Aim To investigate the influence of total fl avones of buckwheat flower (TFBF) on the productivity of the non-enzymatic advanced glycation end products (AGEs) in vivoand vitro. Methods TFBF in different dosages (0.1 g?kg -1?d -1,0.2 g?kg -1?d -1,0.4 g?kg -1?d -1) was taken orally by streptozotocin-induced diabetic rats for 12 wk. After the treatment, blood glucose, fructosamine and AGEs in plasma and kidney were measured. 2.False-negative Possibility in Genetic Test of Congenital Long QT Syndrome by Next-generation Sequencing

Objective: To explore the false-negative possibility in genetic test of congenital long QT syndrome (LQTS) by next-generation sequencing (NGS). Methods: A total of 28 genomic DNA samples were collected from 4 laboratories including 2 commercial medical laboratories using HiSeq2000 platform as Lab1,n=6 and Lab2,n=8; 1 commercial research service laboratory using Ion-torrent platform as Lab3,n=8 and 1 academic laboratory using HiSeq2000 platform as Lab 4,n=6. Sequencing coverage in the exons of protein-coding region in 3 main LQTS pathogenic genes as KCNQ1, KCNH2, SCN5A and possible pathogenic variants were quantitatively analyzed. Results: In Lab1, Lab 2 and Lab 4 with HiSeq2000 platform, above 98% protein coding regions in 3 pathogenic genes were covered with>10-fold reads and 90%-95% were covered with>30-fold reads. In 2 commercial medical laboratories, 3.63% and 9.84% protein coding regions of KCNQ1 gene in 14 samples were covered with<10-fold reads and with<30-fold reads; lower than 10-fold covering region was focused in the 1st exon including about 2% known or likely pathogenic variants. In 2 commercial medical laboratories, 2.64% and 15.76% protein coding regions of KCNH2 gene in 14 samples were covered with<10-fold reads and with<30-fold reads; low covering region was located in multiple exons. For the data from Lab 1, as high as 28.56% protein coding regions of KCNH2 gene were covered with<30-fold reads including 113 (19.79%) known or likely pathogenic variants. SCN5A gene had the best coverage of protein coding region, with no<10-fold reads in all 4 Labs and no<30-fold reads in 2 commercial medical laboratories. Conclusion: Currently, NGS has low coverage region in both KCNQ1 and KCNH2 genes, pathogenic variants could be missed and false-negative possibility should be highly alert.
3.Relationship between Airborne Fungi and Childhood Bronchial Asthma in Huangshi Region

Objective To analyze the relationship between categories,sum and seasonal distribution of airborne fungi and childhood bronchial asthma in Huangshi region.Methods Airborne fungi was investigated through gravitational precipitating method by placing the sample cutter at the 5th floor platform of the surgery department of the First People′s Hospital of Huangshi.Slides were exposed to the air in 24 hours with each day a new slide to be placed.This explosion of slides stayed a whole year from Feb.2007 to Feb.2008.Thus the collected data of fungi were filed,verified and then classified so as to find out the dominant allergic fungi.Multivalent allergen skin tests of fungi were performed on 247 children with bronchial asthma in Huangshi region at the same period and the pattern was explored during invasion season.Results Altogether 365 slides were exposed and 12 667 fungi were collected,which falled to the 22 genres of the 4 sub-categories.And these fungi could be arranged by sum to be alternaria,smut,cladosporium cladosporioides,uredinales,leptosphaeriamaculans and many others including those even without a name.The peak period of fungi distribution in Huangshi city was from Apr to Oct,and it was affected by temperature,wind and rainfall and sunshine.Forty-two point nine one percent were positive reaction to multivalent fungi allergens,of which the hig-hest positive rate to multivalent fungi allergensⅠwas 23.48%.The result showed asthma usually happened during Apr to Aug.Conclusions There were airborne fungi in Huangshi region yearly,and the sensitive season was spring and summer.Dominant fungi were alternaria,ustilaginales and ciadosporium.Forty percent asthma had a close connection to fungi allergens.The attack of children bronchial asthma was corresponded with fungi peak duration.
4.Efficacy of different kinds of artificial tears treatment in patients with xerophthalmia after phacoemulsification combined with intraocular lens implantation

