Objective Currently, the effect of hydrogen peroxide solution( H2 O2 ) flush on the recovery of peripheral nerve injury are still undetermined. This study aims to explore the effects of different H2 O2 flushing time course on recovery of peripheral nerve injury during surgical debridement. Methods 18 New Zealand male white rabbits were randomly divided into isotonic saline group( isotonic saline flush) , H2 O21 min group ( hydrogen peroxide solution flush 1 min + isotonic saline flush) , H2 O23 min group ( H2 O2 flush 3 min + isotonic saline flush. Each group has 6 rabbits. We established rabbit sciatic nerve injury model. After 12 weeks, we executed the animals, drew materials at the suture nerve, detected motor nerve conduction velocity and observed pathological HE staining, as well as measuring the sciatic nerve function indexes( SFI) of rabbits at the 8th and 12th week, respectively. Results Nerve conduction velocity in H2O21min group and isotonic saline group[(25.89±1.42), (26.46±1.67) m/s] were higher than that in the H2O23min group[(23.80±2.91)m/s](P<0.05). Pathological section shows that the isotonic saline group has a better sheathstructure, nerve fibers structure damage is not obvious and myelin degeneration degree is mild in H2 O21 min group. blurred sheath structure and myelin axons obvious deformation in H2 O23 min group. 8 weeks and 12 weeks after operation, SFI in isotonic saline group [(-43.00±0.82), (-39.17±0.91) m/s] was significant higher than H2O21 min group [ (-49.79±1.47), (-46.60±0.79) m/s] and H2O23min group [(-51.66±1.27), (-46.39±0.94) m/s], thedifference was statistically significant (P<0.05). SFI in group of H2O21min [(-49.79±1.47)m/s] was higher than group og H2O23 min [(-51.66±1.27) m/s][P<0.05]. Conclusion H2O2 solution has side effect on the recovery of damaged peripheral nerve inju-ry and with prolonging the H2 O2 flushing time, the damage of peripheral nerve can be aggravated. So in the process of clinical surgical debridement, under the premise of guaranteed sterilization the time of H2 O2 flush should be shortened as far as possible.

Objective To explore the value of free anterolateral thigh flaps in the reconstruction of maxillofacial defects after resection.Methods Patients were recruited from January,2008 to March,2016 in the Department of Oral and Maxillofacial Surgery of Nanjing University.All patients sunder when reconstructive surgery employing anterolateral thigh myocutaneous flaps follow-up of 3 months to 84 months after operation.Results Among 422 cases,survived completely with satisfied configuration,the survival rate was 98.6％.Postoerative vessel thrombosis occurred in flaps,21 patients required operative exploration in the perioperative period including 10 flaps with thrombotic events and 11 flaps with venous thrombosis were complete survival after the salvages,and 6 flaps were failure.The mean follow-up was 36.5 months (range:3 to 84 months).Conclusion The free anterolateral thigh flap is an ideal choice for reconstruction of maxillofacial defects,as it has many advantages such as abundance volume,easily molding and adjusting the thickness,possessing,long vascular pedicle,composition need to be adjusted according to defects,easily concealed donor site,and little donor site complications and malformation.

Objective To observe the changes of peptide YY (PYY) and its receptors in rats with ulcerative colitis (UC) by detecting both the serum level of PYY and jejunum epithelial cells in UC rats. Methods Rats were randomly divided into UC group, diarrhea-irritable bowel syndrome (D-IBS) group and control group. We measured the serum level of PYY by radioimmunoassay and made radioligand analysis of two basic parameters reflecting the characteristics of PYY receptors: dissociation constant (Kd) and maximum binding capacity (Bmax). Results The serum level of PYY was higher in UC and D-IBS groups than in normal group (P<0.001), and it was higher in UC group than in D-IBS group (P<0.001). However, the values of Kd and Bmax in UC group did not differ significantly from those in D-IBS and normal groups (P>0.05). Conclusion The serum level of PYY in UC group was significantly higher than that in normal group and D-IBS group; therefore, we assume that the change of serum PYY level may be related to not only the symptom of diarrhea but also inflammation. Kd and Bmax in neither UC group nor D-IBS group were significantly different from those in normal group, which indicates that the symptom and inflammation in UC may have nothing to do with the changes of PYY receptors.

Objective:To obtain the high expression of Herpes Simplex Virus Type 1(HSV-1)Glycoprotein D gene. Methods:The Herpes Simplex Virus Type 1(HSV-1)Glycoprotein D(gD1) gene fragment containing dominant antigen epitopes confirmed by computer analysis was cloned by PCR technical and inserted into plasmid vector pTrxA. Then the recombinant plasmid was transformed into Rosetta. The expressed product was analyze by SDS-PAGE. Results:930 bp gene fragment was amplified by PCR as anticipated. Nucleotide sequencing showed a 100 % homology with that of the published sequence in GenBank. The molecular weight of the expressed protein was about 48kDa, Western blotting indicated that the antigenicity of the protein was good. Conclusion:The plasmid pTrxA-gd1 was constructed and a high efficiency expression of the gd1 gene from Herpes Simplex Virus Type 1(HSV-1)strain was made. The expressed product shows a good antigenicity.

Objective To establish a rapid fluorescent inhibition test (RFFIT) for testing rabies virus neutralizing antibody and the titer of rabies virus neutralizing antibody.CVS-11 was used as the standard challenge virus,and three generations prepared for the establishment of the virus library.Methods International standard for rabies immunoglobulin was used as the reference serum.RFFIT test was established under consulting the protocol of Institute of Pasteur,and its specificity,stability and reproducibility were validated.Results We established the RFFIT which showed both good specificity ( 100% ) and reproducibility (P＞0.5).Conclusion The establishment of RFFIT test perfected the rabies laboratory techniques and would enhance the overall ability in detecting rabies in China.

