2.Effects and implications of physical modalities on hs-CRP, TNF-α and adiponectin in patients with cerebral infarction

Objective To observe the effects of physical agents therapy on serum hs-CRP, TNF-α andadiponectin in patients with cerebral infarction and the possible underlying mechanisms. Methods Sixty patientswith cerebral infarction were randomly and equally divided into two groups: 30 cases were treated with physical a-gents therapy ( physical therapy group) , and 30 with drugs only ( drug treated group). Thirty normal subjectsserved as the control group. The level of hs-CRP in the serum was determined by latex agglutination reaction, TNF-and adiponectin were determined by using ELISA before and after therapy. Results The levels of serum hs-CRP and TNF-α of patients with cerebral infarction before therapy were much higher than those of the control group,but adiponectin was significantly lower than those of the control group( P < 0.01 ). After therapy, the levels of ser-um hs-CRP and TNF-α were decreased and adiponectin was increased significantly in both treated groups ( P <0.01 ). Comparison with two treated groups showed that the levels of hs-CRP and TNF-α were lower and adiponec-tin was obviously higher in physical agents therapy group than those in the drug treated group ( P < 0.05 ). Con-clusion The patients with cerebral infarction have low level of serum adiponectin. Physical therapy might exertbeneficial effects on patients with cerebral infarction by the decreasing serum hs-CRP and TNF-α, as well as by ele-vating adiponectin.
3.Research on the relationship between recurrence of cryptogenic ischemic cerebrovascular disease and patent foramen ovale

Objective To evaluate the relationship between recurrence of cryptogenic ischemic cerebrovascular disease (CICVD) and patent foramen ovale (PFO),as well as to access the clinical significance of PFO in ischemic cerebrovascular disease.Methods Consecutive patients with CICVD aged 15 to 70 years who were hospitalized in Department of Neurology,Xuanwu Hospital Capital Medical University from January 2008 to March 2011 were prospectively investigated.Identified by transesophageal echocardiography,patients were divided into two groups with respect to outcome:PFO group and non-PFO group.The recurrence of cerebral ischemic events was compared between the two groups after neurological follow-up.Results A total of 91 patients were recruited,including 57 patients with PFO and 34 patients without PFO.The follow-up period of two groups was 695 (506,1142) d.The recurrence rate at 15 months in patients with PFO (24.5% (12/49)) was higher than those without PFO (6.9% (2/29),x2 =4.391,P =0.036).Cum hazard curve indicated that recurrence risk of cerebral ischemic events in patients with CICVD in PFO group was higher than that of patients in non-PFO group during the follow-up period (P =0.044).Cox model used for multivariate survival analysis indicated that PFO was a risk factor for cerebral ischemic event recurrence among patients with CICVD (OR =4.159,95% CI 1.178-14.689,P =0.027).Conclusions PFO is associated with increased recurrence risk of cerebral ischemia in CICVD patients.In addition,PFO may be a significant factor for ischemic cerebrovascular disease.
4.Obsevsation on the effect of docetaxel combined with 5-fluorouracil and cisplatin in the treatment of patients with advanced gastric cancer

Objective To evaluate the efficacy and adverse effects of docetaxel combined with 5-fluorouracil in treating patients with advanced gastrice cancer.Methods 130 patients with advanced gastric cancer were randomly divided into observation group and control group,observation group treated with docetaxel combined with 5-fluorouracil,the control group treated with 5-fluorouracil and cisplatin,the clinical efficacy after the treatment was observed.Results The clinical efficiency rate of the observation group was significantly higher than that of the control group,the rate of leukopenia,thrombocytopenia and gastrointestinal reactions of the observation group was significantly lower than the control group,the quality of life scores of the two groups after treatment were all improved,but that of the observation group was significantly higher than the control group,the difference was statistically significant(P < 0.05).Conclusion Docetaxel combined with 5-fluorouracil was effective in treating advanced gastric cancer.It worth further clinical research and application.
5.Isolation,culture and biological characterization of rat bone marrow-derived endothelial progenitor cells

BACKGROUND:A steady method that can isolate endothelial progenitor cells(EPCs) need to be explored.Studies show that the EPCs riches in bone marrow with strong reproductive activity,which can be easily obtained.OBJECTIVE:To evaluate the isolating culture and biological characterization of EPCs derived from rat marrow.DESIGN,TIME AND SETTING:The in vitro cytology experiment was performed at the central laboratory of the First Affiliated Hospital of Chongqing Medical University from August to December 2008.MATERIALS:Twenty SD rats,with two-week-old,were obtained from Experimental Animal Center of Chongqing Medical University.METHODS:femurs and tibias of rats were isolated under sterile conditions,and then the bone marrow cavity was washed by 4 ℃ 0.01 mol/L phosphate buffer(PBS).Mononuclear cells were collected from marrow suspension by density gradient centrifugation,followed by cultured with M199 culture medium containing 20% fetal bovine serum to obtained cell suspension.The cells were vaccinated into culture flask(coated with fibronectin) with the density of 1?109/L,and incubated at 37 ℃ in 5% CO2 atmosphere.MAIN OUTCOME MEASURES:Morphologic change of cells was observed by an invert microscope,the expression of CD133,CD34 as well as KDR was detected by immunocytochemistry,and the flow cytometry was used to analyze the growth curve.RESULTS:The adherent cells formed clusters that were found to be characterized by density packed round cells surrounded by spindle-likes cells.Adherent cells were identified positive for CD133,CD34 and KDR by immunocytochemistry on days 4,7,10 and 14.CD133-positive cells reduced after 10 days.Flow cytometry analysis indicated that the number of CD133/KDR double positive cells gradually increased to the maximum at day 10 and then declined.KDR-positive cells gradually increased up with the time.EPCs growth curve showed that cells proliferated fast on days 3-6.CONCLUSION:EPCs can be isolated and cultured by density gradient centrifugation from marrow suspension of SD rats.These cells exhibit the characterization of endothelial cells,and can differentiate into endothelial cells.
6.Egr-1 mediates Si0(2)-driven transcription of membrane type I matrix metalloproteinase in macrophages.

