Chinese Journal of Experimental Ophthalmology 2016;34(3):248-253
Background Phacoemulsification with IOL implantation can reduce the light scattering caused by cataract, but many patients with IOLs implantation complained of glare or blurred vision.IOLs from different materials will affect IOL refractive index (RI) and optical design, which affect the value of scattered light of IOLs implantation. Objective This study was to analyze the effect of IOL materials on optical quality in pseudophakic eyes with optical quality analysis system (OQAS).Methods The clinical data of 95 eyes from 65 patients with age-related cataract who received phacoemulcification with IOL implantation in Tianjin Eye Hospital from March to Octomber were retropectively analyzed.Three types of lOL,including AR40e,Acrysof SA60 AT and Rayner 620H,were implanted in 32 eyes,30 eyes and 33 eyes, respectively. The visual quality indexes were detected and evaluated using OQAS,including uncorrected distance visual acuity (UCDVA,LogMAR),objective scatter index (OSI),modulation transfer function cut off (MTF cut off),strehl ratio (SR),Profile width at 50% point scatter function (PSF),Profile width at 10% PSF and contrast visual acuity under the 100%,20%,9% contrasts. Results The UCDVA was ≥ 0.3 in the eyes of AR40e group,SA60AT group and 620H group 1 month after surgery. No significant difference was found in the best corrected distance visual acuity (BCDVA,LogMAR) among three groups (F =1.362,P =0.246).The OSI was 1.72±1.11,1.89±1.07 and 2.13±1.13 in the AR40e group,SA60AT group and 620H group, with a significant difference among the groups (F =3.638,P =0.045),and the OSI was significantly lower in the AR40e group than that in the SA60AT group or 620H group (both at P<0.05).No considerable difference in MTF cut off among the groups (F =2.401,P =0.192).The SR value was significantly elevated and the PSF at both 50% and 10% was declined in the AR40e group compared with the SA60AT group and 620H group (all at P<0.05).A significant difference was found in 9% contrast visual acuity among the three groups (F=3.866,P=0.038),and 9% contrast visual acuity was better in the AR40e group than that in the SA60At group or the 620H group (both at P< 0.05).Negative correlations were found betwee OSI and BCDVA in the AR40e group,SA60AT group and 620H group (r=-0.275,P=0.041;r=-0.287,P=0.042;r=-0.233,P=0.037).Conclusions OQAS can evaluate optical quality of cataract patients after opeartion objectively.Material and RI of IOL have a great effect on visual quality after phacoemulcification with IOL implantation, especially under the low contrast environment. The operated eyes implanted hydrophobic acrylic IOLs with a lower RI can get better optical quality in comparison with the eyes implanted hydrophilic IOLs.

BACKGROUND:The micro-ecological environment has been broken when the ocular prosthesis was inset into the conjunctival sac. The recede of self cleaning function is more conducive to the microbial growth and colonization. The cleaning of plaque biofilm on ocular prosthesis surface affects the patient's wearing comfort and quality of life. It is necessary to seek an effective cleaning method. OBJECTIVE:To compare the clearance effect of five cleaning methods on the palque biofilm on ocular prosthesis surface. METHODS: The conjunctival secretions from 84 patients who were subjected to ocular prosthesis repair were taken for bacterial culture and identification. Fifty pieces of self-curing resin and thermosetting resin artificial eyes were produced. The artificial eyes in each group were randomly divided into five groups, and were cleaned respectively with clear water, volume fraction of 75% ethanol, Boston SIMPLUS, polident and toothpaste. After the completion of the cleaning, the test piece was conducted residual biofilm culture. The clearance effects of different processing modes were evaluated using colony counting method. RESULTS AND CONCLUSION: Eighty-four specimens were submitted for inspection, of which 49 were positive. The Staphylococcusaureus separation rate was 14.29%.Staphylococcus epidermidis separation rate accounted for 13.10%. Maxwel Corynebacterium separation rate accounted for 7.14%. When water, Boston SIMPLUS and toothpaste were used for cleaning, theStaphylococcus aureus colony number in the self-curing resin group was higher than that in the thermosetting resin group (P< 0.05); when ethanol and polident were used for cleaning, there was no difference in the Staphylococcus aureus colony number between these two groups. In self-curing resin, the colony count in the clear water treatment group was higher than that in the other treatment groups (P < 0.05). The colony count in the ethanol treatment group was lower than that in the Boston SIMPLUS group (P < 0.05). There was no significant difference in the colony count between other groups. In thermosetting resin, the colony count in the clear water treatment group was higher than that in the other treatment groups (P < 0.05). There was no significant difference in the colony count between other groups. These results demonstrate that ethanol, Boston SIMPLUS, polident and toothpaste have better cleaning effects onStaphylococcus aureusbiofilms on the surface of two kinds of ocular prostheses than the clear water rinse. Overal, it is encouraged to clean the artificial eyes using polident and Boston SIMPLUS, in order to avoid the occurrence of microbial infection in the conjunctival sac after wearing ocular prosthesis.

