1.The Treatment of Posterior Circulation Ischemia
International Journal of Cerebrovascular Diseases 2006;0(11):-
Posterior circulation iscbemia refers to the transient iscbemic attack and the cerebral infarction in vertebrobasilar artery system, its prognosis is not as good as anterior circulation ischemia, and its treatment is similar to that of anterior circulation ischemia. However, because of the mortality of basilar artery occlusion is higher, usually more aggressive treatment should be considered, such as thromholytic therapy(especially intra-arterial thromholysis)and endovas- cular therapy, and the therapeutic time window is much longer. This article reviews the treatment of anterior circulation ischemia.
2.Meta-analysis of repeated endoscopic sclerotherapy and ligation for prevention of esophageal variceal rehaemorrhagia in cirrhosis
Journal of Shanghai Jiaotong University(Medical Science) 2006;0(04):-
Objective To evaluate the efficacy and safety of repeated endoscopic sclerotherapy injection(ESI) and endoscopic variceal ligation(EVL) in eradication of esophageal varices among patients who survive an episode of first variceal hemorrhage with a high risk of rehaemorrhagia and death. Methods The correlated literatures were identified from Medline,Embsae, the Library Cochrance,PubMed and CNKI.RevMan 4.2 Software provided by the Library Cochrance was used for data analysis. Results A total of 4 randomized controlled trials were included. It was demonstrated that there was no significant difference in the esophageal varices eradication rate between repeated ESI and EVL(OR=0.75,95%CI: 0.48-1.15;P=0.19).However,the rehaemorrhagia rate of EVL after emergency hemostasis was significantly lower than that of ESI(OR=2.19,95%CI:1.44-3.31;P=0.000 2).Meanwhile,there was no significant difference in mortality between ESI and EVL(OR=1.34,95%CI:0.82-2.17;P=0.24).Little publication bias was found with funnel plot analysis. Conclusion EVL outperforms ESL in prevention of rehaemorrhagia in treatment of esophageal varices, while does no better than ESL in eradication of esophageal varices and mortality.

Objective:To evaluate the clinical significance of pattern visual evoked potential (P-VEP) parameters on amblyopic patients with normal-vision atfer pleoptic therapy. Methods:We investigated 60 amblyopic children (8-12 years old) who gained normal-vision atfer pleoptic therapy. hTese patients were assigned to a unilateral amblyopia group (40 patients)and a bilateral amblyopia group (20 patients). Another 20 healthy children served as a control group. All patients underwent a full initial ophthalmologic and orthoptic evaluation. P-VEP test was performed in all. Amplitude and latencies were analyzed and compared among groups. The latencies of P100 waves in the amblyopic eyes were used to generate a multiple linear regression formula from sex, ifrst treatment age, baseline visual acuity, and cycloplegic refraction. Results:hTere was no signiifcant difference in the mean levels of best-corrected visual acuity among groups (P>0.05). A signiifcant prolongation of the latency and a decrease of amplitude of P100 waves were observed in the unilateral amblyopia group and the bilateral amblyopia group compared with the healthy control group (P<0.05). Amplitude and latencies of the fellow eyes in the unilateral amblyopia group were abnormal compared with the healthy control group (P<0.05). Multiple linear regression analysis revealed that the latencies of P100 waves were signiifcantly correlated with the ifrst treatment age, baseline visual acuity, and cycloplegic refraction (R2=0.52, P<0.05). Conclusion:Deifcits exist in the fellow eyes and in normal-vision eyes atfer pleoptic therapy. hTe delayed P100 latency is affected by the ifrst treatment age, baseline visual acuity, and cycloplegic refraction. Traditional amblyopic therapy may be not enough for vision function recovery.

Objective: To establish an HPLC-fluoremetry method for determination of the plasma concentration of mycophenolic acid ( MPA) in renal transplantation patients. Methods:The sample was subjected to precipitate proteins using acetonitrile, and the supernatant (20 μl) was used for the sample injection and determination on a Hypersil BDS C18 (250 mm × 4. 6 mm, 5 μm) column with temperature at 25℃. The mobile phase consisted of acetonitrile-methanol-0. 2 mol·L-1 dipotassium hydrogen phosphate buffer (pH=9. 0)( 18∶2∶80)with the flow rate of 1. 0 ml·min-1. The excitation wavelength (Ex) was 342 nm and the emission wave-length ( Em) was 425 nm. Results:The endogenous plasma impurities and drug combinations had no interference with the determina-tion. The calibration curve was linear over the range of 0. 25-50. 00 mg·L-1(r=0. 999 7), and the lower limit of quantification was 0. 25 mg·L-1 . The mean methodological recovery was 99. 12% and the mean extraction recovery was 93. 27%, the intra-day RSD was less than 2% and the inter-day RSD was less than 6%. Totally 10 cases of renal transplantation patients were with mycophenolate mofetil at the dose of 0. 5-1. 75 g·d-1 , and MPA in plasma was within the range of 0. 43-21. 58 mg·L-1 . Conclusion:The method is rapid, sensitive, accurate and convenient, which can be used in the quantitative determination of plasma concentration of MPA in re-nal transplantation patients.

Objective To investigate the effectiveness of training at morning shift meeting in improving healthcare-associated infection(HAI) prevention and control among health care workers(HCWs) in newly established hospital departments. Methods On July 11-22, 2016, HAI management full-time personnel performed training for HCWs about HAI prevention and control at morning shift meeting of 8 newly established clinical departments and 1 laboratory, the questionnaire survey was conducted before, immediately after, and 3 months after training, training effectiveness was evaluated. Results A total of 239 HCWs were required to receive training, including 85 doctors (35.6%), 150 nurses (62.8%), and 4 technicians (1.7%).The knowledge of HAI prevention and control that most HCWs wanted to know was occupational exposure and occupational precaution(85.6%);most HCWs wanted to obtain knowledge of HAI through training in their respective department by HAI management full-time personnel(87.1%).Total awareness rates of HAI knowledge before, immediately after, and 3 months after training were 45.1%, 96.7%, and 83.9% respectively, difference was significant(P<0.001);comparison among groups showed that HCWs'' awareness in influencing factors of HAI, hand hygiene opportunity, isolation measures, and medical waste classification 3 months after training was significantly lower than immediately after training(all P<0.001);the average scores of HAI prevention and control knowledge among HCWs immediately after and 3 months after training were both higher than before training(P<0.001);average score of HAI prevention and control knowledge among HCWs 3 months after training was lower than that of immediately after training(P<0.001).Conclusion The training at morning shift meeting of clinical departments can improve the awareness of HAI prevention and control knowledge among HCWs.

Objective To study the innervation between baldder and rectum. Methods The retrograde fluorescent double labeling method was used.Propidium Iodide(PI)was injected into the right wall of the rectum of male SD rats and Bisbenzimide(Bb)into their right wall of urinary bladder. Results The fluorescent double labeled neurons were found in the right L6-S3 and L1-L3 dorsal root ganglia(DRG).Conclusion DRG cells at L6-S3 and L1-L3 simultaneously innervate the rectum and the urinary bladder, explaining that the abnormal micturition due to chronic constipation may be caused by axis reflex at the level of the peripheral process of DRG cells.
