1.Four cases report of functional visual loss

Functional visual loss is caused by the functional disturbance of visual cortical area, which is induced by strong emotion impact. With the characteristics of diverse clinic symptoms, functional visual loss is usually hard to be identified and diagnosed. Four cases of functional visual loss are reported in this article. The patients were diagnosed by the query of medical history, observation of behavior and some examinations such as foggy test. The patients received suggestibility treatment, and achieved favourable therapeutic effect.
2.Preparation of a monoclonal antibody against Trichinella spiralis adult and its protective immunity

Objective To prepare a monoclonal antibody(McAb) against Trichinella spiralis (T.s) adult and study its protective immunity. Methods BALB/c mice were immunized with soluble antigens of 3 day adult of T.s. Immnuized spleen cells of BALB/c mice were fused with myeloma cells sp2/0. The hybridoma culture supernatant was isotyped by the Ouchterlony double diffusion technique and the specificity of the McAb was determined by immunoblotting. The protective immunity of the McAb was detected after challenging the mice by the intravenous way. Results A McAb against T.s adult was obtained. The titer of culture fluid and ascetic fluid was 1∶6 400 and 1∶204 800, respectively. This McAb was identified as IgM. Western blotting results showed this McAb can be used to identify 40 000 protein of adult worm, muscle larvae and newborn larvae of T.s. No cross reactions with antigens of Ascaris suum Goeze, Taenia solium Linnaeus, Echinococcus granulosus and Schistosoma were oberserved. The number of muscle larvae was decreased by 55.63% when giving McAb to mice intravenously. Conclusions A species specific McAb against T.s adult was established and its protective immunity was identified. This McAb can be used as a powerful tool in screening Trichinella vaccine candidates.

Cell adhesion molecules(CAMs) play important roles in specifying cell-cell interactions during development,regeneration,and modification of synaptic activity.The close homolog of L1(CHL1) ,a recently identified member of the immunoglobulin superfamily of cell adhesion molecules,is localizably expressed in the nervous system.CHL1 interacts with like molecules(homophilic interaction) and non-like molecules(heterophilic interaction) on neighboring cells or the extracellular matrix to regulate axon outgrowth and fasciculation,neuronal migration and survival,synaptic plasticity and regeneration after trauma.
4.Modified uvulopalatopharyngoplasty for treatment of obstructive sleep apnea hypopnea syndrome

OBJECTIVE To investigate the therapeutic effect of modified uvulopaoatopharyngoplasty(UPPP)on obstructive sleep apnea hypopnea syndrome(OSAHS).METHODS Sixty eight cases of OSAHS were treated with modified UPPP.During operations,the uvula was preserved,the fat tissues of space veli palatine were dissected,palatoplasty and pharyngoplasty were performed.All the patients were followed up for over 6 months.Polysomnography(PSG)was monitored again one year after operation.RESULTS The symptoms of dyspnea,snoring,sleepiness were disappeared or improved obviously after 6 months.The effective rate was 95.6% at one year after operation.CONCLUSION The modified UPPP not only extends the oropharyngeal cavity,but also avoids postoperative complications and improve the effectiveness of UPPP.
5.In vitro culture and influencing factors of mesenchymal stem cells derived from human umbilical blood

