1.Study on the correlation of severity of acute pancreatitis with abnormal glucose metabolism and its prognosis

Objective To investigate correlation between severity of acute pancreatitis and abnormal glucose metabolism and its prognosis.Methods A total of 158 cases of patient admitted with acute pancreatitis in the Department of Digestion of our hospital from December 2004 and December 2012 were selected,and were divided into two groups:mild acute pancreatitis (MAP; n =68) and severe acute pancreatitis (SAP; n =90) according to disease severity.The patient's sugar metabolism such as fasting plasma glucose (FPG),postprandial plasma glucose (PPG) and acute physiology and chronic health evaluation Ⅱ (APACHEII) scores of two groups were detected.Correlation analysis was carried out between glucose metabolism index and APACHEII score.The prognosis of two groups was observed.Results The FPG,PPG,fasting insulin levels (FINS),and insulin resistance index (IRI) levels of SAP group were significantly higher than that of MAP group.The insulin sensitivity index (ISI) level was lower than that of MAP (P <0.01).APACHE Ⅱ score of acute pancreatitis (AP) patient was positively correlated with FPG,PPG,FINS,and IRI (P <0.05,P < 0.01),and was negatively correlated with ISI (P <0.05).The incidence of pseudocyst (infection,abscess,and necrosis),acute lung injury or acute respiratory distress syndrome (ARDS),and death in SAP group were significantly higher than that in MAP group (P < 0.05,P< 0.01).Conclusions There was a close relationship between AP severity and abnormal glucose metabolism.Therefore,in treating AP,ones should be actively controlled acute pancreatitis to prevent and reduce pancreatic necrosis,decrease blood glucose to normal range,and reduce morbidity and mortality.

3.Clinical observation of tension-free vagina l tape procedure as a new surgical treatment method for female urinary incontine nce

Objective To study the clinical effect of tension-free vaginal tape (TVT) procedure as a new surgical treatment method for female urinary incontinence. Methods The outcome of 23 TVT procedures for female urinary incontinence was retrospecti vely analyzed from May 2001 to Feb 2003. Results Out of the 23 cases recruited into this study, 18 (78%) had stress urinary incon tinence (SUI), and 5 (22%) had SUI with mixed motorurge urinary incontinence (MU I). One (4%) case was changed to Kelly′s operation intraoperatively. The success rate of TVT was 96% (22/23). Twenty (91%) of the 22 women were comple tely cured without any complications. Two patients had dysuria caused by MUI a nd infection, respectively, and recovered after bladder catheterization, and exe rcising bladder and medication therapy. The average time of TVT procedure was 49 min. The cure rate was 100%. Conclusions The TVT procedure is a simple, safe and effec tive, minimally invasive operation for female SUI. Prevention and control of in fection, and proper adjustment of TVT tension are the keys to avoid urinary ret ention after operation. SUI with MUI is not an absolute contraindication for TV T.
4.Transcription factor Ets-1 expression in human fetal membranes and its clinical significance

Objective To investigate the role of transcription factor Ets-1 involved in premature rupture of membranes(PROM) by detecting its expression and location in human fetal membranes. Methods Between Feb. and Nov. 2007, 100 pregnant women who delivered in the First Affiliated Hospital, Chongqing Medical University were enrolled in this study. According to gestational weeks, rupture of amuiochorionic membranes and delivery mode, those women were classified into preterm labor group, preterm premature rupture of membranes (PPROM) group, term labor group and term premature rupture of membranes (TPROM) group, matched with elective term cesarean sections women as control group. There were 20 pregnant women in every group. The expression and distribution of Ets-1 protein in fetal membranes were detected by immunohistochemical streptravidin-biotin peroxidase(SP) method and histoscore. In the mean time, 6 cases were chosen from each group randomly and reverse transcription-PCR was used to measure the Ets-1 mRNA expression. Results (1) The expression of Ets-1 mRNA in fetal membranes were 0.342±0.016 in preterm labor group,0.603±0.027 in PPROM group,0.325±0.013 in term labor group, 0.582±0.075 in TPROM group,0.139±0.012 in control group, respectively. When compared with that in control group, Ets-1 mRNA expression were significantly increased in both PPROM and TPROM group(P< 0.05). However, it did not show remarkably difference between preterm group and term labor group, as well as between PPROM and TPROM group (P>0.05). (2) Ets-1 was in stromal layers of amniochorionic membranes and in both cytoplasm and nuclei of trophoblast, which were shown with diffuse intracellular positive granularities (brown-yellow) clearly. No expression of Ets-1 was observed in amniotic epithelial cells. (3) The expression of Ets-1 protein was 0.552±0.018 in preterm labor group, 2.853±0.174 in PPROM group, 0.538±0.042 in term labor group, 2.731±0.090 in TPROM group and 0.214±0.013 in control group, respectively. Ets-1 protein was significantly increased in the stroma of both PPROM and TPROM group than that in control group(P<0.05). However, no remarkable different expression of Ets-1 was observed between preterm and term labor group,so was that between PPROM and TPROM group(P> 0.05). Conclusion Transcription factor Ets-1 is expressed in human amniochorionic membranes and it could be up-regulated in PROM.
5.Willingness of local residents to accept paid influenza vaccine in Chongqing and its influencing factors: a survey

