1.Content Determination of Gallic Acid, Corilagin and Ellagic Acid in Aqueous Extract from Sanguo Decoction by HPLC

Objective To establish an HPLC method for the simultaneous determination of contents of gallic acid, corilagin and ellagic acid in aqueous extract from Sanguo Decoction. Methods The analysis was carried out on an Agilent C18 column (4.6 mm × 250 mm, 5 μm), and the mobile phase was composed of acetonitrile and 0.1%phosphoric acid aqueous with gradient elution. The detection wavelength was 260 nm. The flow rate was 1.0 mL/min at column temperature of 30 ℃. Results Gallic acid, corilagin and ellagic acid were completely separated and the peak shape was good. It showed a good linearity in the concentration range of 0.174–2.61 μg, 0.04–0.60 μg, and 0.052–0.78 μg, respectively. The average recoveries of gallic acid, corilagin and ellagic acid were 99.98%, 99.81%and 100.12%, respectively. The RSD were less than 2.0%. Conclusion This method is rapid, accurate and repeatable, which can provide references for the quality control of Sanguo Decoction.
2.Near Infrared Spectral Hybrid Model Quantitative Analysison Samples with Different Physical States

While the near infrared spectroscopy (NIRS) is used to measure the inhomogeneous samples with diffuse reflection mode, the accuracy and robustness of the calibration model is not quite good for the variation of spectrum scattering and absorption coefficient in those samples.Therefore, an establishment strategy of hybrid model based on homogeneous sample and calibration transfer method was proposed to solve the problem of detection inhomogeneous samples by NIRS.This work was focused on the tobacco leaf samples aspect.Three common calibration transfer methods, including Shenk′s patented algorithm (Shenk′s), piecewise direct standardization (PDS) and calibration transfer based on canonical correlation analysis (CTCCA), were used to construct two hybrid models of tobacco powder mixed with cut tobacco and tobacco powder mixed with tobacco flake samples to predict nicotine content in the samples of cut tobacco and tobacco flake.Experimental results showed that the hybrid model of adding some cut tobacco and tobacco flake samples to the powder model would get preferred prediction ability.Root mean square errors of cut tobacco and tobacco flake samples were reduced by 1.39% and 2.73%, respectively.This showed that the hybrid model was help for the improvement of the predicted results and the robustness of model.Moreover, CTCCA got the optimal performance between these three calibration transfer methods.Therefore, the scheme of building a hybrid model by NIRS homogeneous model and calibration transfer method to determinate the heterogeneous samples is feasible, which can accelerate the development of on-line near infrared spectroscopy technology and will provide reference for the share of near infrared spectral model.
3.Detection and analysis of plasma homocysteine levels and its metabolism related indicators in patients with primary angle-closure glaucoma

?AIM: To explore the relation of homocysteine ( Hcy ) and its metabolic related indicators with primary angle-closure glaucoma ( PACG) .?METHODS: In this study, a total of 150 PACG patients and 150 controls were enrolled. The patients with PACG were diagnosed by applanation tonometer, Humphrey perimetry ( HVF ) , optical coherence tomography ( OCT ) , gonioscope. The normal controls were recruited from physical examination center. Blood samples were collected and the plasma was used to determine homocysteine, vitamin B12 and folic acid. DNA was extracted to determine the methylenetetrahydrofolate reductase ( MTHFR ) C677TT genotype and gene polymorphism.?RESULTS:There was a statistically significant difference (t=2. 04,P=0. 04) in mean homocysteine levels between patients with PACG ( 16. 11 ± 1. 66μmol/L ) and controls (15. 74± 1. 52μmol/L). The level of vitamin B12 in PACG group was 248. 09 ± 119. 07pg/mL and the control group was 230. 21 ± 142. 93pg/mL. No statistically significant difference was found on the vitamin B12 level between the two groups ( t = 0. 84, P = 0. 40 ). The folic acid levels showed no statistically significant differences( t=1. 65,P=0.17) between the PACG group(12. 02±7. 81ng/mL) and the control group (13. 15 ± 6. 25ng/mL). The frequency distribution of the MTHFR C677T genotypes was found to be significantly associated (χ2 =6. 2,P<0. 05) with PCAG (CC 84. 7%, CT 4. 7%, TT 10. 7%) as compared to the controls (CC 94. 7%, CT 2. 0%, TT 3. 3%). The frequency distribution of TT genotypes of MTHFR C677T in the PACG group was significantly higher than that in the control group. Moreover, our findings showed significant difference (χ2=14. 2,P<0. 05) between the allele frequency of the C677T single nucleotide polymorphism within the MTHFR gene in POAG patients ( T 87. 0%, C 13. 0%) and the control group(T 95. 7%,C 4. 3%) .?CONCLUSION: These results show that the Hcy level and the TT genotypes of MTHFR C677T were higher in the PACG patients than in controls. Therefore, our data suggests that high Hcy level and the related indicators are associated with high PACG risk.
4.A sensitive and convenient enzyme-linked immunosorbent assay method in serum MG7 antigen detection in gastric cancer

