1.Effects of delivery mode on postpartum fecal incontinence and urinary incontinence in Chinese women

Objective To investigate the prevalence and associated factors of fecal incontinence (FI) and urinary incontinence (UI) in postpartum Chinese women.Methods Questionnaires about FI and UI symptoms were completed via telephone interviews conducted within six months after delivery.Multi-variant Logistic analysis was applied for relation between delivery mode and FI or UI.Results (1) Two thousand and twelve postpartum women were admitted into this study,among which 14 (0.7%) had FI within 6 months after delivery.Logistic regression analysis showed that FI was significantly associated with forceps delivery (OR =20.09,95 % CI:3.64-110.90,P =0.000),and mediolateral episiotomy (OR=6.11,95% CI:1.29-28.80,P=0.024).(2) Among the 2012 women,the prevalence of UI,stress urinary incontinence (SUI),urgent urinary incontinence (UUI)and mixed urinary incontinence (MUD was 10.04% (n=202),8.15% (n=164),0.94% (n=19)and 0.94 % (n =19),respectively.Logistic regression analysis found that SUI prevalence was related to maternal age (OR =1.07,95% CI:1.04-1.11,P =0.000),maternal weight before delivery (OR=1.04,95% CI:1.02-1.06,P=0.001),neonatal head circumference (OR=1.20,95% CI:1.05-1.39,P =0.010),mediolateral episiotomy (OR =4.96,95 % CI:3.05-8.07,P =0.0005 ),spontaneous vaginal delivery (OR=5.22,95% CI:2.53-10.76,P=0.000) and forceps delivery (OR=9.20,95% CI:4.07-20.79,P=0.000).UUI was related to maternal weight before delivery (OR=1.51,95% CI:1.12-2.05,P=0.008).MUI was related to maternal weight before delivery (OR=1.06,95% CI:1.00-1.11,P=0.049),duration of second stage of labor (OR=1.01,95% CI:1.00-1.03,P =0.010),mediolateral episiotomy (OR =7.76,95% CI:1.42-42.52,P=0.017) and forceps delivery (OR=15.21,95% CI:1.61-143.44,P=0.018).(3) The prevalence of SUI was higher at 4 days and 42 days after delivery (7.95% and 9.10%).Conclusions (1) F1 and UI prevalence is lower in this study than in other reports.(2) Vaginal delivery is a risk factor for women's FI and UI,especially forceps delivery and mediolateral episiotomy.(3) Maternal age,pre-delivery weight,newborn head circumference,spontaneous vaginal delivery,forceps delivery and mediolateral episiotomy might increase the risk of UI.
2.Protective effect of dorsal longitudinal myelotomy at 72 Hours after spinal cord injury in rat model

Incision of the spinal cord (myelotomy) after spinal cord injury (SCI) is a surgical treatment which
is mainly performed within 24 hours after SCI. However, many patients are fi rst seen outside the
established time window of 24 hours. Furthermore, little attention has been given to its effi cacy and
mechanism after 24 hours. In this study, dorsal longitudinal incision of spinal cord on SCI rats was
performed at 72 hours after SCI to remove the liquefi ed necrotic gray matter. The results indicated
that after myelotomy, locomotor function of hindlimbs signifi cantly improved from 21st day after SCI.
There was also increased residual white matter area at the rostral and caudal segments to the injury
center when examined at day 42 after the injury. These results suggested that myelotomy at 72 hours
after injury has protective effect on the spinal cord.
3.Functional Analysis of a Gene Encoding a Chlorotoxin-like Peptide Derived from Scorpion Toxin

A full-length cDNA sequence encoding for the precursor of a venom peptide (named BmKCT) with homology to chlorotoxin has been isolated from a cDNA library made from the venom glands of the Chinese Scorpion Buthus martensii Karsch. The sequence of BmKCT is similar (68 % identities) to that of chlorotoxin isolated from Leiurus quinquestriatus quinquestriatus. To understand the biological function of BmKCT, this peptide was expressed using pGEX expression system and purified using GST affinity column and gel filtration.Whole cell patch-clamping recording showed that BmKCT could significantly inhibit chloride currents of gliomas cells, and the inhibitory effect was reversible. These results suggested that BmKCT might belong to the class of short chain toxins blocking the chloride ion channels.
4.Nutritional risk and nutritional support in gastrointestinal cancer surgical patients

ObjectiveTo investigate the prevalence of nutritional risk and perioperative nutritional support status in the gastrointestinal cancer surgical patients.Methods Adult inpatients from the gastrointestinal cancer surgery department of our hospital were consecutively enrolled from May to September 2010.Nutritional Risk Screening 2002 ( NRS 2002) was performed at admission,and data of the nutritional support and complications during hospitalization were collected.The relationship between nutritional risk and postoperative complications was analyzed.ResultsA total of 195 patients were eligible.At admission,43.6% (85/195) patients were at riskOf the patients at risk,totally 11.7% (10/85) patients received preoperative nutritional support,and 100% (85/85) patients received postoperative nutritional support; of the patients not at risk,no patients received preoperative nutritional support,and 84.5% (93/110) patients received postoperative nutritional support.The overall rates of postoperative complications were 19.1% ( 13/68 ) in the patients at risk while 7.1% (9/127 )in the patients not at risk ( P =0.02).ConclusionsNutritional risk in gastrointestinal cancer surgical patients was high.The application of nutritional support was inappropriate in these patients.The nutritional risk is associated with higher postoperative complications.Dissemination of evidence-based guidelines should be enhanced.
5.Clinical analysis of 25 cases of autologous urological neoplasms in renal transplant recipients

