Objective To investigate the effects of 1,25-dihydroxyvitamin D3[1,25(OH)2D3] on regulatory T cells(CD4+CD25+ Foxp3+ T cell)and airway inflammation in asthmatic rats. Methods Asthmatic models were established in 28 rats with ovalbumin as sensitinogen,and four groups were randomly divided(n=7).1,25(OH)2D3 was orally administered 1 h before challenge in treatment group 1,dexamethasone was subcutaneously injected 1 h before challenge in treatment group 2,both 1,25(OH)2D3 and dexamethasone were given 1 h before challenge in treatment group 3,while no medicine was administered 1 h before challenge in group 4(asthmatic control group).The levels of IL-4 and IL-10 in bronchoalveolar lavage fluid(BALF) were detected,the total cell numbers were obtained and the eosinophil count was performed.The ratios of Treg to CD4+ T cells in peripheral blood and spleen mononuclear cells were measured with flow cytometry,and the mRNA expression of IL-10 and Foxp3 in lung tissues was detected.Wistar rats without asthma were served as normal controls(n=7). Results There were significant differences between asthmatic control group and normal control group in each parameter.Compared with asthmatic control group,the total cell number and eosinophil count decreased,the level of IL-4 decreased and the level of IL-10 increased in BALF in treatment groups.The ratios of Treg to CD4+ T cells in peripheral blood and spleen mononuclear cells significantly increased,and the mRNA expression of IL-10 and Foxp3 in lung tissues increased in treatment groups compared with asthmatic control group.There was no significant difference between treatment groups and normal control group in each parameter. Conclusion Administration of 1,25(OH)2D3 increases the proportion of Treg and expression of Foxp3 mRNA in peripheral blood and spleen in asthmatic rats.1,25(OH)2D3 has a regulatory effect on Treg,and may serve as a potential therapeutic approach against asthma.

Objective To observe the earlier changes of left heart structure and function in patients with Grade 1 hypertension.Methods 65 patients with Grade 1 hypertension were measured on the left atrial diameter(LAD),left ventricular diameter,thickness of ventricular,mass index and evaluate the left ventricular systolic and diastolic function by UCG.Results Compaired to 50 normal controls,the LAD and left ventricular diastolic function of the patients(38.06?3.68 mm,0.72?0.11) was significantly different with those of 50 normal controls(29.86?3.08 mm,1.18?0.08)(P

·AIM: To study the tear function changes in patients with pterygium before and after pterygium transposition.·METHODS: Twenty eyes of 20 patients were enrolled in this study.Schirmer I test and tear break-up time (BUT) were evaluated in patients before and after pterygium transposition.·RESULTS: There was no significant difference in the results of Schirmer I test before and after the surgery.The results of BUT were not significantly different before and up to 4 weeks after surgery. However, BUT prolonged significantly 6 weeks after surgery (P<0.05).·CONCLUSION: Pterygium transposition can improve the tear film function in patients with pterygium.

The bile was collected from fro patients with biliary infections, with the bacterium isolated to study the sensitivity of each kind of the bacterium to several antibiotics in common use. Except G- bacterium, we also found some kinds of G+ bacterium in infection bile. G- bacterium were not sensitive to Clindamycin, G+ bacterium were sensitive to Ciprofloxacin. Escherichia coli, Xanthomonas maltophilia, Enterobacter cloacae, Pseudomonas aeruginosa were sensitive to Ampicillin. G+ bacterium were not sensitive to Azactam. Enterococcus faecalis, Enterococcus faecium, Enterobacter cloacae were not sensitive to Ceftazidime. Enterococcus faecalis, Staphylococcus coagulase negative, Staphylococcus epidermidis, Pseudomonas aeruginosa were not sensitive to Ceftriaxone Sodium. We didn't found any bacterium resistance Imipenem. The possibility of the existence of G+ bacterium as well as drug resistance should be considered n patients with biliary infections. The value of susceptibility test should be respected to avoid drug abuse of antibiotics.

