1.Determination of Baicalin in Jinchaihuang Granules by RP-HPLC

[Objective] To establish the contents determining means of baicalin in Jinchaihuang granules.[Methods] Baicalin was carried out by HPLC.[Results] Baicalin showed a good linear relatiaoship at a range of 0.157~1.256?g,r=0.9997.The average recovery was 101.31%,and RSD was 1.26%.[Conclusion]The methods are accurate and quick,and can be used for the quality control of Gujingdan granules.
2.An experimental study of rh-endostatin in combination with radiation on human lung adenocarcinoma A549 cell in mice

Background and purpose:There were reports that radiotherapy might intensify angiogenesis and result in the resistance of tumor to radiotherapy.Endostar,a new type rh-endostatin,may specifically inhibit angiogenesis and tumor growth,so it may reduce the resistance to radiotherapy.This study was to evaluate the inhibitory effect of Endostar in combination with radiation compared with cisplatin(DDP) plus radiation on lung adenocarcinoma A549 in BALB/c nude mice.Methods:We established A549 lung adenocarcinoma xenograft models.When the largest diameter of tumor reached 1.0 cm,all mice were randomized into 4 groups(n=6):control,radiation alone,radiation plus DDP,radiation plus Endostar.We measured the largest diameter and the vertical diameter of the tumors at different time points after treatment.At the 16th day,mice were executed and the tumors were subjected to removal for the analysis of apoptosis,and the expression level of basic fibroblast growth factor(bFGF) mRNA by reverse transcription-polymerase chain reaction(RT-PCR) and vascular endothelial growth factor(VEGF) were also detected by immunohistochemistry.Results:The tumor growth velocity of control,radiation alone,radiation plus DDP,radiation plus Endostar were(162?6)%,(131?8)%,(104?7)% and(108?11)% respectively(P0.05).The apoptosis rates of control,radiation alone,radiation plus DDP,radiation plus Endostar were(12.2?1.1)%,(16.5?0.8)%,(24.4?1.5)% and(20.5?1.9)%,respectively(P
3.Application of the Luo’s fornix cup in laparoscopic total hysterectomy

Objective To explore the clinical value of the Luo’s fornix cup in laparoscopic total hysterectomy. Methods A total of 7 patients with indications for hysterectomy from October 2004 to February 2005 was given laparoscopic total hysterectomy. During the operation, the Luo’s fornix cup was used for the orientation and dilatation of the vaginal fornix. Then the anterior vaginal fornix was circlewise incised by using a harmonic hook to extract the Luo’s fornix cup. Afterwards the posterior fornix was opened to remove the uterus by way of the vagina. And finally the vaginal stump was closed. Results All the operations were successfully completed. The operation time was 90~250 min. With the use of the Luo’s fornix cup, the operator could easily identify the top of the vaginal fornix, and avoid the damage of important neighbouring organs. No injuries of bladder, ureter, and rectum were noted. Conclusions The Luo’s fornix cup is a rational accessory instrument in laparoscopic hysterectomy, being worthy of recommendation.
4.Double versus single internal thoracic artery in coronary artery bypass grafting: role in sterna wound healing

Objective To investigate the effect of double versus single internal thoracic artery on sterna wound healing in coronary artery bypass grafting (CABG).Methods Patients undergoing CABG were randomly divided into experimental group (double internal thoracic artery group) and control group (single internal thoracic artery group) according to the number of internal thoracic artery used in operations.Experimental group:skeletonized double internal mammary arteries were harvested in all 50 cases (37 males and 13 females).Control group:skeletonized left internal mammary artery was harvested in all 50 cases (34 males and 16 females).Sterna wound healing and post-operational complications were observed.Results The incidence of poor sterna wound healing and volume of drainage after operation in the experimental group were more than those in the control group [volume of drainage on the first day:(350.67±56.86)ml vs (240.72 ±48.35)ml;total volume of drainage:(595.65 ± 102.35) ml vs (398.85 ± 75.84) ml;poor sterna wound healing rate:20% vs 0;P < 0.05 or P < 0.01].The sterna wound healing rate of high-risk patients between two groups had significant difference (8 cases vs 0 case,P < 0.05).Conclusions Compared to the single internal mammary artery grafting,the double internal mammary artery grafting increased the risk of sternal wound healing.
5.Intervention of Tert-butyl Hydroperoxide-Induced Injury in Human Umbilical Cord Derived Mesenchymal Stem Cells with Ginkgo Biloba Extract

Objective To observe the intervention effect of Ginkgo biloba extract EGb761 on tert-butyl hydroperoxide ( t-BHP)-induced injury in human umbilical cord-derived mesenchymal stem cells ( hUC-MSCs) . Methods Proliferation of hUC-MSCs after primary culture was detected by methyl thiazolyl tetrazolium (MTT) assay, and then the optimal concentrations of t-BHP and EGb761 for oxidative stress injured MSC model were screened. Cell apoptosis was assessed by flow cytometry after Annexin V-fluorescein isothiocyanatel propidium iodide ( Annexin V-FITC/PI) staining. Content of malondialdehyde ( MDA) and superoxide dismutase (SOD) activity in hUC-MSCs were evaluated, and the expression levels of p53 and p21Cip1/Waf1 were analyzed by real-time fluorescence PCR. Results Pretreatment with 10~200 mg/L of EGb761 for 3 hours reduced the sensitivity of hUC-MSCs to t-BHP ( 100 μmol/L) induced proliferation inhibition, while EGb761 over 100 mg/L had no significant effect on enhancing the protection of hUC-MSCs . EGb761 at 100 mg/L prohibited hUC-MSCs apoptosis and MDA accumulation in hUC-MSCs induced by 100μmol/L of t-BHP acting for 6 hours, maintained the enzymatic activity of SOD, and decreased the expression of p53 and p21Cip1/Waf1 in hUC-MSCs with t-BHP-induced injury. Conclusion EGb761 is capable of protecting hUC-MSCs against oxidative stress injury, and its mechanism is probably related with the modulation of p53/p21 signal pathway.
6.Analysis of thigh pain after treatment of femoral trochanteric fractures by proximal femoral nail an-tirotation Ⅱ

