1.Effect of electro-needling at acupoints of the yangming meridian on the expression of PAC-1 and lower limb functions in acute cerebral infarction patients.

OBJECTIVETo observe the effect of electro-needling at acupoints of the yangming meridian on the expression of platelet associated complement-1 (PAC-1) and lower limb functions in acute cerebral infarction (ACI) patients.
METHODS58 ACI patients were randomly assigned to the treatment group and the control group. Conventional therapies were given to all patients. Additionally, the electro-needling at acupoints of the yangming meridian was given to patients in the treatment group. Changes of PAC-1 were detected using flow cytometry. Effect of lower limbs functions of ACI patients before and after electro-needling was assessed using Fugl-Meyer Index. Meanwhile, 20 healthy subjects were selected for reference value.
RESULTSIn the acute stage, the PAC-1 level in ACI patients were significantly higher than that in healthy subjects (P<0.05). The PAC-1 level in the electro-needling group was obviously lowered after treatment (P<0.01). There was no significant difference in the control group between before and after treatment. Significant difference was found in Fugl-Meyer index in the same group between before and after two-week treatment (P<0.05). It was higher in the electro-needling group than in the control group, showing significant difference (P<0.01).
CONCLUSIONSPlatelet activation exists in the acute stage of ACI. Electro-needling at acupoints of the yangming meridian showed obvious inhibition on PAC-1 levels, could improve lower limbs functions of ACI patients. It was inferred that electro-needling at acupoints of the yangming meridian promoted the recovery of paralyzed lower limbs at the early stage mainly by regulating PAC-1 levels, thus postponing the progress of ACI.

2.Management of solitary iliac aneurysms: report of 19 cases

Objective To report our surgical experience in treating 19 cases of solitary iliac aneurysms (SIA). Methods The clinical data of 19 consecutive patients with SIA between January 1985 and January 2008 were retrospectively reviewed. There were 18 men and 1 woman, aging from 39 to 77 years ( mean 62 ± 7 years). Results There were 30 SIAs in the 19 patients, including 25 ( 83.3% ) common iliac aneurysms, 4 (13.3%) internal ihac aneurysms and 1 (3. 3% ) external iliac aneurysm. Eleven patients ( 57.9% ) had multiple ancurysms, with 9 patients ( 47.4% ) having bilateral SIA. Two patients had coexistent peripheral vascular occlusive disease. There were 2 patients suffering form ruptured SIA, one was saved by emergency operation and one died before an surgery could be attempted. Seventeen patients underwent successful open aneurysmectomy and artificial graft implantation leaving no ischemic complications of the pelvic organs. One patient with right common iliac aneurysm underwent endovascular repair without endoleak. There was no operative death during porioperative period. The surviving patients remained stable and had good patency of grafts during the follow-up period. Conclusions Early management of SIA is important, CT angiogarphy (CTA) is necessary not only to evaluate the SIAs, but also to detect multiple aneurysms or arterial occlusive disease. Close and long-term follow-up is mandatory for the early detection of the formation of new anearysms.
3.Survey on health-promoting lifestyle of kidney transplant recipients

Objective By discussing the state of lifestyle as well as affecting factors of kidney transplant recipients,to provide more health education content and basis for their physical and psychological health.Method With internationally accepted Health-Promoting Lifestyle Profile (HPLP),we surveyed the lifestyle of the selected 104 long-term follow-up cases of kidney transplant recipients.At the same time,163 sex-and age-matched healthy volunteers,who had no obvious abnormalities in the medical test,were chosen.Result For HPLP scores among the 104 cases of recipients,20 cases were excellent,67 cases good,17 cases common,and none was inferior.As a whole,its excellent rate was 83.65%.As for 129 healthy volunteers,their overall excellent rate was 70.55%.The HPLP scores for the nutrition behavior ranked top in kidney transplant recipients,followed by healthy responsibility behavior,and lowest for exercise behavior.For the healthy volunteers,the HPLP scores for interpersonal support behavior ranked top,followed by nutrition behavior,and lowest for healthy responsibility.Correlation analysis revealed that the HPLP scores in kidney transplant recipients were significantly and positively correlated with age (r =0.307,P =0.002) and educational level (r=0.370,P =0.000),and not with gender,ethnicity,occupation and kidney sources (P>0.05).The HPLP scores in idney transplant recipients were higher than those in healthy volunteers,among which self-actualization and healthy responsibility showed statistically significant differences (P < 0.05),there was no significant difference in exercise,nutrition,interpersonal support and stress management between recipients and healthy volunteers (P>0.05).Conclusion The HPLP scores in kidney transplants was higher than in healthy volunteers,thereinto,stress management behavior and exercise behavior were relatively weak,which were the focused improvement projects of lifestyle of kidney transplant recipients.
4.The relationship between serum testosterone and metabolic syndrome in middle-aged and elderly men

