Objective To investigate the expression of phosphodiesterase 5 (PDE5) in clitoral corpus cavernosum and effect of icariin on cyclic GMP (cGMP) levels in vitro. Methods The presence of mRNA coding for PDE5 in clitoral corpus cavernosum was detected by means of RT PCR analysis.The effect of icariin with various concentrations on cGMP levels,which is the target of PDE5 in vivo,was measured by 125 I radioimmunoassay with sildenafil as positive control. EC 50 of icarrin and sildenafil was calculated. Results There was a lane around 800 bp on the agar electrophoresis of the production of RT PCR,which was as long as we designed according to cDNA sequence of PDE5.In the presence of SNP,icariin increased cGMP concentrations in rabbit clitoral corpus cavernosum with increasing dose. Conclusions The results support the proposal that icariin may influence cGMP level by NO cGMP signaling pathway as an inhibitor of PDE5.

Objective To understand the morbidity of human brucellosis in Wulanchabu City of Inner Mongolia in order to provide a basis for development of prevention and control measures.Methods According to the requirements in Baseline Survey Programme of Human Brucellosis in Inner Mongolia Autonomous Regionbetween November and December in 2010,stratified cluster sampling method was used to selected three townships according to the conditions of brucellosis(mild,moderate and serious) in 11 flags(cities,counties,districts) of the city.Three villages were extracted from each township.There were at least 200 persons aged 10 and older were investigated in every township.At least 600 people were investigated in every flag(city,county,district).Respondents were investigated about their awareness rate of basic knowledge about brucellosis,risk factors and common sense of prevention and treatment of the disease by using health education questionnaire.Based on the principle of informed consent,we collected respondents venous blood for preliminary screening using Hu red tablets.Brucellosis was confirmed with standard tube agglutination test and positive result was confirmed if 1:100 antibody concentration was two + or more.Prevalence,false negative rate as well as different age,gender,occupational incidence was calculated based on the network reported results of brucellosis in 33 townships of Wulanchabu in 2010.Results We distributed 6998 questionnaires and 6763 questionnaires were effective.The witting rate of basic knowledge about brucellosis was 57.99％(15 687/27 052); the witting rate of risk factors about brucellosis was 44.33％ (29 978/67 630); the witting rate for prevention and control of brucellosis was 41.66％(28 176/67 630),and total witting rate was 45.49％(73 841/162 312).The morbidity of brucellosis in Wulanchabu City was 91.39/10 000 (785/85 894) in 2010.Five hundred and ninety-one were serologically positive and the infection rate was 8.48％ (591/69 972).Nine hundred and three cases of patients were diagnosed with brucellosis and the prevalence rate of brucellosis was 105.13/10 000 (903/85 894).Missing report 377 cases,and the false negative rate was 48.03％ (377/785) in 2010.Men infection rate was 71.07％(420/3755) and women infection rate was 28.93％(171/3217).The infection rate[97.63％(577/591)] of people aged 31 and older was significantly higher than that [2.37％(14/591)] of the people aged 30 and younger.Infection rate of people engaged in aquaculture was 8.61％ (582/669).The infection rate of people working in livestock processing industry was 2.91％ (2/103).The infection rate of people working in animal by-product circulation was 2.85％ (2/70).The Infection rate of people working in other industries was 4.59％(5/109).Conclusions In Wulanchabu City,the morbidity of brucellosis and missing report rate are both high.The infection rate of people engaged in aquaculture is high and man morbidity is high.The awareness rate about prevention and treatment knowledge of brucellosis is low.We should carry out health education to inhabitants in endemic areas to improve their self-protection awareness and reduce the incidence of brucellosis.

Objective To introduce a new economical method which can be used for determination of urinary iodine of batch samples with low cost of arsenic trioxide,and decrease environmental pollution.Methods A catalytic spectrophotometry for measuring iodine content in a small volume of urine samples with low cost of arsenic and cerium was established based on the principle of ammonium persulfate digestion As3+-Ce4+ catalytic spectrophotometry.Ninety-six well polypropylene microplate was utilized as digestion and catalysis reaction vessel,conventional laboratory oven was used as a tool to digest and heat,ice plate was used to cool,and the absorbance was read with the Multimode Reader.The accuracy of the new method was evaluated with the current standard method (WS/T 107-2006) by simultaneous determination of urinary iodine of 24 urine samples.Results The linear range of this method was 0-300 μg/L,the linear correlative coefficient (r) was higher than 0.999,and the detection limit was 8.67 μg/L.The coefficient of variations was 2.15％,4.33％ and 3.48％ when measuring urine samples with high,medium and low iodine concentration,respectively.The test results of three national standard urinary iodine samples were all within the given value range and the relative deviation (RD) was-0.16％,1.81％ and-2.82％,respectively.The average recovery of the low concentration was 97.51％,and that of the high concentration was 96.01％.The two methods correlated well (r =0.995,P ＜ 0.01).Conclusions This method greatly reduces the arsenic waste,environmental pollution,consumables and labor.The new method is simple and efficient,accurate and reliable;it is suitable for application as a supplementary method for analyzing urinary iodine of a large number of samples.

