1.Development of hospital infection data sharing platform based on J2EE technique

Objective:To establish hospital infection data sharing and service platform for collecting and issuing the hospital infection data. Methods:The relational database was used to realize data storage and J2EE light structure was employed to enhance the development and the operating efficiency and flexibility of the system according to the principles of Web information system.Results and Conclusion:An open and sharing hospital infection data platform for data processing, retrieving and analyzing was established. The established hospital infection data set and surgery infection data set contained more than 20 000 records. The system provided an effective software platform support for the utilization of hospital infection data and the prevention and control of hospital infection. This article introduces key technologies of the design and realization of the system.
2.The diagnosis value of TI-RADS and sonographic characteristics for papillary thyroid carcinoma and follicular thyroid carcinoma

Objective To investigate the value of TI-RADS for sonographic diagnosis of papillary thyroid carcinoma (PTC) and follicular thyroid carcinoma (FTC) and to evaluate the sonographic findings of FTC compared to PTC.Methods The clinical and sonographic data of 363 cases of pathology confirmed thyroid nodules were retrospectively analyzed.The thyroid nodules were classified by TI-RADS grading criteria,the accuracy of TI-RADS and sonographic findings between FTC and PTC was analyzed.Results In 363 thyroid nodules,114 cases were confirmed as benign lesions;249 cases were confirmed as carcinomas,including 50 FTC cases and 199 PTC cases.The sensitivity,specificity,PPV,NPV,and accuracy of PTC were 97.0% (193/199),85.1% (97/114),91.9% (193/210),85.8% (97/113),95.7% (290/303) respectively.The sensitivity,specificity,PPV,NPV,and accuracy of FTC were 97.0%(193/199),85.1% (97/114),91.9% (193/210),85.8% (97/113),95.7% (290/303) respectively.There were significant differences of nodule size,shape,margin,echogenecity,calcification,halo and flow between FTC group and PTC group (P < 0.05).Conclusion TI-RADS has higher value in the sonographic diagnosis of PTC than that of FTC.FTC are greatly different from PTC in many sonographic findings making it less valuable for the diagnosis of FTC.
3.Outcome and safety analysis of colonoscopy in 1 249 patients aged 80 years and over

Objective To investigate the value and safety of colonoscopy in patients aged 80 years and over.Methods Clinical and endoscopic data of 1 249 patients aged 80 years and over collected from December 2005 to December 2015 at PLA army General Hospital were analyzed retrospectively.Results The average age was 83.03 years.Reasons for receiving colonoscopy included constipation with abdominal distension(19.38 %)and hematochezia(10.57%).The completion rate of colonoscopy for the entire length was 94.50 %.There were no abnormal findings in 492 cases (39.39 %).Colonic polyps (31.62 %) and colorectal cancer (14.25 %) were among the major lesions detected with colonoscopy.Colorectal neoplasms were found in 58.33% of the 132 patients with hematochezia.The total complication rate from colonoscopy and treatment was 0.72%,with the complication rate from treatment at 1.32%.Conclusions Constipation with abdominal distension and hematoehezia are the main reasons for undergoing colonoscopy for very elderly patients.Colon polyps and colorectal cancer are common disorders in elderly patients over 80 years of age and people with hematochezia in this age group are at high risk of having colorectal cancer.Colonoscopy is a safe and effective procedure and an important examination method for very elderly patients with hematochezia.
4.Biocompatibility in vitro between fascia fibroblasts and fibrin glue

BACKGROUND:Fibrin glue is a natural biodegradable scaffold, which can be used for tissue-engineered scaffolds, and is increasingly used as seed cel carrier for tissue engineering repair. OBJECTIVE:To investigate the biocompatibility in vitro of rabbit fascia fibroblasts and fibrin glue. METHODS:Tissue explants adherent method was used to culture fibroblasts from subcutaneous deep fascia tissue of New Zealand white rabbits. The fibroblasts could be passaged with trypsin digestion method. Suspension of passage four fibroblasts was co-cultured with fibrin glue. Morphology and proliferation of fibroblasts on the surface of fibrin glue were dynamical y observed under the inverted phase contrast microscope. At 5 days after co-culture, fibroblasts were identified by immunofluorescence staining under the laser scanning confocal microscope. The fibroblast growth and adhesion were observed under the scanning electron microscope. RESULTS AND CONCLUSION:There was no significant difference in fibroblast morphology between co-culture fibroblasts and pure culture fibroblasts with inverted phase contrast microscope. Scanning electron microscope demonstrated that fibroblasts ful y extended in fibrin glue surface, and showed a good adhesion between the“pseudopodium”and fibrin glue, and secreted matrix material. It is clear that the fibrin glue did not alter the morphologic features of fibroblasts. Laser scanning confocal microscope revealed that fibroblasts were positive for vimentin. These verified that properties of fibroblasts did not change after they were seeded in fibrin glue surface and did not be induced to differentiate. There is a very good biocompatibility between fascia fibroblasts and fibrin glue in vitro.
5.Effects of platelet-rich plasma (PRP) on the periodontal healing of replanted teeth