Objective To analyze the causes for the thigh pain after treatment of femoral trochanteric fractures by proximal femoral nail antirotation Ⅱ( PFNA Ⅱ) . Methods Included in this ret-rospective study were 236 patients who had been treated by us for femoral trochanteric fracture from October 2011 to December 2015. They were 103 men and 133 women, aged from 42 to 86 years (average, 50. 3 years) . According to AO classification, 13 cases belonged to type 31-A1. 2, 32 to type 31-A1. 3, 35 to type 31-A2. 1, 27 to type 31-A2. 2, 33 to type 31-A2. 3, 38 to type 31-A3. 1, 39 to type 31-A3. 2 and 19 to type 31-A3. 3. All the fractures were single, fresh and closed and treated with PFNAⅡinternal fixation. Results This cohort was followed up for 8 to 26 months (average, 13. 2 months). Nonunion occurred in one case who had to accept artificial hip replacement. The remaining 235 cases obtained bony union after 22 to 39 weeks (average, 29. 3 weeks). By the Harris evaluation at final follow-ups, the affected hips scored from 81 to 93 points (average, 85. 1 points) . Post-operative thigh pain was reported in 19 cases (8. 05%) . The causes included varied anatomic morphology of the proximal femur in 6 cases, distal defects of the intramadullary nails in 4, insufficient stability of internal fixation or uneven biomecanical distribution in 3, unskillful operation in 2, and severe oesteoporosis in 4. Avascular necrosis of femoral head was not observed during follow-ups. Conclusions Postoperative thigh pain is worthy of serious atention from orthopaedists following PFNA Ⅱtreatment of femoral trochanteric fractures. PFNA Ⅱshould be modified according to the specific Chinese features of the proximal femur, especially in the respects of anterior arch and distal structure of the main nail and lateral declination as well.
7.Comparison of the effect between the hip replacement and the hip replacement after failed internal fixation in elderly patients with displaced femoral neck fracture

Objective To compare the clinical effect of the primary total hip replacement (THR)and the secondary THR after failed internal fixation in elderly patients with displaced femoral neck fracture,so as to investigate the optimal treatment for displaced femoral neck fractures in elderly patients.Methods From January 2006 to December 2012,22 patients treated with a secondary THR after failed internal fixation were chosen as the observation group and 30 patients treated with a primary THR were chosen as the control group,the operation time,blood loss,postoperative Harris scores for hip joint and health-related quality of life (~ index score) were observed and analyzed in two groups.Results Both of the groups were followed up successfully and each of the groups had 1 patient died.There were no joint infection,re-operations and any other complications in both groups.The operation time in observation group was longer than that in control group [(114.82 ±32.13) min vs.(90.63 ± 16.24) min],blood loss was more than that in control group [(551.73 ± 241.62) ml vs.(314.46:± 156.72) ml],there was significant difference (P < 0.05).But the Harris score and KPS index score between two groups had no significant differences (P > 0.05).Conclusion The primary THR in elderly patients with displaced femoral neck fracture has lower risk in operation tocompare with the secondary THR after failed internal fixation,but the two ways are similar in the hip joint function recovery and improve patients quality of life.

The early of nutritional support (parenteral and enteral) in the very/extremely low birth weight infants improve the survival rate and avoid extrauterine growth restriction.The practice of early nutritional strategy,appropriate enteral feeding and the individualized care and assessment program can result in catch-up growth and avoid growth and development deficits.
9.The nursing of the surgery for the dysembryoplastic neuroepithelial tumors

Objective To explore the observation and nursing of the surgery of dysembryoplastic neuroepithelial tumors (DNT) in the function zone of brain.Methods A retrospective analysis of the clinic data of 5 patients receiving the surgery for the dysembryoplastic neuroepithelial tumors (DNT) in the function zone of brain was carried out,the clinical nursing experience was summarized.Results The EEG was re-examined 1 week after surgery.4 cases showed roughly normal EEG,one case with rare slow wave.The re-examined EEG 3 months after surgery suggested 3 cases were normal and 2 cases were almost normal.No epilepsia occurred after 6-month follow-up.Conclusions For DNT patients,microsurgical resection of the tumor is effective and enhancing the patients' perioperative observation and nursing has a significant meaning to increase the success of surgery,reduce complications,promote the rehabilitation of patients and improve the quality of life of patients.
10.First line gefitinib monotherapy for brain metastases patients with advanced non-small cell lung cancer

Objective To evaluate the efficacy and toxicity of gefitinib on the treatment of advanced NSCLC with brain metastases.Methods 17 cases received gefitinib 250 mg/m2/d,30 days a cycle.Results Objective response rate of 35.3% was achieved,and DCR was 70.6% ,the toxic reactions could be controlled with relative therapy.Conclusion Gefitinib is effective and well tolerable for advanced NSCLC with brain metastases.It is worth to be used.
