1.ADM LIPOSOME THERAPY IN THE TREATMENT OF FORTY FIVE PATIENTS WITH ADVANCED LIVER CANCER

Objective:To compare the survival rate and side effects of doxorubic in (ADM) liposome therapy and ADM therapy for liver cancer.Methods:87 patients with advanced liver cancer were divided into two groups:Group A was treated by ADM liposome chemotherapy and group B by ADM chemotherapy.Results:The mean survival and 1 year survival rates showed no obvious difference in both groups.However,patients in Group A showed better immediate curative effect and lower immediate side effects than Group B.Conclusion:ADM Liposome chemotherapy increase the immediate curative effects and decrease the general reactions.
2.Morphine by supersound atomizer for postoperative pain relieve in gynecology and obstetrics patients

Objective To evaluate the efficacy and safety of morphine by supersound atomizer for the management of postoperative pain in gynecology and obstetrics patients.Methods Eighty ASA class III patients scheduled for elective mixed gynecology and obstetrics surgeries were randomly and single-blindly located into 4 groups.Patients in each group received different analgesia therapy 30 rain before the end of the surgeries,as inhaling morphine 15 nag in group MI,inhaling morphine 20 nag in group M2,inhaling normal saline 10 ml in group N and intravenous patient-controlled analgesia (PCA) in group PCA.Open-label rescue analgesia of intramuscular injection pethidine 50 mg or intravenous PCA was also available as needed.Pain scores were measured at baseline,15 min,30 min,1 h,2 h,3 h,4 h,6 h,8 h and 24 h after the extubation using visual analng score ( VAS ),vital signs,adverse events,and the uses of rescue analgesia were also recorded.Results The VAS of group M2 were significantly lower than that of group N and group M1.The VAS at 15 min,1 h,6 h,8 h,24 h of group M2 were significantly lower than that of group PCA(P < 0.05 or <0.01 ).The VAS at 15 min,30 min,1 h,2 h,4 h,6 h of group M1 were significantly lower than that of group N (P < 0.05 or < 0.01 ) postoperatively,which were significantly higher than that of group PCA except for 30 min.The morbidity of postoperative,nausea and vomiting in group M1 and group M2 were significantly higher than those in group N and group PCA.The rescue analgesia was more performed in group M 1 and group N than that in group M2 and group PCA.Conelusions Inhalation of morphine by supersound atomizer via intratracheal tube may produce safe and satisfying analgesic effect in postoperative pain model of gynecology and obstetrics patients.More studies are needed to determine what,if any,the optimum dose of morphine is for postoperative pain relieving and the possible mechanism.
3.Quality Control of Taraxacum Mongolicum Based on Comprehensive Score Method

Objective: To study the effects of different origins, collection and processing methods on the quality of Taraxacum mongolicum. Methods:The HPLC fingerprints of Taraxacum mongolicum were established. Totally 11 batches of Taraxacum mongoli-cum were analyzed by similarity evaluation and cluster analysis. Results:According to the results of HPLC, 11 batches of Taraxacum mongolicum had good baseline separation and showed 5 common peaks. Based on the established HPLC method, the quality of different batches of Taraxacum mongolicum showed difference according to the results of similarity evaluation and cluster analysis. The quality of batch 121231-1 was the best. Conclusion:The origin, collection and processing method show notable influence on the quality of Ta-raxacum mongolicum, and the comprehensive score method can be applied in the quality evaluation of Chinese herbs.
4.Antibacterial Use by Line Category in Pediatric Outpatient and Emergency Departmeats of a Hospital:A Survey

OBJECTIVE To know the clinical antibacterial use by line category in pediatric outpatients and emergency departments.METHODS The antibacterial use by line category from the 38998 prescriptions of pediatric outpatient and emergency departments of a hospital from Jul 1st to Dec 31st 2008 according to the medicament amount and frequency were surveyed and analyzed.RESULTS First-line antibacterial drug DDDs accounted for 37.4% and 24.7% of the total amount;second-line drug DDDs accounted for 48.7% and 40.8% of the total amount and the third-line drug DDDs accounted for 13.9% and 35.0% of the total amount.The top three by total amount were cefmetazole sodium injection,azithromycin sodium injection and Cefuroxime sodium injection.According to the sorting method of DDDs,the top three were cefprozil dry suspension,azithromycin sodium injection and amoxicillin sodium clavulanate potassium injection.CONCLUSIONS Frequency of the third-line antibacterial drug using is relatively high.Clinicians have a tendency to skip a line category in using drugs.Effective measures should be taken to strengthen the management of antibacterials.
6.To study expression and recurrence significance of uniting soluble E-cadherin and AFP pre-and postoperative in hepatocellular carcinoma

