two groups were observed. Results After treatment, the objective response rate in treatment group (76.93%) was significantly higher than that in control group ( 42.31%, P<0.05 ); the quality of life in treat-ment group was significantly better than that in control group ( P<0.05 ).The incidence rate adverse reactions in treatment group were significantly lower than that in control group ( P<0.05 ).Conclusion Capecitabine combined with oxaliplatin for the treatment of patients with advanced colorectal cancer had a good clinical efficacy , low incidence of adversereactions , which can improve the quality of life.
