1.Clinical Observation on Acupuncture Combined with Western Medicine in Treating Acquired Paralytic Strabismus

Objective To observe clinical curative effect of acupuncture combined with western medicine in treating acquired paralytic strabismus.Methods Randomly divided 60 patients with acquired paralytic strabismus into a treatment group and a control group,with 30 cases in each group.The control group accepted compound anisodine injection for subcutaneous injection beside superficial temporal artery and oral administration of neurotrophic drugs.The treatment group accepted acupuncture treatment in addition to the treatment of the control group.Both groups were treated for 4 weeks.Before and after treatment,measured the ocular symptom scores,including eye position deviation,eyeball movement obstacle,diplopia,the first squint angle being greater than the second,compensatory head posture and evaluated the clinical curative effect.Results The clinical curative rate in the treatment group was 40.0% and the total effective rate was 93.4%,which in the control group were 10.0% and 66.7% respectively.The total effective rate and curative rate in the treatment group were significantly higher than those in the control group (P< 0.05).After treatment,the improvement in the total ocular symptom score and diplopia,deviation of eye position,eyeball movement disorder scores were superior to those in the control group (P < 0.05).Conclusion Acupuncture combined with western medicine was superior to simple western medicine in treating acquired paralytic strabismus,and could significantly improve the eye symptom.
2.Relaxant effect of salianic acid A isopropyl ester on rat pulmonary artery and its mechanism

Objective To investigate the relaxant effect of salanic acid A isopropyl ester(isopropyl 3-(3,4-dihydroxyphenyl)-2-hydroxypropanoate)and its mechanism.Methods Isolated rat pulmonary artery was perfused and the tension of the vessel was measured.To observe the relaxant effect of salianic acid A isopropyl ester on the pulmonary artery precontracted by noradrenaline(NE)and the role of endothelial cell and vascular smooth muscle cell on vasodilation.Results Salianic acid A isopropyl ester relaxed the endothelium-intact artery precontracted by NE in a concentration-dependent manner,and the effect was depressed with denuded endothelium.Salianic acid A isopropyl ester right-shifted the concentration-response curve of KCl and suppressed the contractions induced by NE and CaCl2 in the Ca2+-free-K-H solution.The relaxant effect of salianic acid A isopropyl ester was inhibited by N?-nitro-L-arginine methyl ester hydrochloride(L-NAME),Indometacin,CsCl,Tetraethylammonium(TEA),Glibenclamide,and BaCl2,but not that by Propranolol,Metoprolol,and Salbutamol.Conclusion Salranic acid A isopropyl ester could significantly relax the pulmonary artery of rats,which may derive in part from endothelium.The relaxant effect of salianic acid A ispropyl ester may relate to NO pathway,prostacyclin(PGI2)pathway,calcium channel,and potassium channel.
4.Blood oxygenation level dependent functional MRI study on the changes of motor cortex in patients with amyotrophic lateral sclerosis

Objective To study the changes of motor cortex in patients with amyotrophic lateral sclerosis(ALS)while executing sequential finger tapping movement by using blood oxygenation level dependent(BOLD)functional MRI.Methods Fifteen patients with definite or probable ALS and 15 age and gender matched normal controls were enrolled in the BOLD study,and all the subjects were right-handed with no other diseases or any recent medication history.A 3.0 T MR scanner was employed and gradient echo EPI(GRE-EPI)sequence was used to acquire the functional images.Subjects executed sequential finger tapping movement at a frequency of 1-2 Hz during a block design task.fMRI data were analyzed by using statistical parametric mapping(SPM)2.Volume of activated brain areas was compared with the use of a Student's t-test.Results Bilateral primary sensorimotor cortex(PSM),bilateral posterior aspect of premotor area(PA),bilateral supplementary motor area(SMA),contralateral inferior lateral premotor area (ILPA),bilateral parietal region(PAR),and ipsilateral cerebellum showed activation in both ALS patients and normal controls when executing the same motor task.The activation areas in bilateral PSM and bilateral posterior aspect of PA(right hand ipsilateral activation:ALS(924.5±141.1)mm3,control(829.9±98.4)mm3,P=0.05;right hand contralateral activation:ALS(9143.8±702.8)mm3,control(8638.8±506.4)mm3,P<0.05;left hand ipsilateral activation:ALS(1162.5±357.4)mm3,control(902.5±184.2)mm3,P<0.05;left hand contralateral activation:ALS(8255.2±870.2)mm3,control (5934.6±616.4)mm3,P<0.05),bilateral SMA(right hand bilateral activation:ALS(6564.3±720.6)mm3,control(4710.7±416.3)mm3,P<0.05;left hand bilateral activation:ALS(6970.5±961.8)mm3,control(3688.9±672.3)mm3,P<0.05),and ipsilateral cerebellum(right hand ipsilateral activation:ALS(2720.0±1154.2)mm3,control(254.3±84.4)mm3,P<0.05;left hand ipsilateral activation:ALS(4794.4±1237.0)mm3,control(1689.0±719.6)mm3,P<0.05)were significantly larger in ALS patients than in normal controls.Extra activation areas including ipsilateral ILPA,contralateral cerebellum and bilateral posterior limb of internal capsule were only detected in ALS patients.Conclusions Similar activation areas were seen in both groups while executing the same motor task,but the activated areas were more prominent in ALS group.The increased activation areas in ALS patients may represent neural reorganization.while the extra activation areas in ALS patients may indicate functional compensation.
5.Urine biomarkers after acute kidney injury in rats induced by gentamycin

