2.Clinical observation of retrobulbar injection of triamcinolone acetonide combined 577 nm laser macular grid photocoagulation for the treatment of cystoid macular edema

AlM:To evaluate the efficacy of retrobulbar injection of triamcinolone acetonide ( TA ) combined with 577nm laser macular grid photocoagulation for the treatment of cystoid macular edema.
METHODS: Fifty-eight cases ( 66 eyes ) with cystoid macular edema caused by different diseases were recruited in this study. The included patients were treated with both retrobulbar injection of triamcinolone acetonide and 577nm laser macular grid photocoagulation. The best corrected visual acuity, macular thickness, fundus and intraocular pressure were observed in the 1wk, 1 and 3mo after the treatment in all of the included cases.
RESULTS: After treatment, all of the 66 eyes showed cystoid macular edema partially or completely subsided according to optical coherence tomography and fluorescence fundus angiography; 54 eyes ( 82%) visual acuity improved, 12 vision remained the same.
CONCLUSlON: Retrobulbar injection of triamcinolone acetonide combined with 577nm laser macular grid photocoagulation has good curative effect, simple operation procedure and rare complications in the management of cystoid macular edema.

Objective To explore the clinical features,diagnosi s and prognosis of incontinentia pigmenti.Methods Analyzing and summarizing the clinical characteristic, diagnosis and prognosis of neonatal incontinentia pigmenti in 6 neonatal infants that were hospita- lized in our department during the period from January 1 998 to December 2003 were studied,and some relevant literature were reviewed. Results 1.Three of 6 infants were male which was unusual;2.Four infants had typical skin lesions at birth and 1 case at 6 days old.Four cases had typical 3 stages o f skin lesions including the erythematous and vesicular inflammatory stage,verr ucous lesions and hyperkeratosis stage,macular hyperpigmentation stage,but the re was overlap;3.Four infants were complicated by central nervous system involv ement (two cases presented mental retardation,2 infants were temporary damage). Two cases were complicated by ocular manifestations ( one case had optical nerve atrophy and blind in left eye,the other had severe bilateral retinal lesions); 4.On specific examination 5 infants were diagnosed by skin biopsy.Gene analysis was made in 1 case,but we didn′t find the mutations of NEMO. Conclusions Incontinentia pigmenti is a rare X-linked dominant multisystem disease.It may be misdiagnosed in the initial stages.Except typical clinical features,skin biops y and gene analysis are main evidence for diagnosis.Early detection and interven tion are important for prognosis. 5.Case of Infant with Congenital Dermal Sinus Complicating Multiple Intramedullary Spinal Abscess

Objective To improve the recognition of intramedullary spinal abscess by a case of congenital dermal sinus with intramedullary spinal abscess and reduco the incidence of congenital dermal sinus with intramedullary spinal abscess.Methods Clinical,laboratory data and image of a confirmed case about one infant of congenital dermal sinus with multiple intramedullary spinal abscess were investigated,the related literature was reviewed.Results In this case,when the infant with congential dermal sinus had infection,he failed to gain antibiotic therapy, timely surgical treatment,his infection had diffused, and multiple intramedullary spinal abscess flared up.Conclusions Intramedullary spinal abscess is a rare disease.If treatment is delayed, the prognosis is poor and the mortality rate is high.MRI is the ideal investigation for diagnosis.Intramedullary spinal abscess can happen subsequent to congenital dermal sinus with infection, and cause neurological sequela. So an infant with congenital dermal sinus should be offered to avoid complication caused by infection.
6.Mongolian medicine cha gan beng ga regulated activity of biomarker PGC-1α.

OBJECTIVETo investigate the regulation of Cha Gan Beng Ga on the activity of biomarker PGC-1α in vivo and in vitro, and lay the foundation for studying the efficacy result of Cha Gan Beng Ga on xenograft tumor model and extracting active constituents.
METHOD(1) The coarse powder of Cha Gan Beng Ga was extracted with 70% ethanol solution through heating and refluxing, and finally was used to freeze dry powder. (2) 50 mg x kg(-1) of freeze-dried power was orally administrated to KM and C57BL/6J mice once daily, lasting for 5 consecutive days; different concentrations of extracted materials was given to non-small cell lung cells A549. (3) The expression level of PGC-1α mRNA was quantitatively determined in lung tissue of mice and non-small cell lung cells A549.
RESULTThe expression levels of PGC-1α in lung tissue of different mice strains had an increasing tendency. Furthermore, the expression levels of PGC-1α in non-small cell lung cells A549 also had an increasing tendency, showing dose and time-dependent relationships.
CONCLUSIONMongolian Medicine Cha Gan Beng Ga could induce the over-expression of PGC-1α mRNA in lung tissue of mice and in non-small cell lung cells A549. The present results will lay foundation for studying the efficacy result of antitumor and active constitutes in future.

7.Sodium and potassium disturbances and their risk factors in 9870 elderly inpatients

