3.Detection and analysis of human bocavirus in children with acute respiratory infection in Nanjing

Objective To investigate the possible existence of HBoV in children with acute respiratory infections in Nanjing area and explore its relationship with clinical characteristics.Methods A total of 397 nasopharyngeal secretion samples were collected from children with acute respiratory infection,admitted from July 2009 to June 2010 in Nanjing Children'S Hospital affiliated to Nanjing Medical University,and 50 cases of children without symptoms of respiratory infection were recruited as control group,whose nasopharyngeal secretion samples were also collected.HBoV was determined by real-time fluorescence quantitative PCR.MP and CT were detected by real-time fluorescence quantitative PCR in those HBoV-positive samples.RSV,ADV,IVA,IVB,PIV-1,PIV-2,PIV-3 and hMPV were detected by direct antigen-specific immunofluorescence assays.HBoV NP-1 fragments were amplified and sequenced in 5 HBoV positive samples randomly selected.The results were compared with the known GenBank sequence,and thereby the phylogenetic tree was established.The epidemiological characteristics,clinical presentation and the final clinical diagnosis of HBoV were analyzed according to the clinical data of the HBoV-positive patients.Results Thirty-three HBoV-positive cases were detected by real-time fluorescence quantitative PCR method with a positivity rate of 8. 3% ( 33/397 ). Among the 33 HBoV-positive cases, 19 cases (57.6%) were multiple infections with HBoV and other pathogens, the top three of which were MP (27.3% ,9/33 ),RSV (24.2% , 8/33 ) and PIV-3 ( 12. 1% ,4/33 ). Affected children aged from 7 to 36 months old accounted for 75.8% of the total ( 25/33 ). The measured HBoV NP-1 gene sequences of 5 specimens were consistent,indicating a high homology (99% to 100% ) with the stl, st2 and WHL-1. Conclusions HBoV is one of the pathogens of children's acute respiratory infections in Nanjing. HBoV NP-1 gene is highly conserved,with little variation in different seasons and in different regions and therefore can be used as a marker for real-time fluorescence quantitative PCR and other methods.
4.Cyclic fatigue performance of different types of stainless steel root canal files

BACKGROUND: Root canal files fracture is a kind of common oral clinical complications. Therefore, it is of clinical importance to study the cyclic fatigue of different types of stainless steel root canal files.OBJECTIVE: To explore the effect of the bending angle and cross-sectional area of root canal instruments on cyclic fatigue of stainless steel files.METHODS: Thirty 25# stainless steel K files (25 mm in length) were selected, the same to 30# and 35#. And these three kinds of files were respectively randomized into three groups (n=10 per subgroup). In the nine subgroups, the files were placed into self-made root canals at the bending angle of 45°, 60°, 90°, and driven by a 16:1 reducer (350 r/min).Fracture time in each subgroup was recorded and converted into the number of rotations. The microstructure of the fracture end of the files was observed under scanning electron microscope. Fractured files were collected and the crack tip length was measured by an electronic caliper.RESULTS AND CONCLUSION: At the same bending angle, the number of rotations resulting in file fracture was significantly reduced with the increasing of the cross-sectional area of the files (P < 0.05). When the type and cross-sectional area were same, the number of rotations resulting in file fracture was significantly reduced with the increasing of the bending angle of the root canals (P < 0.05), indicating a reduction in the cyclic fatigue performance of the files. Toughness fracture occurred in all the files, and circular or oval roughness nests with different sizes and microcavities formed on the fracture surface. In addition, brittle surfaces could be detective between the roughness nests. The diameter of roughness nests was gradually increased with the increasing of the bending angle of the root canals. For 30# and 35# files, the number of brittle surfaces was gradually increased with the increasing of the bending angle of the root canals. For 25# files, the crack tip length was gradually decreased with the increasing of the bending angle of the root canals, but there was no significant difference (P > 0.05). For 30# and 35# files, the crack tip length was significantly shortened with the increasing of the bending angle of the root canals (P < 0.05). But there were no significant changes in the 35#-60° and 35#-90° groups (P=0.095). At the same bending angle, there were no regular changes in the crack tip length in the different types of files. In conclusion, with the increasing of the cross-sectional area, the cyclic fatigue performance of the files with the same length and taper is reduced and the files become easy to be fractured; with the increasing of the bending angle of the root canals, the files with the same types are apt to be fractured near the root tip, indicating the shorter length of the crack tip indicates the more difficulty in the removal of the broken files.
5.Fingerprint of Flos Lonicerae by HPLC-FPC

AIM To establish the HPLC-FPC of Flos Lonicerae . from different growing areas. METHODS The gradient was adopted elution with methanol and 1 % acetic acid, at detection wavelength was at 254 nm. RESULTS The constitutents of Flos Lonicerae were well separated on HPLC, and the HPLC-FPC of 10 batchs of samples was established. The results showed that this method had a good repeatability, and the 11 common peak’s had an exclusive properties. CONCLUSION The method developed can evaluate the quality of Flos Lonicerae conveniently.
6.Safety evaluation of Dengzhanxixin injection in 986 elderly inpatients