A total of 450 residents from three neighborhood committees of Ba'nan were selected to assess their willingness to accept paid influenza vaccine and the influencing factors.Among them,273(60.7%) were willing to accept paid influenza vaccine.Age,economic status,health,flu recognition and vaccine approval were the major influencing factors of vaccination.
6.Development and Application of Cost-Benefit Analysis of Medical Equipment on HIS

Objective In order to realize the real time collection of the data related to the usage of medical equipment, input of the running cost and the cost-benefit analysis, the software with the cost-benefit analysis of medical equipment is developed. Methods On the basis of No.1 Military Medical Project, the software was interlinked to the subsystem of it such as the pricing system, paying system and report system etc. Results The evaluation system of management of medical equipment was established based on the income of large-scale medical equipment and expendables, the running cost, the ratio of cost -benefit, the evaluation. Conclusion The scientific and standard management of medical equipment is accelerated with strengthening the information management of medical equipment.
7.Research progress of optimizing vancomycin individual administration with the PK/PD model

e: Vancomycin is a glycopeptide antibiotic separated from streptomycete, having been used as the ifrst choice to treat methicillin-resistant Staphylococcus aureus infection so far. The studies show that because of the individual difference in the metabolism of vancomycin, it is dififcult to get the trough concentration of pediatric patients severely ill or complicatedly in-fected to reach the target range (15—20 mg/L). However, with the help of therapeutic drug monitoring (TDM) of vancomycin and the pharmacokinetic/pharmacodynamic parameter (PK/PD) mode, the PK/PD parameters to achieve the precise control can be acquired by using the Bayes feedback. By a stretched review from clinical medication guide of vancomycin to the latest evidence from research, this paper fully demonstrates that renal function, weight, age, and disease state are the principal parameters to impact pediatric patient’s vancomycin metabolism and that the area under the concentration-time curve divided by the minimum inhibitory concentration (AUC/MIC)≥400 is the better cut-off value to determine vancomycin efifcacy and toxicity.
8.Functions and characteristics of calcium-sensing receptorsand effects of CaSR on characteristics of adult stem cells

Calcium-sensing receptor(CaSR) is a widely distributed G-protein coupled receptor.The activated CaSR plays an important role in many kinds of signaling pathway regulation, such as Ca2+ signaling pathway.It not only maintains the body calcium balance, but also is involved in the regulation of a variety of cell secretion, proliferation, apoptosis, and ion channel opening processes.CaSR expression is involved in stem cell migration, adhesion and homing in hematopoietic stem cells and mesenchymal stem cells.The activated CaSR also regulated the function of itself and characteristics in stem cells through a variety of cell signaling pathways.We introduce the functions and characteristics of CaSR, the relationship between CaSR and disease, and review the effects of the biological characteristics on hematopoietic stem cells and mesenchymal stem cells.
9.Protective effect of hydrogen sulfide on pressure ulcer rats with ischemia/reperfusion injury and its mecha-nisms