Objective:To evaluate the influence of platelet-rich plasma(PRP) on periodontal healing of replanted teeth in 6 Beagle dogs.Methods:The second incisors were pulled out and re-implanted after 60 min dry storage.In group A,PRP were applied before replantation,and 7days later the teeth received root canal treatment.In group B,PRP were applied before replantation either,but without root canal treatment.In group C,PRP were not applied and 7 days later,the teeth received root canal treatment.In group D,no PRP were applied and no root canal treatment.All the dogs were sacrificed after 12 weeks and the histological parameters were observed.Results:There were no significant differences between group A and C.Group B showed a lower incidence of ankylosis,and higher incidence of healed PDL compared with group D.Conclusion:Under the definite condition,PRP can help to decrease the incidence of ankylosis and increase the incidence of healed PDL in groups that the pulps were not removed.
6.Perioperative treatment and protection in human immunodeficiency virus infected patients with osteoporotic vertebral compression fracture

Objective To investigate the effect of perioperative treatment and protection in human immunodeficiency virus (HIV) infected patients with osteoporotic vertebral compression fracture by percutaneous kyphoplasty (PKP).Methods From February 2010 to December 2015,206 cases with osteoporotic vertebral compression fracture treated by PKP were admitted,including 13 HIV infected patients.Blood routine,biochemical indexes,liver and kidney function,blood coagulation function,immune index and preoperative nutritional status were detected before operation.Highly active antiretroviral therapy (HAART),reasonable antibiotics,nutritional and immune support were given to the HIV infected patients.Standardized operation process and meticulous operative manipulation were applied during operation.The prevention of occupational exposure protection standardization process was strictly enforced.The postoperative complications,improvement of immune function and nutritional status,vertebral height,VAS,ODI and exposure occupation of medical personnel were observed.Results All 13 HIV infected patients had successful operations,without bone cement leakage and toxic reaction.One patient had perficial infection and the others had operative incisionprimary healing.There was no death.The levels of vertebral height,VAS scores and ODI scores were significantly improved after intervention and at the end of follow-up (P < 0.05),but there were no significant differences between after the intervention and at the end of follow-up (P > 0.05).Eleven malnutrition patients with HIV infected were received nutritional and immune support,and the levels of CD4 +,ALB and Hb were significantly improved:(289.00 ± 54.29) × 106./L vs.(237.25 ± 72.70) × 106/L,(38.04 ± 5.17) g/L vs.(33.73 ± 7.16) g/L,(112.87 ± 15.54) g/L vs.(100.68 ± 18.95) g/L,P < 0.05.The levels of PLT and WBC had no significantly change (P > 0.05).Two medical personnel had occupational exposure,and none had HIV infection.Conclusions Perioperative adjuvant treatment in HIV infected patients with osteoporotic vertebral compression fracture can achieve satisfactory outcomes.Strict implementation of protective standardization process should be focused.
7.Perioperative treatment of patients with human munodeficiency virus undergoing spinal surgery