Objective To explore a highly sensitive and highly specific method to detect the serum MG7 antigen (Ag) level for early gastric cancer diagnosis.Methods The serum MG7-Ag level was detected by enzyme-linked immunosorbent assay (ELISA) method in 116 preoperative gastric cancer patients,63 postoperative gastric cancer patients,41 patients with precancerous lesion,37 patients with precancerous diseases,50 healthy individuals and 281 patients with other cancers.Meanwhile,the expression of MG7-Ag was also examined with immunohistochemistry in patients with gastric cancer or precancerous lesion.Chi-square test was used for comparing positive rates of the two detection methods.Results The positive rate of MG7-Ag determined by ELISA was 83.6%(97/116) of preoperative gastric cancer patients,45.2%(28/62) of lung cancer patients,45.5%(20/44) of rectal cancer patients,17.6% (12/68) of colonic cancer patients,14.2% (6/42) of breast cancer patients,47.6% (30/63) of postoperative gastric cancer patients,19.5 % (8/4 1) of patients with precancerous lesions,5.4 % (2/37) of patients with precancerous diseases and 0 of healthy individuals.The sensitivity of ELISA (83.6 %) was similar with that of immunohistochemistry (94.0%)(P>0.05).However,the false positive rate of ELISA (12.8 %) was lower than that of immunohistochemistry (51.3 %) (x2 =26.491,P<0.01).There was statistically significant difference in MG7 Ag expression in gastric cancer with different clinical stages (x2=15.564,P<0.01).Conclusion This ELISA method might be a non-invasive screening method for population with high risk of gastric cancer.
5.A comparative study on transilluminated powered mini-phlebectomy (TriVexTM) and pointed phlebectomy for the treatment of varicosis of the great saphenous vein of the lower limbs

ObjectiveTo compare the therapeutic effects of a new surgical endoscopic technique, the transilluminated powered phlebectomy (TriVex System, Smith+Nephew) and pointed phlebectomy in the treatment of varicosis of the great saphenous vein of the lower limbs. Methods Thirty-nine patients (46 limbs) received TriVex operations under spinal, or epidural anesthesia. The powered vein resector and an irrigated illuminator device-a minimally invasive system was used for varicose vein surgery. During the same period, 41 patients (46 limbs) underwent pointed phlebectomy. Results The average postoperative hospital stay was 4.6 days in patients receiving TriVex, and 8.1 days for pointed phlebectomy. Compared with the pointed phlebectomy, the incision of TriVex procedure was shorter and the number less. Conclusion This new surgical device is easy to operate, minimally invasive, efficacious and time saving with satisfactory results.

7.Status and equity of mental health resources allocation in Shanghai Municipality

Objective:
To analyze the current status and equity of mental health resources allocation in Shanghai Municipality, so as to provide data supports to formulate mental health action plans and relevant policies. .
Methods:
The data pertaining to mental health institutions, actual beds opened, certified or assistant psychiatrists and registered nurses was collected from the Survey of the Current Status on Mental Health Resources in Shanghai Municipality in 2020. The equity of mental health resources allocated by population and geographical area in Shanghai Municipality was evaluated with Lorenz curve and Gini coefficient
Results:
There were 96 mental health institutions, 15 060 actual beds opened, 257 certified or assistant psychiatrists and 2 887 registered nurses in Shanghai Municipality in 2020, with a physician-to-nurse ratio of 1∶2.30. The greatest numbers of actual beds opened in the department of psychiatrics, the number of certified or assistant psychiatrists and the number of registered nurses per 10 000 residents and per km2 were all found the central urban areas. The numbers of actual beds opened in the department of psychiatrics, the number of certified or assistant psychiatrists and the number of registered nurses per 10 000 residents were 6.06 beds, 0.51 physicians and 1.16 nurses, with Gini coefficients of 0.36, 0.42 and 0.44, respectively, and the numbers of actual beds opened in the department of psychiatrics, the number of certified or assistant psychiatrists and the number of registered nurses per km2 were 2.38 beds, 0.20 physicians and 0.46 nurses, with Gini coefficients of 0.72, 0.76 and 0.75, respectively.
Conclusions
There was a gross equity in mental health resources allocated by population and geographical area in Shanghai Municipality in 2020, which showed an improvement as compared to 2015. The equity in mental health resources allocated by geographical area was lower than that by population in Shanghai Municipality.
8.Methylation of p16 and hMLH1 genes in esophageal squamous cell carcinoma and reflux esophagitis.