BACKGROUND:Meta-analysis results have shown that Duhuojisheng Decoction can improve the symptoms and relieve the pain of patients with knee osteoarthritis, but the mechanism and pathway are not very clear.OBJECTIVE:To further verify the curative effect of Duhuojisheng Decoction for primary knee osteoarthritis in macaca fascicularis. METHODS: Under natural conditions, 6 clean young macaca fascicularis (aged 3-5 years, normal group) and 6 elderly macaca fascicularis (aged more than 20 years, model group) were selected from 24 macaca fascicularis. The models of primary knee osteoarthritis were established in the model group,then aged animals were randomly divided into control and intervention(given the treatment of Duhuojisheng Decoction) groups. The number of whole blood leukocytes, C-reactive protein, erythrocyte sedimentation rate, properties of the joint fluid and expression level of tumor necrosis factor-α were compared between the control and intervention groups. RESULTS AND CONCLUSION: The aged macaca fascicularis was characterized by a typical osteoarthritis similar to human. Treatment with Duhuojisheng Decoction significantly down-regulated the expression level of tumor necrosis factor-α and significantly reduced the levels of whole blood leukocytes, and C-reactive protein. Our results suggest that the aged macaca fascicularis model of primary knee osteoarthritis is an effective animal model to simulate the occurrence and development of human primary knee osteoarthritis.Duhuojisheng decoction alleviates the progression of primary knee osteoarthritis probably by inhibiting the expression level of tumor necrosis factor-α and decreasing the levels of inflammatory factors in the articular cartilage. Additionally, the levels of whole blood leukocytes and C-reactive protein are important parameters for the prediction of curative effects.

Objective To study the efficacy and safety of estradiol and drospirenone tablets (Angeliq)in treatment of menopausal symptoms among postmenopausal Chinese healthy women.Methods Total 244 postmenopausal Chinese healthy women who had moderate to severe hot flushes were randomly assigned for 16 weeks in this randomized multi-center double-blind placebo-controlled study.During the trial.the follow-up visits were conducted at week 4,8,12,16 of treatment and 2 weeks after treatment respectively.Height,weight,vital signs,hot flushes,other relevant menopausal symptoms and vaginal bleeding were observed in each follow-up visit,while the clinical global impression scale Was assessed at 16 weeks as well.Results It showed that hot flushes were reduced significantly more in observation group than that in placebo group ( P＜0.01 ), although both treatments were effective. The absolute values of mean severity index of total hot flushes decreased by - 0. 6± 0. 5 in observation group and - 0. 4 ± 0. 4 in placebo group from baseline respectively, which reached significant difference ( P ＜ 0. 05 ). However, the absolute values of mean severity index of moderate to severe hot flushes decreased by - 0. 6± 0. 8 in observation group and -0. 3± 0.6 in placebo group from baseline respectively, which had no significant difference (P ＞ 0. 05 ).After 16 weeks treatment, it also showed that estradiol and drospirenone had significant better efficacy than placebo on moderate to severe sweating, vaginal dryness and clinical global impression scale (P ＜0. 01 ).During the trial, blood pressure in observation group was stable. The rate of vaginal bleeding in observation group was higher than that in the placebo group, especially during the week 4 to week 8 when 48. 9% (87/178) in observation group and 10. 7% (6/56) in placebo group of patients bled. Although the cumulative amenorrhea rate of observation group was lower than that of placebo group in each cycle (28 days), it increased gradually along with duration of the treatment. The commonest adverse event in observation group was breast tenderness which accounted for 12.0% (22/183 ). The level of serum potassium was in the normal range in observation group mostly. Meanwhile, the other adverse events rate was low. Serious adverse events reported in this trial were assessed as not study drug related or as unlikely study drug related. Conclusion Estradiol and drospirenone tablets which could effectively alleviate menopausal symptoms in postmeuopausal Chinese healthy women is a novel hormone replacement therapy regimen with high safety and efficacy.

