1.Experimental study about the rat model of diffuse brain injury and the pathophysiological characteristics in the rats after injury

Objective To develop-the rat model of diffuse brain injury(DBI)and to observe the pathophysio logical alteration of the rats after.traumatic injury.Methods A modified Marmarou's traumatic device W88 used to establish the animal model of diffuse brain injury with different degree through changing the impact condition that was applied on the rat brain(including 80cm 450g,120 cm 450g and 180cm 400g);the animal model was evaluated by observing the variations of nervous system signs,vital signs,sensorimotor function,and morphological changes of brain tissue in the rats that have received different impacts.Results The significant changes in nervous system signs,vital signs and sensorimotor function had been observed in all rats that were subjected to the different impact.The pathologic changcs of DBI in rats brain could he found in all groups.Moreover,this characteristic changes would have became more significant following the traumatic iaiury when the impact energy was increased.Conclusion The rat model of graded diffuse brain injury have been developed successfully.The moderate impact(120cm 450g)could produce low animal death rate and result in obvious pathologieal alterations in the injured rat brain.This impact condition would be suitable to be used in the study about DBI.
3. Preparation of donepezil sustained release microspheres and pharmacokinetic research in rabbits

OBJECTIVE: To prepare and evaluate donepezil sustained-release microspheres by ultrasound technique. METHODS: Preparation technology of donepezil biodegradable microspheres by ultrasound technique was established and optimized. In vitro evaluation of donepezil microspheres was carried out. The pharmacokinetics of donepezil microspheres was investigated by LC-MS/MS after subcutaneous injection in rabbits at a dose of 30.0 mg · kg. RESULTS: Donepezil microspheres with drug loading of 12.1% and mean particle size between 40 to 130 μm were successfully prepared by ultrasound technique. The donepezil microspheres displayed a one-month sustained-release character in vitro. The pharmacokinetic parameters were as follows: pmax 40.63 μg · L-1, tmax 2.47 d, MRT 14.81 d, and AUC0→∞ 646.96 μg · d · L-1. CONCLUSION: Ultrasound technique is successfully applied in the preparation of donepezil sustained-release microspheres.
4.Bone cement kyphoplasty for repair of rheumatoid arthritis combined with Kümmell’s disease:a follow-up addressing vertebral height and spinal stabilization

BACKGROUND:For patients with rheumatoid arthritis and Kümmel ’s disease, how to effectively control back pain, to recover patient’s locomotor activity and to avoid a vicious cycle of disuse osteoporosis is a key therapeutic target. Kyphoplasty is a recently developed new technology of minimal y invasive spine surgery. Few reports concerned the kyphoplasty for rheumatoid arthritis and Kümmel ’s disease. OBJECTIVE:To assess the clinical outcome of bone cement kyphoplasty for the treatment of Kümmel ’s disease combined with rheumatoid arthritis. METHODS:From June 2012 to July 2013, 11 female patients at the age of 65.4±5.1 years with Kümmel ’s disease combined with rheumatoid arthritis, who suffered from severe back pain, were treated with bone cement vertebroplasty. Back pain and imaging indexes were compared and observed before surgery and during fol ow-up. Imaging indexes contained preoperative and postoperative anterior height of vertebral body after fractures, the ratio of anterior height to posterior height of the vertebral body, and local kyphosis angle (Cobb method). RESULTS AND CONCLUSION:No patients were lost to fol ow up. 11 patients were fol owed up for 6 to 12 months. Significant differences in fol ow-up and preoperative Visual Analogue Scale scores, anterior height of vertebral body after fractures, the ratio of anterior height to posterior height of the vertebral body, and local kyphosis angle were detected (P<0.05). Two patients experienced bone cement leakage. No severe complications appeared such as pulmonary embolism or neurological dysfunction. These data confirmed that bone cement vertebroplasty for rheumatoid arthritis combined with Kümmel ’s disease can effectively lessen back pain, partial y restore the height of vertebral body after fracture, rebuild spinal stabilization, reduce local kyphosis, and is a safe effective repair method.
5.Study on distribution and drainage of lymphatic vessels of tongue.

OBJECTIVETo investigate the distribution and drainage of lymphatic vessels of tongue, and to provide anatomical evidence for treatment of tongue cancer.
METHODSIndirect lymphatic injection was employed, combined with clearing method with winter green oil and corrosive cast technique, to study the distribution of lymphatic vessels of tongue. Anatomical methods were used to detect the sentinel lymph nodes in different region of tongue.
RESULTSThe lymphatic vessels of dorsal mucosa composed of lymphocapillary vessels and anstomosing side branches were present by superficial and deep capillary networks. The distribution of lymphatic networks extend from tip to base and from one board to another, and was not influenced by the sulcus tenninalis and median lingual sulcus. Lymphatic vessels in the muscular portion communicated with lymphocapillary network of dorsal and ventral mucosa, which made the lymphatic vessels of tongue to be an integrity network structure. These characters of distribution influenced the lymphatic drainage of tongue. The results showed principal sentinel lymph nodes (SLNs) for anterior part of tongue were submental lymph nodes, submandibular lymph nodes and juguloomohyoid lymph nodes, for lateral part and middle part of tongue were submandibular lymph nodes, jugulodigastric lymph nodes and thyroid lymph nodes, and for root part of tongue were jugulodigastric lymph nodes. SLNs for every injection region were all presented at bilatral neck, but the frequency of stained SLNs at homolateral neck was more than that at contralateral neck.
CONCLUSIONThe lymphatic vessels of tongue arranged like a network, which made the lymphatic drainage at various ways and made the distribution of sentinel lymph nodes to be bilateral and dispersive.

