OBJECTIVE To investigate the therapeutic effect of modified uvulopaoatopharyngoplasty(UPPP)on obstructive sleep apnea hypopnea syndrome(OSAHS).METHODS Sixty eight cases of OSAHS were treated with modified UPPP.During operations,the uvula was preserved,the fat tissues of space veli palatine were dissected,palatoplasty and pharyngoplasty were performed.All the patients were followed up for over 6 months.Polysomnography(PSG)was monitored again one year after operation.RESULTS The symptoms of dyspnea,snoring,sleepiness were disappeared or improved obviously after 6 months.The effective rate was 95.6% at one year after operation.CONCLUSION The modified UPPP not only extends the oropharyngeal cavity,but also avoids postoperative complications and improve the effectiveness of UPPP.

OBJECTIVE: To explore the possibility mechanism of non-side population cells (NSP) of Hep-2 be induced into stem-like cancer cells by chemotherapy drug--cisplatin. METHOD: Hep-2 cell lines were sorted by fluorescence-actived cell sorting. The acquired NSP cells in trail group were co-cultured with cisplatin for more than 48 hours,while the control group with normal saline(NS). Then identified the percentage of the side population (SP) cells by flow cytometer. The β-catenin, notch-1 mRNA in trial and control group were detected using quantitative realtime PCR, and the β-catenin, notch-1 protein in two groups were compared by Western blot. RESULT: The percentage of side population cells in two groups were (17.16 ± 0.18)%, (10.05 ± 1.20)%, respectively. There was significant difference between two groups (t = 5.844, P < 0.01). The expression of β-catenin, notch-1 was higher in trail group by qRT-PCR; the protein levels of β- catenin, notch-1 was found to inceased in the trail group by Western blot (t = 5.155, P = 0.031; t = 5.977, P = 0.004). Statistical analysis showed significant difference between two groups (P < 0.05). CONCLUSION NSP cells can be differentiated into stem-like cancer cells after being treating with cisplatin. The supposed mechanism is maybe through wnt/β-catenin, notch signaling transduction pathway abnormalities.

Saw palmetto fruit extract (SPE), as a herbal product, is widely used for the treatment of benign prostatic hyperplasia (BPH) and lower urinary tract symptoms (LUTS). Recent studies show that SPE also has some therapeutic effects on chronic prostatitis, prostate cancer, sexual dysfunction, and so on. This article presents an overview on the application of SPE in the treatment of BPH, prostate cancer, and chronic prostatitis/chronic pelvic pain syndrome, with a discussion on its action mechanisms.

Objective To understand the molecular epidemiology characteristics and its drug resistance of methicillin-resistant Staphylococcus aureus (MRSA) in the urban area of Jilin and to provide important basis for guiding the clinical medication and prevention of the MRSA infection. Methods One hundred and three strains of MRSA from July 2013 to July 2014 in the urban area of Jilin were selected. The polymerase chain reaction (PCR) technology and multiple polymerase chain reaction were used to detect mecA gene and Staphylococcus chromosomal cassette mec typing (SCCmec) genotype of MRSA. The drug sensitivity test for 13 kinds of clinical common antibacterial drugs were detected by using the K-B method. And the source of the strains were analyzed. Results The results of SCCmec genotype of MRSA showed that SCCmecⅢtype were 62 strains, accounting for 60.2%;SCCmecⅡtype were 39 strains, accounting for 37.9%; failing to parting were 2 strains,accounting for 1.9%. Drug susceptibility test results showed that all of 103 MRSA strains were resistant to cefoxitin, cefazolin, penicillin and benzene, and drug resistance rate was 100.0%. The resistant rate to erythromycin, levofloxacin, ciprofloxacin, tetracycline, gentamicin and rifampin were 96.1%, 93.2%, 95.1%, 91.3%, 90.3%and 55.3%receptively;the resistant rate to sulfamethoxazolewas was only 1.9%;and the resistant strains to vancomycin and teicoplanin were not detected. The top three department of the distribution of the strains source were department of neurosurgery (31.1%), ICU (19.4%) and burn plastic surgery (17.5%). Conclusions The SCCmecⅢtype is the main MRSA epidemic strains, and SCCmec type II is a minor epidemic strainin the urban area of Jilin. The antibiotic resistance of MRSA is a serious problem with multiple drug resistance, but MRSA is sensitive to vancomycin and teicoplanin.

After a period of time developing the group psychotherapy of TCM and the community health education, Psychotherapy of TCM in community were promoted. In the community hospitals the psychotherapy work of TCM was carried out smoothly, and has been widely recognized and praised.