Background and purpose: E-cadherin (EC) is a type of glycoprotein, which could maintain epithelial polarity and cell-cell junction. It is thought to be associated with the development and progression of hepatocellular carcinoma (HCC). The study aimed to detect expression of uniting detection sEC and AFP before and after operation in patients, and to monitor recurrence of postoperative. Methods:The levels of sEC and AFP in 105 HCC patients were tested by enzyme linked immunosorbent assay (ELISA) and radioimmunoassay (RIA) before and after operation as well as in 70 healthy controls. Theirs correlation with clinical and pathological factors was analyzed. We analyzed the level of plasma sEC and AFP of 47 patients who had been suffered from the hepatocellular carcinoma operation and relapse. Results: The levels of sEC and AFP in HCC patients before operation were significantly higher than in controls, which declined significantly one week after operation. The sEC level before operation showed significant correlation with TNM staging (P<0.01), edmondson grade, the carcerous thrombi within the portal vein and the capsule of carcinoma (P<0.05), but not with age of patients or the tumor size (P>0.05). While AFP only correlation with TNM staging (P<0.05), not with others factors (P>0.05). When the recurrence was confirmed, the level of sEC was much higher significantly than the level postoperation (P<0.01). Conclusion:The levels of sEC in HCC patients before operation are closely related with prognosis and recurrence of HCC. The unites detection of sEC+AFP may be a predictive factor for recurrence of postoperation.
7.Effects of nimodipine on reducing the complications after microvascular decompression in patients with hemifacial spasm

Objective To investigate whether nimodipine could reduce complications after microvascular decompression in patients with hemifacial spasm. Methods 220 patients with hemifacial spasm , admitted into our hospital from May 2011 to Oct. 2013 , were all treated by microvascular decompression. They were divided randomly into two groups. Patients in the treatment group were treated with nimotop 50 mg/d after the operation for the first 3 days, then was orally taken 30 mg/d for a week. Patients in the control group were treated with the same after the operation without nimodipine. The postoperative complications including hearing deficit and peripheral facial paralysis were closely observed within three months after operation. Results The total effective rate was 100% in both groups. The incidence rate of peripheral facial paralysis and hearing deficit in treatment group was significant lower than that of the control group (P < 0.05). Conclusion Nimodipine has significant effect on reducing the incidence rate of peripheral facial paralysis and hearing deficit after microvascular decompression in patients with hemifacial spasm.
8.Effect of Overexpression of NAD+-dependent Malic Enzyme on Anaerobic Mixed Acid Fermentation of E.coli FMJ39

In order to investigate the effect of malic enzyme on anaerobic mixed acid fermentation,the gene encoding NAD+-dependent malic enzyme was amplified from E.coli DH5? genome by using PCR and cloned into vector pTrc99a to give an expression vector pTrc99a-sfcA.Malic enzyme was overproduced by E.coli FMJ39(ldh,pfl)harboring pTrc99a-sfcA under the condition of IPTG induction.The results of anaerobic fermentation indicated overexpression of malic enzyme would have influence on the pathway of formate,acetate and succinate of FMJ39.The concentration of formate and acetate was 17.58% and 15.27% higher than FMJ39,the succinate concentration was reduced by 26.87%.There was no obvious citrate concentration change.The experimental results also indicated that high concentration of L-Thr and L-Ser would induce the Tdc operon to convert pyruvate into formate and acetate even when pfl gene encoding the pyruvate formate lyase was inactivated.
9.Primary Study of Producing UMP by Overexpressing URA 5 and URA 3 Genes in Saccharomyces cerevisiae

To increase the biotransfomation efficiency from the orotic acid to the uridine 5'-monophosphate(UMP),URA5 gene encoding orotate phosphoribosytransferase was amplified from Saccharomyces cerevisiae BY4742 by PCR,then it was inserted into the expression vector pYX212(contained orotidine monophosphate decarboxylase gene URA3)and the pYX212-URA5 was transformed into Saccharomyces cerevisiae BJX12 by electroporation.The recombinant strain was elementarily used to convert orotic acid to UMP.The results showed that pYX212-URA5/BJX12 could accumulate 7mmol/L UMP from 32mmol/L orotic acid in 26h,significantly higher than both control groups pYX212/BJX12(2.7mmol/L) and BJX12(2.4 mmol/L).
10.Determination of valsartan in human plasma by HPLC

AIM To establish a high performance liquid chromatographic method (HPLC) to determine the concentration of valsartan in human plasma. METHODS Separation was achieved on the lichrospher C 18 column. The mobile phaseconsisted of pH 3 1 phosphate buffer acetonitrile (53∶47, V/V) was used at a flow rate of 1 0 ml?min -1 . The fluorimetric excitation and emission wavelengths were set at 265 nm and 378 nm, respectively. The plasma samples were acidified with HCl, extracted with ethyl acetate. Separate the organic phase, remove the solvent and then residue was dissolved in mobile phase. RESULTS The retention time of valsartan was 12 5 min. The calibration curves were linear in the range of 5 9~ 2 360 ?g?L -1 . The precision values (RSD) of intra day and inter day were determined to be 2 83%~7 07% and 1 57%~8 41% respectively. The absolute recovery rate were 80 30%?5 13%. The method was applied to determine the peak and valley concentrations in plasma of the hypertensive treated with 80mg valsartan per day. CONCLUSION The assay was sensitive and simple. It is suitable for the study of the pharmacokinetics of valsartan.
