Objective To observe the effects of repetitive transcranial magnetic stimulation (rTMS) combined with rehabilitation training on motor cortex in children with motor retardation. Methods Sixty children with motor retardation were divided into treatment group and control group by random digits table method with 30 cases each. The children in control group were treated with rehabilitation training, and the children in treatment group were treated with rTMS combined with rehabilitation training. Two groups were treated for 3 courses. The Gesell pediatric neuropsychological scale and gross motor function measure (GMFM) scale before and after treatment were evaluated. Results There was no headache, dizziness and abnormal pathological reaction in the 2 groups. The motor behavior development quotient scores of Gesell pediatric neuropsychological scale after treatment in control group and treatment group were significantly higher than those before treatment: (64.37 ± 16.37) scores vs. (62.37 ± 14.21) scores and (74.50 ± 13.32) scores vs. (61.90 ± 13.76) scores, but the score in treatment was significantly higher than that in control group, and there were statistical differences (P<0.05). There was no statistical difference in language behavior development quotient score between after treatment and before treatment in control group: (79.57 ± 5.93) scores vs. (79.07 ± 5.75) scores, P>0.05. The language behavior development quotient after treatment in treatment group was significantly higher than that before treatment:(80.70 ± 5.38) scores vs. (78.57 ± 5.72) scores, and there was statistical difference (P<0.05). The A, B, C and D area scores of GMFM scale after treatment in control group and treatment group were significantly higher than before treatment, which in control group were (76.43 ± 19.18)% vs. (75.40 ± 19.38)%, (50.53 ± 27.63)%vs. (49.60 ± 28.22)%, (31.07 ± 24.93)%vs. (28.40 ± 23.70)%and (1.60 ± 1.33)%vs. (0.89 ± 0.37)%, and in treatment group were (85.80 ± 13.73)%vs. (79.13 ± 16.87)%, (65.77 ± 26.27)%vs. (49.37 ± 29.67)%, (49.60 ± 28.22)%vs. (28.83 ± 23.19)%and (2.10 ± 1.60)%vs. (1.07 ± 0.43)%, and there were statistical differences (P<0.05); the A, B and C area scores after treatment in treatment group were significantly higher than those in control group, and there were statistical differences (P<0.05); but there was no statistical difference in D area score after treatment between 2 groups (P>0.05). Conclusions The rTMS combined with rehabilitation training is better in gross movement than the single rehabilitation training for children with motor retardation. The rTMS can improve the motor function and language ability of children.

Objective To report the therapeutic results of Hybrid external fixator in treatment of open metaphyseal fractures of the tibia. Methods From March 2000 to July 2004,19 cases of proximal tibial meta- physeal fractures and six cases of Pilon fractures were treated with Hybrid external fixators.By Gustilo classification, eight cases were of typeⅡ,10 typeⅢA,five typeⅢB and two typeⅢC.The wounds were closed directly after thorough debridement in 21 cases, and closed by pedicled flap transposition or free flap transplantation in four. Fasciotomy was performed in three.Closed indirect reduction of the fractures was done in 11 cases, while open reduction was carried out through an extended wound or a limited incision in 14.Anatomical reduction was done for the articular fractures,if any,and Hybrid external fixators were employed to stabilize fractures in all the cases in this series. Results All the patients had a mean follow-up of 15.7 months (ranging from 9 to 26 months) which re- vealed bone union of the fracture in all cases with a mean healing time of 8.8 months. Two patients had their external fixators replaced with plates and iliac bone graft to get fracture union. The fixators were removed 2.9 to 21 months (mean,9.4 months) after operation.Except for two cases of secondary healing, all the wounds healed primarily without serious complication, such as infection. The final follow-up revealed a knee motion range of more than 90?in 15 cases, less than 70?in four, whose knee motion range was improved to more than 90?after knee release performed one year later. In all the six cases of Pilon fractures, the ankle joint recovered normal range of motion.Traumatic arthritis occurred in two cases of Piion fracture 11 months after operation.Evaluation by Rasmussen's functional and radiological scoring system demonstrated excellent knee function in 13 cases, good in three, fair in two and poor in one, with a good to excellent rate of 84.2%.The ankle function was excellent in three cases, good in two and poor in one, with a good to excellent rate of 83.3%.Conclusions In treatment of open tibial metaphyseal fractures, the Hybrid external fixator can well maintain the stability of the fragments with a low incidence of soft tissue com- plications and insignificant interference with the knee motion. However the bone healing time can be prolonged with a definite rate of nonunion.

Asparaginyl endopeptidases (AEPs) in plants belong to the family of cysteine protease that undergo self-activation in the form of zymogen in acidic vacuole and play important physiological roles in maturation of seed storage proteins, protein degradation, programmed cell death and host defense. Bioprocessing enzymes (peptidyl Asx-specific ligases, PALs) that promote the maturation of cyclotides have recently been isolated and identified from several cyclotide-rich plants. PALs derived from AEPs can site-specifically catalyze the formation of asparagine or aspartate peptide bonds. Due to the advantages of relatively traceless peptide bonds and broad substrate spectrum and high catalytic efficiency, they have been playing important roles in the cyclization and modification of peptides and proteins, and are powerful tools for improving the stability of peptide drugs. This review describes the physiological functions of AEPs in plants and summarizes the discoveries, structural characteristics, catalytic mechanism and protein engineering of PALs, as well as the limitation of their applications and future trends. In addition, the applications of PALs in cyclotides biosynthesis and the development of macrocyclic peptides are highlighted, with the aim of providing a new idea for the biocatalytic synthesis of cyclic peptides.

Objective To investigated the effect of procyanidin (PC) on the expression of cysteine proteinase -3 (Caspase -3) in type 2 diabetes mellitus SD rats with focal cerebral ischemia. Methods Following the random principle, 40 healthy Sprague - Dawley (SD) rats were numbered sequentially and randomly divided to normal rats with focal cerebral ischemia group, type 2 diabetes mellitus SD rats with focal cerebral ischemia group,PC low/ middle/ high -dose groups, with 8 rats in each group. The type 2 diabetes mellitus - MCAO model was set up. The doses of PC for low, middle and high - dose groups were 50 mg/ kg,100 mg/ kg,200 mg/ kg. Immunohistochemistry method was used to measure the activity of Caspase - 3. Results Compared with that in the normal rats with focal cerebral ischemia group[(11. 42 ±2. 52)],the expression of Caspase -3 increased in the type 2 diabetes with ischemia group[(15. 00 ± 2. 38)](t = 2. 17,P < 0. 01). Compared with that in the type 2 diabetes with ischemia group, the expression of Caspase - 3 decreased in the PC groups[(9. 38 ± 2. 00),(7. 71 ± 1. 55),(6. 96 ± 1. 57)](t = 2. 86,3. 13,3. 36,all P < 0. 01),whereby the middle and high - dose groups showed more significant decrease (t = 1. 92,2. 03,all P <0. 01) and with no statistically significant difference between the two groups(t = 1. 13,P > 0. 05). Conclusion PC can decrease the expression of Caspase - 3 protein in type 2 diabetes mellitus SD rats with focal cerebral ischemia, finally may inhibit the apoptosis.
