2.Effect of Air Formaldehyde Exposure of Medical Students on DNA-Protein Crosslinks in Their Buccal Mucosa Cells

Objective To study the effect of formaldehyde exposure on DNA-protein crosslinks (DPC) in buccal mucosa cells in students who were taking anatomy course.Methods The modified SDS-KCl precipitation assay published by Zhitkovich and Costa in 1992 was applied to detect DNA-protein crosslinks in human cells.And this method has been used to explore DPC induced by different pollutants in numerous studies.37 medical students (20 males and 17 females) aged 19.24?1.09 (mean?standard deviation) and 40 students (20 males and 20 females) in natural science college aged 19.55?0.99 (mean?standard deviation) were studied.The frequency of exposure to formaldehyde was 6 h per week in exposed group.Results Concentrations of formaldehyde in anatomy laboratory ranged from 0.42 to 1.57 mg/m3.Exposure to formaldehyde resulted in an increase of DNA-protein crosslinks.The percentage (mean?standard deviation) of DNA-protein crosslinks in exposed and nonexposed students were 25.72%?6.48% and 22.88%?5.34% respectively(P0.05).Conclusion The result of the present study suggests that formaldehyde exposure in medical students increases the frequency of DNA-protein crosslinks in buccal mucosa cells and females may be more sensitive to formaldehyde exposure.
3.EST combined with LC in the treatment of cholecystolithiasis with common bile duct stones

Objective To discuss the feasibility and superiority of EST combined with LC in treating cholecystolithiasiswith common bile duct stones.Methods Patients underwent LC(laparoscopic cholecystectomy) preceded by EST(endoscopic sphincterotomy) and removal of common bile duct stones.ENBD(Endoscopic nasal biliary drainage tube)was placed if EST failure and then the patient underwent LC+laparoscopic biliary duct exploration(LCBDE),or open operative bile duct exploration.Results In 91 of the 99 cases,LC preceded by EST and stone removal was successful,while EST following LC was successful in 3 cases,and stone removal by EST was unsuccessful in 3 cases.Two cases less than 15 years of age underwentLC+LCBDE through the cystic duct and did not undergo EST.Three cases,who had EST failure,underwent LC+LCBDE with primary suture of the common bile duct or LCBDE with T-tube drainage of commonbile duct,or open bile duct exploration with primary suture of the common bile duct(ENBD was in place).All patients were discharged from hospital without serious complications.Conclusions EST combinedwith LC in treating cholecystolithiasis with common bile duct stones is a safe and effective method.Combination of endoscopic and laparoscopic procedures fully reflects the advantage of minimally invasive therapy.

5.Clinicopathological significance and expression of Numb and α-catenin in gastric carcinoma

Objective To detect the expressions of Numb and α-catenin in the gastric cancer and matched normal gastric mucosa,and to analyze their relationship with clinicopathological factors of gastric cancer and explore their roles in gastric carcinogenesis and progression.Methods Immunohistochemical method was used to detect the expressions of Numb and α-catenin in 109 cases of tumor specimens and 97 cases of matched normal gastric mucosa.The correlations between Numb,α-catenin and clinicopathological factors were analyzed.Results The positive rates of Numb and α-catenin in gastric cancer were significantly lower than those in normal gastric mucosa (60.6% ∶ 100.0%,x2 =48.361,P =0.000;76.1% ∶ 100.0%,x2 =26.480,P=0.000).The expression of Numb protein was correlated with Lauren type (x2 =17.018,P =0.000) and tubular adenocarcinoma differentiation (x2 =17.586,P =0.000).The expression of α-catenin protein was correlated with Borrmann type (x2 =6.700,P =0.035),Lauren type (x2 =11.098,P =0.001),tubular adenocarcinoma differentiation (x2 =8.203,P =0.017) and lymph node metastasis (x2 =6.402,P =0.011).The expressions of Numb and α-catenin were statistically correlated in 109 cases of gastric cancer (rk =0.184,P =0.028).Conclusion Compared with normal gastric mucosa,both Numb and α-catenin expressions are down-regulated and the two expressions are correlated in gastric cancer.Numb and α-catenin may be involved in the regulation of histological differentiation of gastric cancer by certain passages,Numb protein may be adjusted by α-catenin pathway which is involved in Wnt-β-catenin pathway,and thus plays an important role in promoting cell proliferation,invasion and metastasis in human gastric cancer.
6.Characteristic and treatment of ankle fracture combined with Tillaux-Chaput and Volkmann fractures

