2.Relationship between polymorphism of cytochrome P450 2C19 and clinical response to clopidogrel

Objective To discuss the relationships of the genotype of cytochrome P450 2C19 (CYP2C19) gene and and clopidogrel responsiveness.Methods The study enrolled 110 patients with acute myocardial infarction who were treated with clopidogrel in Tiantan Hospital of Beijing from November 2012 to May 2014.Patients were treated by aspirin and clopidogrel.CYP2C19 polymorphisms were detected by genotype testing kits.Platelet inhibition rates were measured by thrombelastography to represent the antiplatelet effect of clopidogrel.The platelet inhibition rates of clopidogrel were compared among different genotypes.Results Compared with carriers of CYP2C19 * 2 or * 3 reduced-function allele,CYP2C19 * 1 wild type had higher platelet inhibition rate of clopidogrel ((35.73 ± 19.29)% vs.(48.30± 20.84)%),and the difference was significant(t =3.264,P<0.05).There was no difference between intermediate metabolizer((35.72± 19.27)%) and poor metabolizer((35.75±19.89)%;P>0.05).The frequency of wild-type homozygotes CYP2C19 * 1/* 1 was higher in responders than in low responders(frequency of low reaction group CYP2C19 * 1/* 1 14 cases (40.0%),* 1/.2 10 cases(28.6%),* 1/* 3 4 cases(11.4%),* 2/* 2 5 cases(14.3%),* 2/* 3 2 cases (5.7%),frequency of reaction group were 45 cases (60.0%),15 cases (20.0%),4 cases (5.3 %),7 cases (9.3%),4 cases(5.3%);x2 =3.838,P =0.05).Conclusion Polymorphism of gene CYP2C19 is associated with different responses to clopidogrel.CYP2C19 * 2/ * 3 reduced-function allele is associated with low response to clopidogrel.

Persicae Ramulus decoction, as the first prescription in Treatise on febrile Diseases, has been recommended by physicians of successive generations. It is also the general prescription for harmonizing yingfen and weifen, yin and yang, qi and blood. Although it only consists of five herbal medicines, it has a wider range of application and more categorized formulas than other prescriptions. Though Persicae Ramulus decoction was originally formulated to treat taiyang apoplexy, it has functions beyond the treatment of exopathic diseases. This formula is also effective in treating internal diseases, surgical diseases, gynecologic diseases, paediatric diseases, etc. KE Yun-bo praised it as the No. 1 formula among ZHANG Zhong-jin's formulas as well as the general prescription for harmonizing Yin and Yang, yingfen and weifen, resolving fleshy exterior and inducing perspiration. Professor SHI Xin-de has been expert at treating intractable diseases by using Persicae Ramulus-associated prescriptions, such as Xiaojianzhong decoction and Baohe pill for children's chronic eczema, Persicae Ramulus and Puerariae Lobatae Radix decoction and Yupingfeng powder for chronic nephritis, and Persicae Ramulus and Longgu Muli decoction for insomnia. Instead of being restricted to Chinese or Western disease names, he prescribed appropriate formulas according to syndromes, thereby achieving a good efficacy.

4.An analysis of Clinic, CT and MRI in 150 child patients with cerebral palsy

Objective To search for the CT and MRI morphological changes in the child patients with cerebral palsy. Method The CT scranning and MRI were carried out in 150 child patients with cerebral palsy, andthe results were analysed. Results In 150 cases, 97 cases (54.6 per cent) have abnormal findings of CT scranning. Diffuse ence-phalatrophy (42 cases), encephalomalacia focus (24 cases) porencephaly (9 cases), and localized ence-phalatrophy (7 cases) ect are the common CT morphological changes in the patients with cerebral palsy.In the 53 patients with normal findings of CT scranning, MRI was carried out for 30 cases and 25 cases(83.3 per cent) were abnormal. The delayed myelinogeny of white matter of brain is the main MRI mor-phalogical abnormality. Conclusion CT scranning and MRI have important value on the etiological diagnosis for child patientswith cerebral palsy. The combination of them can improve the diagnostic accordance rate.
7.Surgical treatment of ascending aortic aneurysm caused by Marfan Syndrome:a report of 16 cases