6.Formulation and analysis of comprehensive quality competency evaluation scale for medical graduates

Objective:To meet the demand of medical system for talents, the training of medical students' competency has become a new direction of medical education. This study aims to evaluate the effectiveness of training quality in medical graduates through the competency scale.Methods:Taking "attitude", "skill" and "knowledge" as the evaluation dimensions, the competency development was divided into four levels of "state", "explain", "apply" and "transfer", and we proposed the competence concept of "A.S.K.-SEAT" and formulated an evaluation scale. Questionnaires and behavior event interviews (BEI) were conducted in medical graduates of Shantou University in 2018. Reliability and validity of the questionnaire were evaluated and current situation of different competency items were analyzed.Results:A total of 155 questionnaires were collected with good reliability and validity, and 15 graduates participated in BEI. A total of 21 A.S.K. competency items (including five basic competency items and two discriminating competency items) and SEAT textual descriptions were finally established.Conclusion:A.S.K.-SEAT scale can provide valid references for the competency evaluation of medical graduates.
7.The oxidative stress enhanced adenoviral E1A-involved transactivation of nuclear factor-kappaB.

OBJECTIVEThe relationship between latent adenovirus infection and airway inflammation have not been well documented. The aim of this study is to illustrate the roles of adenovirus E1A protein on the level of glutathione (GSH) in response to oxidative stress and the effect of the oxidant/antioxidant imbalance upon the transactivation of NF-kappaB triggered by E1A protein.
METHODSRat alveolar epithelial cell stably expressing adenoviral E1A or control plasmid were developed. For isolation of nuclear extracts, 5 x 10(5) cells were plated and grown overnight in 60 mm dishes. Experiments were repeated three times. The cell model of stably expressing adenoviral E1A was stimulated by H2O2. The level of GSH were measured. E1A positive clone was stimulated by LPS or TNF-alpha and treated with L-Buthionine-sulfoximine (BSO). The expression of NF-kappaB was measured by Western blot. Differences between groups were assessed for significance by Student' t test; multiple comparisons by the one-way ANOVA.
RESULTSThere is no difference of GSH level without stimulation between E1A-positive clones and E1A-negative clones. For E1A-positive clones, the level of GSH did not increase in response to H2O2 as E1A-negative clones. The quantitation by densitometry of the NF-kappaB expression in E1A-positive clones were (79.3 +/- 4.6), (80.3 +/- 3.8) respectively without treatment and were (81.8 +/- 3.9) - (89.9 +/- 1.6) and (94.1 +/- 1.9) - (99.8 +/- 1.6) respectively under LPS or TNF-alpha stimulation, which were significantly higher than that of the control group (68.3 +/- 3.8), (69.4 +/- 4.3) respectively without stimulation and (70.1 +/- 2.8) - (80.8 +/- 3.6), (73.4 +/- 4.9) - (83.2 +/- 6.7) respectively under stimulation. The quantitation by densitometry of the NF-kappaB expression in E1A-negative clones were (1.25 +/- 0.18) and (1.69 +/- 0.19) respectively under LPS and TNF-alpha-stimulation and (1.22 +/- 0.16) and (1.75 +/- 0.13) respectively upon treatment for LPS and TNF-alpha with BSO preincubation. There did not show difference upon treatment with LPS or TNF-alpha with or without BSO in E1A-negative cell clone. The quantitation by densitometry of the NF-kappaB expression in E1A-positive clone were (1.75 +/- 0.10) and (2.26 +/- 0.21) respectively upon treatment for LPS and TNF-alpha with BSO preincubation which were significantly higher than that of LPS or TNF-alpha-stimulation alone (1.35 +/- 0.12), (1.80 +/- 0.14) respectively.
CONCLUSIONThese results indicate that E1A protein decreased GSH levels in oxidant stress and upregulated NF-kappaB transcription activity. The oxidant/antioxidant imbalance in rat alveolar epithelial cells enhances E1A-modulated transcriptional activation of NF-kappaB. The mechanism underlying transactivation of NF-kappaB involved by E1A may be related to oxidative stress.

8.DNA vaccine encoding Der p 2 allergen generates immunologic protection in recombinant Der p 2 allergen-induced allergic airway inflammation mice model.

