3.Detectable rate and target antigens of antineutrophil cytoplasmic antibodies in Chinese patients with clinically suspected small vessel vasculitis

Objective To study the detectable rate of antineutrophil cytoplasmic antibodies (ANCA) in Chinese patients with clinical suspected small vessel vasculitis,and the target antigens.Methods All 5 604 sera sent to the authors′ laboratory for ANCA screening were tested by indirect immunofluorescence (IIF),anti myeloperoxidase (MPO),and anti proteinase 3 (PR3) ELISA.Then the positive sera with IIF ANCA but negetive with MPO and PR3 ANCA were further tested by antigen sepecific ELISA using other five highly purified known ANCA antigens as solidphase ligands.The known antigens included human leukocyte elastase (HLE),lactoferrin (LF),cathepsin G (CG),bactericidal/permeability increasing protein (BPI) and azurocidin (AZU).Results Of the 5 604 sera,298(5 3%) were IIF ANCA positive,and 390/5 604(7%) were ANA positive.Patients′ANCA presented mainly in summer or winter.Of the IIF ANCA positive patients,213 were anti MPO positive,32 were anti PR3 positive and 5 were positive for both.Of the 48 sera positive by IIF,yet negative by MPO and PR3 ANCA,13 sera(27%) recognized other target antigens,7 sera recognized BPI,5 recognized HLE,one recognized CG and one recognized AZU,and none of the sera recognized LF.One serum recognized both BPI and HLE.There was no difference in gender(M∶F=1∶1 12) in ANCA positive patient with a mean age of 53 1 years.However,the M/F ratio was 1 17∶1 for patients over 60 years,and was 1∶4 for patients less than 20 years.Conclusion ANCA related diseases are not rare in China and the major antigens are MPO and PR3.When IIF technique is used to detect ANCA,ANA should be carefully distinguished.Some sera also recognized other known ANCA antigens,but for a few sera the target ANCA antigens are still unkown.
4.Construction and effect identification of RNAi eukaryotic expression vectors of iASPP

Objective To construct the RNAi eukaryotic vector of inhibitory member of the ASPP family(iASPP) gene and observe the interfering effect in Jurkat cell line after the vector transfection.Methods The specific siRNA sequence was designed according to the iASPP sequence in GenBank.The sequence was cloned into PGCsilencer~(TM) H1/Neo/GFP and then sequence analysis was performed.The recombinant plasmid was transfected into Jurkat cell by liposome.The iASPP expression was analyzed by RT-PCR and cell apoptosis was detected by FCM.Results The sequence of templet and specific siRNA was correct by sequence analysis.The iASPP expression in Jurkat cells decreased and the apoptosis rate increased from 11.81% to 33.15% after RNAi transfection.Conclusion The RNAi eukaryotic vector PGCsilencer~(TM)H1/Neo/GFP/RNAi was constructed successfully.The RNAi inhibitory effect on the Jurkat cells is confirmed.The successful construction of iASPP RNAi makes it possible to further study the interaction between iASPP and p53.
5.Chronic Pulmonary Complications Associated with Toxic Epidermal Necrolysis Caused by Carbamazepine

Objective To explore the clinical manifestations of toxic epidermal necrolysis(TEN) and its rare pulmonary complications.Methods Clinical symptoms,treatment and prognosis of 1 child with TEN caused by carbamazepine were analyzed.Radiological images were reviewed to evaluate the manifestations and the outcome of chronic pulmonary complications associated with TEN.Results The patient had high fever shortly after a dosage increment of carbamazepine.A confluent erythematous exanthema developed rapidly into painful blistering with skin erosion,denudation and involvement of conjunctive and oropharyngeal mucosa.The diagnosis of TEN was made.The mucocutaneous damage was gradually recovered with steroid plus intravenous immunoglobulin for 3 weeks.However,the patient presented with respiratory failure in the recovery phase of TEN.The computer tomography revealed pulmonary bullae and pneumothorax in the right lung.Lung parenchyma was squeezed and pulmonary bullae ruptured with pneumothorax and atelectasis,which were absorbed gradually through thoracic drainages.The patient′s lung function and pulmonary bullae were partly improved during a 7-month follow-up.Conclusions TEN is a severe form of blistering skin di-sease which is characterized by an extensive loss of epidermis and mucous membrane.Chronic pulmonary complications may occur in recovery phase of TEN.Pulmonary bullae,which might be caused by mucous damage and respiratory obstruction,is a rare complication of TEN.
6.Influence on cultured human keratocytes by liposome