Objective To observe the effect of perioperative treatment on complications in human immunodeficiency virus positive patients undergoing spinal surgery, and summarize effective perioperative management to make these patients to get better treatment. Methods Forty-eight HIV positive patients (experimental group) and 79 HIV negative patients (control group) undergoing spinal surgery from November 2010 to March 2016 were retrospectively reviewed. The two groups were comparable in age, gender, type of spinal surgery and basic diseases. Nutritional support and the uses of antibiotics and blood transfusion were strengthened in patients of experimental group. Immunomodulatory drugs, the treatment with HAART and occupational protection were also used in patients of experimental group. The patients in control group only received conventional perioperative treatment. Wound healing, opportunistic infection, the failure of internal fixation and death after the operation was observed. The perioperative treatment methods were summarized. Chi square test was used in comparison of the rate and P < 0.05 was considered statistically significant. Results Surgery was successfully completed in all patients in experimental group and control group. Two cases (4.2%) in experimental group and 4 cases(5.1%) in control group showed delayed healing of incisions. All incisions were healed after debridement and no incision infection happened. CD4+T lymphocyte count of 2 cases in the experimental group was in stage 3. Among them, 1 patient underwent emergency surgery and opportunistic infection appeared after the operation. The other patients for elective surgery took operation when CD4+T lymphocyte counts were adjusted to 2 and no complications appeared. The two groups had no internal fixation failure cases and deaths. Conclusions HIV positive patients with spinal diseases are special patients. HIV positive patients with spinal surgery can reduce complications, and achieve good clinical curative effect by reasonable preoperative assessment, optimization of perioperative treatment, proper type of operation and active prevention of opportunistic infections after surgery.
8.Bailing Capsule in Preventing Epithelial- Mesenchymal Transition in Rats with Tubulointerstitial Fibrosis

Objective To explore the effect of Bailing capsule on epithelial-mesenchymal transition( EMT) in rats with adenine-in-duced tubulointerstitial fibrosis. Methods Tubulointerstitial fibrosis animal models were established and SD rats were divided into mo-del group ( n = 30), treatment group ( n = 30) and control group( n = 30), randomly. Experimental rats were harvested at 7 w, 12 w,17 w after onset of experiment and functional evaluations were performed. Histology, immunohistology were examined to investigateboth histolopathology changes and the expression of bone morphogenic protein-7 (BMP-7), transforming growth factor-β1 (TGF-β1 )and a-smooth muscle actin (α-SMA) in kidneys at three time points mentioned above, respectively. Results Compared with controlgroup, 24 h urinary protein in model group lost increasingly and significantly difference appeared at three time points relative to controlgroup ( P < 0.01 ). Urinary NAG in model group was markedly higher than that in control group from 7 w after onset (P < 0.01 ) andwas increasingly raised at 12 w and 17 w (P<0.01). The value of blood BUN and Cr in model group increased at 7 w (P>0.05) rel-ative to control group. There was significant difference at 12 w and 17.w (P < 0.01 ). Histologically, kidneys in model group, at 7 w,exhibited tubular casts and gently tubular dilation, granuloma in cortex, mononuclear cells infiltration in tubulointerstitial areas, andmild interstitial fibrosis. At 12 w, the degree of tubular injury and tubulointerstitial fibrosis gradually aggravated. Up to 17 w, diffusetubular dilation or atrophy was observed and focal tubules disappear. Diffuse interstitial fibrosis was exhibited. In normal kidneys, im-munohistochemistry suggested that the light expression of BMP-7 was detected in proximal renal tubular epithelial cells and marked ex-pression was identified in distal tubule, collecting duct, and renal tubular epithelial in junction area between cortex and medulla. How-ever, the expression of BMP-7 in kidneys of model group significantly decreased with increasing tubulointerstitial fibrosis and was nega-tive correlation with the expression of TGF-β1(r = -0. 981 P<0.01) and α-SMA (r= -0.975 P<0.01). Bailing capsule ad-ministration protected the expression of BMP-7 and reduced TGF-β1 and α-SMA expression before 12 w(P< 0.01 ). Conclusions Ourstudy shows an anti-fibrotic reno-protective function of Bailing capsule in rats with tubulointerstitial fibrosis via prevention of epithelial-mesenchymal transition at early stage. However, the beneficial effect lost with increasing tubulointerstitial fibrosis.

10.Antioxidating and energy metabolism improving effects of Qiangjing Decoction on oligospermia and asthenospermia: An experimental study.