Objective To investigate the rationality and safety of dengzhanxixin injection used in elderly inpatients. Methods The clinical data of 986 inpatients including 620 males and 366 females were collected, and questionnaires containing age, sex, discharge diagnosis, symptoms, drug dosage, course of treatment, laboratory examination, adverse drug reaction and drug effect were analyzed. Results For the 986 cases, the average age was(74.3±7.5)years. The average dose of dengzhanxixin injection was (38.2±4.4) ml, once daily by intravenous drip, and the average period of treatment was (10.8±5.2) days. The adverse reaction rate was 0.81%. The levels of blood glucose and hemoglobin were decreased after treatment(t orμ=1226.5,2620.0, both P<0.05), but there were no statistical differences in the levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), creatinine (CREA), blood urea nitrogen (BUN) and white blood cell count (WBC) before and after treatment (t or μ=122.5, 405.0, 513.5, 996.5, 956.5, all P>0.05). Conclusions It is safe to use dengzhanxixin injection according to the medication description for elderly inpatients.
7.Analgeic effect of patient control subcutaneous morphine combined with small-dose ketamine on moderate/severe terminal cancer pain

Background and purpose:The pain is not only the chief symptom of cancer but also the main factor of survival quality for the cancer patient.So it is the most difficult for how to control the pain of moderate to severe late-stage cancer and reduce the rate of side effects.We observed the analgesia effects of micro-dosage ketamine associating morphine patient control skin analgesia(PCSA) for latter cancer pain patients.Methods:52 cases middle, latter cancer pain patients were divided two groups randomly,the first group used the morphine PCSA and the second group used micro-dosage ketamine associating morphine PCSA.The VAS(Visual Analogue Scale) was used to evaluate the pain level,then statistic the side effects rate of two groups.Results:Two groups patients'pain all alleviated fundamentally,the VAS of every period have not statistic difference,but the PCSA pressing times in the first group was more than in the second group.The second group patients'life satisfactory rate was higher obviously than the first group patients';Side effects:the incidence of nausea,vomiting,indulge in sleep and skin titillation in the first group was higher than in the second group;the incidence of respiration depression and hallucination had not difference between two groups.Conclusion:Micro-dosage ketamine associating morphine patient control skin analgesia(PCSA)approach appears to provide patient with safe and effective pain relief and reduce side-effect for moderate to severe late-phase cancer pain,easy to perform and have no process complication to used in clinic.
8.Successful treatment of a patient with severe H1N1 Flu anti multiple organ dysfunction syndrome

Objective To investigate the causes of severe H1N1 Flu with multiple organ dysfunction, and measures to reduce mortality. Method The data of the patient, who was diagnosed as severe H1N1 Flu and mul-tiple organ dysfunction syndrome in First People's Hospital Affiliated to Shanghai Jiaotong University in September 2009, were retrospectively analyzed. The patient was male, 35 year-old, obese, high fever, sore throat, cough, progressive dyspnea, severe hypoxemia and hypotension. Effective measures were carried out, including protective lung ventilation, recruitment maneuver, vasopressor support, limited fluid resuscitation, appropriate corticosteroid, anfiviral plasma, anticoagulafion and antiviral medicine (Oseltamivir)in early stage and full dose. Results After one-month intensive care, clinical symptoms was improved obviously, oxygen pressure reached 74 mmHg without oxygen supply, CT scan showed diffused interstitial ehange. Neuromyopathy developed at approximately 3 weeks after the onset of H1N1. Conclusions H1N1 Flu can develop in healthy adults, and obesity is one of the inde-pendent risk factors. Effective measures should be taken as soon as possible to reduce the mortality.
9.Application of information system in material management of sterile supply

Objective Hospital material management system of sterile supply has its particularity which is in close relation with medical quality.The study was to greatly improve the quality and efficiency of sterile supply department with the application of material management system. Methods The material management system integrated with the hospital net was applied in the man-agement of material distribution, inventory and statistics. Results It provided exact and detailed data for sterile supply department and clinical departments. Conclusion The application of information system in hospital net can provide exact and overall cost ac-counting information for involved departments, greatly improving the efficiency of material management.
10.Expression of CD133 and CD44 proteins in gastric stromal tumors and their clinical significances

Objective To investigate the expression of CD133 and CD44 proteins in gastric stromal tumors (GST) and their clinical significances. Methods The expression of CD44 and CD133 proteins in the GST tissues of 112 patients was detected by immunohistochemical staining. The relation between the expression of CD44 and CD133 proteins and the clinicopathological characters was analyzed. The survival and prognosis of GST were also analyzed. Results Both CD44 and CD133 were expressed on the cell membranes. The expression rates of CD44 and CD133 were 58.04 % (65/112) and 42.86 % (48/112) separately; the co-expression rate of CD44 and CD133 was 27.68 % (31/112). CD44 and CD133 were negative in normal peritumoral tissues. No correlation was found between CD44 and CD133 and the clinicopathological parameters including gender, age and lymphatic vessel invasion (all P>0.05), but the expression levels of CD44 and CD133 in patients with the mitotic count ≥ 5/50 high-power field, large diameter and vascular invasion were significantly higher (all P<0.05). No correlation was found between co-expression of CD44 and CD133 and the clinical clinicopathological parameters including gender, age, the mitotic count ≥ 5/50 high-power field and vascular invasion (all P>0.05), but the co-expression level of CD44 and CD133 in patients with tumor diameter ≥5 cm was significantly higher than that in patients with tumor diameter < 5 cm (χ2=5.040, P=0.025). The overall survival rate of the patients with co-expression of CD44 and CD133 was shorter than that in other groups (χ2 = 8.758, P= 0.001). No correlation was found between CD44 and CD133 expression (r=0.126, P=0.210). Multivariate analysis with the Cox regression models showed that the tumor diameter ≥5 cm (P=0.042) and co-expression of CD44 and CD133 (P=0.003) were significantly associated with poor prognosis. Conclusion CD44 and CD133 as robust cancer stem cell markers in GST might be the prognostic factors.
