1.Noninvasive differentiation of pulmonary arterial hypertension and pulmonary venous hypertension by echocardiography

Objective To determine whether pulmonary arterial hypertension(PAH) and pulmonary venous hypertension (PVH) can be differentiated noninvasively by echocardiography. Methods Fifty-six patients with pulmonary arterial systolic pressure(PASP) ≥40 mmHg by echocardiography were involved,and cardiac catheterization performed within 7 days of each other. Based on left ventricular end-diastolic pressure or pulmonary capillary wedge pressure(PCWP) ,30 patients were classified as PAH group and 26 patients as PVH group. The early(E) and late(A) diastolic mitral inflow velocities,E/A ratios,deceleration time(DT),early dastlic mitrial annular velocity(E') and E/E' ratios were measured by conventional and Doppler tissue imaging echocardiography in the two groups. Results Compared with PVH group,the PAH group had significantly higher A,DT,PASP and E',and significantly lower E,E/A ratio and E/E' ratio (P < 0. 01 or P <0. 001). E/E' and E/A ratio was optimal indexes for differentiation of PAH and PVH,the area under receiver operating characteristic curve was 97% and 91 %, respectively. Optimal cutoff for diagnosing PVH was E/E'>9.2 (sensitivity 95%, specificity 97%) and E/A> 1. 7 (sensitivity 75%,specificity 92%). Conclusions PAH and PVH imaging can be reliably differentiated by echocardiography.
2.Interaction between bovine serum albumin and pullulan acetate nanoparticles

Objective To investigate the interaction of pullulan acetate nanoparticles (PANs) and bovine serum albumin (BSA),and to provide the basis for in vivo pharmacokinetic study of PANs.Methods Mixed solutions of different concentration of PANs and BSA solution was prepared,the interaction was studied by fluorescence quenching method and circular dichroism (CD) measurement.Apparent quenching constant (Kq) between the PANs and BSA was calculated,and the anti-stress effect induced by urea and heating of PANs-BSA solution was observed.Results The fluorescence spectrum of BSA was affected by PANs on density-related manner,and Kq calculated by the modified Stern-Volmer plot increased from 2.64×104 to 3.55 ×l05 with the concentration of PANs increased from 0.015 mg/ml to 0.25 mg/ml.With the denaturation of urea or heating,the CD spectrum of PANs-BSA and free BSA had the similar variation tendency.Conclusions The fluorescence spectrum display results show that interaction between PANs and BSA exists,but the interaction does not protect the BSA from degeneration by urea and heating.
3.Fire prevention management and practice of data room in hospital

Objective To explore the fire prevention management to ensure the operating of the data room in the hospital. Methods The hidden risks in the fire prevention of the data room were pointed out in auto fire fighting system,precision air-conditioning equipment, UPS and battery pack, compatibilization and expansion, high-voltage power supply circuit, lightningproof grounding and etc.It's indicated that the maintenance staff had deficiency in mastering related knowledge on gas fire-extinguishing system.Results The maintenance had to be strengthened for the auto fire fighting system,precision air-conditioning equipment,UPS and battery pack,and the detection should be reinforced for the high-voltage power supply circuit and lightningproof grounding system. It's suggested that standardized construction be executed during data room compatibilization and expansion,corresponding management system be established,and the knowledge be mastered on the composition,operation and announcements.Conclusion The fire prevention management of the data room is enhanced in the hospital,and the hidden risks are eliminated for fire fighting.
4.A crossover control study of prophylactic treatment of chemotherapy-induced constipation by senna extract.

OBJECTIVETo observe the prophylactic effects of senna extract 40 mg/kg on chemotherapy-induced constipation.
METHODSEighty-two patients suffering from constipation after chemotherapy were assigned to Group AB and Group BA. Group AB referred to patients who first took senna extract in the 1st chemotherapeutic course and the crude fiber diet in the 2nd chemotherapeutic course. But the sequence was just the opposite in Group BA. The effective rates of relieving chemotherapy-induced constipation by senna extract and by the crude fiber diet were observed. The differences of the digestive tract reaction and the hematotoxicity reaction were compared. The conditions of patients' abdominal pain and stool properties were observed after they took senna extract.
RESULTSThe effective rate of constipation by taking senna extract was 92.68% and that by the crude fiber diet was 10.93%, with statistical difference shown (P < 0.01). There was no statistic difference in adverse reaction rate such as decreased neutrophils over degree II, decreased hemoglobin, decreased platelet, nausea, vomit, etc. (P > 0.05). The occurrence rate of abdominal pain over degree II after taking senna was 8.54%. In the distribution of stool properties, the rate of loose stool was 35.53%.
CONCLUSIONSenna extract 40 mg/kg was effective and safe in treating chemotherapy-induced constipation.

7.Determination of 1, 1-dichloro-l-nitroethane in air of workplaces by gas chromatography.

