2.Total laparoscopic splenectomy and pericardial devascularization for the treatment of portal hypertension

Objective To evaluate total laparoscopic splenectomy combined with pericardial devascularization. Methods In this study,40 patients with portal hypertension and hypersplenism underwent total laparoscopic splenectomy combined with pericardial devascularization,in which 26 cases had a history of up-GI bleeding. Results Procedures were successful in 36 patients.There was four conversions to open surgery.The operations were completed in a mean time of (224 ± 54) min,with a mean estimated blood loss of (296 ± 209) ml and with no serious complication. Conclusion Total laparoscopic splenectomy combined with pericardial devascularization is a safe and effective available minimally invasive procedure in patients with portal hypertension and hypersplenism.
4.Application value of Provider-Initiated HIV Testing and Counseling in dermatology.

OBJECTIVETo explore the clinical application value of Provider-Initiated HIV Testing and Counseling (PITC) by analyzing the positive rate of HIV tests for people in need of PITC and that of routine HIV tests.
METHODSWe retrospectively analyzed the demographic and epidemiologic data about the patients seeking PITC services or undergoing routine HIV tests in Nanjing Drum Tower Hospital between January and December 2013.
RESULTSThe positive rate of initial HIV screening was 1.98% in the PITC group and 0.24% in the routine test group, while that of confirmed HIV was 0. 40% in the former and 0.07% in the latter, both with statistically significant differences between the two groups (P < 0.01). The positive rate of HIV was markedly higher in males than in females, particularly in the PITC group.
CONCLUSIONPITC has a high clinical value in HIV detection for targeted subjects and therefore deserves general application in dermatology.

5.Design and implementation of a medical record management subsystem as a part of a telemedical system.

This paper introduces the work on the design and implementation of a medical record management subsystem, which is a part of a telemedicine system based on Microsoft .NET. First, an analysis is given on components and structure of medical record, together with an explanation for what is needed in a teleconsultation, how they should be organized and realized in a database, and how to deal with consultation data granularity using XML in combination with database; second, the work done during the realization stage is expatiated, including system analysis and design, database design, and system implementation.

6.Clinical features of unspecified functional bowel disorder in servicemen from a Chinese army unit

Objective To investigate clinical manifestation of unspecified functional bowel disorder (UFBD), the features of coexistence with functional gastrointestinal disorder (FGID) and its relationship with psychological factors and sleep disturbance in the Chinese Army servicemen.Methodsc FGIDs were diagnosed based on the RomeⅢ Modular Questionnaire. The subjects were 189 servicemen with UFBD (UFBD group) and 372 without FGID (control group). All subjects completed symptom checklist 90 (SCL-90) and Pittsburgh Sleep Quality Index (PSQI) questionnaire.Results'Have to rush to the toilet when having a desire to defecate' was the most frequent symptom of UFBD (93.7%). More than one half of UFBD patients had the symptom 'a feeling of incomplete emptying as bowel movements' or 'straining during bowel movements'. Twenty-eight percent of UFBD subjects had combined FGID (namely cFGID). Among them, the most frequent was proctalgia fugax (7.9%), followed by cyclic vomiting syndrome (6.3%), functional fecal incontinence (6.3%), functional dyspepsia (4.8%) and belching (4.8%). The UFBD group scored significantly higher than the control group in the global severity index (GSI) and in all SCL-90 subscales (P<0.05). The scores of the four domains (sleep quality, sleep latency , sleep disturbance and daytime function disorder), total PSQI score and proportion of poor sleeping quality were significantly higher in the UFBD group than in the control group (P<0.05). The subjects scored significantly higher in combined FGID group than in UFBD group in GSI and in all of SCL-90 subscales, except for phobic anxiety subscales (P<0.05). However, there was no significant difference in each domain, total PSQI and proportion of poor sleeping quality between the cFGID group and UFBD group (P>0.05).ConclusionPathogenesis of UFBD may be closely correlated with psychiatric and psychological factors and sleep disturbance. cFGID are associated with an increased severity of psychopathological features.
7.Comparison between harmonic scalpel as major instrument and conventional operation for tonsillectomy.

