2.Surgical management of venous drug-induced infective endocarditis

Objective To summarize the experience of surgical treatment of venous drug-induced infective endocarditis(IE). Methods The 17 cases of drug-induced IE were found to have valvular vegetations,16 patients had involvement of tricuspid valve, 1 mitral valve associated with ventrieular septal defect. Preoperative blood culture was positive in 8 patients. The operations included tricuspid valve repair (6 cases),tricuspid valve replacement(8 cases), mitral valve replacement and VSD closure(1 cases). Results All patients were alive and discharges. After follow-up of 6 months to 6 years, 1 case had severd tricuspid alve regurgitation, 1 had haemoptysis because of incorrect anticoagulation. Conclusions This study indicated surgical intervention is effective treatment of venous drug-induced infective endocarditis.
3.Research on the promoting role of apelin-13 in proliferation, migration and capillary-like tube formation of RF/6A cells

AIM: To investigate the effects of apelin-13 on proliferation, migration and capillary-like tube formation of a monkey choroid / retinal endothelial cell line, RF/6A, to clarify whether apelin-13 could promote retinal angiogenesis in vitro.METHODS: RF/6A cells in good conditions were administrated with DMSO (the control group), apelin-13 at 0.1μmol/L (low dose group) or apelin-13 at 1μmol/L (high dose group).Cell proliferation, migration and capillary-like tube formation were detected by using the MTT assay, scratch assay and matrigel assay, respectively, at 24h after plating the cells.RESULTS: Cell proliferation was promoted in both low and high dose apelin-13 groups compared to the control cells (P<0.05);the cell migration distance of both apelin-13 groups was significantly greater than that of the control group (P<0.05);and the number of capillary-like tube structures of both apelin-13 groups was significantly larger than that of the control cells (P<0.05).In addition, cell proliferation, migration and tube formation increased as the concentration of apelin-13 increased.CONCLUSION: Apelin-13 could obviously promote the angiogenesis capacity of RF/6A cells, suggesting that apelin-13 was an important pro-angiogenic factor in retinal endothelial cells.

Hypertension is one of main risk factors for the occurrence and death of stroke and coronary heart disease. Its prevalence rate is rising year by year. It severely threatens the health of the human beings. The acupuncture method of "activating blood and dispersing wind, harmonizing Gan-Pi" for treating hypertension launched by Academician SHI Xue-min has aroused great attention due to good cur- ative effect and less adverse reactions. In this paper principles of the circular motion covered by the acupuncture method of "activating blood and dispersing wind, harmonizing Gan-Pi" were clarified.

5.The application of EVAR in the treatment of octogenarian patients with abdominal aortic aneurysm

Objective To analyze the effect of EVAR in the treatment of octogenarian patients with abdomi-nal aortic aneurysm.Methods 16 octogenarian patients aged over 80 years who treated with EVAR were collected. Mean age was (87.0 ±6.2)years and included 11 cases in male,2 cases in emergency.All the cases were infrarenal abdominal aortic aneurysm and 11 cases (69%)were the complex aortic anatomy.According to ASA classification, 11 cases (69%)were class Ⅲ and 5 cases (31%)were class Ⅳ.All patients were followed up with CT for 1 month to 3 years.We evaluated technical success rate,perioperative complications and the application characteristics of EVAR in the treatment of octogenarian patients with abdominal aortic aneurysm were analyzed.Results All the patients were operated with EVAR,the success rate was 100%,12 Endurant endografts and 4 Talent endografts were used.Mean operation time was (92 ±25)min and mean hospital stay length was (17 ±9.5)days.1 patient died with heart failure in 3 days after operation.1 patient died due to the Ⅰ endoleak and 4 patients died due to other fators. Conclusion For the octogenarian abdominal aortic aneurysm patients,EVAR is the preferred treatment with accepta-ble procedural success and perioperative morbidity.Those patients have characteristics in the physiological function and anatomy,so we should take measures to prevent the complications.The medium and long -term results suggest that EVAR may be of limited in some patients who are aged over 80 years.
6.The value of gemstone spectral imaging in assessment pathological features of esophageal carcinoma

Objective To evaluate the clinical value of gemstone spectral imaging (GSI) in preliminary assessment of esophageal carcinoma pathology features.Methods 58 patients were analyzed which were diagnosed with histological pathology as esophageal carcinoma underwent GSI enhanced scans before surgery.The iodine concentrations (IC) in the lesions were measured on the iodine-water based material-decomposition images.The results of IC value were evaluated retrospectively with different pathological grading,locations and pathological morphology according to the final pathologic findings.Results 52 cases patients were squamous cell carcinoma and 6 patients were adenocarcinoma.The IC values were (14.75±4.24) mg/ml and (12.86±5.09) mg/ml.The IC value between the two different pathological types had not statistically difference (P =0.35).The IC of different pathological grading:Well differentiation was (20.08± 4.66)mg/ml,n =19.Medium was (14.13±3.39) mg/ml,n =25.Poor was(11.73±3.21) mg/ml,n =14.The IC values between pathological grading had significant difference(P =0.00).There were four different pathological morphology including m edullar (n =16),m ushroom type (n =21),ulcer (n =13) and narrow type (n =8).Their IC values respectively were (16.34±2.56) mg/ml,(18.70±3.03) mg/ml,(14.31±4.60) mg/ml and (11.18±2.09) mg/ml.The IC value between mushroom and narrow type had statistical difference (P =0.04).The Other types had no statistically difference (P =0.19).Conclusions The results of this study demonstrate that GSI has a certain ability of pathologic stage of esophageal cancer.The GSI has a certain clinical value in guiding treatment and judging prognosis of esophageal carcinoma.
7.Associaiton of irisin and vaspin with clinical presentations of metabolic syndrome in patients with type 2 diabetes mellitus

