1.Role of airway inflammation in the pathogenesis in patient with aspirin-induced asthma

The numbers of eosinophils and mast cells expressing 5-lipoxygenase of airway in aspirin-induced asthma (AIA) increased. LTC 4 synthase, the terminal enzyme for cys-LT production, is overexpressed markedly in bronchial biopsy specimens of most patients with AIA. These characterized the unique airway inflammation of AIA which may illuminat the pathogenesis of AIA to some extent.
2.Advances in clinical diagnosis and treatment of severe acute respiratory syndrome

It has been proved that severe acute respiratory syndrome (SARS) is caused by a novel coronavirus. Detecting disease in its early stage, understanding its pathways of transmission and implementing specific prevention measures for the disease are very important in the course of disease control. China has established the definitions of suspected and confirmed cases of SARS. But the laboratory tests and definitions for the diagnosis are limited.Now the primary measures for SARS include isolation, therapeutics application with ribavirin and corticosteroid, mechanical ventilation, etc. Convalescent plasma is being explored.There still are many problems to be solved in the course of conquering SARS.

4.Construction and screening of cDNA library of alveolar macrophages

Objective To construct cDNA library of alveolar macrophages after lipopolysaccharide (LPS) and dexamethasone (Dex) treatment for exploring the protein molecule interactive with glucocorticoid receptor (GR) in condition of inflammation. Methods After the cultured AMs were treated with LPS (10mg/ml) and Dex (10 -5 mol/L) for 4h, total RNA was extracted from AMs, then the cDNA was synthesized from total RNA of AMs and amplified using primers SMARTⅢ TM and CDSⅢoligo (dT) as the base of recombination. The purified PCR products as well as the linearized plasmid pGADT7-Rec were co-transformed into the competent yeast AH109. They were recombined by yeast homologous recombinase in the yeast cells and became the active cyclic plasmid. The transformed yeasts grew in the SD/-Leu plates. All the growing clones were harvested and then constituted the cDNA library. Furthermore, the bait pGBKT7-rGR transformed the yeast AH109 of library was constructed, and the protein molecule interactive with GR was screened. Results cDNA library of AMs was constructed with high multiplication and good capacity. 1.07?106 recombinants were obtained from the cDNA library. The amplified PCR fragments were between 0.3-1.5kb in size. One true positive clone, obtained by screening the cDNA library, was confirmed to be BAG-1 by sequencing and BLAST. Conclusion The yeast two-hybrid cDNA library of AMs was successfully constructed by Clontech SMART method; GR can interact with BAG-1 in yeast cells.
5.DRGs-based medical performance evaluation of tertiary hospitals in Yunnan

Objective To identify objective methods for medical service performance evaluation,and means to improve the efficiency of medical services.Methods By means of collection of medical records from 10 tertiary hospitals in Yunnan in 2012,and Diagnosis-related groups of Beijing version(BJDRGs)as a risk-adjustment tool,medical service performance was made to such hospitals.Results The capacity,efficiency and medical safety of hospital of the hospitals were subject to a comprehensive evaluation of their medical service capabilities.In terms of discharged inpatients and total weight,hospital A were found to be the highest(77 458 cases,104 227.5 respectively),and hospital I the lowest(25 450 cases,38 162.28)among these hospitals in 2012; compared with other hospitals,DRG groups of hospitals A,b and D were found higher,while the case mix index(CMI)of hospitals e and I were higher.The expenditure index of hospital C is the lowest(0.9359,0.8514); the mortality in cases of low risk of hospitals B,c and D were the lowest,while hospital F was the highest.Conclusion DRGs as a risk adjustment tool,can significantly improve the reliability,science and continuity of medical service performance evaluation,providing reliable basic data for hospitals accreditation in Yunnan.
6.Combined application of a self-expanding neurovascular stent and endovascular coils in the management of wide-necked intracranial aneurysms

