2.Comparison of conventional and conformal radiotherapy and different irradiation technique by 3D-TPS for esophageal carcinoma : a dosimetric study

Purpose:To obtain the optimum irradiation technique in different plans of conventional radiotherapy(2DRT)and conformal radiotherapy(3DCRT)and between different conformal radiotherapy techniques for esophageal carcinoma as compared by 3D-TPS.Methods:10 patients with esophageal carcinoma were enrolled in this study.The conventional treatment plan and four conformal technique plans were created for each patient.Plan 1 was the same as the 2DRT,plan 2 consisted of anterior and two posterior oblique fields,plan 3 consisted of posterior and two anterior oblique fields,plan 4 consisted of 2 dynamic arc fields.Different plans were compared using DVH.Results:①On average,PTV of 2DRT was the same as that of 3DCRT in DVH.The difference in maximum dose to spinal cord was obvious.The left lung dose in D_(mean)?D_(min)?V_(30) in 2DRT was significantly increased as compared to 3DCRT(P
3.Oxidative stress and inflammatory changes in the lung caused by cigarette smoking exposure in mice and the effect of smoking cessation

Objective To observe the changes of pulmonary oxidative stress after cigarette smoking exposure,its re-lationship with inflammatory cytokines,and the effects of smoking cessation. Methods Fifty male BALB / c mice were randomly divided into the smoke exposure group,smoke cessation group,and the controls. Mice in smoke cessation group were exposed to cigarette smoking for 16 weeks. On 4,8,and 12 week after smoking cessation mice were executed and the bronchoalveolar lavage fluid(BALF)and lung tissue were collected. The morphologi-cal alternations of lung tissue were observed. Mean length of interval and mean alveolar number were measured. Total cell numbers in BALF were counted. Superoxide dismutase(SOD)activity was measured with hydroxylamine method,malondialdehyde(MDA)level was measured with TBA method. The levels of pulmonary interleukin-8 (IL-8)and tumor necrosis factor-α(TNF-α)in BALF and lung tissue homogenate were measured with ELISA. Re-sults Compared with the mice in the controls,emphysematous changes were remarkable in the lung of cigarette ex-posed mice,the total cell numbers in BALF were increased significantly(P < 0. 05)and reduced gradually after smoking cessation(P < 0. 05). SOD and MDA levels increased remarkably in the cigarette exposure group(P <0. 05),and declined gradually after smoking cessation. The levels of IL-8 and TNF-α in BALF and lung tissue ho-mogenate in the smoke exposure group increased significantly( P < 0. 05),and lowered time-dependently after smoking cessation,but not reached to normal level even 12 weeks after smoking cessation. SOD and MDA levels were positively correlated with the cytokine changes. Conclusion Abnormal oxidative stress in the airways caused by cigarette smoking exposure was merely partially reversed after smoking cessation. And the inflammation remains persistent concomitantly.
4.Changes of interleukin-17A and interleukin-22 in the lung of cigarette smoke exposed mice and the effect of N-acetylcysteine

Objective To investigate the changes of interleukin-17A(IL-17A)and interleukin-22(IL-22)in the lung of cigarette smoke exposed mice and the effect of smoking cessation and N -acetylcysteine (NAC )treatment . Methods BALB /c mice in experimental groups were exposed to cigarette smoke.Then the smoke-exposure was stopped and mice were treated with NAC gavage.The mice were executed 1,2,and 3 months after smoking cessa-tion.Lung tissue sample and bronchoalveolar lavage fluid(BALF)were collected.HE staining was used to observe the pathologic changes of the lung and ELISA was used to measure the level of IL-17A and IL-22.Results Con-siderable emphysematous changes was found in the lung of mice exposed to cigarette smoke.Compared with the controls,the level of IL-17A and IL-22 elevated remarkably in pulmonary tissue and BALF after smoking exposure and declined gradually after smoking cessation.Additional NAC gavage treatment enhanced the decline tendency. Conclusion IL-17A and IL-22 might play a complex role in the chronic inflammatory changes of lung in mice ex-posed to cigarette smoke.
5.Progression of treatment in locally advanced breast cancer

Neoadjuvant chemotherapy combined with surgery and/or radiotherapy is the common method in treating locally advanced breast cancer.This article reviewed the progress on the rationales for neoadjuvant chemotherapy,regimens and courses of chemotherapy,the factors relative to the outcome of the treatment and prognoses,advantage and disadvantage of neoadjuvant chemotherapy.The progress in diagnosis,surgery,radiotherapy and endocrine therapy for locally advanced breast cancer are also summarized.
6.Application of the improved peripherally inserted central career technique in chemotherapy for 169 patients with lung cancer