BACKGROUND: Multipotential mesenchymal stem cells (MSCs) can be differentiated into bone, fat, cartilage, muscle and endothelial cell at the specific conditions, so it draws great attentions of the related study.OBJ ECTTVE: To establish the method of in vitro culture and expansion of human umbilical blood-derived MSCs and investigate their influencing factors.DESTGN: Randomized and controlled trials.SETTING: Department of Tissue Engineering and Cell Biology in Wuxi Third People's Hospital.MATERTALS: Seventy cases of human umbilical cord blood (HUCB) of healthy neonates (selected from Parturient Room, Department of Gynecology and Obstetrics, Wuxi Third People's Hospital, with the informed consents of the puerpera and their relatives), 40 mL each; dulbecco's minimal essential medium (DMEM) of low glucose (LG) at the dose of 50 g/L, fetal bovine serum (FBS), mycillin and trypsin (all were from Gibco), FITC-labeled mice anti-human CD29, CD105 and CD106 (Ancell) monoclonal fluorescent antibody, PE-labeled mice anti-human CD34, CD44, CD45, CD19, HLA-DR (Immunotech)monoclonal fluorescent antibody, Ficoll separation medium (Pharmacia).METHODS: The experiment was carried out in Department of Tissue Engineering and Cell Biology in Wuxi Third neonatal cord blood were anticoagulated by 25 u/mL heparin, and 60 cases of isolated HUCB mononuclear cell were precipitated and suspended by cell culture fluid (50 g/L DMEM-LG + 50 g/L FBS + 10 g/L mycillin), while other 10 cases were purified with DMEM of high glucose (100 g/L) to produce adherent layer. Once the cells reached fluences of FBS coating on adhesive rats of MSCs: Some of the purified HUCB MSCs at logarithmic phase were growth curve of MSCs: The growth of cells were observed by phase contrast microscope (OLYMPUS CK40), and were detected by EPICS-ALTRA flow cytometry.FBS coating and without FBS coating at the different time points, and their adhesive rates.RESULTS: All 70 cases of HUCB MSCs were adopted in this study, however, 10 samples failed to obtain the ideal MSCs cord blood MSCs after culture. During 2 weeks of culture, primary cells began to double for 3-4 days and reached conand CD105, but did not express antigens CD34, CD45, CD106 and HLA-DR, which was identical to surface labeling of those without FBS coating (P<0.01).CONCLUSrON: MSCs in HUCB can be cultured and expanded in vitro, and are regarded as an alternative source of MSCs for experimental and clinical applications. Moreover, high glucose can depress the growth and proliferation of MSCs in HUCB.
6.A clinical report of five cases of central nervous system miliary tuberculomas first diagnosed by neurologists

To explore the clinical characteristics and imaging features of miliary tuberculomas in central nervous system (CNS).A total of 5 cases diagnosed with tuberculosis in CNS first diagnosed by neurologists in Navy General Hospital of PLA were enrolled in the study.All clinical and imaging data were collected and analyzed retrospectively.The main initial symptoms were fever and headache (4/5).Multiple diffused miliary lesions were shown by brain MRI,with maximum diameter ranged from 1-4 mm and ringshape or nodular enhancement after gadolinium injection.As mycobacterium tuberculosis could seldomly be found in serum and cerebrospinal fluid,contrast MRI remains the effective method for detecting miliary tuberculomas in CNS.
7.Diagnosis and treatment of cerebral schistosomiasis:a report of 166 cases

Objective To discuss the clinical features diagnosis and treatment of cerebral schistosomiasis. Methods A to?tal of 166 patients with cerebral schistosomiasis were treated and their clinical data were collected and analyzed retrospectively. Results In 166 cases of cerebral schistosomiasis the confirmative diagnoses of 156 cases were diagnosed according to the clin?ical manifestation etiology immunology and auxiliary examination CT MRI .In among 74 cases were confirmed by pathologi?cal examination 10 cases were diagnosed through to the diagnostic treatment. Totally 102 patients received the oral medication of praziquantel and they all improved and discharged 14?16 days later 64 patients received the craniotomy and praziquantel medication after the operation and 48 patients significantly improved others did not improve or aggravated. There was no opera?tive mortality. Conclusions Neuroimaging and laboratory tests are valuable in the diagnosis of cerebral schistosomiasis. The praziquantel treatment is selected firstly when the diagnosis was established. However in the case of serious intracranial hyper?tension intractable epilepsy and praziquantel treatment fails the surgical treatment is required.
8.A comparative study of difference internal fixation methods for calcaneal fracture