OBJECTIVETo explore the mechanisms of Qianjing Decoction in the treatment of oligoasthenospermia (OAS).
METHODSWe randomly divided 100 SPF male rats into five groups of equal number: normal, model, Huangjingzanyu, levocarnitine, and Qiangjing. OAS models were established in the animals followed by intragastrical administration of normal saline, ornidazole, Huangjingzanyu Capsules (200 mg per kg body weight per day), levocarnitine (100 mg per kg body weight per day), and Qianjing Decoction (10 g per kg body weight per day), respectively, qd, for 4 successive weeks. Then, we detected the concentration and motility of the epididymal sperm, obtained the contents of superoxide dismutase (SOD), malonaldehyde (MDA), glutathione peroxidase (GSH-Px), lactate dehydrogenase (LDH), α-glucosidase, and fructose in the epididymis, and determined the mRNA expressions of nuclear factor erythroid 2-related factor 2 (Nrf2) and succinate dehydrogenase (SDH) in the epididymal tissue of the rats by real-time PCR.
RESULTSThe concentration and motility of the epididymal sperm in the model, Huangjingzanyu, levocarnitine, and Qianging groups were (35.34 ± 4.22) x 10(6)/ml and (40.04 ± 7.05)%, (48.12 ± 5.56) x 10(6)/ml and (62.46 ± 7.12)%, (47.14 ± 4.87) x 10(6)/ml and (63.23 ± 6.34)%, and (50.25 ± 5.08) x 10(6)/ml and (66.34 ± 7.58)%, respectively, all significantly lower than in the normal group ([53.05 ± 4.55] x 10(6)/ml and [70.20 ± 8.54]%) (P < 0.05), but remarkably higher in the Huangjingzanyu, levocarnitine, and Qiangjing groups than in the model rats (P < 0.05). Compared with the thinned epididymal lumen walls, decreased sperm count, and disorderly and loose arrangement of the lumens in the OAS models, the rats in the Huangjingzanyu, levocarnitine, and Qiangjing groups showed evidently thicker epididymal lumen walls, with the lumens full of sperm cells and arranged regularly and compactly, similar to those of the normal rats. The levels of SOD and GSH-Px were significantly lower but that of MDA markedly higher in the model rats ([84.12 ± 23.25], [10.56 ± 3.02], and [14.04 ± 2.06] nmol/mg) than in the normal group ([110.04 ± 19.56], [17.25 ± 3.56], and [8.87 ± 1.35] nmol/mg) (P < 0.05), while the former two indexes remarkably higher and the latter one significantly lower in the animals treated with Qiangjing Decoction ([120.56 ± 23.68], [16.34 ± 3.12], and [8.45 ± 1.56] nmol/mg), Huangjingzanyu Capsules ([115.34 ± 21.35], [15.23 ± 3.67], and [8.33 ± 1.54] nmol/mg), and levocarnitine ([116.67 ± 22.67], [15.35 ± 3.45], and [8.05 ± 1.78] nmol/mg) than in the models (P < 0.05). The levels of fructose, LDH and α-glucosidase were decreased markedly in the OAS models ([100.22 ± 12.12] mg/[ ml x g], [322 ± 46.13] U/[ ml x g], and [10.48 ± 2.33] U/[ml x g]) as compared with the normal rats ([128.12 ± 13.45] mg/[ml x g], [428 ± 35.12] U/[ml x g], and [15.34 ± 3.12] U/[ ml x g]) (P < 0.05), remarkably higher in the rats treated with Qiangjing ([130.23 ± 13.67] mg/[ml x g] [455 ± 51.50] U/[ml x g], and [18.56 ± 4.67] U/[ml x g]), Huangjingzanyu ([124.16 ± 14.02] mg/[ml x g], [ 419 ± 43.14] U/[ml x g], and [17.64 ± 4.08] U/[ml x g]), and levocarnitine ([123.34 ± 14.02] mg/[ml x g], [430 ± 31.80] U/ [ml x g], and [16.85 ± 5.55] U/[ml x g]) than in the models (P < 0.05). The Nrf2 mRNA expression was significantly reduced in the models as compared with the normal rats (P < 0.05) but remarkably increased in the Huangingzanyu, Qiangjing and levocarnitine groups as compared with the model and normal animals (P < 0.05). The SDH mRNA expression was significantly lower in the model than in the normal rats (P < 0.05) but markedly elevated in the Huangjingzanyu, Qiangjing and levocarnitine groups as compared with the model and normal animals (P < 0.05), remarkably higher in the Qiangjing than in the Huangjingzanyu group (P < 0.05).
CONCLUSIONOrnidazole induces OAS in rats, which is closely associated with excessive oxidation and energy metabolism dysfunction. Qiangjing Decoction can improve and even reverse ornidazole-induced OAS in rats as well as improve the ultrastructure of their testicular and epididymal tissues. Antioxidation and improvement of energy metabolism are probably the action mechanisms of Qiangjing Decoction in the treatment of OAS.