Objective To summarize the clinical features,diagnosis and treatment of autologous urologic neoplasms in renal transplant recipients.Methods A retrospective analysis on the clinical data of 25 renal transplant recipients was done in our center.The onset time of new neoplasms was between 29 to 72 months after transplantation,with an average of 48.2 months.Intermittent hematuria was the first symptom in 23 patients,and the rest two cases were diagnosed through routine examination. The pathological diagnoses of thee cases were renal carcinoma,which were treated by transperitoneal laparoscopic radical nephrectomy.Eight cases were diagnosed as having renal pelvic tumor,which was treated by radical resection for the renal pelvic carcinoma.Fourteen cases were diagnosed as having bladder cancer,which was treated by transurethral resection of bladder tumor (13 cases) or radical cystectomy (one case).All patients were subjected to surgical treatment.The dosage of MMF,CSA/Tacrolimus was decreased to 1/2-2/3 of their original dosage. Sirolimus was used in place of calcineurin inhibitors in four patients.Immunosuppressive regimes and adjuvant therapy were given after surgery treatment.Results Twenty-five patients were followed up for 12-84 months.Contralateral renal carcinoma combined with lung and chest multiple metastases occurred in one case after radical nephrectomy,who died after targeted therapy 6 months later.Two patients with lymph node metastasis died 14 months and 20 months after surgery respectively.The rest 22 patients were closely followed up,whose creatinine remained 98-163μmol/L.Conclusion More attention should be paid to patients with hematuria after renal transplantation to screen the autologous urinary neoplasms.Patients should be treated with surgical procedures,and immunosuppressive regimens should be adjusted postoperatively.
6.Preparation of herpetin lyophilized liposome and evaluation on its safety and pharmacodynamics.

In this study, the herpetin (HPT) lyophilized liposome was prepared, and its saftey and pharmacodynamics were evaluated. HPT lyophilized liposome was prepared by thin-film ultrasonication method. The lyoprotectant was optimized using particle size and encapsulation efficiency as indexes. Then, the influencing factors of HPT lyophilized liposome were investigated. In addition, preliminary safety and therapy efficiency of HPT lyophilized liposome to liver injury induced by CCl4 in the mice. The optimal lyoprotectant was 5% sucrose plus 5% lactose and the dispersed HPT lyophilized liposomes were spherical with the mean diameter of (107.0 ± 1.2) nm and the mean encapsulation efficiency of (99.7 ± 0.50)%. The lyophilized powder was sensitive to temperature, humidity and illumination. None of hemolysis, hemagglutination and vein irritation was observed after intravenous injection of HPT lyophilized liposomes into rabbits. HPT lyophilized liposome showed obviously therapy efficiency to liver injury induced by CCl4 in the mice. The improvements of ALT, AST and ALP were better than that in HPT free drug. The obtained HPT lyophilized liposome met the standard of CP with fine particle size and encapsulation efficiency after dispersion. The HPT lyophilized liposome showed good safety and enhanced the treatment efficacy of HPT. The HPT lyophilized liposome should be stored in low temperature, sealed condition far away from light.

7.Advances on pharmacokinetics of traditional Chinese medicine under disease states.

In recent years, more and more research shows that the pharmacokinetic parameter of traditional Chinese medicine can be affected by the disease states. It's possible that drug metabolic enzymes, transporters, cell membrane permeability and the change of microbes group could be interfered with physiological and pathological changes, which enables the pharmacokinetics of traditional Chinese medicine in the body to be altered, including the process of absorption, distribution, metabolism and excretion, and then the pharmacokinetic parameters of traditional chinese medicine are altered. It's found that investigating the pharmacokinetic of traditional Chinese medicine in the pathological state is more useful than that of in normal state because the great part of traditional Chinese medicine is mainly used to treat disease. This article reflects the latest research on the pharmacokinetic of traditional Chinese medicine in the disease state such as diabete, cerebral ischemia, liver injury, inflammatory disease, nervous system disorders and fever in order to provide certain reference for clinicians designing reasonable administration dose.

