3.Angiographic observation of the effect to prevent and reverse cerebral vasospasm by lumbar subarachnoid space continuous drainage following subarachnoid hemorrhage

Objective In this study,the authers investigate the effect of lumbar arachnoid space continuous drainage to prevent and reverse cerebral vasospasm (CVS) angiographically following experimental subarachnoid hemorrhage (SAH).Methods SAH model was achieved by cisternal magna twice injections of autolugous blood.Thirty adult mongrel dogs presented with SAH were randomly divided into five groups: early-drainage group,later-drainage group,early-puncture group,later-puncture group and control group. The lumbar puncture two groups started to puncture and exclude blood cerebral spinal fluid(CSF) 2 hours later following the second injection and on day 5 respectively,while the drainage two groups started to drain at the same time.The degrees of CVS were analyzed through cerebral angiography(%reduction of basilar artery diamiter,%RBAD). Results ①Compared with early puncture group and control group,early drainage group had less cases of CVS and the degrees of CVS were more slight which relieved within shorter period than other two groups.②Compared with later puncture group and control group,later drainage group had less cases of CVS and the period of CVS was shorter.③Compared with later drainage group,there were less cases of CVS and the degrees of CVS were more slight in early drainage group.Conclusions Lumbar subarachnoid space continuous drainage may prevent and reverse the CVS following SAH.Early lumbar subarachnoid space continuous drainage should be presented with better consequence.
4. Development and Revision of the General Principles of Physical and Chemical Analysis in the Chinese Pharmacopoeia 2020 Edition Volume

OBJECTIVE: To provide references for the correct understanding and accurate implementation of the physical and chemical analysis general chapters of the Chinese Pharmacopoeia 2020 Edition volume . METHODSE: The principles, processes, and main considerations of the development of the physical and chemical analysis general chapters of the Chinese Pharmacopoeia 2020 edition volume IV are introduced, and the content and main characteristics are summarized and analyzed. RESULTS: According to the goals and key tasks of the Chinese Pharmacopoeia 2020 edition, the principles of scientificity, versatility, operability, gradual and sustainable development, and harmonization with international standards, the physical and chemical analysis committee of the Chinese Pharmacopoeia commission carry out the development of the physical and chemical analysis general chapters of the Chinese Pharmacopoeia 2020 edition volume . Further expand the application of advanced and mature analytical techniques, strengthen the applicability of analytical methods and the development of the external pollution control methods for Chinese medicinal materials, and improve the harmonization of analytical techniques and impurity control requirements with international technical requirements. CONCLUSION: The physical and chemical analysis general chapters of Chinese Pharmacopoeia 2020 edition volume provide method and technical support to ensure the safety, effectiveness and controllable quality of China's pharmaceuticals, and play an important and positive role in encouraging the application of advanced analytical technology, strengthening drug regulatory measures, and enhancing the international influence of the Chinese Pharmacopoeia.
5. Overview and Analysis of General Chapters (Appendices)of Physical and Chemical Testing Methods in Pharmacopoeias

OBJECTIVE: To provide reference for introducing new methods and technologies to the physical and chemical testing methods general chapters in Chinese Pharmacopoeia Volume . METHODS: By reviewing the general chapters of drug physical and chemical testing methods (appendices)in the new editions of European, US, Japanese and British Pharmacopoeias, the harmonization results of the Pharmacopoeia Discussion Group (PDG)and the guidelines of the International Council for Harmonization of Technical Requirements for Pharmaceuticals for Human Use (ICH), the authors summarized the overview and new, revised and harmonized texts of physical and chemical testing methods, and made a preliminary comparison with the related testing methods in Chinese Pharmacopoeia (2015 edition) Volume . RESULTS: The new and revised text on the physical and chemical testing methods in the new editions of European, US, Japanese and British Pharmacopoeias are mainly concentrated in elements impurities, spectrometry and chemometrics. Chinese Pharmacopoeia can learn from the technology of identification tests, spectrometry and chromatography, impurities analysis and functionality-related characteristics of pharmaceutical excipients. CONCLUSION: Chinese Pharmacopoeia should be based on the actual level of China's pharmaceutical industry, follow the principles of progressive and sustainable development, introducing new drug testing technologies and methods to strengthen the quality control of drugs and improve drug controllability.
6. Anti-tumor constituents from the roots of Actinidia valvata

Objective: To study the anti-tumor constituents from the roots of Actinidia valvata Dunn. Methods: Column chromatographic techniques were used to isolate and purify the chemical constituents of the plant and their structures were elucidated by spectroscopic analysis, including 1HNMR, 13CNMR and MS. The cytotoxic activities of some individual compounds were examined by MTT assay. Results: Ten triterpenoids: 2α, 3α, 24-trihydroxyurs-12-en-28-oic acid(1), asiatic acid(2), corosolic acid(3), 2α, 3α, 23, 24-tetrahydroxyurs-12-en-28-oic acid(4), 2α, 3α, 13β, 24-tetrahydroxyurs-11-en-28-oic acid-13-lactone(5), 2α, 3α, 24-trihydroxyolean-12-en-28-oic acid(6), 2α, 3α, 19α, 24-tetrahydroxyurs-12-en-28-oic acid(7), 2α, 3α, 24-trihydroxyurs-12, 20(30)-dien-28-oic acid(8), 2α, 3β, 24-trihydroxyurs-12-en-28-oic acid(9), and oleanolic acid (10), and one plant sterol β-citosterol(11) were isolated from the CHCl3 fraction of the roots of Actinidia valvata Dunn. The cytotoxic activities of five compounds (1, 2, 3, 4, and 7) against A549, LOVO and HepG2 cell lines were evaluated. Conclusion: Compounds 3-8 are isolated from this plant for the first time. Compounds 2 and 3 possess cytotoxic activity against LOVO and HepG2 cell lines, and the cytotoxic activity decreases with the increase of polarity of individual compound.
7.Analysis of hyponatremia incidence in patients with ruptured intracranial aneurysms

