The cancer stem cell hypothesis posits that cancer stem cells,which are rare among the tumor cell population and share many common properties with normal stem cells,and have been positively identified and successfully isolated from some cancers,are responsible for tumorigenesis and implicated in multistage cancer progression,particularly with respect to metastasis.A better understanding may provide insights into the development of cancer stem cell-targeting therapeutic strategies.The examines what is known regarding cancer stem cells' concept,comparison with normal stem cell,potential origins,assays for isolation and their roles in metastasis.
2.Expression and Clinical Significance of Soluble E?cadherin before and after Operation in Breast Cancer Patients

Objective To detect the expression of serum soluble E?cadherin(sEC)before and after operation in breast cancer patients,and to ex?plore its clinical significance. Methods The levels of sEC in 42 patients with breast cancer and 18 patients with benign breast diseases were tested by enzyme?linked immunosorbent assay(ELISA)before and after operation as well as in 30 healthy controls. Its correlation with clinical and patho?logical characteristics was analyzed. Results The levels of sEC in breast cancer patients before operation were significantly higher than patients with benign breast diseases and controls,which declined significantly after operation. Moreover,the levels of sEC in those with high clinical grading and lymphatic metastasis were also higher than in the ones with low grading and negative lymphatic metastasis(P<0.05). Conclusion The levels of sEC in breast cancer patients before operation are specific and closely related with pathological grade and lymphatic metastasis,which may be a pre?dictive factor for prognosis of breast cancer.
3.The Study on the Activity of Protein Kinase C in Platelets of Maternal Vein and Umbilical Blood in Pregnancy Induced Hypertension Syndrome Patients

Objective To investigate the relationship among the activity of protein kinase C (PKC) in platelets of maternal vein and umbilical blood , the pathophysiological changes of pregnancy induced hypertension (PIH) and fetal growth restriction (FGR) in PIH patients. Methods Activities of PKC in membrane and plasma of platelets from maternal vein and umbilical blood taken from 35 PIH patients and 20 normal pregnant women were measured with substrate phosphorylation method. Results No difference was shown in the PKC activities between the mild PIH patients and normal pregnant women in both maternal and cord blood.The PKC activities in moderate and severe PIH patients were significantly higher than those of the normal pregnant group.In normal pregnant women, the PKC activity in membrane and plasm of the platelets had no significant difference. 4.Study on the changes in pulmonary artery pressure in elderly Chronic kindney disease patients without end-stage renal disease

Objective To prospectively investigate the characteristics and correlative influential factors for the changes in pulmonary artery hypertension (PAH)in chronic kidney disease (CKD) patients without end-stage renal disease.Methods Complete clinical and laboratory data of 133 outpatients without ESRD (CKD stage 2,3 and 4)in Beijing Hospital between January 2012 and January 2013 were retrospectively analyzed.Pulmonary artery systolic pressure(PASP)was assessed by echocardiography according to the guideline from the American Society of Echocardiography in 2010,and PASP more than 35 mmHg(1 mmHg=0.133 kPa)was diagnosed as PAH.Results PAH was found in twelve(9.0 %) patients,including 7 females and 5 males,with mean age(82.9 ± 5.4 years).The prevalence of PAH was 2.3%,10.9% and 17.6% in the patients of CKD stage 2,3 and 4(P< 0.05) respectively.PASP could be calculated in 67 patients in whom tricuspid regurgitation was present.The mean PASP was (27.1 ± 5.4) mmHg,(31.0 ± 8.3) mmHg and (37.3 ± 13.8) mmHg in patients with CKD stage 2,3 and 4,respectively(P < 0.05).Body mass index(BMI),eGFR and BUN showed significant differences between PAH group and non-PAH group with statistical significance (P<0.05).Multivariate logistic regression analysis revealed that the lower BMI and the higher stage of CKD were independent risk factors for PAH (P < 0.05).Conclusions PAH is not a rare complication in the CKD patients without end-stage renal disease.The incidence of PAH is significantly associated with lower BMI and the higher stage of CKD.Doppler echocardiography should be performed periodically to detect PAH in CKD patients in order to improve the prognosis of these patients.
5.Comparison between the intraocular pressure fluctuations measured at home versus in the clinic