Objective To summarize the experience of surgical treatment and to explore the oper-ation method choice of ascending aortic aneurysm caused by Marfan Syndrome. Methods The clinical da-ta of 16 patients from 2005 January to 2011 November were retrospectively analyzed. Results Among all, there was no operative mortality,but 6 cases of early postoperative complications(37. 5%),including 2 ca-ses of arrhythmia,2 cases of pulmonary infection,1 case of renal insufficiency and 1 case of pneumothorax. The follow-up time was 12~63(25 ± 9)months. During follow-up,2 patients died(12. 5%),1 due to rup-tured abdominal aortic aneurysms and 1 due to renal failure. Besides there were 2 cases of anticoagulation complications and 1 case of coronary heart disease. Conclusion Although sparing aortic root replacement of aortic valve has similar curative effect as Bentall operation and avoids the mechanical valve-related com-plications,the indications are relatively strict. Bentall operation is still the first choice for severe patients.
8.Drug resistance ananlysis of Acinetobacter baumannii strains isolated during 2014 in a hospital of Beijing

Objective To investigate the drug resistance of Acinetobacter baumannii strains isolated during 2014 in a hospital of Beijing.Methods Isolated bacteria were cultured by routine method,identified and performed drug susceptibility test by bacteria a-nalysis system.Statistical analysis of Acinetobacter baumannii was conducted by Whonet5.6 software.Results A total of 226 Acin-etobacter baumannii strains were isolated from January 2014 to December 2014 in Fengtai Teaching Hospital of Capital Medical U-niversity.These strains showed the lowest resistance rates to minocycline and Cefoperazone-sulbactam(23.0% and 30.1%).The other antimicrobial resistance rates were more than 50.0%.The resistance rate of Acinetobacter baumannii varied from one depart-ment to another.Pandrug-resistant and multi-drug resistant Acinetobacter baumannii were 71.2%,1 9.0% respectively. Conclusion The resistant rates of Acinetobacter baumannii strains to a variety of antibiotics are high.It is essential to strengthen monitoring the drug resistance of Acinetobacter baumannii,as well as use antibiotics reasonable and separate patients to control Acinetobacter baumannii infection.
9.Study on the Retrovirus Mediated Transfer of GM-CSF cDNA in Hematopoietic Progenitor Cells

In order to investigate the feasibility of granulocyte-macrophage colony stimulating factor (GM-CSF) gene therapy, murine GM-CSF cDNA recombinant retrovirus pLXSN/GM was transfered into retrovirus-packaging cell line PA3 17 by electroporation, and the transfected cells were used to infect hematopoietic progenitor cells rich populations. Transfective efficiency of NeoR gene was detected by G418 resistant CFU-GM test, and the results showed 36% them. In the genome of the infected target cells, integrated NeoR gene and GM-CSF cDNA were identified successfully by PCR and Southern Blot analysis respectively. The recombinant plasmids were showed to be capable of expressing GM-CSF mRNA in hematopoietic cells by in situ hybridization. In Dexter culture system, the present of GM-CSF-producing transduced cells inscreased mature nonadherent cell numbers as compared to controls. These results demonstrated that recombinant plasmids were successfully transfected into hematopoietic progenitor cells, and expressed in the cells. Therefore, it provided a basis for further investigation of gene therapy.
10.Changes of the expression of adhesion molecules on CD34~+ cells during the rhG-CSF mobilization

Objective To study the changes of the expression of adhesion molecules on peripheral blood CD34+ cells during recombinant human granulocyte colony stimulating factor(rhG-CSF) mobilization,and to study the influence of rhG-CSF on donors.Methods Fifteen healthy blood donors were subcutaneously injected with rhG-CSF(10?g?kg-1?d-1)for 4 to 6 days.The expressions of very late antigen 5(VLA-5,CD49e) and L-selectin(CD62L) on CD34+ cells were examined by flow cytometry before mobilization,the fourth day during mobilization and the seventh day after mobilization.Results The percentage of CD34+ cells,especially CD34+CD49e+ cells,increased significantly during mobilization,reaching the peak on the forth day,but declined to normal level when the mobilization stopped.The percentage of CD34+CD62L+ cells didn't show any significant change during the mobilization.Conclusion rhG-CSF could increase the percentage of CD34+CD49e+ cells in peripheral blood,but the percentage was reduced one week after the mobilization.rhG-CSF doesn't affect the percentage of CD34+CD62L+ cells.