Objective To explore the clinical characteristics and operative methods of the ankle fracture combined with Tillaux-Chaput and Volkmann fractures.Methods The data of 15 patients who were suffered the ankle fracture combined with the simultaneous fractures of Tillaux-Chaput and Volkmann between Septenber 2005 and January 2012 were analyzed.There were 8 males and 7 females with an average age of 27.3 years (range,16-57 years).All had medial malleolar and fibular fracture with Tillaux-Chaput and Volkmann tubercle avulsed fracture.X-ray film shows that there were 8 cases in which the patients had Tillaux-Chaput fracture,among whom 7 were diagnosed by CT scan.Classified by Lauge-Hansen system,ankle fracture could be divided into pronation-exterual rotation type in 4 cases,and pronation-abduction type in 11 cases.The fracture of fibula and the Volkmann tubercle were treated through the posterolateral approach.The fibular fracture was fixed with plate.The medial malleolar fracture was explored from medial curve approach.Tillaux-Chaput fracture was treated by the mierotubule cut.All patients were assessed with the ankle hindfoot clinical rating system of the American Orthopaedic Foot and Ankle Society (AOFAS).Results All patients were followed up for 8-20 months (average,12.5 months).The fracture healed 12-36 weeks later in all the 15 patients,with an average of 23.5 weeks.Twelve patients walked normally and 3 with mild claudication in the last follow-up.The AOFAS score was average 85.4 points,9 were excellent,4 were good,2were fair,with the good-excellent rate being 86.7% (13/15).The movement of ankle joint limited in 3 patients,tenderness occurred when weight bearing in 3 patients.Conclusion The ankle fracture include the simultaneous fractures of Tillaux-Chaput and Volkmann were more seen in the type of pronation-abduction.It is often misdiagnosis by the photograph; CT scan should be helpful to find the fracture fragments.The exact reduction and stable fixation were needed in the treatment of the ankle fracture combined with Tillaux-Chaput and Volkmann fractures.Inferior tibia fibular screw was not routinely used in this fracture.
7.Effect of early food intake for patients undergoing non-spinal orthopedic surgery

Objective To observe the effect of early food intake for the patients undergoing orthopedic surgery under intravertebral anesthesia or general anesthesia.Methods A total of 52 cases of orthopedic surgeries under intravertebral anesthesia or general anesthesia from the orthopedic department in the First Hospital of Tsinghua University were randomly divided into an experiment group (n=30) and control group (n=22).The experiment group followed the orders of their doctor,allowed to eat food any time after the anesthesia wore off (without exhaustion beforehand) and the control group was asked to eat after 6~8h.The two groups were compared in terms of the first time for exhaustion,the feeding amount for the first time and the symptoms for the first time.Results The first time for exhaustion in the experiment group was earlier than that of the control group,showing significant difference between them (P<0.05).The amount of food intake of the two groups was insignificantly different (P>0.05).After the first feeding,the patients in the experiment group had no nausea/vomiting and abdominal distension,and the incidence of thirst and hunger was significantly lower in the experiment group than that in the control group (P<0.05).The discharge time was significantly earlier than that in the control group (P<0.05).Conclusion It is feasible for patients undergoing orthopedic surgery to eat food early.Patients are able to eat food once they are conscious and it is not necessary to wait until gastrointestinal function fully recovered.

10.Cavins: new sights of caveolae-associated protein.

Caveolae are specialized lipid rafts that form flask-shaped invaginations of the plasma membrane. Many researches show that caveolae are involved in cell signaling and transport. Caveolin-1 is the major coat protein essential for the formation of caveolae. Recently, several reports indicated that the other caveolae-associated proteins, Cavins, are required for caveola formation and organization. It's worth noting that Cavin-1 could cooperate with Caveolin-1 to accommodate the structural integrity and function of caveolae. Here, we reviewed that the relationship between Cavins and Caveolins and explore the role of them in regulating caveolae.