OBJECTIVE To investigate the time and dose relation of new urine bio markers in rat model of acute kidney injury induced by genta mycin (GM)to search for more sensitive,noninvasive and specific markers than traditional approaches to monitor nephrotoxicity.METHODS SD Rats were im treated with GM5,20,80 mg·kg -1 or saline once daily.Rats were randomly divided into 20 subgroups:treated for 1 ,3,7,14 d and 14 d followed with 28 d recovery period.Ten rats per group (5 rats per sex)were scarified at 24 h after the last dosing or the end of recovery period.Blood sa mples were col-lected for blood urea nitrogen (BUN)and creatinine(CRE)analysis.Urine was collected at each nec-ropsy for urine protein by dry che mistry method,for kidney injury molecule-1 (KIM-1 )analysis by ELISA, and for β2-microglobulin (β2-MG)analysis by ELISA.Kidneys were obtained for histological exa mination after HE stains.RESULTS Positive protein(3 +)was noted for several fe male animals treated for 7 or 14 d at 80 mg·kg -1 and the tendency of recovery were noted at the end of recovery period.Co mpared with those in saline control group treated for 7 d,the seru m BUN and CRE levels for fe males and the CRE level for males were significantly increased at 80 mg·kg -1 (P <0.05),and the BUN level showed the tendency of increase for males at 80 mg·kg -1 (P >0.05).When treated for 14 d,the seru m BUN and CRE levels for fe males and males at 80 mg·kg -1 and the seru m CRE level for fe males at 20 mg·kg -1 were significantly increased when compared with those in saline control group(P <0.05). The seru m BUN and CRE recovered to base line for all animals treated for 14 d followed by a 28 d recov-ery period.Histopathological observation of kidney tissues indicated that focal tubule dilatation was noted for animals treated for 3 d at 20 and 80 mg·kg -1 ,infla mmatory cell infiltration and focal tubule dilatation were noted at 20 mg·kg -1 and focal renal tubular epithelial cell degeneration,infla mmatory cell infiltra-tion,focal casts (lightly)were noted at 80 mg·kg -1 for animals treated for 7 or 14 d.For animals treated for 14 d followed by a 28 d recovery period,only basophilic tubules and renal casts were noted at 80 mg·kg -1 .New urine bio markers determination indicated that KIM-1 level was significantly increased at 20 and 80 mg·kg -1 for animals treated for 3,7 or 14 d when compared with that in saline control group (P<0.05).For animals treated for 14 d followed by a 28 d recovery period,the KIM-1 level was still significantly higher than saline control group for males and fe males at 80 mg·kg -1 and males at 20 mg·kg -1 (P <0.05 ),but there was evidence for reversal.The β2-MG level was significantly increased at 80 mg·kg -1 for animals treated for 3 d(P<0.05),at 20 or 80 mg·kg -1 for animals treated for 7 or 14 d(P<0.05 or P<0.01 ),when compared with that in the saline control group.For animals treated for 14 d followed by a 28 d recovery period,the β2-MG level was still significantly higher than saline control group for males and fe males at 80 mg·kg -1 and fe males at 20 mg·kg -1 (P <0.05),but there was also evidence for reversal.CONCLUSION Urine KIM-1 and β2-GM are more sensitive and specific markers for early diagnosis of kidney injury induced by GM when compared with the traditional approaches to monitor nephrotoxicity.
6.The effect of Brandt-Daroff exercises on positional vertigo with atypical positioning nystagmus