AIM:To observe the effects on human keratocytes by cationic liposome LipofectamineTM 2000(LF2000).to investigate the efficiency and safe range applied in human keratocytes,and establish basis for gene therapy of human keratocytes.METHODS: Human keratocytes cultured in vivo within 3 to 5 passages were used in experiment after being identified.The effects on proliferation of cultured human keratocytes by LF2000 with different concentrations and time were evaluated By MTT:the effects of LF2000 on the survival rate and its relation with 5,10,20,40.80mg/L concentration and time were detected by trypan blue staining.related with concentration and time.The cellular proliferation and survival rate declined when concentration of LF2000 was above certain level,and this effect increased as time became longer.LF2000 had no effect with concentration under 40mg/L for 24 hours. CONCLUSION:LF2000 did ont cause cytotoxicity during a concentration range"tested",and it is hoped to play an important role in gene therapy of human keratocytes.
7.Cage bone graft combined with pedicle screw fixation fusion in treatment of lumbar spondylolisthesis

Objective To study the effect of different approaches of Cage bone graft combined with pedicle screw fixation fusion in trea-ting lumbar spondylolisthesis. Methods From January 2011 to December 2012,120 patients with lumbar spondylolisthesis in our hospital were randomly divided into anterior lumbar interbody fusion (ALIF) group(n=40),postero lateral fusion (PLF) group(n=40) and posteri-or lunbar interbody fusion (PLIF) group(n=40). The related index and body’s function were observed. Results In PLIF group,the opera-tion time was(82. 3 ± 12. 1) h,intraoperative bleeding volume was (60. 3 ± 9. 5) mL,postoperative ambulation time was (5. 5 ± 0. 7) d, NRS score was(1. 5 ± 0. 2),ODI score 3,6,9,12 months after operation were respectively (24. 1 ± 3. 4),(19. 4 ± 3. 1),(13. 5 ± 2. 5), (9. 8 ± 1. 6),which was significantly lower than ALIF group and PLF group;the intervertebral height(9. 3 ± 1. 2)mm were significantly high-er than those of ALIF group and PLF group. Conclusion PLIF has positive clinical value in treating lumbar spondylolisthesis as it can re-duce the operation wound,promote postoperative recovery,and improve the long-term function of body.

10.Malignant transformation of sinonasal inverted papilloma: retrospective analysis of 32 cases.

OBJECTIVETo investigate the clinicopathological features and prognosis of patients with the malignant transformation of sinonasal inverted papilloma.
METHODSThirty-two consecutive cases encountered between January 1991 and January 2008 were retrospectively reviewed. Survival rates and prognostic factors were calculated with SPSS 17.0 software using the Kaplan-Meier method and multivariate Cox model survival analysis.
RESULTSThe malignancy accounted for 8.99% of all types of sinonasal inverted papilloma. There were 25 males and 7 females, the median age was 56.5 years. The sites of tumor included 22 cases in nasal cavity and ethmoid sinuses, 10 cases in maxillary sinuses. The tumors included 21 high grade tumors, 8 intermediate grade tumors and 3 low grade tumors. Thirty-two patients were staged as T1(3/32), T2(10/32), T3(16/32), T4(3/32). According to the percentage of the malignant cell in the entire tumor tissue, 5 patients were in grade I, 5 patients were in grade II, 8 patients were in grade III, 14 patients were in grade IV. There were 3 distant metastasis in 32 patients; 19 patients underwent surgery plus postoperative radiotherapy, 10 underwent surgery alone and 3 underwent radiotherapy alone. The 5-year overall survival were 72.5%, and the median overall survival time was 62.2 months. Kaplan-Meier univariate survival analysis indicated that the clinical stages and treatment modalities were prognostic factors, and multivariate Cox model survival analysis confirmed that the clinical stages and treatment modalities were independent factors for overall survival (HR were 4.211 and 0.312, all P < 0.05).
CONCLUSIONSThe morbidity of sinonasal inverted papilloma-associated malignancy is low, the clinical features were not specificity. The clinical stages and treatment modalities may affect the prognosis. Surgery plus postoperative radiotherapy are main treatment means.