Objective To explore the changes in serum concentrations of irisin,vaspin and reactive oxygen species (ROS) in patients with type 2 diabetes mellitus (T2DM) and metabolic syndrome (MS),and to investigated the correlation of irisin and vaspin with clinical parameters of MS.Methods A total of 260 T2DM patients were enrolled.Age and gender were recorded,anthropometrics,biochemical parameters,and levels of irisin,vaspin and ROS in fasting serum were measured,and homeostatic model assessment of insulin resistance (HOMA-IR) calculated.Wilcoxon rank sum test,correlation analysis,Logistic regression analysis,multiple linear regression analysis and receiver operating characteristic (ROC) curve analysis were performed.Results Compared to T2DM patients without MS,T2DM patients with MS had lower serum level ofirisin [male:112.81 (86.96-191.84) μg/Lvs.156.23 (110.61-225.97) μg/L,female:141.09 (77.52-175.55) μg/L vs.172.15 (95.69-240.37) μg/L,P <0.01],higher levels of vaspin and ROS [male:1.13 (0.95-1.38) μg/Lvs.0.36 (0.21-0.82) μg/L,1 540 (1 250-1 860) kU/Lvs.1 020 (920-1 350) kU/L;female:1.52 (1.13-1.80) μg/Lvs.0.51 (0.47-1.08) μg/L,1 650 (1 320-1 940) kU/Lvs.1 120 (980-1 420) kU/L,P <0.01].In the T2DM patients,serum irisin level was negatively correlated with vaspin (r =-0.382,P < 0.01) and ROS (r =-0.410,P < 0.01),while vaspin was positively correlated with ROS (r =0.400,P < 0.01).Multiple linear regression analyses showed that irisin was significantly correlated with body mass index (BMI),waist circumference and triglyceride,while vaspin was correlated with gender,BMI,and waist circumference (all P < 0.05).Logistic regression analysis revealed that irisin,vaspin and ROS were all associated with MS (OR =0.77,95 % CI 0.608-0.978;OR=1.39,95% CI 1.252-1.539;OR=1.38,95% CI1.112-1.718,all P<0.05).ROC analysis demonstrated that irisin and vaspin had significant area under the curve (AUC =0.931,P <0.01;AUC =0.777,P < 0.01) for the prediction of MS.Conclusions Serum irisin level was significantly decreased,while vaspin and ROS were significantly increased in T2DM patients with MS.Irisin and vaspin were associated with clinical presentations of MS,suggesting that irisin and vaspin might be valuable predictors of MS.
8.The use of European Organization of Research and Treatment of Cancer Risk Tables to predict the prognosis of patients with T1 non-muscle-invasive bladder cancer

Objective:To assess the accuracy of the European Organization of Research and Treatment of Cancer (EORTC) Risk Tables in predicting the prognosis of patients with T1 non-muscle-invasive bladder cancer (NMIBC) treated in the Tianjin Medical University Can-cer Institute and Hospital (TMUCIH). The prognostic factors of T1 NMIBC are also explored, and a new risk scoring model suitable for T1 NMIBC is determined. Methods:We retrospectively reviewed the clinicopathologic characteristics of 108 patients with T1 NMIBC who underwent transurethral resections in TMUCIH from January 2011 to June 2013. We scored patients based on the number of ad-verse factors. Afterwards, divided them into different risk groups by the limits determined using receiver operating characteristic curve (ROC) analysis, and created a new risk scoring model. Results:In a group of 108 patients, 90 (83%) were male and 18 were female (17%). The median age was 65 years old (ranging from 24 to 88). Furthermore, 21 patients (19.4%) had a recurrence and 11 cases (10.2%) progressed to muscle-invasive disease. Conclusion:The EORTC cannot accurately predict the recurrence and progressive rate of T1 NMIBC. The most important prognostic factors for recurrence were tumor size and prior recurrence rate. Tumor grade and prior recurrence rate are independent prognostic factors for tumor progression. The new risk scoring model is more accurate in predicting the recurrence risk and progression of T1 NMIBC.
9.Association of serum Irisin with 25 ( OH) VD3 and other metabolic indicators in patients with polycystic ovarian syndrome

