1.Efficacy on dysphagia induced by bulbar palsy treated with acupuncture at Tiantu (CV 22) with different needling techniques.

OBJECTIVETo compare the efficacy between elongated needle therapy and regular needle therapy at Tiantu (CV 22) on the basis of xingnao kaiqiao (activiting brain and regaining consciousness) acupuncture therapy so as to explore the effective therapeutic method in treatment of dysphagia induced by bulbar palsy.
METHODSSeventy one cases of dysphagia induced by bulbar palsy were randomized into two groups. The xingnao kaiqiao acupuncture therapy was applied at Shuigou (GV 26), Neiguan (PC 6), Sanyinjiao (SP 6) and the others in the two groups. In the elongated needle therapy group, on the basis of xingnao kaiqiao acupuncture therapy, the elongated needle was used to puncture Tiantu (CV 22). In the regular needle therapy, the regular acupuncture technique was used at Tiantu (CV 22). In both groups, the treatment was given once a day in a week except Sunday and lasted for 4 weeks totally. Before and after treatment, the swallowing condition and the standardized swallowing assessment (SSA) were observed in the patients and the efficacy was compared between the two groups.
RESULTSThe total effective rate was 97.2% (35/36) in the elongated needle therapy group, which was better than 77.1% (27/35) in the regular needle therapy group (P<0.05). After treatment, SSA score was reduced significantly as compared with that before treatment in the two groups (both P<0.05). SSA score in the elongated needle therapy group was reduced much more apparently as compared with that in the regular needle therapy group after treatment (P<0.05).
CONCLUSIONOn the basis of the xingnao kaiqiao acupuncture therapy, the elongated needle therapy at Tiantu (CV 22) achieves the superior effect on bulbar palsy-induced dysphagia as compared with the regular acu- puncture at Tiantu (CV 22).

2.Determination of Environmental Estrogens in Surface Water and Tap Water

Objective To understand the environmental estrogens contamination of tap water and source water. Methods Environmental estrogens in the water samples were concentrated by macroporous adsorption resin H103. The effect of environmental estrogens in concentrated samples was assessed by the recombinant yeast system. Results The results showed that some source water and tap water really contained environmental estrogens. The activity of environmental estrogens in concentrated samples of some source water increased as the volume of concentrated sample increased. The concentrate of 100 ml water sample could show the estrogenic effect. Conclusion Some source waters and tap waters in China have been contaminated by environmental estrogens. Environmental estrogens in source water can not be removed completely by drinking water treatment system of city.
4.Pharmacodynamical mechanism study of “Heat Asthma Formula” in treatment of bronchial asthma

Objective: In this paper the pharmacodynamical mechanism of Heat Asthma Formula(HAF)(Herba Ephedrae, Rhizoma pinelliae, Flos Lonicerae, etc) in the treatment of bronchial asthma was discussed. Methods: By means of the radio labelled aglycone, applying phlogistic medium TXB2, ? receptor on bronchus and cyclic nucleotide (cAMP/cGMP) in lung tissue used as observed targets. Results: The results showed HAF could inhibit the release of phlogistic medium TXB2, improve the conjugate rate of ? receptor, adjust the proportion between cAMP and cGMP. Conclusion: The treatment on bronchial asthma of “Heat Asthma Formula” was perhaps concerned with above mentioned contents.
5.Study of ceiling effect of commonly used Chinese recognition materials in post-lingual deafened patients with cochlear implant.

OBJECTIVE:
To evaluate and compare outcomes effects of Chinese recognition materials in post-lingually deafened patients with cochlear implant.
METHOD:
Thirty-two post-lingually deafened cochlear implant users participated in the study. Each one of them was given a series of speech recognition tests including the speech recognition score of monosyllable words, spondee words, Chinese BKB sentences and MHINT in quiet, as well as speech recognition threshold of HOPE corpus in babble noise. Speech recognition scores and thresholds were compared among these test materials using histograms , scatter diagrams and statistical methods.
RESULT:
Recognition scores for spondee words, Chinese BKB sentences and MHINT were affected by ceiling effects, with 1,14 and 4 cases scored 100% respectivelty. Meanwhile, 17, 26 and 14 cases scored more than 85% correspondingly. On the other hand, speech recognition scores for monosyllable words in quiet and speech recognition threshold for HOPE corpus in babble noise were not affected.
CONCLUSION
For a considerable part of post-lingually deafened patients using cochlear implant, some Chinese speech recognition materials have demonstrated different degrees of ceiling effects. While speech recognition scores for monosyllable words in quiet and recognition threshold for HOPE corpus in noise can evaluate the performance of speech recognition abilitymore objectively.