AIM: To compare intraocular pressure (IOP) fluctuations measured at home and in the clinic over a 24-hour period.METHODS: A prospective investigational study.A total of 120 Chinese participants were selected from five communities in the Chengdu area.Patients underwent a clinical interview and IOP was measured both at home and in the clinic.IOP were measured at 8 a.m., 10 a.m., 12 a.m., 2 p.m., 4 p.m., 6 p.m., 8 p.m., 10 p.m., 2 a.m., 6 a.m.using the same pneumatonometer.Measurements were taken in the sitting position.RESULTS: The average 24-hour IOP measured in the clinic was slightly lower than that at home.The mean difference in 24-hour IOP measurements between home and clinic was 0.27 mmHg.The IOP fluctuation in the clinic was higher than at home (the mean difference was 0.01 mmHg).There was no statistically significant difference in the average 24-hour IOP measured at home vs in the clinic.The average IOP measured at 2 p.m.at home (16.04±5.95 mmHg) was significantly higher compared with the measurement in the clinic (15.43±5.16 mmHg) (P<0.05).The overall agreement between 24-hour IOP measurements made in the clinic and at home in diagnosis of primary open angle glaucoma was 85.0% (K coefficient: 0.68).CONCLUSION: The 24-hour IOP measured in the clinic was similar to that measured at home, and the method of measuring IOP in the clinic is acceptable in diagnosing primary open angle glaucoma.
6.Effect of Duhuojisheng Decoction on primary knee osteoarthritis in macaca fascicularis

BACKGROUND:Meta-analysis results have shown that Duhuojisheng Decoction can improve the symptoms and relieve the pain of patients with knee osteoarthritis, but the mechanism and pathway are not very clear.OBJECTIVE:To further verify the curative effect of Duhuojisheng Decoction for primary knee osteoarthritis in macaca fascicularis. METHODS: Under natural conditions, 6 clean young macaca fascicularis (aged 3-5 years, normal group) and 6 elderly macaca fascicularis (aged more than 20 years, model group) were selected from 24 macaca fascicularis. The models of primary knee osteoarthritis were established in the model group,then aged animals were randomly divided into control and intervention(given the treatment of Duhuojisheng Decoction) groups. The number of whole blood leukocytes, C-reactive protein, erythrocyte sedimentation rate, properties of the joint fluid and expression level of tumor necrosis factor-α were compared between the control and intervention groups. RESULTS AND CONCLUSION: The aged macaca fascicularis was characterized by a typical osteoarthritis similar to human. Treatment with Duhuojisheng Decoction significantly down-regulated the expression level of tumor necrosis factor-α and significantly reduced the levels of whole blood leukocytes, and C-reactive protein. Our results suggest that the aged macaca fascicularis model of primary knee osteoarthritis is an effective animal model to simulate the occurrence and development of human primary knee osteoarthritis.Duhuojisheng decoction alleviates the progression of primary knee osteoarthritis probably by inhibiting the expression level of tumor necrosis factor-α and decreasing the levels of inflammatory factors in the articular cartilage. Additionally, the levels of whole blood leukocytes and C-reactive protein are important parameters for the prediction of curative effects.
7.Case-control study on the treatmentof the fifth metatarsal base fractures by cardboard compression pad versus short leg plaster.