Objective To validate the 2010 Edition kidney cancer TNM staging in China.Methods Data were collected from 695 operated kidney cancer patients from Jan.1981 to Dec.2003.These patients were staged based on the 6th Edition and 7th Edition kidney cancer TNM staging system,respectively.The Kaplan-Meier method,log-rank test and Cox regression models were used to analyze survival functions.Results Compared to the 6th edition staging system,the number of patients with stage T3a,T4changed from 98 to 88,from 29 to 42 in new staging system,respectively.According to the 6th edition staging system,there was significant difference comparing perirenal fat invasion only with adrenal gland only ( P =0.08 ),there was no significant difference comparing Gerota fascia invasion ( stage T4 ) only with adrenal gland invasion only (P =0.412).According to the new staging system,there was no significant difference comparing stage T2b with stage T3a ( P =0.714). Conclusion The new kidney cancer staging system has better staging specificity and clinical application results in high accuracy.

Objective To explore the clinical characteristics,diagnosis,treatment and outcomes of anti-N-methyl-D-aspartate receptor(anti-NMDAR) encephalitis in children.Methods Six children diagnosed as anti-NMDAR encephalitis were recruited at the Department of Neurology,Capital Institute of Pediatrics,from December 2011 to April 2013.The data of clinical characteristics and laboratory examinations were retrospectively analyzed.All the children had long-term follow-ups and the prognosis was assessed.Results (1) Age and course of the disease at the time of the admission:the mean age of the 6 patients (2 female) was 3 years and 5 months,ranging from 2 years and 2 months to 6 years and 8 months.The course of the disease at the time of the admission ranged from 15 to 80 days,with a mean time of 39 days.(2)Clinical characteristics:5 cases had afebrile convulsion and 1 case had speech impairment at the onset of disease.Convulsion occurred in all the 6 cases,4 cases of whom had persistent convulsion,and 5 cases had impaired consciousness.All the 6 cases exhibited aphasia,and complicated with mental or emotional abnormalities,irritability or shouting.Five cases developed into sleep disorders such as sleep deprivation.Five cases had limb and facial involuntary movement,in which 2 cases had stereotyped action.Prominent autonomic nervous dysfunction including hidrosis was found in 1 case.(3) Laboratory examination:cerebrospinal fluid test was normal in 6 cases,and 1 case had slightly increased white blood cell level.Specific anti-NMDAR antibody was positive in serum and/or cerebrospinal fluid in the 6 cases.Electroencephalograph of the 6 cases showed slow wave background during lucid interval,and 5 cases had interictal epileptiform discharges.The skull MRI showed cerebral atrophy 4 cases,and 2 cases of them were complicated with encephalomalacia.No tumor was found in the patients.(4) Treatment and follow-ups:6 cases received gamma globulin or methylprednisolone or other immunotherapy.Three cases received combined therapy with Rituximab,1 case received plasmapheresis,and 1 case received Cyclophosphamide.Follow-ups lasted for 2 to 31 months.Three patients had clinical recovery,and varying degrees of neurological complications were found in 3 cases.Conclusions (1) Anti-NMDAR encephalitis is common in children.(2) The specificity of its clinical symptoms is not strong.The incidence of convulsion is high,and different degrees of consciousness disorders may occur in some of the severe patients.Degeneration of language function and emotional changes can be observed.Most pediatric patients have abnormal movement,and the symptoms of automatic nervous system are not prominent.(3) The disease can be confirmed by the specific anti-NMDAR antibody in the spinal fluid or plasma.(4) The time of clinical recovery is long,and an early immunotherapy is associated with a better prognosis.

Purpose To confirm the effects of acupuncture, and Chinese medicines in controlling the withdrawal symptoms from the opium-like drugs. Method 96 heroin-dependent subjects were divided into four groups,which were treated respectively by western medicine (Agroup), acupuncture (B group), Chinese herbs (Cgroup), and acupuncture & Chinese herbs (D group).Before, during and after treatment, the concentration of serum testosterone and prolactin, and immune functions (serum CD3+ 、CD4+ 、CD8+ and CD4+/CD8+ ) were tested.Results After treatment, the concentration of serum testosterone in A and B group were higher than before and during treatment, and in C and D group, during treatment were higher. In the four groups, the concentration of serum prolactin before treatment was the highest. The levels of CD3+、CD4+、 CD8+ and CD4+/CD8+ were lowest before treatment and highest after treatment. Conclusion Acupuncture and Chinese medicines effective in relieving spasm and pain can control the opium-like drug withdrawal symptoms to different degrees, especially acupuncture. However, acupuncture cannot ease the withdrawal symptoms completely. Acupuncture does not strikingly cooperate with the Chinese medicines effective in relieving spasm and pain (including M-receptor antagonists). In detoxification, the Jiaji points are the primary ones and symptom-based points the secondary ones.

Heroin addicts were treated with acupuncture, Chinese herbs, and acupuncture plus Chinese herbs respectively, their effects were compared with that of Western medicine.The scores of withdrawal symptoms were observed continuously for 20 days. The effect of acupuncture was better than that of Western medicine 48 hours after the withdrawal treatment (P＜ 0.05), and better than that of other three methods 72 hours after the withdrawal treatment, (P ＜ 0.01). Acupuncture, Chinese herbs, and acupuncture plus Chinese herbs have the detoxification effects of varying degrees, in particular acupuncture' effect was the best. But all the therapies could not completely improve the withdrawal symptoms in the first 3 days of treatment, and acupuncture plus Chinese herbs had no synergetic effect in treating withdrawal symptoms.