AIM:To explore efficacy of the different kinds of artificial tears treatment in patients with xerophthalmia after phacoemulsification combined with intraocular lens implantation.METHODS:Totally 280 patients (280 eyes) with xerophthalmia after operation in our hospital from January 2015 to June 2016 were selected.According to the different treatment methods, they were divided into control group (n=70;treated with tobramycin and dexamethasone eye ointment 3 times per day for 1wk, levofloxacin eye drops 3 times per day for 1wk, pranoprofen eye drops 4 times per day for 1mo), polyacrylic acid group (n=70;besides the treatment of control group, polyacrylic acid was used 4 times per for 1mo), polyethylene glycol group(n=70;besides the treatment of control group, polyethylene glycol was used 4 times per for 1mo) and sodium hyaluronate group (n=70;besides the treatment of control group, sodium hyaluronate was used 4 times per for 1mo).The tear film break up time (BUT), Schirmer Ⅰ test (SⅠt), symptoms of dry eye and corneal staining in four groups were observed.RESULTS:(1) BUT:The BUT of the four groups significantly increased after treatment(P<0.05);that of polyacrylic acid group, polyethylene glycol group, and sodium hyaluronate group was different compared with control group(P<0.05);the BUT in sodium hyaluronate group was significantly higher than the other groups after 2wk of treatment(P<0.05).(2) SⅠt:SⅠt of the four groups significant increased after treatment(P<0.05);that of polyacrylic acid group, polyethylene glycol group, and sodium hyaluronate group was different compared with control group(P<0.05);and the SⅠt in sodium hyaluronate group was significantly higher than other groups (P<0.05).(3) Xeroma score:the scores of dry eye significantly decreased after treatment in the four groups(P<0.05);that of polyacrylic acid group, polyethylene glycol group, and sodium hyaluronate group was different compared with control group(P<0.05);and the scores of the sodium hyaluronate group after 3wk was significantly lower compared with other groups(P<0.05).(4) Corneal staining score:the scores significantly decreased after treatment in the four groups(P<0.05);at 1 and 2wk after treatment the corneal staining score had not statistically different among the four groups (P>0.05);sodium hyaluronate group was significant lower than other groups in corneal staining score at 3wk and 1mo after treatment(P<0.05).CONCLUSION:Artificial tears in the treatment of xerophthalmia after cataract phacoemulsification combined with intraocular lens implantation has better clinical efficacy, which contains sodium hyaluronate may be the better than others.
5.Clinical analysis on 64 cases of testicular torsion and literature review

Objective To investigate the clinical characteristics of testicular torsion,and to provide the strategies for its early diagnosis and timely therapy.Methods The clinical data in the patients with testicular torsion diagnosed by surgical exploration from March 2011 to December 2016 were reviewed.The clinical characteristics were compared between the testis-preserving group and testis-resection group.The latest advances in the diagnosis and treatment of testicular torsion were summarized.Results A total of 64 cases of testicular torsion were included in this study.There were 27 cases in the testicle-preserving group with the age range of(8-40) years old,the average age was(19.00±5.28) years old,the onset time range was(1-24) h,the average time was (8.78 ± 6.73) h;the intraoperative sper matic cord twist range was (180-540) ° and average (290.00 ±103.92) °;6 cases were misdiagnosis with the misdiagnosis rate of 22.22 %.There were 37cases in the testicle-resection group with the age range of(10-34),the average age was(19.00 ± 7.45) years old;the onset time range was (12-168) h,the average time was(66.92 ± 47.01) h,the intraoperative spermatic cord twist range was(180-720)° and average(457.00± 168.88)°;22 cases were misdiagnosed with the misdiagnosis rate of 59.46 %.The onset time,spermatic cord twist degree and misdiagnosis rate had statistically significant difference between the two groups(P<0.05).Conclusion Testicular torsion is an important urinary emergency condition which can not be ignored.Timely and effective treatment is the key to save the testicles.
6.Inhibitory effect and kinetic analysis of sodium quercetin-7,4'-disulphate on recombinant human protein kinase CK2 holoenzyme.

AIMTo study the direct effect and kinetics of sodium quercetin-7,4'-disulphate (SQDS) on recombinant human protein kinase CK2 holoenzyme.
METHODSThe recombinant human CK2 holoenzyme activity was assayed by detecting incorporation of 32P of [gamma-32P] ATP into the substrate in various conditions.
RESULTSThe recombinant human CK2 was a second messenger (Ca2+, cAMP and cGMP) independent protein kinase. The characterization and function of the reconstituted holoenzyme were consistent with those of native CK2. SQDS was shown to strongly inhibit the holoenzyme activity of recombinant human protein kinase CK2 with an IC50 of 4.4 mumol.L-1, which was more effective than DRB and A3, known CK2 special inhibitors. Kinetic studies of SQDS on recombinant human CK2 showed: the inhibition was competitive with ATP and noncompetitive with casein.
CONCLUSIONSQDS is a potent inhibitor of protein kinase CK2. This study provide experimental basis for the development of more effective inhibitors of CK2 and for clinical application of SQDS in the future.

7.Release kinetics of oridonin self-microemulsifying drug delivery system in vitro.

OBJECTIVETo investigate the release characteristics and mechanism of oridnonin self-microemulsifying drug delivery system (SMEDDS) in vitro.
METHODThe concentration of oridonin was determined by HPLC. In vitro release studies were conducted by reverse dialysis technique. The effects of release medium, agitation rate and preparations on the oridonin release were studied. The similarity factor (f2) was applied to the release profile comparisons. Model fitting was used to determine the kinetics and mechanism.
RESULTThe release media and agitation rate from 50-100 r x min(-1) had no distinctive effect on the oridonin release kinetics, which the similarity factors were greater than 50. The oridonin release profiles for oridonin SMEDDS and oridonin ethanol solution were similar. 65% of oridonin were released in 30 min for oridonin SMEDDS in pH 7.8 PBS. Oridonin SMEDDS fit the Hixson-Crowell model best.
CONCLUSIONThe release data from oridonin SMEDDS showed it release fast. The deduced release mechanism is that the surface and particle sizes of self-microemulsion in water solution are changing during the process of release and the drug penetration through membrane is a passive diffusion process.