OBJECTIVETo establish a gas chromatography method for detecting the concentration of 1,1-dichloro-1-nitroethane in air of workplaces.
METHOD1,1-dichloro-1-nitroethane in air of workplaces was collected by activated charcoal tube, absorbed using carbon disulfide and analyzed by Gas Chromatography (FID) with FFAP capillary column.
RESULTSThe linear rang of 1,1-dichloro-1-nitroethane in this method was 4.0-858.2 microg/ml, the linear regression formula was Y = 283X-1076, the correlation coefficient was 0.9999, the lowest detection concentration was 0.4 mg/m3 (3L sampling air), the relative standard deviation (RSD) was 1.8%-4.1%, the desorption efficiency was 88.5%-90.6%, the breakthrough volume was > 0.7 mg, the sampling efficiency was 100%, the samples could be kept at ambient temperature for at least 7 days.
CONCLUSIONThe indicators of this method were conformed to the requirements of "Guide for establishing occupational health standards--Determination methods of air chemicals in workplace". This method could be used to detect 1,1-dichloro-1-nitroethane in air of workplaces.

8.Fibrous dysplasia: molecular clonality analysis of 21 cases.

OBJECTIVETo investigate the neoplastic nature of fibrous dysplasia by molecular clonality approaches.
METHODSTwenty-one cases of fibrous dysplasia were examined by clonality assays based on X-chromosomal inactivation mosiacism. Lesional and non-lesional tissues were microdissected from paraffin sections followed by DNA extraction. The DNA was predigested by HpaII or HhaI, and then amplified by nested PCR targeting phosphoglycerate kinase (PGK) and androgen receptor (AR) genes. Single nucleotide polymorphism (SNP) at the PGK locus was identified by incubation of the PCR products with Bst XI and agarose gel electrophoresis, and CAG repeat length polymorphism at AR locus was determined by denaturing polyacrylamide gel electrophoresis and visualized by silver staining.
RESULTSMicroscopically, all 21 cases showed characteristic features of fibrous dysplasia, including spindle fibrous cell proliferation and immature bone trabeculae at various proportions. DNA polymorphisms at AR locus and SNP of PGK gene were found in 15 of 21, and 4 of 21 cases, respectively. All 19 cases were monoclonal in nature. Two cases showed no polymorphism at either AR or PGK gene locus.
CONCLUSIONSFibrous dysplasia is likely a clonal, neoplastic process. Additional studies of larger number of cases are needed for a definitive conclusion.

9.Clinical utility of the ratio between circulating fibrinogen and fibrin (ogen) degradation products for evaluating coronary artery disease in type 2 diabetic patients.

BACKGROUNDWe investigated whether and to what extent the ratio between circulating fibrinogen (Fg) and its degradation products (FDP) reflects the severity of coronary artery disease (CAD) in type 2 diabetic patients.
METHODSPlasma levels of Fg and FDP were determined, and Fg/FDP ratio was calculated in 344 consecutive patients with type 2 diabetes and chest pain on exertion undergoing coronary angiography. The severity of CAD was evaluated by the number of significant CAD (>50% luminal diameter narrowing) and Gensini score.
RESULTSPlasma Fg was higher, but Fg/FDP ratio was lower in patients with significant CAD (n = 255) compared with those without (n = 89), due to a disproportionate increase in FDP. Fg and FDP correlated positively, while Fg/FDP ratio negatively, with the number of diseased coronary arteries and the tertile of Gensini score (all P values for trend < 0.01). After adjusting for age, sex, risk factors for CAD, lipid profiles, glycosylated hemoglobin A1c, creatinine, leukocyte count, and high-sensitivity C-reactive protein, Fg/FDP ratio remained an independent determinant for multivessel coronary disease (MVD) (odds ratio [OR], 0.869; 95% confidence interval [CI], 0.788-0.958, P = 0.005) and high tertile of Gensini score (OR, 0.797, 95% CI, 0.682-0.930, P = 0.004). The area under the curve of Fg/FDP ratio was larger than that of Fg for predicting the presence of MVD (0.647 vs. 0.563, P = 0.048) and Gensini score ≥ 30 (0.656 vs. 0.538, P = 0.026).
CONCLUSIONSElevated plasma Fg and FDP level and reduced Fg/FDP ratio are associated with presence of CAD, and Fg/FDP ratio is superior to Fg in reflecting severe coronary atherosclerosis for patients with type 2 diabetes.

10.Isolation and characterization of a human enterovirus 74 isolated in Shandong Province, China.

Human Enterovirus HEV 74 is a new member of species Human enterovirus B (HEV-B). To understand its evolution and restructuring characteristics, we report the complete genome sequence of a HEV74 strain 05293/SD/CHN/2005(abbreviated as 05293) isolated from an acute flaccid paralysis (AFP) case in Shangdong Province, China, 2005. Analysis of the complete genomic sequence of 05293 showed that its genome was collinear with that of previously described 2 HEV74 strains, except for insertions and deletions at the 5'NTR and the 3 NTR regions. The complete genome sequence of strain 05293 displayed 80. 8% nucleotide and 96% amino acid identity to the prototype strain USA/CA75-10213, and 80. 6% and 95. 9% to another isolated strain Rikaze-136. The P1, P2 and P3 coding regions of strain 05293 displayed 81. 5%, 80. 0%, 79. 7% nucleotide and 95. 9%, 96. 0%, 96.2% amino acid identity to the prototype strain USA/CA75-10213, and 81. 9%, 78. 8%, 79. 5% and 95. 9%, 96. 1%, 95. 7% to strain Rikaze-136, respectively. The phylogenetic tree and Simplot analysis on 05293 and HEV-B genome sequences were performed, and the result indicated frequent recombination within HEV-B.