OBJECTIVE Identify the effect of Brandt-Daroff exercises for positional vertigo with atypical positioning nystagmus.METHODS 11 cases of positional vertigo during 2006~2007 with atypical positioning nystagmus when carrying Dix-Hallpike and roll test,the nystagmus had the follwing characters:poly-positions,atypical direction,long lasting(more than 3 mins),no fatigue.After ruling out centrul neural system disorders,the patients were instructed to do Brandt-Daroff exercises 2-3 times/day,and 6-10 times every time.RESULTS After doing the exercises for 3-4 days,the vertigo of all the 11 cases were all relieved.After 1-2 weeks,10 cases were cured,syptom of the other 1 was relieved significantly,and the nystagmus was alleviated.CONCLUSION Cases with atypical positioning nystagmus may present a different kind of BPPV related with the otolith and vestibular nerve disease.Brandt-Daroff exercises was a good method for the atypical positioning nystagmus cases.
7.Detection and analysis of differential gene expression between benign lymphoepithelial lesion of lacrimal gland and orbital cavernous hemangioma

Background Benign lymphoepithelial lesion of lacrimal gland is not a common orbital disease in clinic,which mainly presented as symmetrical and painless enlargement of bilateral lacrimal glands.However,the etiology and pathogenesis of this disease is still unclear now.Objective This study was to screen the differentially expressed genes between benign lymphoid epithelial lesion of lacrimal gland and orbital cavernous hemangioma and explore the pathogenesis of benign lymphoepithelial lesion of lacrimal gland at the molecular level.Methods Nine patients diagnosed as benign lymphoepithelial lesion of lacrimal gland in Beijing Tongren Hospital,Capital Medical University were enrolled from September 2010 to April 2013,and nine patients with orbital cavernous hemangioma served as control group.The intraorbital tissue was collected during surgery.Whole-genome gene expression microarray was used to detect the expressed genes,and limma algorithm was used to analyze the differentially expressed genes between the benign lymphoepithelial lesion of lacrimal gland and the orbital cavernous hemangioma.Real-time PCR was used to verify differentially expressed genes,Fisher method and gene ontology (GO) functional analysis were performed to realize function and signaling pathways analysis.This study complied with Helsinki Declaration and the protocol was aproved by Institutional Review Board of Beijing Tongren Hospital,and informed consent was obtained.Results Total 5 260 differentially expressed genes were screened between benign lymphoepithelial lesion of lacrimal gland and orbital cavernous hemangioma.The Fisher function and signaling pathways analysis showed that 109 GO terms were significantly upregulated and 101 GO terms were significantly downregulated,and 32 relevant signaling pathways were significantly upregulated and 25 signaling pathways were significantly downregulated in the benign lymphoepithelial lesion of lacrimal gland.GO analysis showed that the expression enrichment of complement receptormediated signaling pathway was high,then following the upregulation of T cell and B cell signaling pathways and downregulation of mitogen-activated protein kinase (MAPK) and transforming growth factor-β (TGF-β) signaling pathways.Real-time PCR results showed that the expressions of TIPRL,TLR7 and TLR10 genes were significantly higher in the benign lymphoepithelial lesion of lacrimal gland than that in the orbital cavernous hemangioma,with significant differences between the two diseases (Z =-2.03,-2.32,-2.32;all at P<0.05),which was consistent with the microarray data.Conclusions Gene expression profiles are significantly different between benign lymphoepithelial lesion of lacrimal gland and orbital cavernous hemangioma.Those differentially expressed genes play roles in the upregulation of T cell and B cell signaling pathways,downregulation of MAPK and TGF-β signaling pathways and the change of complement system.It is implied that a comprehensive effect of various genes and pathways participates in the pathogenesis of benign lymphoepithelial lesion of lacrimal gland.
8.Photoprotective effect of a self-made sunscreen cream: an evaluation in vitro and in vivo by using guinea pigs

[Objective] To determine the sun protection factor (SPF) and protection factor of ultraviolet A (PFA) of a self-made sunscreen cream.[Methods] The sample of a self-made sunscreen cream and a commercial sunscreen cream were painted to an artificial skin (3M film) at a dose of 2 μl/cm2,and a tester was used to evaluate their photoprotective effect.A total of 48 albinism guinea pigs were classified into 8 groups to remain unprotected or be protected by the self-made or commercial sunscreen cream.A solar ultraviolet light simulator (SUV-1000) was used to simulate the ultraviolet rays in sunlight to irradiate the guinea pigs,and the photoprotecfive effect of these sunscreen creams was determined.[Results] The in vitro evaluation revealed that the SPF value of the self-made sun-screen cream and commercial cream was 32.26 ± 2.42 and 30.87 ± 2.57respectively (n =5,t =0.94,P > 0.05),and the PFA value was 24.28 ± 2.44 and 17.53 ± 2.28 respectively (n =5,t =4.52,P< 0.01 ).As the in vivo experiment showed,the SPF value of the self-made sun-screen cream and commercial cream was 30.39 ± 6.65 and 28.79 ± 7.36,respectively (n =12,t =0.38,P> 0.05),and the PFA value was > 8.91 and > 8.93 respectively.[Conclusion] The photoprotective effect of the self-made sunscreen cream is similar to that of the commercial cream.