Objective To explore the effect of extracurricular activities based on specialist nursing clinic. Methods Thirty-four undergraduate nursing students of grade 2013 in the Capital Medical University were enrolled in the study. After learning of nursing theories for senile dementia, the extracurricular activities based onspecialist nursing clinicwere carried out. Before and after the second class activities, the caring ability inventory scale and self scale-rating for self-directed learning (SRSSDL) were used to assess 34 students. Result After the activities, students' caring ability and self-directed learning ability were statistically significantly improved (P<0.01). Conclusion The extracurricular activities based onspecialist nursing cliniccan stimulate the learning interest of nursing students and improve their self-directed learning ability and humanistic care ability.

Lactobacillus casei was selected as an antigen delivery vehicle for the development of oral vaccine to express recombinant LTB and porcine parvovirus (PPV) VP2 protein. The fusion protein gene encoding PPV VP2 protein and LTB, was cloned into the surface expression vector pPG, and then the recombinant expression vector pPG-VP2-LTB was electrotransformed into Lactobacillus casei 393, generating recombinant strain pPG-VP2-LTB/L. casei 393. After induced by 2% Lactose in MRS broth, an about 78 kD protein was detected in the recombinant Lactobacillus casei by SDS-PAGE. The result of Western blot indicated that the protein possessed the antigenic specificity same as the native virus protein. The result of the whole bacteria cell ELISA indicated that the LTB protein was expressed at the same time. The results of indirect immunofluorescence test and immuno-gold electron microscopy showed that the interest protein was expressed on the surface of L. casei 393. The results provide potential for the development of lactic acid bacteria oral vaccine of PPV, which used LTB as mucosal adjuvant.

OBJECTIVETo understand the role of KCNE2 in functional regulation of Kv4.3, the major alpha subunit of transient outward current (I(to)) in human heart.

METHODSThe cDNAs of Kv4.3 or Kv4.3 plus KCNE2 were transfected into COS-7 cells and 24-36 h after the transfection, the channel proteins were expressed in the surface membrane of the cells and the channel currents were recorded with patch-clamp technique in whole-cell mode.

RESULTSKCNE2 played an important role in modulating the channel function. The recorded current density was decreased in cells co-expressing KCNE2 and Kv4.3 to 152.96-/+33.71 pA/pF (n=16) as compared with Kv4.3-expressing cells with a mean current density of 375.13-/+112.87 pA/pF (n=11). At the recording voltage of 60 mV, KCNE2 increased the time to peak (TTP) of the current. TTP in only Kv4.3-expressing cells was 4.82-/+0.32 ms (n=11), significantly shorter than the TTP of 20.41-/+2.13 ms (n=16) in cells co-expressing Kv4.3 and KCNE2 (P<0.05). In the presence of KCNE2, the voltage-dependent inactivation of Kv4.3 showed a positive shift. The voltage of half maximum inactivation (V(0.5)) was decreased significantly from -53.62-/+1.24 mV (n=8) in Kv4.3 group to -46.58-/+1.6 mV (n=10) in KCNE2 co-expression group (P<0.05). KCNE2 accelerated the recovery of the channel from inactivation, reducing the recovery time constant (tau) from 193.43-/+17.98 ms to 137.71-/+18.29 ms.

CONCLUSIONKCNE2 might serve as an important beta subunit and play a role in the regulation of I(to) function in human heart.

BACKGROUNDHeterotopic pregnancy (HP) is defined as a pregnancy in which one or more embryos is viably implanted in the uterus while the other is implanted elsewhere as an ectopic pregnancy. The occurrence of HP rises dramatically with the increased use of assisted reproductive technology. HP of interstitial pregnancy is one special situation which needs more concern. Here we evaluate the efficacy of local aspiration and instillation of hyperosmolar glucose in the treatment of live interstitial pregnancy complicated with live intrauterine pregnancy after in vitro fertilization and embryo transfer.

METHODSFive female patients were diagnosed with live interstitial pregnancies complicated with intrauterine pregnancies. They were treated with transvaginal ultrasound-guided aspiration of interstitial pregnancy and instillation of hyperosmolar glucose at the Center for Reproductive Medicine of Peking University Third Hospital from January 1st, 2008 to May 30th, 2011.

RESULTSGemmule embryos in all 5 cases were aspirated successfully and there was no abdominal hemorrhage, threatened abortion or infection in any of the cases. The sac of interstitial pregnancy continued to progress after aspiration and stopped growing between 11 to 20 weeks. By the 30th week of pregnancy, 80% of the interstitial masses had disappeared. Four cases have delivered and one is still in on-going pregnancy. All of the four cases underwent cesarean section and there were nothing special detected in the corner of the uterus.

CONCLUSIONLocal aspiration and instillation of hyperosmolar glucose may be an effective way to treat live interstitial pregnancy when coexisting with a live intrauterine pregnancy.