OBJECTIVE: To analyses the causes and prevention of systemic complications of functional endoscopic sinus surgery (FESS) in patients with chronic rhinosinusitis. METHOD: Three typical cases were reported including their medical history, preoperative diagnosis, medications during preoperational period, complications and treatment. The causes and preventive measures of systemic complications were analyzed. RESULT: Three patients were all suffered from chronic rhinosinusitis with nasal polyps (CRSwNP). After FESS, 1 case was complicated with coma and hyponatremia, 1 case with acute myocardial infarction, and 1 case with lower extremity deep venous thrombosis. The patient with coma and hyponatremia was soon waked after intravenous infusion of 10% sodium chloride. Two patients with acute myocardial infarction and lower extremity deep venous thrombosis were soon completely rehabilitated after emergency thrombolytic therapy and endovascular intervention. Three patients were completed recovered from their systemic complications without any severe sequela. CONCLUSION Systemic hemostatic drugs should be banned in patients with hypercoagulable state in perioperation period of FESS in order to avoid severe systemic complications. Timely vascular interventional treatment can prevent severe sequels.

Objective To explore the clinical characteristics of vestibular migraine in patients from the clinic and ward of the neurological department in comprehensive hospitals.Methods A total of 226 patients diagnosed as vestibular migraine were enrolled in the study.Clinical data were collected and analyzed,including the medical history,clinical symptoms and signs,as well as the result of diagnostic examinations.Results The mean age of the patients at the visit was 51.7 years old,with the male to female ratio of 1:1.48.The occurrence of vertigo and migraine varied in order,with 53.1％ (120/226) patients presented migraine several years before vertigo.The duration time of vertigo ranged from seconds to days,with 1.8％ (4/226) patients presented no headache during the whole course.Several punctate long T2 or high FLARE (fluid attented inversion recovery) signals scattered at the centrum ovale.Conclusions Vestibular migraine has complex mechanism and presents multiple clinical manifestations with certain regularities of the onset.Differential diagnosis should be made from the similar diseases.

ObjectiveTo analyze the differences of United States (US) and France (Fr) Risser grading systems in determining the growth potentiality of girls with adolescent idiopathic scoliosis (AIS).Methods Girls with AIS undergoing posterior spinal correction with autogenous bone graft were recruited.The Risser grades were recorded for the non-dominant side iliac crest apophysis according to the US and Fr grading systems.Iliac crest cartilage biopsies were harvested during surgery then standard histologic grades(HGs) of proliferation activity of iliac cartilage were analyzed on histological section.The agreement of two grading systems was analyzed by kappa statistics while the correlation between Risser grades and HGs were analyzed by Spearman correlation analysis.Receiver Operating Characteristic (ROC) curve was employed for selecting the best parameter in determining growth cessation of girls with AIS.ResultsFifty-three AIS girls with an average age of 14.0 years were recruited.The agreement of two grading systems was poor.The US system showed higher correlation with HGs compared to Fr system.Growth cessation could be determined by Risser grade 5 of US system or Risser grade 4 of Fr system,also older than 16 years or three years after menarche.When combining the results of ROC curve analysis,the Risser grade 4-5 of US system with two years after menarche could be used in determining growth cessation with the highest sensitivity,specificity and accuracy.Furthermore,the mean age of patients with growth cessation determined by this method was 15.2 years,showed 0.9-1.4 years earlier than the age determined by other methods.ConclusionThe US Risser grading system is better in determining maturity of AIS girls compared to Fr Risser grading system.By combining years since menarche,the accuracy of Risser grades in determining growth cessation could be enhanced,and the time of weaning from a brace could be advanced.

Objective To evaluate the effect of self-management interventions on the quality of life of patients with heart failure(HF).Methods The randomized controlled trials(RCTs)taking self-management interventions were collected from the databases of EMBASE,Medline,VIP,CNKI and WanFang.Data were analyzed with RevMan 5.1 software.Results 12 articles met the inclusion criteria.There was considerable heterogeneity across the analysis,which might be result from length of intervention,age and NYHA classification according to subgroup analysis.Self-management interventions could significantly improve the quality of life of patients with HF,including both physical and emotional aspects.Conclusions Self-management interventions can improve the quality of life of patients with heart failure.