Objective To investigate whether nimodipine could reduce complications after microvascular decompression in patients with hemifacial spasm. Methods 220 patients with hemifacial spasm , admitted into our hospital from May 2011 to Oct. 2013 , were all treated by microvascular decompression. They were divided randomly into two groups. Patients in the treatment group were treated with nimotop 50 mg/d after the operation for the first 3 days, then was orally taken 30 mg/d for a week. Patients in the control group were treated with the same after the operation without nimodipine. The postoperative complications including hearing deficit and peripheral facial paralysis were closely observed within three months after operation. Results The total effective rate was 100% in both groups. The incidence rate of peripheral facial paralysis and hearing deficit in treatment group was significant lower than that of the control group (P < 0.05). Conclusion Nimodipine has significant effect on reducing the incidence rate of peripheral facial paralysis and hearing deficit after microvascular decompression in patients with hemifacial spasm.

Objective To compare the effect of Buzhong Yiqi Decoction (BYD) containing different doses of Radix Astragali on fibulin-3 expression in the hemorrhoid tissues of stage Ⅲ internal hemorrhoids patients with spleen deficiency and sinking of qi, and to evaluate its therapeutic efficacy and possible mechanism. Methods Fifty-five qualified patients were randomly divided into control group(N = 15), Chinese medicine group 1(N =20), and Chinese medicine group 2(N=20). All of the 3 groups were treated with operation, and additionally, Chinese medicine group 1 was given BYD containing Radix Astragali 20 g, and Chinese medicine group 2 was given BYD containing Radix Astragali 50 g orally after operation. The scores of anal pendant expansion and anal prolapse were evaluated, and the expression level of fibulin-3 in the hemorrhoid tissues was detected by Western blot method. Results After treatment, the symptoms of anal pendant expansion and anal prolapse were improved in the 2 Chinese medicine groups (P < 0.05) , and the improvement of anal pendant expansion symptom in Chinese medicine group 2 was superior to that of Chinese medicine group 1(P<0.05). Compared with the control group, the expression level of fibulin-3 in the hemorrhoid tissues of the two Chinese medicine groups was increased (P < 0.05); the expression level of Chinese medicine group 2 showed an increasing trend as compared with that of Chinese medicine group 1, but the difference was statistically insignificant (P > 0.05). Conclusion BYD with large dose of Radix Astragali exerts stronger therapeutic efficacy for the treatment of prolapsed hemorrhoids than BYD with small dose of Radix Astragali, and its therapeutic mechanism has no obvious relation with promoting the increase of fibulin-3 expression.

Objective: To explore the correlation among serum prealbumin (PAB) level, red cell distribution width (RDW) and cardiac function in advanced aged male patients with chronic heart failure (CHF). Methods: Research objects included 60 patients with heart failure（28 cases with NYHA class Ⅱ and 32 cases with NYHA class Ⅲ-Ⅳ, heart failure group）and 30 cases with normal cardiac function admitted in the same period（normal control group）. Serum PAB, hemoglobin (Hb) concentration and RDW, red blood cell (RBC) count were measured. Patients all received echocardiography examination. Changes of all indexes were compared among patients with different cardiac function class. Results: ① Compared with normal control group, there was significant decrease in serum PAB level [(252±49) mg/L vs. (185±36) mg/L] and significant increase in RDW [(13.84±0.60) % vs. (15.79±1.33) %] in heart failure group, and compared with class Ⅱ group, there was significant decrease in serum level of PAB and significant increase in RDW in class Ⅲ-Ⅳ group, P<0.01 all; ② Compared with normal control group, there were significant decrease in E/A [(1.02±0.36) vs. (0.75±0.18)] and left ventricular ejection fraction [LVEF, (59±9) % vs. (49±11) %] in heart failure group, and the worse cardiac function was, the lower these two indexes were, P<0.01 all; ③ Pearson linear correlation regression analysis indicated that PAB was positively correlated with E/A and LVEF (r=0.451, P<0.05; r=0.596, P<0.05), and RDW was negatively correlated with E/A and LVEF (r=-0.391, P<0.05; r=-0.574, P<0.05) in heart failure group. Conclusion: Prealbumin and red cell distribution width are closely correlated to cardiac function class in advanced aged male patients. Both of them can indirectly reflect cardiac function in advanced aged patients with chronic heart failure.

Objective To explore the clinical characteristics of vestibular migraine in patients from the clinic and ward of the neurological department in comprehensive hospitals.Methods A total of 226 patients diagnosed as vestibular migraine were enrolled in the study.Clinical data were collected and analyzed,including the medical history,clinical symptoms and signs,as well as the result of diagnostic examinations.Results The mean age of the patients at the visit was 51.7 years old,with the male to female ratio of 1:1.48.The occurrence of vertigo and migraine varied in order,with 53.1％ (120/226) patients presented migraine several years before vertigo.The duration time of vertigo ranged from seconds to days,with 1.8％ (4/226) patients presented no headache during the whole course.Several punctate long T2 or high FLARE (fluid attented inversion recovery) signals scattered at the centrum ovale.Conclusions Vestibular migraine has complex mechanism and presents multiple clinical manifestations with certain regularities of the onset.Differential diagnosis should be made from the similar diseases.