8.Clinical observations of proliferative diabetic retinopathy with chronic myeloid leukemia of five cases

Objective:To observe and analyze the clinical features and prognosis of proliferative diabetic retinopathy (PDR) with chronic myeloid leukemia.Methods:A retrospective case series study. From May 2011 to December 2020, 5 patients (10 eyes) were included in this study in Eye-ENT Hospital of Fudan University. Basic information about the patient's age, gender, diabetes history and CML history were collected. The endocrine and hematological indexes of all patients were evaluated. All the patients were undertaken visual acuity, intraocular pressure, slit lamp and fundus examination and other examinations to observe the eye conditions. Ophthalmic treatments included panretinal laser photocoagulation, intravitreal injection of anti-vascular endothelial growth factor, vitrectomy. During the follow up period from 5 months to 6 years, prognosis was observed at each office visit. During the follow up period, patients' vision, intraocular pressure, anterior segment and retinal status were observed.Results:There were 4 males and a female in 5 patients. The ages were from 27 to 49 years, with the mean age of 39 years. All patients were bilateral. All patients suffered type 2 diabetes for 3 months to 13 years. Four of them were diagnosed as chronic myeloid leukemia before visiting to ophthalmologists, while the other visited to ophthalmology first due to poor vision. The initial visual acuity ranged from light perception to 0.4 and 6 eyes were less than 0.1. In addition to the typical manifestations of diabetic retinopathy, such as venous tortuous dilation, exudation, microaneurysm and neovascularization, patients also presented with Roth spot as leukemic fundus manifestations. All eyes developed to PDR stage. Abnormal thickening of the neovascular membranes may occur in the lower part of the retina, with secondary traction retinal detachment. All the eyes were treated with pan retinal photocoagulation and 9 eyes underwent pars plana vitrectomy. After treatment, retina of 8 eyes kept flat. The best corrected visual acuity ranged from no light perception to 1.0, and only 4 eyes reached more than 0.2. Unfortunately, one eye lost vision because of secondary neovascular glaucoma.Conclusions:PDR patients with CMLof fundus not only have venous tortuous dilation, exudation, microaneurysm and neovascularization, also present with Roth spot as leukemic fundus manifestations. Diabetic retinopathy combined with CML could progress rapidly, and its aggravating complications such as hyperplastic membrane, vitreous hemorrhage and traction retinal detachment may result in poor visual prognosis. Early screening and treatment can help improve the prognosis of patients.
9.Effects of continuously using eyes at near on eyes with different refractive status

?AIM: To investigate the changes of nearwork induced transient myopia ( NITM ) in different refractive status after continuous near tasking.?METHODS:Prospective study. Thirty subjects ( aged 18-24, average 20. 9 ± 2. 1, 12 males and 18 females) were recruited in this study. They were divided into 3 groups according to the subjective refraction: 10 with hyperopia (H), 10 with emmetrope (E) and 10 with myopia (M). All the subjects with soft contact lens watched videos on a panel computer at near distance (33cm ~ 40cm). Five measurements of distance refraction in the right eye were performed by using an infrared optometer before, after 30min and 60min sustained viewing task, and the mean of 5 refractive values was recorded as spherical equivalent. Then distance refraction of right eyes was done every 5s followed by stopping near tasking until NITM was disappeared completely and the decay time of NITM was recorded for each subject. The value of NITM was the difference of refractive values between before and after near tasking. Paired-t test was used to compare the changes of refractive values in the same group. ANOVA was used to determine the differences of NITM and its decaying time among three groups.?RESULTS: Compared with pre - task, no significant refractive changes were found in hyperopic group ( t =1. 627,P= 0. 138 ); While subjects with emmetropia and myopia showed more myopic shifts at the two time points (tE = 2. 699, PE = 0. 024;tM = 4. 930, PM = 0. 001 ). With continuous viewing until the 30th min and 60th min, significant differences of averaged NITM were found between myopic group and other 2 groups (P<0. 05), but no difference was found between hyperopic group and emmetropic group (P>0. 05). Significant differences of the decay time of NITM can be seen among the three groups after near tasking (F=787. 983,P<0. 001).? CONCLUSION: Subjects with myopia are more susceptible to produce NITM than other 2 groups during sustained nearwork for the same time and the decaying time of NITM is longer in myopia group after near tasking, thus it is suggesting that NITM might be attributed to the development and progression of myopia.
10.Relationship between levels of miR-125b and miR-181c in cerebrospinal fluid before joint replacement and postoperative delirium in elderly patients

Objective To evaluate the relationship between the levels of microRNA-125b (miR-125b) and miR-181c in cerebrospinal flnid (CSF) before joint replacement and postoperative delirium (POD) in elderly patients.Methods Fifty-two patients of hoth sexes,aged ≥ 65 yr,weighing 45-70 kg,of American Society of Anesthesiologists physical status Ⅰ-Ⅲ,scheduled for elcctive hip or knee replacement under spinal anesthesia,were included in the study.CSF was collected after successful puncture to measure the levels of miR-181c and miR-125b by fluorescent quantitative real-time polymerase chain reaction.The patients were divided into POD group and non-POD group using the Chinese reversion of Confusion Assessment Method on postoperative days 1 and 2.Results The incidence of POD was about 28% in the patients underwent hip or knee replacement.Compared with non-POD group,the preoperative level of miR-181c in CSF was significantly increased (P<0.05),and no significant change was found in the preoperative level of miR-125b in CSF in POD group (P>0.05).Conclusion The level of miR-181c in CSF before joint replacement is related to POD in elderly patients,and the preoperative level of miR-181c in CSF is a risk factor for POD.