ObjectiveTo explore the causes for descending of the serum testosterone (T) in middle-aged and elderly men with metabolic syndrome (MS).MethodsFifty-six male patients aged between 45 and 83 years old were investigated.Blood pressure,height and weight were measured,and body mass index (BMI) was calculated.Biochemical indice in fasting state [ fasting blood sugar (FBG),total cholesterol (TC),triglycerides (TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol (HDL-C) ],fasting insulin (FIN),and the level of serum testosterone) were detected.An insulin resistance index (IR) was calculated using homeostasis model assessment (HOMA).Participants were divided into the MS group and the non-MS group according to CDS diagnostic standard.The relationships between the level of serum Testosterone and each index were analyzed.ResultsThe level of serum Testosterone was significantly lower in the MS group than that in the non-MS group [ (9.97 ± 3.87 ) nmol/L vs.( 13.73 ± 3.93 ) nmol/L,t =3.337,P < 0.01 ].Multiple regression analysis showed that the level of serum testooterone was negatively correlated with age,waist circumference and HOMA-IR ( regression coefficients:- 0.214,- 0.329,- 0.317; standardized regression coefficients:- 0.730,- 0.597,- 0.313 ; t =- 5.833,- 4.681,- 2.686 respctively; P < 0.01 ).ConclusionThe level of serum testosterone descends in middle-aged and elderly men with MS.The level of serum testosterone is closely related to age,waist circumference and insulin resistance
5.Oxidative stress injury of myocardial cells induced by emphysema with intermittent hypoxia in rats

Objective To observe the effect of exposure of emphysema with intermittent hypoxia on oxidative stress injury of myocardial cells in rats. Methods Sixty male Wistar rats were divided randomly into four experimental groups(each n=15). The normal control group was bred normally. The emphysema group was exposed to cigarette smoke twice a day(once 30 minutes). The intermittent hypoxia(IH)group was exposed to intermittent hypoxia circumstance 8 hours/day,and the emphysema with IH group was exposed to cigarette smoke twice a day (once 30 minutes)and intermittent hypoxia circumstance 8 hours/day. Each group was exposed for 8 weeks. At the beginning of 9 weeks,the blood gas analysis was performed in 5 rats selected randomly from each group,and the rest rats were sacrificed and their hearts and lungs were taken. Under light microscope,the lung tissues stained with hematoxylin-eosin(HE)were examined. The lung pathology and the results of blood gas analysis showed that the emphysema with IH rat model was established successfully. The levels of malonaldehyde(MDA)and superoxide dismutase(SOD)in rat myocardium were measured by enzyme-linked immunosorbent assay(ELISA),and the subunit p22phox mRNA expressions of nicotinamide adenine dinucleotide phosphate(NADPH)-oxidase were detected by real-time reverse transcription-polymerase chain reaction(RT-PCR). Results Compared with the normal group, the MDA levels and p22phox mRNA expressions were increased obviously in emphysema group, IH group and emphysema with IH group〔MDA(μmol/g):2.93±0.54, 3.58±0.63, 4.51±0.72 vs. 1.75±0.56, p22phox mRNA:0.043±0.004,0.067±0.015,0.123±0.016 vs. 0.018±0.002,all P<0.05〕,but the activities of SOD were decreased significantly(U/mg:36.07±4.79,33.51±7.12,24.29±5.36 vs. 46.08±5.12,all P<0.05). In emphysema with IH group,the increase of MDA levels and p22phox mRNA expressions and decrease of SOD levels were more remarkable compared with those in emphysema group and IH group(all P<0.05). The expression of p22phox mRNA was positively correlated with MDA level(r=0.734,P<0.001). Conclusion The myocardial tissue oxidative stress injury in rats induced by emphysema with intermittent hypoxia exposure is more serious than that induced by exposure of either emphysema or intermittent hypoxia alone,NADPH oxidase possibly being the important medium of myocardial cell response to oxidative stress.
6.Clinical Analysis of 11 Patients with Congenital Esophageal Atresia and Tracheoesophageal Fistula