Objective To investigate the sodium and potassium disturbances in 9870 elderly inpatients and to analyze their risk factors.Methods Clinical data of sodium and potassium levels and the possible risk factors were collected in the elderly inpatients from a single center.The incidence of sodium and potassium disturbances and their risk factors were analyzed by multiple Logistic regression,and the correction of the imbalance was investigated.Results A total of 6027(61.6%)times of sodium and potassium disturbances were found in the 9870 elderly inpatients on admission and during hospitalization,and the total incidence of this disturbances was 61.6%.The incidences of hyponatremia,hypokalemia,hypernatremia and hyperkalemia were 27.9% (2729/6027),9.7% (951/6027),15.4%(1506/6027) and 8.6% (841/6027),respectively.Heart failure was the common risk factor for the two electrolyte disturbances.T2DM caused hyponatremia,hypokalemia and hypernatremia.Among the medication risk factors,diuretics were the common risk factor for various electrolyte disturbances.Patients taking antiepileptics,antidementia drugs,antidepressants and benzodiazepines were more likely to have hyponatremia.The correction rate of mild,moderate and severe hyponatremia/ hypokalemia were 79.2%(1253/1582),68.1% (535/786),45.1% (163/361) and 79.5%(776/976),66.4%(217/327),40.1%(81/203),respectively.The time for the correction of the above degrees of hyponatremia/ hypokalemia were(3.7±2.7) d,(4.1±2.3) d,(8.9±1.6) d and(2.5± 1.4) d,(3.2 ± 1.5) d,(6.1 ± 1.2) d.The supplement amounts of sodium chloride and potassium chloride were(5.98±3.67) g,(9.45±3.02) g,(10.26±1.32) g and(2.23±0.93) g,(5.12± 1.53) g,(8.07 ± 2.46) g,respectively.Conclusions The incidences of electrolyte disturbances are high in elderly inpatients with combined diseases and application of various drugs,and the correction of disturbances is difficult.More attention should be paid to the electrolyte disturbances,which should be corrected positively.
8.Effect of modified bowel preparation in patients with constipation undergoing colonoscopy

Objective To evaluate the effect of modified bowel preparation-Polyethylene Glycol Electrolytes Powder (SF-PEG, Shu Taiqing) in combination with magnesium sulfate in patients with constipation undergoing colonoscopy. Methods Sixty cases from mild to moderate constipation were randomly divided into two groups:Group A: SF-PEG (438.4 g, 4 L), 30 cases; Group B: received low-dose SF-PEG (219.2 g, 2 L) and 50.00%magnesium sulfate 50 ml, and then 250 ml water, 30 cases. Boston Bowel Preparation Scale scores (BBPS) and bubbles in bowel lumen score were assessed by the same endoscopist blindly to the preparation regimen, and the time of colonoscopy and the tolerability was recorded. Results All patients underwent completely the bowel preparation and colonoscopy. The time of colonoscopy of group B was shorter than that of group A (P < 0.05). The BBPS score of group B was higher than that of group A (P < 0.05). The bubbles in bowel lumen were less in Group B than in Group A (P < 0.05). The willingness to retake in Group B were superior to those in Group A (P < 0.05). No significant difference was found in score of total adverse effects and taste score between the two groups (P > 0.05). Conclusion SF-PEG in combination with magnesium sulfate in patients with constipation for colonoscopy can improve tolerance to bowel preparation, cleanliness and effects of examination. It's safe to be recommended in clinical practice.
9.Surgery for left ventricular aneurysm after myocardial infarction: techniques selection and results assessment.

BACKGROUNDThe most appropriate surgical approach for patients with post-infarction left ventricular (LV) aneurysm remains undetermined. We compared the efficacy of the linear versus patch repair techniques, and investigated the mid-term changes of LV geometry and cardiac function, for repair of LV aneurysms.
METHODSWe reviewed the records of 194 patients who had surgery for a post-infarction LV aneurysm between 1998 and 2010. Short-term and mid-term outcomes, including complications, cardiac function and mortality, were assessed. LV end-diastolic and systolic dimensions (LVEDD and LVESD), LV end-diastolic and end-systolic volume indexes (LVEDVI and LVESVI) and LV ejection fraction (LVEF) were measured on pre-operative and follow-up echocardiography.
RESULTSOverall in-hospital mortality was 4.12%, and major morbidity showed no significant differences between the two groups. Multivariate analysis identified preoperative left ventricular end diastolic pressure > 20 mmHg, low cardiac output and aortic clamping time > 2 hours as risk factors for early mortality. Follow-up revealed that LVEF improved from 37% pre-operation to 45% 12 months post-operation in the patch group (P = 0.008), and from 44% pre-operation to 40% 12 months postoperation in the linear group (P = 0.032). In contrast, the LVEDVI and LVESVI in the linear group were significantly reduced immediately after the operation, and increased again at follow-up. However, in the patch group, the LVEDVI and LVESVI were significantly reduced at follow-up. And there were significant differences in the correct value changes of LVEF and left ventricular remodeling between linear repair and patch groups.
CONCLUSIONSPersistent reduction of LV dimensions after the patch repair procedure seems to be a procedure-related problem. The choice of the technique should be tailored on an individual basis and surgeon's preference. The patch remodeling technique results in a better LVEF improvement, further significant reductions in LV dimensions and volumes than does the linear repair technique. The results suggest that LV patch remodeling is a better surgical choice for patients with post-infarction LV aneurysm.

10.Inhibitory effect of sinomenine on H2O2-induced apoptosis in neonatal rat cardiomyocytes.

OBJECTIVETo study the effects of sinomenine on apoptosis in cutured neonatal rat cardiomyocytes induced by H2O2 and its possible mechanism.
METHODH2O2 was used to build an oxidative stress-induced injury model in neonatal rat cardiomyocytes after being treated with sinomenine (10, 30, 100 micromol L(-1)), the apoptosis rate, the content of malondialdehyde (MDA), the activity of superoxide dimutase (SOD), the activity of lactate dehydrogenase (LDH) and expression of NF-kappaB protein of the Cardiomyocytes were examined.
RESULTCompared with the model group, the apoptosis rate and the content of MDA, LDH decreased greatly (P < 0.01), and the activity of SOD increased distinctly (P < 0.01) after being treated by sinomenine (10, 30, 100 micromol x L(-1)).
CONCLUSIONSinomenine can inhibit the apoptosis induced by H2O2 in neonatal rat cardiomyocytes. The protective mechanism could be related to its ability to reduce lipid pexosidation and to inhibit cardiomyocyte expression of NF-kappaB protein.