BACKGROUNDDNA immunization is a promising novel type of immunotherapy against allergy. An estimated 79.2% patients with asthma, wheezing and/or rhinitis suffer from Dermatophagoides pteronyssinus group 2 (Der p 2) allegen. The aim of the present study was to determine whether DNA vaccine encoding Der p 2 could generate immunologic protection in recombinant Der p 2 (rDer p 2) allergen-induced allergic airway inflammation mice model and to understand the role of DNA vaccination in specific-allergen immunotherapy for asthma.
METHODSAfter DNA vaccination, BALB/c mice were sensitized by intraperitoneal injection (i.p) and challenged by intranasal instillation of rDer p 2. The lung tissues were assessed using hematoxylin and eosin. Mucus-producing goblet cells were identifed using periodic acid-Schiff (PAS)/alcian blue. The total cell number and composition of bronchoalveolar lavage samples were determined. The levels of the cytokines IL-4 and IFN-gamma, as well as IgE and IgG2a in the serum were determined by enzyme-linked immunosorbent assay. Allergen-specific IL-4 and IFN-gamma production by spleen cells were also measured by enzyme-linked immunosorbent assay. Expression of signal transducer and activator of transcription 6 (STAT6) in splenocytes were determined by Western blot.
RESULTSDNA vaccine encoding Der p 2 allergen inhibited extensive infiltration of inflammatory cells and production of mucin induced by allergen. The influx of eosinophils into the lung interstitium was significantly reduced after administration of DNA vaccine. Significant reductions of IL-4 and increase in levels of IFN-gamma in bronchoalveolar lavage fluid were observed. The allergen-specific IgE was markedly decreased in mice receiving DNA vaccination. Allergen could induce higher IFN-gamma, weaker IL-4 in cultured spleen cells from mice receiving DNA vaccine. DNA vaccination inhibited STAT6 expression of spleen cells induced by allergen.
CONCLUSIONThese results indicated that DNA vaccine encoding Der p 2 allergen generates immunologic protection in recombinant Der p 2 allergen-induced allergic airway inflammation mice model with regulating the immune response towards a Th1-type reaction.

9.Study on clinical effect and mechanism of jianpi qingre huayu recipe.

OBJECTIVETo study the effects of Jianpi Qingre Huayu Recipe in curing gastric ulcer and to preliminarily probe into its pathogenic mechanism.
METHODSFifty patients with gastric ulcer of Pi -insufficiency and stasis-heat syndrome type were assigned to the treated group (30 patients) and the control group (20 patients). They were treated respectively with JQH and Ranitidine. At the same time, another group consisting of 20 healthy persons was set up for normal control. The clinical effect on gastroscopic figure and traditional Chinese medicine (TCM) syndrome were observed. Changes of T-cell subsets and interleukin-8 (IL-8) in serum as well as IL-8 in mucosa around the gastric ulcer were determined before and after treatment by flow cytometry and ELISA.
RESULTSComparison of the total effective rate on gastroscopic figure in the treated group and the control group (86.7% vs 80.0%) showed insignificant difference, but the cure rate and markedly effective rate in the former (50.0% and 20.0%) was higher than that in the latter (40.0% and 15.0%) respectively. Comparison of the total effective rate on TCM syndrome in the treated group and in the control group (96.7% vs 70.0%) showed insignificant difference, but the cure rate and markedly effective rate in the former (63.3% and 23.3%) was higher than that in the latter (50.0% and 20.0%) respectively. Serum levels of CD3+, CD4+, CD8+ got restored to normal range in the treated group after treatment but it was not so in the control group. IL-8 level in gastric mucosa was improved in both groups but the improvement in the treated group was better.
CONCLUSIONJQH could effectively treat gastric ulcer and partly reduce its recurrence through improving patients' immune function.

10.The Investigation of Students' Entrepreneurial Psychological Quality in Kunming Medical University

Objective To investigate the status of psychological quality of college students ' innovation and entrepreneurship, identify the elements of the psychological quality and problems and countermeasures against the psychological quality training in medical schools. Methods A questionair was made and administered to Kunming Medical University students and Chi-square analysis was used to analyze the gender, major, grade, and entrepreneurial intention differences between different groups of students so as to analyze the psychological factors affecting College students ' undertaking intention. Results A total of 1150 questionnaires,1099 was recovered,the effective recovery rate was 95.6%. Psychological quality score was (76.9 ±9.0), good mental qualities only was 8.2%, entrepreneurial environment, factors of psychological quality of good-willed persons were only 0.5%and 15.5%. Female entrepreneurial intentions were better than those of men (<0.001) . Science students' undertaking intention was superior to liberal arts students ( =0.025), knowledge of entrepreneurship difference between knowledge of business groups and <0.005,there was a statistically significant difference, about the environment worse group entrepreneurial intention superior knowledge of business groups. Conclusions As students' psychological qualities are poor,have poor psychological quality,in teaching we should strengthen the cultivation of medical students' entrepreneurial ability and correct understanding of entrepreneurship, create a good business environment, increase the entrepreneurship policy advocacy efforts, and create a good business environment and atmosphere so as to strengthen the entrepreneurship education, improve entrepreneurship courses so that students fully understand the cognitive and psychological qualities, stimulate the entrepreneurial motivation, lay the Foundation for entrepreneurship.