Objective To explore the protective effect and mechanism of exogenous hydrogen sulfide ( H2 S) on pressure ulcer rats with ischemia/reperfusion ( I/R) injury. Methods Forty SD rarts were randomly divided into 4 groups,namely normal control group,I/R model group,low-dose H2 S intervention group and high-dose H2 S interven-tion group. Each group had 10 animals. The low-and the high-dose H2 S intervention groups were injected intraperitone-ally 10 and 30μmol/kg of sodium hydrosulfide ( NaHS) for 1 week,respectively;however,the normal control group and I/R model group were treated with same volume 9 g/L saline. Then,the normal control group did not sustain any pres-sure,but other 3 groups were pressurized. After 3 circulations of ischemia/reperfusion,all rats were sacrificed,and path-ological changes in pressure ulcer rats were evaluated by HE staining. Serum H2 S contents were analyzed through meth-ylene spectrophotometric method. Serum levels of tumor necrosis factor-α(TNF-α),interleukin-6 (IL-6),intercellu-lar adhesion molecule -1 (ICAM-1),and myeloperoxidase (MPO) were detected by enzyme linked immunosorbent assay ( ELISA ) . The activities of malondialdehyde ( MDA ) , superoxide dismutase ( SOD ) , glutathione-peroxidase ( GSH-Px) and catalase ( CAT) in the homogenate of muscle were examined. Apoptotic index ( AI) was calculated by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling ( TUNEL) . The expressions of Bcl-2 and Bax in muscle tissue were evaluated by Western blot. Results In comparison with the normal control group,serum TNF-α, IL-6,ICAM-1 and MPO levels,MDA activity in the homogenate of muscles,AI and Bax expression level were in-creased (all P<0. 01),while serum H2S levels,the activities of SOD,GSH-Px and CAT as well as Bcl-2 expression levels were reduced in the I/R model group (all P<0. 01). Upon intervention therapy of low-or high-dose H2S,path-ological scores,serum TNF-α,IL-6,ICAM-1 and MPO levels,MDA activity in the homogenate of muscle,AI and Bax expression levels were gradually reduced while serum H2 S levels,the activities of SOD,GSH-Px and CAT and Bcl-2 expression levels were gradually enhanced when compared to those of the I/R model group (all P<0. 05,0. 01). Con-clusions Exogenous H2 S has better protection against pressure ulcer rats with I/R injury. The mechanism may be as-sociated with inhibition of inflammatory reaction and enhancement of antioxidative ability as well as reduction of apopto-sis.
10.Quantitative analysis of interleukine-2 receptor ? mRNA expression in ankylosing sondylitis by fluorescence quantitative reverse transcription polymerase chain reaction and clinical significance

Objective To construct and evaluate the real-time fluorescence quantitative polymerase chain reaction for detecting IL-2R? mRNA in ankylosing spondylitis(AS) based on TaqMan technique.Methods A pair of primers and a TaqMan probe were designed by sequence in GenBank.Total RNA isolated from the fresh peripheral blood monocytes (PBMC) of homo sapiens was amplified by the real-time FQ-RT-PCR.The product was collected by agarose gel electrophoresis and sequencing analysis then ligated with pUCm-T.The recombined plasmid was transcribed to cRNA by T7 RNA polymerase in order to prepare serial standard materials.A new method was created to quantify IL-2R? mRNA in ideal condition.Sensitivity, reproducibility and efficiency were evaluated and used, combined with sIL-2R,for clinical application of AS.Results The linear range was (7~107) cells/ml.The intra-and-inter-assay coefficient variation was 8.4% and 9.6% respectively. Recombined plasmid contained the target fragment was specific and accurate by BLAST.Standard reference was constructed successfully.The RT-PCR product in AS with HLA-B27 positive groups was higher than that with HLA-B27 negative groups and health controls (P0.05).The sensitivity of IL-2R? mRNA was 96.7%.Conclusions Real Time FQ-RT-PCR of IL-2R? mRNA is constructed successfully.This is an easy,rapid,sensitive, accurate and reliable method for quantifing IL-2R? mRNA. There is highly statistical significance, compared with sIL-2R, on the expression of IL-2R? mRNA and inflammatory states between AS and control group and effective information for administration of patients.