The up-regulation mechanism of membrane type I matrix metalloproteinase (MT1-MMP) in macrophages stimulated by silica in vitro and the contribution of early growth response 1 (Egr-1) transcription factor in the gene expression pathway were investigated. Macrophages stimulated by silica were treated with Egr-1 antibody or Egr-1 decoy oligodeoxynucleotides (ODN). The levels of MT1-MMP proteins were determined by Western blot and the expression of MT1-MMP mRNAs was detected by RT-PCR. The results showed as compared with control macrophages, silica-stimulated group showed up-regulated gene expression of MT1-MMP via Egr-1 (P<0.01). Compared with silica-stimulated macrophages untreated with antibody, the cells treated with 5 microg/mL Egr-1 antibody were associated with reduced expression of MT1-MMP protein (P<0.01) and mRNA (P<0.01). Compared with silica-stimulated untransfected group, the Egr-1 "decoy" ODN group was associated with reduction in the expression of MT1-MMP protein and mRNA (P<0.01). It was concluded gene expression of MT1-MMP which may play a critical role in silicosis was up-regulated by silica in macrophages. Egr-1 participated in the expression of MT1-MMP and positively regulated the expression. Both Egr-1 antibody and Egr-1 decoy ODN suppressed the expression of MT1-MMP through the Egr-1 pathway and may become a potential therapeutic tool in the management of silicosis in the future.
7.Application of Position Vector in Balance Evaluation of Stroke Patients with Hemiplegia

Objective To investigate the application of position vector in balance evaluation of stroke patients with hemiplegia.Methods The static balance of 30 healthy subjects and 30 stroke patients with hemiplegia was measured by the balance instrument when standing on a force plate with eyes' open.There were 10 patients tested before and after rehabilitation training.Results The position vectorgram of the healthy subjects showed anterior-posterior sway and the patients showed right-left or diagonal sway.The patients exhibited much longer value in position vector than healthy subjects(P<0.01).After rehabilitation training,10 patients showed the changes in position vector at six directions,and there was a significant difference compared with those before rehabilitation training(P<0.01).Conclusion The position vector is clinically valuable in balance evaluation and monitoring the effect of treatment.
8.Related Factors of Ischemic Cerebrovascular Disease in Patients with Patent Foramen Ovale

ObjectiveTo investigate the related factors of ischemic cerebrovascular diseases in patients with patent foramen ovale (PFO). Methods42 patients with PFO,aged ≤55 years old, with cerebral infarction or transient ischemic attack and treated with aspirin were involved. All patients were examined with transesophageal echocardiography. 6 cases relapsed and 36 cases did not. The size of PFO, serum fibrinogen level, and combination of atrial septal aneurysm (ASA) were compared. ResultsThey were followed up for (512.51±127.38) d. The serum fibrinogen level was (3.33±1.11) g/L in no relapse group, and was (4.40±1.49) g/L in relapse group (P=0.032). There was not significant difference in the size of PFO (P=0.618) and combination ASA (P=0.554) between these groups. Logistic regression showed that patients with ischemic cerebrovascular diseases history involved in relapse (OR=46.913,95%CI:2.371~928.254), but the serum fibrinogen level did not (OR=2.656,95%CI:0.977~7.219). ConclusionThe PFO patients with history of ischemic cerebrovascular diseases incline to relapse. It is not sure the influence of the serum fibrinogen level on it.
9.Treatment of nonunion of humeral shaft fracture with plate fixation and bone graft

Objective To evaluate the operative treatment of nonunion of humeral shaft fracture with in- ternal plate fixation and autogenous cancellous hone graft.Methods Forty-one cases of nonunion of humeral shaft fracture operatively treated from February 2002 to December 2004 were analyzed retrospectively.There were 32 males and nine females.Their average age was 37.5 years(range,17 to 67 years).Sixteen nonunions were defined as hypertrophic and 25 as atrophic.We followed all the patients and obtained their complete medical information. Results Our average follow-up was 22.6 months(range,8 to 42 months).Forty fractures(97.6%)were united within an average of 5.8 months(range,3 to 12 months).Complications included iatrogenic radial nerve injury in three patients,wound infection in one patient and fracture nonunion in one patient.At the final follow-up,shou]der and elbow functions were found to be satisfactory.Conclusion Open reduction and plate internal fixation sup- plemented with autogenous cancellous bone graft is an effective treatment for nonunion of humeral shaft fracture.
10.Biological Control of Alternaria solani by Bacillus subtilis NJ-18

Strong inhibition of Bacillus subtilis strain NJ-18 on mycelia growth of Alternaria solani was observed in the antagonistic tests by cylinder plate methods, and the inhibition width was 21.5 mm. Observation under microscope found that the supernatant of fermentation from NJ-18 could make the pathogen hyphae cells malformed and swelled, and consequently the growth of the pathogen was inhibited. Determining of the colonization in potato plants by the signed rifampicin-resistance in NJ-18 showed that it could colonize well in the plants, the colonization quantity of NJ-18 in the root and stem of the potato detected 30 days after fermentation irrigation was 103 CFU/g plant’s fresh weight. In pot experiment, we inoculated the tomato plants with the spore suspensions of Alternaria solani after spraying the fermentation of NJ-18, the results were investigated in 14 days and the efficacy in controlling the disease was 72.9%, which was significantly higher than 45.7%, the efficacy resulted from spray treatment of 2000 fold dilution of 50% iprodione wetable powder.