Objective To investigate the level of Irisin in polycystic ovarian syndrome ( PCOS) patients and analyze its potential correlation with 25 (OH) vitamin D3 and other metabolic indicators , with an attempt to explore the role of Irisin in PCOS and thus inform the diagnosis and treatment of this condition .Methods Totally 67 PCOS patients presenting in the reproductive infertility center of the First Hospital of Shanxi Medical University from April 2015 to January 2016 were enrolled as the PCOS group;meanwhile, 72 women of child-bearing age from the health check-up center were included as the control group .We measured the height , body mass, waist circumference, abdominal girth, hip circumference, and blood pressure and calculated their body mass index (BMI) and waist-to-hip ratio (WHR).The serum biochemical indicators including Irisin , 25(OH) vitamin D3, total cholesterol (TC), triglycerides (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol ( LDL-C ) , fasting plasma glucose ( FPG ) , fasting insulin ( FINS ) , follicle-stimulating hormone (FSH), luteinizing hormone (LH), total testosterone (TES) , and Homeostatic Model Assessment of Insulin Resistance ( HOMA-IR ) were measured .Results The body mass [ ( 66.074 ± 12.952) kg vs.(57.671 ±8.806) kg, P =0.000], BMI [ (25.340 ±4.149) kg/m2 vs.(22.201 ± 3.585 ) kg/m2 , P=0.003 ] , waist circumference [ ( 88.829 ±12.212 ) cm vs.( 80.550 ±10.343 ) cm, P=0.007], hip circumference [ (100.805 ±8.614 ) cm vs.(92.007 ±9.325 ) cm, P=0.000], WHR (1.279 ±0.082 vs.0.857 ±0.071, P=0.000), diastolic blood pressure [ (78.548 ±8.936) mmHg vs. (73.071 ±10.614) mmHg, P=0.036], TG [ (2.263 ±1.142) mmol/L vs.(1.330 ±1.329) mmol/L, P=0.019], FPG [ (5.128 ±0.392) mmol/L vs.(4.809 ±0.394) mmol/L, P=0.008], FINS [ (13.319 ± 8.122) mU/L vs.(4.921 ±2.028) mU/L, P=0.000], LH [ (8.153 ±2.271) IU/L vs.(3.811 ±2.607) IU/L, P=0.000], TES [ (63.068 ±25.704) ng/dl vs.(34.811 ±11.827) ng/dl, P=0.000] and, HOMA-IR (2.245 ±0.853 vs.1.021 ±0.153, P=0.000) in PCOS group were higher than those in the normal control group.Serum irisin level in PCOS group was significantly elevated when compared with control group [ (831.542 ± 98.484) ng/ml vs.(612.345 ±86.308) ng/ml, P=0.006], while HDL-C [ (1.278 ±0.345) mmol/L vs. (2.493 ±0.262) mmol/L, P=0.011], and 25 (OH) VD3 concentrations [ (17.181 ±5.078) nmol/L vs. (20.823 ±5.305) nmol/L, P=0.009] were decreased significantly .Linear regression analysis showed that iri-sin was positively associated with body mass (r=0.396, P=0.028), BMI (r=0.712, P=0.000), waist circ-umference (r=0.462, P =0.021), hip circumference (r =0.478, P =0.006), WHR (r =0.686, P =0.002), TG (r=0.397, P=0.036) and HOMA-IR (r=0.467, P=0.019) in the PCOS patients and there was a negative correlation between irisin and 25 (OH) VD3 (r=-0.654, P=0.004).Multiple regression analysis in PCOS group with irisin as dependent variable and other anthropometric and biochemical indicators as independent variables revealed that there was linear regression relationship between serum irisin and BMI (β=0.822, P<0.001), TG (β=0.076, P=0.035), and 25 (OH) VD3 levels (β=-0.242, P=0.027). Conclusions Serum Irisin level is increased in PCOS patients and it is correlated with 25 ( OH) vitamin D3 , BMI, HOMA-IR, and TG.Irisin provides a new method for the diagnosis and treatment of PCOS .
10.Changes of Endogenous Hydrogen Sulfide in Plasma of Newborn Infant with Pulmonary Hypertension

Objective To explore the changes of hydrogen sulfide(H2S) levels in plasma of newborn infants with pulmonary hypertension(PH) and its relationship with pulmonary hypertension,and provide scientific evidence for the decision of treating neonatal PH.Methods Sixteen children with PH and 16 children without PH in ICU from Mar.2005 to Mar.2006 were selected.Ultrasonic cardiogram(UCG) examination was performed for eachpatients.Pulmonary artery pressure(PAP) was measured.The plasma concentrations of H2S,cysteine and PAP of each patient were measured.Results PAP was 4.27-9.73 kPa[(6.49?1.79) kPa] in neonatal PH group,and PAP in control group was normal.The plasma levels of cysteine and H2S in neonatal PH group significantly increased compared with those of control group [(11.94?6.65) ?mol/L vs(6.43?2.08) ?mol/L,t=2.630 P=0.016;(26.99?1.33) ?mol/L vs(24.92?1.36) ?mol/L,t=4.373 P=0].Conclusions Endogenous H2S and cysteine were up-regulated during the development of neonatal PH;it might play an improtant role in the development of PH.H2S possibly depress the PAP by dilating the pulmonary artery to protect the patients with pulmonary hy pertension.