Objective To explore early diagnosis and treatment of congenital esophageal atresia and tracheoesophageal fistula.Methods Clinical data of 11 patients with congenital esophageal atresia and tracheoesophageal fistula were analyzed retrospectively.Definite diagnoses were made for all cases in 24 hours through making an esophagus opacification with acetrezoic acid or iodinated oil.And an esophagus anastomosis outside pleura was made.Some experience of diagnosis and therapy were summarized.Results All of the 11 cases underwent operation.Among them,9 cases were cured (81.8%),and 2 cases died(18.2%).Anastomosis stenosis of esophagus was found in 1 case after operation,which was cured through esophagus dilatation.Tracheoesophageal fistula was found in another case after operation and it was cured through combined treatment including anti-infection, nutritional support and sufficient draining.Conclusion Early diagnosis and surgical treatment, postoperative care,prevention and cure of complication are very important to improve the survival rate of the patients with congenital esophageal atresia and tracheoesophageal fistula.
7.Effect of delayed preconditioning induced by Sini decoction on myocardial cell apoptosis and its mitochondrial mechanism.

OBJECTIVETo investigate the mechanism of myocardial cell apoptosis during delayed preconditioning induced by Sini decoction (SND).
METHODSD rats were divide into four groups: control, sham, I/R and SND groups. The rats in I/R group, left anterior descending coronary artery (LAD) was occluded for 1h and reperfused for 1 h. The rats in SND group were pretreated with Sini decoction (5 g x kg(-1) x d(-1)) for three days, the last treatment was pretreated 24 h before the index occlusion. Cell apoptosis was measured by flow cytometry, cytochrome C and bcl-xl were detected by Western blotting and the activity of caspase-3 was detected by assay kit.
RESULTAs compared with I/R group, apoptosis rate of myocardial cell, the release of cytochrome C from mitochondria and the activity of caspase-3 were significantly decreased, and the expression of bcl-xl protein was elevated in SND group.
CONCLUSIONThe delayed preconditioning induced by Sini decoction decreased myocardial cell apoptosis. The mechanism may be related to the inhibition of mitochondria signal pathway of apoptosis.

Objective To investigate the therapeutic effect of interleukin-2(IL-2)on experimental autoimmune encephalomyelitis(EAE)mice.Methods After establishment of the EAE(experimental autoimmune encephalomyelitis) mouse models with MOG35-55 polypeptides,the mice were grouped according to the neurological function score and divided into control group,EAE group and low dose IL-2 treatment group.A double blind method was used to evaluate the neuro-logical impairment in mice.On the 29th day,pathological experiments were carried out in the mice's brain and spinal cord, hematoxylin-eosin staining was used to evaluate the scoring of inflammatory cell infiltration and luxol fast blue staining was used to evaluate the scoring of demyelinating.The proportion of regulatory T cells(Treg)and NK cells(natural killer cell, NK)was detected by flow cytometry,and the immunohistochemical method was used to detect the expressions of glial fibril -lary acidic protein(GFAP)and myelin basic protein(MBP)in the spinal cord.Results Compared with the EAE group, the neurological function score, the inflammatory cell infiltration score and the demyelinating score of the low dose IL-2 treatment group were reduced.The proportion of Treg cells in the low dose IL-2 treatment group was significantly higher than that in the EAE group,and the proportion of NK cells in the low dose IL-2 treatment group was slightly higher than that in the EAE group The expression of GFAP and MBP was detected by immunohistochemistry.The expression level of GFAP in low dose IL-2 treatment group was significantly lower than that in the EAE group,while the expression level of MBP was higher than that in the EAE group.Conclusion Low dose IL-2 has significant therapeutic effect on EAE mice.