8.Liver pathology in idiopathic portal hypertension.

OBJECTIVETo study the histopathological changes of livers in idiopathic portal hypertension (IPH).
METHODSLiver specimens from 29 cases with idiopathic portal hypertension were studied. Histological preparations of the livers were stained with haematoxylin eosin and Masson's trichrome; reticular fibers in the liver tissues were demonstrated. The slides were also stained using some immunohistochemistry methods, and the pathological changes of the livers were analyzed.
RESULTSThe characteristic changes found in these IPH livers were dense portal fibrosis; obliteration, with or without phlebitis, of the branches of the portal vein; dilatation of the sinusoids; atrophy and nodular hyperplasia of liver cells.
CONCLUSIONSHistopathological changes of the livers in IPH are dense portal fibrosis, portal vein branch obliteration and nodular hyperplasia of liver cells. These are the main features for a histopathological diagnosis of IPH.

9.Observation of neural electrophysiology and morphology in traumatic optic neuropathy in cat model.

OBJECTIVETo establish the animal model of traumatic optic neuropathy similar to clinical feature by injuring the optic nerve of the pipeline through the ethmoid and sphenoid sinus. The electrophysiology and morphology were observed.
METHODSThe optic nerve in 2 cats was anatomically observed. Thirty healthy adult cats were divided into 1 h, 1 d, 3 d, 1 w, 2 w, 4 w injury groups. The right eye was selected as the experiment eye, and the left as the control. The optic canals were damaged by the injury instrument. The changes of optic nerve were observed using light and electron microscope. The pattern reversal visual evoked potential (PR-VEP) was used to determine the succeed model and pupil was monitored. PR-VEP and neural tissue morphology were examined at intervals of 1 h, 1 d, 3 d, 1 w, 2 w, 4 w after trauma.
RESULTSEight cats had Marcus-Gunn's pupil, and 22 cats did not have after injury. Neural tissue was physaliphorous degeneration in light microscope. The neural fibers swelled gradually and fibers physaliphorous degeneration in the first day after damage. At the seventh day, the bubble enlarged to be a big flat. After two weeks, the big flat bubble came out to be a bigger bubble. Under the electronic microscope, axons were physaliphorous degeneration, crack emerged between axon membrane and myelin sheath and myelin sheath were loose in the first day. At the third day, axon swelled, physaliphorous degeneration rose and myelin sheath were loose obviously. At the seventh day, axon and myelin sheath appeared loop in half cycle and bubble-like broke up. A lot of pieces came out and axon became physaliphorous and uniform. Microtubule and microfilament disappeared. After two weeks, myelin sheath became onion-like. On fourth week, myelin sheaths enlarged and squeezed to center. Axoplasm disappeared and neural tissues collapsed.
CONCLUSIONSThis animal model was similar to clinical optic nerve decompression through ethmoid and sphenoid sinus. The change of PR-VEP and neuromorphology were distinct before and after trauma. The pathological changes of optic nerve catching vibrated injury are mainly degeneration, so earlier decompression of optic nerve may improve the visual function.

10.Clinical efficacy of montelukast sodium in the treatment of chronic obstructive pulmonary disease combined with obstructive sleep disordered breathing overlap syndrome

Objective To evaluate the clinical efficacy of montelukast sodium combined with Bi-level positive airway pressure ( BiPAP) respi-rator in treatment of chronic obstructive pulmonary disease combined with obstructive sleep disordered breathing overlap syndrome.Methods A total of 103 patients with overlap syndrome were divided into treatment group(n=51) and control group(n=52).Patients in treatment group were received montelukast sodium combined with BiPAP respirator, while patients in control group received BiPAP respiratoronly.All patients were treated for 4 weeks.The clinical symptom score, C -reactive protein ( CRP ) , arterial blood gas analysis parameters and pulmonary function parameters were recorded before and after therapy in two groups were ob-served.Results The level of CRP after therapy was significantly lower in two groups than before therapy ( P <0.05 ) , treatment group lower than control group(P<0.05).After therapy, the levels of apnea hypop-nea index ( AHI) ,partial pressure of carbon dioxide( PaCO2 ) were signif-icantly lowerthan before therapy ( P<0.05 );andpartial pressure of oxy-gen ( PaO2 ) , forced expiratory volume in one second ( FEV1 ) and FEV1 /( forced vital capacity, FVC ) were significantly higher than before therapy ( P<0.05).Conclusion Montelukast sodium combined with BiPAP respirator had a good effect in overlap syndrome patients.