Objective To evaluate the influence of pressing technique on the malposition of catheter dur- ing the operation of peripherally inserted central catheter (PICC). Methods Totally, 169 patients with lung cancer were randomly divided into treatment group (n = 88), in which the external and internal jugular veins were pressed at the supraclavicular fossa by the assistant when the catheter was navigated in 25 cm, and control group (n = 81), in which the patients were called to turn their angle of mandible to supraclavicular fossa when the cathe- ter was sent in. Results There were only one patient whose catheter was sent in external jugular vein in treatment group and nine patients whose catheters were sent in external and internal jugular vein in control group. The inci- dence of malposition of catheter was significantly different between two groups (1.1% vs. 11. 1% , X2 =7. 3636, P < 0. 01). Conclusion The pressing technique can reduce the incidence of malposition of catheter into external and internal jugular vein during the operation of PICC.

8.Clinical observation on treatment of aplastic anemia patients by zaizhang decoction combined with Western medicine.

OBJECTIVETo study the effect of Composite Zaizhang decoction (ZZD) on immune function of chronic aplastic anemia (CAA) patients.
METHODSTo determine the levels of T-lymphocyte subset, serum tumor necrosis factor (TNF-alpha) and interleukin-2 (IL-2) in CAA patients before and after treatment, and to analyse the regulation of their changes comparatively with the group treated with conventional treatment and a normal control group.
RESULTSBefore treatment, CD4 was lower, CD8 higher, CD4/CD8 ratio also lower, TNF-alpha and IL-2 were higher in the ZZD treated group than those in the normal control group, with significant difference (P < 0.05), but after ZZD treatment, these indexes were all inversely changed, as compared with those before treatment and with those in the control group, the difference was significant (P < 0.05).
CONCLUSIONZZD might relieve the hemopoietic inhibition so as to promote the recovery of hemopoietic function through improving the T-lymphocyte subset and reducing the release of hemopoietic negative ragulatory factors such as TNF-alpha and IL-2 etc.

9.Study on quality standard of Eucommia ulmoides based on absorbed active components in rat plasma.

The method was established for determination of geniposidic acid, chlorogenic acid, pinoresinoldiglucoside, which are three kinds of constituents of Eucommia ulmoides absorbed into the blood components. LC-MS/MS technique was applied to determine the blood components of the bloodstream after administration of E. ulmoides extract. At the same time, HPLC was used for detection of the ingredients content of the blood sample from 23 batches of E. ulmoides. The results showed that geniposidic acid, chlorogenic acid and pinoresinoldiglucoside are prototype into the blood in rats after oral administration of E. ulmoides extract, The linear range of geniposidic acid, chlorogenic acid and pinoresinoldiglucoside was good, and the average recoveries geniposidic acid, chlorogenic acid and pinoresinoldiglucoside were 98.69%, 100.8% and 98.39%, respectively. The method is simple and feasible with good reproducibility.

10.Circulatory sleep apnea: Preliminary report of clinical observation on sleep apnea in patients with chronic heart failure.

OBJECTIVEThe aim of this study is to investigate the occurrence and mechanism of Cheyne-Stokes breathing pattern in patients with heart failure.
METHODSFifty-six patients who performed polusomnography sleep testing at National Center of Cardiovascular Diseases Fuwai Hospital from March to May in 2015. We divided them into chronic heart failure (CHF) group and non-CHF group.
RESULTSThe occurrences of sleep apnea in two groups were high. In CHF group (n = 11) , there were 10 patients with apnea hypopnea index (AHI) > 5; and their AHI was 23.93 ±14.63. In non-CHF group (n = 45), there were 33 patients whose AHI > 5; and their AHI was 16.20 ± 18.76. The ratio of center sleep apnea to all gross sleep apnea ratio in CHF group was higher than that in non-CHF group (80.21% ± 30.55% vs 27.16% ± 35.71%, P < 0.01 ).
CONCLUSIONBased upon the new theory of holistic integrative physiology and medicine, we explain the mechanism of circulatory dysfunction induce the oscillation breathing in patients with CHF. The sleep apnea and C-S respiration in CHF should be called circulatory sleep apnea, rather than central sleep apnea.