7.Clinical analysis of radio-frequency ablation in the treatment of advanced pancreatic cancer

Objective To explore the clinical effect of radio-frequency ablation in the treatment of advanced pancreatic cancer.Methods Retrospectively analyzed the clinical data of 28 cases of radio-frequency ablation in the treatment of advanced pancreatic cance between January 2008 and December 2012 in 304 hospital.And choosed made 140 cases patients who only underwent the conservative treatment as the control group at the same period.Then compared the data of two groups patients.Results The mean tumor diameter was (4.8 ± 1.6) cm.The average radio-frequency time was (17.7 ± 4.3) minutes.There was no patient occurred of pancreatic fistula or postoperative bleeding,and no patient dead.Peri-operation pain relief rate was 88.9%.Twenty-three patients were followed up,and the follow up rate was 82.1%.The patients' survival rate who underwent radio-frequency ablation of 1 month,6 months and 1 year were 95.7%,65.2% and 38.2%.The average survival time was (14.6 ± 2.2) months.And the data was significantly higher than that of the control group patients (97.4%,45.3%,19.1%,P =0.0306).Conclusions The radio-frequency ablation was one of an effective treatment method for the advanced pancreatic cancer patients.It can prolong the survival time of pancreatic cancer patients and alleviate the patient's pain.

ObjectiveTo compare the therapeutic effects of a new surgical endoscopic technique, the transilluminated powered phlebectomy (TriVex System, Smith+Nephew) and pointed phlebectomy in the treatment of varicosis of the great saphenous vein of the lower limbs. Methods Thirty-nine patients (46 limbs) received TriVex operations under spinal, or epidural anesthesia. The powered vein resector and an irrigated illuminator device-a minimally invasive system was used for varicose vein surgery. During the same period, 41 patients (46 limbs) underwent pointed phlebectomy. Results The average postoperative hospital stay was 4.6 days in patients receiving TriVex, and 8.1 days for pointed phlebectomy. Compared with the pointed phlebectomy, the incision of TriVex procedure was shorter and the number less. Conclusion This new surgical device is easy to operate, minimally invasive, efficacious and time saving with satisfactory results.
10.Analgeic effect of patient control subcutaneous morphine combined with small-dose ketamine on moderate/severe terminal cancer pain

Background and purpose:The pain is not only the chief symptom of cancer but also the main factor of survival quality for the cancer patient.So it is the most difficult for how to control the pain of moderate to severe late-stage cancer and reduce the rate of side effects.We observed the analgesia effects of micro-dosage ketamine associating morphine patient control skin analgesia(PCSA) for latter cancer pain patients.Methods:52 cases middle, latter cancer pain patients were divided two groups randomly,the first group used the morphine PCSA and the second group used micro-dosage ketamine associating morphine PCSA.The VAS(Visual Analogue Scale) was used to evaluate the pain level,then statistic the side effects rate of two groups.Results:Two groups patients'pain all alleviated fundamentally,the VAS of every period have not statistic difference,but the PCSA pressing times in the first group was more than in the second group.The second group patients'life satisfactory rate was higher obviously than the first group patients';Side effects:the incidence of nausea,vomiting,indulge in sleep and skin titillation in the first group was higher than in the second group;the incidence of respiration depression and hallucination had not difference between two groups.Conclusion:Micro-dosage ketamine associating morphine patient control skin analgesia(PCSA)approach appears to provide patient with safe and effective pain relief and reduce side-effect for moderate to severe late-phase cancer pain,easy to perform and have no process complication to used in clinic.