Objective To observe the therapeutic effect of titanium plate and Kirschner wire system for treatment of calcaneal fracture.Methods The data of 189 patients with calcaneal fracture treated with operation were retrospectively studied,of which 102 patients of calcaneal fracture were treated with titanium plate fixation (titanium plate group),87 patients were treated with Kirschner wire system (Kirschner wire group).The operative time,blood loss,postoperative complication,healing time,before and after treatment Bohler angle,Gissane angle and middle calcaneal width,and Maryland foot function score were observed.Results The patients of the 2 groups were observed with 12-36 (24.0 ± 9.5) months follow-up.The operative time and blood loss in Kirschner wire group were significantly lower than those in titanium plate group [(42.3 ± 12.5) min vs.(67.5 ± 23.1) min and (137.7 ± 42.8) ml vs.(245.0 ± 55.6) ml],there were statistical differences (P < 0.05).The Bohler angle,Gissane angle and middle calcaneal width in the 2 groups after treatment were significantly better than those before treatment [titanium plate group:(32.80 ± 6.35)° vs.(7.81 ± 8.75)°,(130.42 ±6.93)° vs.(175.80 ± 6.05)°,(30.77 ± 3.56) mm vs.(42.60 ±5.24) mm; Kirschner wire group:(30.11 ±4.91)° vs.(9.54 ±8.90)°,(132.89 ±9.54)° vs.(173.11 ±9.05)°,(32.00 ± 4.18) mm vs.(42.78 ± 4.96) mm],there were statistical differences (P< 0.05).The excellent rate of Maryland foot function score after treatment in titanium plate group was significantly higher than that in Kirschner wire group [92.2% (94/102) vs.81.6% (71/87)],there was statistical difference (P < 0.05).Therewere 6 cases appeared infection of incisional wound in titanium plate group,there was no complication in Kirschner wire group,there was statistical difference (P < 0.05).Conclusions Titanium plate and Kirschner wire system are the commonly used methods in treatment of calcaneal fractures,each has its advantages and disadvantages,titanium plate has long time operation,much blood loss and prone to infection,but the excellent rate is higher.Also note that the incision location,restoration and reconstruction of calcaneal anatomy subtalar joint.

10.Hip arthroplasty for failed internal fixation of femoral neck fractures.

OBJECTIVETo investigate the result of hip arthroplasty for failed internal fixation of femoral neck fractures.
METHODSFrom June 2007 to January 2014, 29 cases who underwent hip arthroplasty for failed of internal fixation of femoral neck fractures were reviewed. There were 12 males and 17 females. The mean age was 60.3 years (ranged 43 to 83 years) at the time of the fracture. Left hip was in 16 cases, right hip was in 13 cases. The average interval from fracture to arthroplasty was 23.3 months (ranged, 3 to 48 months).
RESULTSAll of 29 cases were performed total hip arthroplasty. There were 20 cases of cementless cup,7 cases of cementless cup with bone graft, 2 cases of cemented cup with bone graft; 13 cases of cementless stem, 16 cases of cemented stem. There were no complications occurred such as intraoperative fracture of the greater trochanter. The average operative time was (115 ± 38) minutes,the mean intraoperative blood loss was (420 ± 175) ml, the average postoperative drainage volume (240 ± 119) ml, intraoperative blood transfusion was (200 ± 220) ml, intraoperative fluid volume was (2,200 ± 400) ml, the average postoperative blood transfusion was (300 ± 200) ml. There was 1 case get postoperative dislocation. All patients were followed up for 14.7 months in average (ranged, 5 to 24 months). There was no revision for mechanical failure. Harris Hip Score significantly was improved from 51.1 ± 7.5 before the conversion to 88.5 ± 6.4 points at the final follow-up.
CONCLUSIONThe effect of the hip replacement for patients with failed internal fixation of femoral neck fractures was confirmed. This method can shorten the time on the bed and reduce the complications. It benefits the patients earlier functional recovery, but it must control operation indication. The long term efficacy is necessary to further observation.