Objective: To investigate the hyponatremia incidence in patients with ruptured intracranial aneurysms and the correlation with Fisher grading and cerebral vasospasm. Methods: Totally 150 patients with ruptured intracranial aneurysms from March 1998 to May 2001 were divided into anterior communicating artery (ACoA) group,posterior communicating artery (PCoA) group and middle carotid artery(MCA) group according to the sites of aneurysms.The patients were treated with GDCs interventionally and the incidences of hyponatremia were analyzed. Results: The incidences of hyponatremia in 3 groups were 48%(32/67),15%(8/52) and 16%(5/31),respectively; The incidences of severe hyponatremia of 3 groups were 30%(20/67),6%(3/52) and 6%(2/31), respectively. In ACoA group, the incidences of hyponatremia in Fisher grade Ⅲ-Ⅳ and Ⅰ-Ⅱ were 67%(20/30) and 32%(12/37) and the incidences with and without vasospasm were 71%(24/34) and 24%(8/33), respectively. Conclusion: The ACoA group has a significantly higher incidence of common and severe hyponatremia than the other groups. In ACoA group,hyponatremia is significantly more often in Fisher grade Ⅲ-Ⅳ cases and those with vasospasm.
8.Determination Optimization of Total Amino Acids in Runing Pills by Orthogonal Design

Objective:To optimize the determination technology for the total amino acids in Runing pills. Methods:The process was optimized by L9 (34 ) orthogonal design using the hydrolysis temperature, hydrolysis time and hydrochloric acid concentration as the evaluation factors,and the content and yield of total amino acids as the evaluation indices. Results: The optimal determination condi-tions were as follows:the hydrolysis temperature was 110℃, the hydrolysis time was 22 h and the hydrochloric acid concentration was 6 mol·L-1 . Conclusion:The optimal determination technology can be applied in the total amino acids from Runing pills,which pro-vides the basis for the further research of Runing pills.
9.Efficacy analysis of S-1 combined with radiotherapy for locally advanced esophageal cancer

Objective To compare the efficacy for aged patients with locally advanced esophageal cancer who accepted 3D-CRT with or without S-1.Methods 62 aged patients with locally advanced esophageal cancer were randomized divided into three groups in Central Hospital of Kaifeng since March,2009.The S-1 combined with radiotherapy group was 26 patients as combined group,the single radiotherapy group was 20 patients and single drug group was 16 patients.S-1 combined group patients accepted 3D-CRT,and the patients were taken S-1 from the first day,the dose was 40 mg/m2 twice a day continually 14 days and then stop 7 days.There were 2 cycles during radiotherapy.The 20 patients accepted 3D-CRT in single radiotherapy group,and the patients in single drug group were taken S-1 only,the dose was 40 mg/m2 twice a day continually 28 days and then stop 14 days within 28 days.Results The response rate was 92.31% in S-1 combined group,with 13 patients CR,11 patients PR,and 1 patient SD.The response rate was 60.00 % in single radiotherapy group,with 4 patients CR,8 patients PR and 6 patients SD.The response rate was 31.25 % in single drug group,with 1 patients CR,4 patients PR and 6 patients SD.The effect of the S-1 combined with radiotherapy group was significant better than the others (P < 0.05).The 1,2 and 3 year survival rates in each group were 76.92 %,57.69 %,42.31%,75.00 %,55.00 %,40.00 % and 68.75 %,50.00 %,25.00 %,which there is no significant difference in the three groups.The adverse reaction of hematologic toxicity in S-1 combined with radiotherapy group was little higher than the others,still there was no significant difference in the incidence rate of radiation esophagitis and radiation pneumonitis between S-1 combined with radiotherapy group and single radiotherapy group.Conclusion S-1 plus three dimensional conformal radiotherapy treatments were reliable,and the adverse reactions were mild and well tolerated for elderly patients with locally advanced esophageal cancer.
10.Role of Head Band for Nosocomial Infection Control in Operating Room

OBJECTIVE To discuss the role of head hygiene for nosocomial infection control in operating room.METHODS The head band was weared to improve head hygiene,and enhance the consciousness in control nosocomial infection.RESULTS From Mar 2004 to Mar 2006 there were 2534 cases operated and 23 cases suffered incision infection(0.91%) when surgeon was without use of head band;but from Mar 2004 to Mar 2006 there were 3014 cases operated and the incision infection rate was 0.40%(12 cases) when surgeon was with use of head band.It was an effective method for reducing sweat-inducing contamination by wearing head band to control nosocomial infection in operating room.CONCLUSIONS To improve the head hygiene by wearing head band is arelatively simple practice,it can control nosocomial infection in operating room.