OBJECTIVETo compare the effect,safety,and advantage of flexible fixation with paperboard and pad versus short leg plaster in treating the fifth metatarsal base fracture,and establish the standard of diagnosis and treatment of the fifth metatarsal base fractures in flexible fixation with paperboard and pad.
METHODSFrom June 2010 to March 2013,59 patients with the fifth metatarsal base fracture were treated with paperboard and pad fixation or short leg plaster. Patients were enrolled and divided into paperboard and pad treatment group (paperboard group) and short leg plaster treatment group (plaster group) randomly according to the random number table. In paperboard group,there were 29 cases including 9 males and 20 females with an average age of (51.79±11.40) years old; the average course of injury was (11.59±6.58) hours. In plaster group, there were 30 cases including 9 males and 21 females with an average age of (52.13+17.34) years old ;the average course of injury was (11.03±7.06) hours. According to whether the fracture line across the articular surface, in paperboard group there were 14 cases of type A,15 of type B; in plaster group,16 of type A, 14 of type B. According to the degree of dislocation,in paperboard group there were 16 cases of degree I ,13 of degree II ; in plaster group,20 were degree I ,10 were degree II. Fracture was restored according to the type in manual. Patients in paperboard group were treated with paperboard and pad, and patients in plaster group were treated with short leg plaster. Fracture was fixed for 4 to 6 weeks according to fracture healing. On the 2nd, 4th,6th, 8th week and 3rd, 6th month after fixation, patients were followed up, and the foot function score was used to evaluate the function of injured foot. X-ray of injured foot was taken on the 2nd, 4th, 6th and 8th week were used to assess fracture healing.
RESULTSAll patients got complete follow-up. The X-ray result showed that all fracture reached at clinical healing on the 8th week after fixation without skin ulcer,nonunion and displacement of fracture. From the 4th to 8th week after fixation, paperboard group had a higher X-ray score than plaster group, but the difference between two groups had no statistically significance. Repeated analysis result showed that there was interact at different time point and between groups,the difference had statistically significance (P<0.01). The foot function score showed that at all time point, paperboard group had a higher score than plaster group, and on the 2nd, 4th, and 6th week, it had statistically significant difference(P<0.01) between two groups. On the 6th months after fixation,the excellent and good rate of paperboard group was 93.10%, higher than that of plaster group, which was 86.67%. But it had no statistically difference(P=0.483) between two groups.
CONCLUSIONUsing paperboard and pad fixation to treat the fifth metatarsal base fracture has the advantage of simplicity operating,reliable fixation, satisfactory effects, easily obtainable material.

9.Causes of death in patients with subarachnoid hemorrhage induced by ruptured cerebral aneurysms

Objective To analyze the risk factors of death in patients with subarachnoid hemorrhage induced by ruptured cerebral aneurysms and provide some therapeutic strategies. Methods A retrospective analysis of the clinical data and causes of death in 24 patients died from subarachnoid hemorrhage induced by ruptured cerebral aneurysms from January 2003 to December 2010 was performed. Results Eleven patients died from intra-cerebral re-bleeding: 5 patients had re-bleeding before the interventional embolization,3 had bleeding during the surgery,and 3 had re-bleeding after the surgery. Eight patients died from vasospasm or cerebral infarction after the surgery, including 3 with cerebral infarction under early-stage CT (within 3 d of surgery) and 5 with vasospasm or cerebral infarction under late-stage CT (3 d after the surgery); 1 died from respiratory arrest after the operation; 3 died from pulmonary infection; and 1 died from renal failure. Conclusion Re-bleeding,vasospasm/cerebral infarction and Non- neurological complications are the major causes of death in patients died from aneurysmal subarachnoid hemorrhage induced by ruptured cerebral aneurysms.Increase the awareness of causes of death will help reduce the mortality.
10.Preliminary study on general safe medication regularity of Chinese patent orthopedic medicines based on adverse reaction/event literature analysis.

Chinese patent orthopedic medicines feature complex components, mainly including desperate and toxic herbal pieces, narrow safety window, more clinical contraindications and frequent adverse drug reaction/events (ADR/ADE). To study the general safe medication regularity of Chinese patent orthopedic medicines, define key points in the medication education and ensure rational clinical medication, the authors took 80 types of commonly used Chinese patent orthopedic medicines as the study objects, collect 237 cases from 164 ADR/ADE documents through a system retrieval strategy, make a multidimensional literature analysis to determine the common risk factors for safe and rational medication of Chinese patent orthopedic medicines and establish an ADR/ADE prevention regularity. First, in the aspect of clinical symptoms, skin allergy is the most common ADR/ADE and closely related to the toxic ingredients, particularly accumulated liver or kidney damage caused by some drugs. Second, there are three time nodes in the ADR/ADE occurrence; The ADR/ADE occurred in 30 minutes is closely related to the idiosyncrasy; the ADR/ADE occurred between several months and half a year is related to the drug-induced liver and kidney damages; The most common ADR/ADE was observed within 7 days and predictable according to the pharmacological actions; Third, toxicity is an important factor in the occurrence of ADR/ADE of Chinese patent orthopedic medicines. Fourth, emphasis shall be given to the special medication factors, such as the combination with western medicines and Chinese herbal decoctions, overdose and long-course medication and self-medical therapy. In conclusion, the general ADR/ADE prevention regularity for Chinese patent orthopedic medicines was summarized to provide supports for clinicians in safe and rational medication and give the guidance for pharmacist in medication education.