Objective To assess the technical feasibility and efficacy of the combined application of a flexible,self-expanding neurovascular stent(Neuroform)and Gugliebni detachable coils(GDC)in the management of wide-necked intracranial aneurysms in humans.Methods Sixty-five wide-necked aneurysms which underwent 65 endovascular procedures were performed by using intracranial stent and GDC.There was a total of 30 aneurysms at basilar artery including 16 at the basilar tip,9 at the basilar trunk and 5 at the beginning of the basilar artery.And there were 30 aneurysms located at the posterior communicating artery, and 5 aneurysms located at the vertebral artery.The Neuroform stents were deployed to cover the neck of aneurysms.Another microcatheter was introduced into the aneurysm sac through the stent interstices and then detachable coils were released to embolize the aneurysms.Results The combined procedures were successful in all of the 65 patients with wide-necked aneurysms.The stent could pass smoothly through the intracranial artery and got released.Complete occlusion was achieved in 60 patients and incomplete occlusion in 5 patients.In-stent thrombosis occurred in 2 patients.All patients recovered well.Forty-two patients had followe-up angiography at 3 to 6 months after the procedure.Among them,no filling was found for the 39 aneurysms which were densely packed,and 3 aneurysms had neck remnant.Conclusion The implantation of Neuroform stent as a complimentary device to GDC coiling is easy and safe for embolization of wide-necked intracranial aneurysms.It has great advantage for treatment of wide-necked intracranial aneurysms.
7.Kinematics Modeling and Analysis of Central-driven Robot for Upper Limb Rehabilitation after Stroke.

The present paper proposed a central-driven structure of upper limb rehabilitation robot in order to reduce the volume of the robotic arm in the structure, and also to reduce the influence of motor noise, radiation and other adverse factors on upper limb dysfunction patient. The forward and inverse kinematics equations have been obtained with using the Denavit-Hartenberg (D-H) parameter method. The motion simulation has been done to obtain the angle-time curve of each joint and the position-time curve of handle under setting rehabilitation path by using Solid Works software. Experimental results showed that the rationality with the central-driven structure design had been verified by the fact that the handle could move under setting rehabilitation path. The effectiveness of kinematics equations had been proved, and the error was less than 3° by comparing the angle-time curves obtained from calculation with those from motion simulation.

9.Aspergillus fumigatus conidia inhibit early apoptosis of alveolar epithelial cells

Objective To observe the impact of Aspergillus fumigatus conidia on the early apoptosis of alveolar epithelial cells.Methods A549 cells was first stimulated with tumor necrosis factor-α(TNF-α)or staurosporine (STS)to induce cell apoptosis,and then co-cultured with Aspergillus fumigatus conidia of three different concentrations (5 × 10 5 ,5 × 10 6 ,5 × 10 7 )simultaneously.The changes of the apoptosis rate of A549 cells were detected by flow cytometry.Results The early apoptosis of A549 cells was increased markedly after stimulated with TNF-α ([14.94 ± 0.82 ]% vs [3.26 ± 0.74]%,χ2 =9.18,P <0.05)and STS ([18.10±1 .00]% vs [3.26±0.74]%,χ2 =12.32,P <0.05). When Aspergillus fumigatus conidia of different concentrations were added,the apoptosis rate was decreased (χ2 =31 .23,P <0.05 ).The early apoptosis of A549 cells was negatively correlated with the concentration of Aspergillus fumigatus conidia (rTNF = - 0.92,r STS = - 0.94,both P < 0.05 ). Conclusion Aspergillus fumigatus conidia inhibits the early apoptosis of A549 cells induced by TNF-αor STS and the inhibit effect increases with the concentration of Aspergillus fumigatus conidia.
10.Analysis of risk factors for stroke in elderly patients

Objective To analyze the relative factors for the prognosis in elderly patients with stroke,and provide theoretical basis for improving prognosis.Methods 188 elderly patients with stroke were treated in our department from Jan.2011 to Nov.2013,and clinical data was analyzed.The prognosis and relative risk factors for stroke were statistically analyzed.Results 99 cases had good prognosis,89 cases had poor prognosis,and the recovery rate were 52.7% (99/188) and 47.3% (89/188) respectively.The age was older in group of good prognosis than in group of poor proghosis (t=1.7930,P<0.05).Univariate analysis showed that the age,position of cerebral infarction,size of lesions,hypertension,diabetes,drinking and smoking were associated with the prognosis of stroke (t=1.7930,x2 =8.6473,15.046,13.1334,4.1536,5.7004,5.9043,respectively,all P<0.05),while the prognosis was not associated with gender,a family history of stroke,hyperlipidemia and heart disease.Multivariate logistic regression analyses revealed the prognosis of stroke was related with risk factors of age (OR=1.072),size of the lesion (OR=3.481),hypertension (OR=2.034),diabetes (OR=1.631) and smoking (OR=3.144).Conclusions It is beneficial to the prognosis of patients with stroke to stop bad habits and to treat hypertension and diabetes actively.