OBJECTIVETo study the promoter methylation pattern of p16 and hMLH1 genes in esophageal squamous cell carcinoma and reflux esophagitis, and to correlate the results with clinical and pathologic findings.
METHODSTwelve cases of normal esophagus, 13 cases of esophageal squamous cell carcinoma, 43 cases of reflux esophagitis with basal cell hyperplasia and 21 cases of reflux esophagitis with dysplasia, as confirmed by endoscopic and pathologic examination, were enrolled into the study. Genomic DNA was extracted. The promoter methylation status of p16 was measured by methylation-specific polymerase chain reaction. The promoter methylation status of hMLH1 was measured by sodium bisulfite-restriction enzyme digestion. Immunohistochemical study for p16 and hMLH1 proteins was also carried out.
RESULTSThe rates of p16 methylation in normal esophageal epithelium, basal cell hyperplasia, dysplasia and esophageal squamous cell carcinoma were 0/12, 14.0% (6/43), 38.1% (8/21) and 6/13, respectively. The p16 methylation correlated with the progress of esophageal lesions. On the other hand, the hMLH1 methylation was not observed in the normal esophageal epithelium and reflux esophagitis. One case of esophageal squamous cell carcinoma showed the presence of hMLH1 methylation. The hMLH1 promoter hypermethylation did not correlate with the clinical and pathologic features.
CONCLUSIONSThe p16 methylation may be one of the earliest events in the pathogenesis of esophageal squamous cell carcinoma and is also observed in reflux esophagitis. Reflux esophagitis may be related to the development of esophageal squamous cell carcinoma in Chinese population. In contrast, hMLH1 methylation may not be directly involved in the tumorigenesis of esophageal squamous cell carcinoma.

9.The clinical and mammographic features of plasma cell mastitis

Objective To investigate the clinical and mammographic features of plasma cell mastitis.Methods Twenty-five patients(28 lesions)with histologically confirmed plasma cell mastitis, aged from 26 to 70 years(mean age 41 years),were examined with X-ray mammography.The clinical manifestations and imaging features were retrospectively reviewed.Results No case was in lactation.The painful irregular masses,ranged from 1.3 to 8cm in size,were found in 22 patients,while 3 patients with acute episode.Recurrent episodes of breast masses were noted in 4 patients.Based on the mammographic appearances,the plasma cell mastitis were classified as the following four types:inflammation-like type (2/28),ductal ectasia type(3/28),focal infiltration type(10/28)and nodular type(13/28).The valuable radiogyaphic signs:(1)An asymmetrically increased density along the lactiferous duct with a flame-like appearance,inhomogeneous low density tubular structures and scattered stick-shape calcifications.(2) Architectural distortion and oil cysts formation in adjacent area,(3)Subareolar ductal ectasia.Conclusions The clinical and mammographic characteristics of plasma cell mastitis are critical to avoiding unnecessary surgery.Histopathological result is needed for the diagnosis in patients highly suspected of malignancy.
10.Nitroglycerin tolerance aggravates arterial ischemia/reperfusion injury by increasing nitrotyrosine and ONOO- production.

OBJECTIVETo investigate the influence of nitroglycerin tolerance (NT) on arterial ischemia (90 min) and reperfusion (120 min).
METHODSMale Sprague-Dawley rats were infused with nitroglycerin (GTN) or saline for 12 h and ascending aorta was rapidly isolated. The isolated aorta was subjected to one of the following treatments: stimulative ischemia/reperfusion, stimulative ischemia/reperfusion (I/R) plus glutathione (GSH, 0.1 mmol/L) during reperfusion, or control solution (Kreb's solution for 3.5 h).
RESULTSCompared with I/R group, contractile function, vasorelaxation responses to Ach, NO production were significantly decreased and CK, LDH activity as well as nitrotyrosine formation in reperfusion solution were significantly increased in I/R + NT group and these effects could be prevented with addition of GSH in I/R + NT aortas.
CONCLUSIONSOur results demonstrated that NT could aggravate arterial I/R injury by increasing the production of ONOO- and GSH may play a cardioprotective role against NT-induced myocardial injury by attenuating the formation of ONOO-.