Background The vitreoretinal traction plays a critical role in the formation of macular hole and cystoid macular edema.Enzymatic vitreolysis has potential in relieving vitreoretinal traction as a simple and less invasive method in comparison with pars plane vitrectomy.ObjectiveThis study is to investigate the effects of plasmin mutant with kringle domains deficiency(Plm△K)on vitreoretinal interface in new Zealand white rabbits. Methods Plm△K was prepared through activating plasminogen mutant with Kringle domains deficiency (Plg△K) by tissue plasminogen activator (tPA).100μL of Plm△K at the dose of 0.5,1.0 and 1.5μmol/min was injected respectively into the vitreous of 48 New Zealand white rabbits and 16 eyes for each dose.B-scan and optical coherence tomography (OCT) were performed to detect the structure variety at the vitreoretinal interface in 1 day and 7 days after injection. The gross anatomy analysis with triamcinolone acetonide fine particle suspension,as well as histopathological examinations by scanning electron microscopy, was performed in the different time points mentioned above. Results Two peptide chains were determined with the relative molecular weight about 26000 and 5000 by the gel imaging analysis of reduced SDS-PAGE.Separation of the posterior vitreous cortex from retina was found after intravitreous injection under the B-scan and OCT.The ultrastructure change of vitreoretinal interface as well as the examination of fine particle suspension by triamcinolone acetonide demonstrated the same outcome. The degree of remnants of vitreous cortex showed the negative correlation with the dosage of Plm△K (r=-0.9516,P=0.048).No significant correlation was found between the degree of remnants of vitreous cortex and the action time(r=-0.720,P=0.470).There was no obvious morphological difference in outer layer of retina between control eyes and Plm△K-treated eyes.No drug-related adverse event was found after intravitreous injection of Plm△K.Conclusion Intravitreous injection of Plm△K alone can induce complete separation of vitreous from retina. This procedure is safe and effective.

Objective To develop a rapid ultra high performance liquid chromatography-tandem mass spectrometry (UPLC-MS/ MS) for determination of 11 sedative-hypnotics in human plasma. Methods The plasma samples were extracted with dichloromethane:n-hexane:acetoacetate (5∶4∶1). The separation was performed on a Waters ACQUITY UPLC HSS T3 column (2.1 mm×100 mm,1.8 μm) using the mobile phase composed of acetonitrile-0.1% ammonium solution at a flow rate of 0.2 mL?min-1 in gradient elution mode. The detection was performed on a triple quadrupole tandem mass spectrometer by multiple reaction monitoring (MRM) via electrospray ionization (ESI) source in positive mode. Results Good linear relation was obtained over the investigated concentration range, with all correlation coefficients higher than 0.99. The limit of detection was 200 pg?mL-1 for dexzopiclone, and 10- 20 pg?mL-1 for other sedative-hypnotics. The intra- and inter-day RSD of 11 sedative-hypnotics were no more than 15.0%. The recoveries were in the range of 72.3%-108.0%, and the matrix effects were approximately 0.86- 1. 12. Conclusion The method is rapid, sensitive and reliable, and suitable for the simultaneous determination of 11 sedative-hypnotics in human plasma.