OBJECTIVETo investigate the feasibility of applying harmonic scalpel in tonsillectomy by comparing it with the conventional tonsillectomy.
METHODSThirty-five patients were treated by harmonic scalpel assisted tonsillectomy (harmonic scalpel group) and 35 patients were treated by conventional tonsillectomy (control group). The operating time and blood loss during operation and the degree of pain and post-tonsillectomy hemorrhage as well as the time of defining were observed.
RESULTSThe data of the operating time and blood loss during operation and the degree of pain in harmonic scalpel group were lower than that in the conventional group, the harmonic scalpel group had less perioperative bleeding (median 1 ml), short operation time (median 20 min), postoperative pain during 5 days with visual analogue scale was lighter (median from 5 scores down to 1 score), there was statistical difference (P < 0.01). Neither primary hemorrhage nor secondary hemorrhage occurred in harmonic scalpel group, while one primary hemorrhage and one secondary hemorrhage occurred in conventional group. No difference was found in the beginning time of defining (P > 0.05).
CONCLUSIONThe application of harmonic scalpel in tonsillectomy was safe, efficient and feasible, which had a nice application prospect in the field of otolaryngology.

8.DNA damage and changes of antioxidative enzymes in chronic benzene poisoning mice.

OBJECTIVETo study the damage effect of benzene on DNA and its mechanism and the changes of antioxidative enzymes in vivo.
METHODSDNA break in bone marrow cells and peripheral blood lymphocytes of mice exposed to benzene by 4 h static inhalation per day at different concentrations for two months were analyzed with single cell gel electrophoresis (SCGE). Meanwhile, the activity of SOD, GSH-Px and the level of MDA in liver, spleen and brain were detected.
RESULTSIn low and high dosage groups, the rate of DNA migration of bone marrow cells (83.56% +/- 10.28%, 92.54% +/- 15.93%) and peripheral blood lymphocytes (41.27% +/- 6.03%, 65.79% +/- 11.62%) were higher than those in control (4.13% +/- 0.52% and 2.21% +/- 0.31% respectively, P<0.05]. The activity of SOD in liver [(754.33 +/- 116.30), (694.26 +/- 116.30) U/mg pro] and GSH-Px [(22.52 +/- 3.31), (18.56 +/- 4.97) U/mg pro] were lower than those in control [(999.92 +/- 188.24) and (35.31 +/- 6.63) U/mg pro respectively, P<0.05, P<0.01]. But there was no significant difference between the two dosage groups. The activity of GSH-Px in spleen of both groups [(31.38 +/- 2.71), (25.30 +/- 7.44) U/mg pro] were lower than that of control [(37.11 +/- 3.42) U/mg pro, P<0.05] and there was significant difference between the two dosage groups. The activity of GSH-Px in brain of both groups [(5.70 +/- 0.84), (5.24 +/- 1.19) U/mg pro, P<0.05] were lower than that of control [(7.10 +/- 0.46) U/mg pro, P<0.05], but there was no significant difference between the two dosage groups. The level of MDA in brain of high dosage group [(3.99 +/- 1.15) nmol/mg pro] was higher than that of control [(2.58 +/- 0.53) nmol/mg pro, P<0.05].
CONCLUSIONChronic benzene poisoning may result in DNA break in bone marrow cells and peripheral blood lymphocytes and decrease in the activity of antioxidative enzymes.

9.The Role of Hydrogen Sulfide in Acute Liver Injury Induced by Traumatic Stress in Rats.

OBJECTIVE:
To explore the role of hydrogen sulfide (H2S) in acute liver injury induced by crushing hind limbs of rats.
METHODS:
The rats were randomly divided into the following groups: control, crushing, H2S donor sodium hydrosulfide (NaHS) + crushing, H2S inhibitor propargylglycine (PAG) + crushing group. The acute liver injury model was established by 'crushing the hind limbs of rats with standard weight. Rats were sacrificed at 30 min and 120 min after the crush. The activities of serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were measured by colorimetric method, and the content of H2S in plasma and the contents of malondialdehyde (MDA), protein carbonyl, glutathione (GSH) in the liver and the activity of H2S generating enzyme (cystathionine y-lyase, CSE) were determined by chemical method. The expression of CSE mRNA in liver was detected by RT-PCR.
RESULTS:
For crush injury group, the levels of AST and ALT in serum, MDA and protein carbonyl in liver increased. The levels of GSH, CSE, CSE mRNA in liver and H2S in serum decreased. The administration of NaHS before limbs crush could attenuate the changes of liver injury, but the pre-treatment with PAG could exacerbate the changes.
CONCLUSION
The decrease of H2S production could involve in mediating the acute liver injury induced by traumatic stress in rats.
