1.Progress in diagnosis and treatment of diabetic retinopathy with leukemia

Combined with leukemia is a risk factor for aggravating diabetic retinopathy. A combination of diabetic retinopathy and leukemia can be expected to have a rapid progression and patients often visit the department of ophthalmology first. In addition to the typical manifestations of diabetic retinopathy such as retinal venous tortuous dilation, microaneurysm, retinal hemorrhage and exudation, patients may also be associated with leukemic retinopathy. Areas of extensive capillary non-perfusion and neovascularization may appear in the early stage of mild microangiopathy. Moreover, severe complications such as vitreous hemorrhage, neovascular membranes and traction retinal detachment appear earlier, which may be a prognostic indicator for poor vision. The causes of leukemia aggravating diabetic retinopathy include retinal ischemia due to hyperviscosity, anemia and thrombocytopenia, direct infiltration of tumor cells, chemotherapy and radiotherapy, high level of vascular endothelial growth factor. In clinic, rapidly progressing diabetic retinopathy should alert the ophthalmologist to the underlying hematological disorder. Patients with both diabetes and leukemia need to be screened much earlier and followed up at shorter intervals. Early detection and aggressive management may help preserve visual acuity in such cases.
2. Identification of serum biomarkers for diagnosing stage I lung adenocarcinoma by MALDI-TOF mass spectrometry

Objective: To identify specific biomarkers that could improve early diagnosis of lung adenocarcinoma using matrix-assisted laser desorption/ionization (MALDI) technology. Methods: Serum samples were isolated from 17 patients with stage I lung adenocarcinoma and 17 age- and sex-matched healthy controls, and the serum proteomic profiles were obtained by matrix-assisted laser desorption ionization time of flight (MALDI-TOF) mass spectrometry. Results: Compared with healthy control group, two highly expressed potential biomarkers were identified with the relative molecular weights of 6631.64 Da and 4964.21 Da. The two best novel protein peaks were automatically chosen for the system training and the development of the constructed model. The constructed model was then used to test an independent set of masked serum samples from 15 lung adenocarcinoma patients and 22 healthy individuals. The analysis yielded a sensitivity of 93.3%, and a specificity of 95.5%. Conclusion: These results suggest that MALDI-TOF-MS ProteinChip technology is a quick, convenient, and high-output analyzing method that is capable of selecting several relatively potential biomarkers from the serum of lung adenocarcinoma patients and may have a clinical value in the future, and will provide clues to identifying new serologic biomarkers of lung adenocarcinoma.

5.The clinical use of interventional chemotherapy and intravesical instillation for preventing recurrence of superficial bladder cancer

Objective To study the therapeutic efficacy of combined interventional chemotherapy and intravesical instillation of mitomycin on preventing bladder cancers from recurring after local ablation. Methods 28 patients with superficial bladder cancers were randomized into combined interventional chemotherapy and intravesical instillation of mitomycin or intravesical instillation of mitomycin alone for preventing recurrence after local ablation.The result was assessed by ? 2 test. 6.Antitumor Effects of Adenovirus-Mediated CD/5FC Gene Therapy in Established Colon Adenocarcinoma- Bearing Mice and Its Immunological Mechanisms

To further investigate whether the suicide gene therapy affect the immune system, CT26 colon adenocarcinoma of BALB/c mice was adapted as experimental tumor model. The growth of CT26 tumor was suppressed by the adenovirus-mediated CD/5FC system significantly both in vitro and in vivo. But the tumors seemed very difficult to be cured and developed again soon after the end of treatment. After adenovirus-mediated CD/5FC gene therapy, there were about 40%of treated mice were cured from the tumor burden, survived longer and resisted further CT26 cell challenge. These mice also showed improvement of splenic CTL cytotoxicity. Costimulatory molecule B7-2, dendritic cell marker NLDC-145, MHC-I, and F4/80 were present in tumor mass in mice. These data suggested that in vivo suicide gene therapy could induce specific antitumor immunity, but this effect is not strong enough to eradicate established tumors. Further study for more efficient induction of antitumor immunity in CD/5FC gene therapy is warranted.
7.Anti-contactin-associated protein-1 antibody associated chronic inflammatory demyelinating polyradiculoneuropathy: a case report

Chronic inflammatory demyelinating polyneuropathy (CIDP) with positive anti-contactin-associated protein-1 (Caspr1) antibody is a rare autoimmune antibody mediated peripheral neuropathy. A 62-year-old male patient was reported in this article, whose clinical manifestations were subacute onset, abnormal distal limb motor sensation, and increased cerebrospinal fluid protein level. The patient had a good response to plasma exchange. Electromyography of lower limbs showed that motor involvement was dominant, motor conduction velocity slowed down, compound motor active potential (CMAP) and sensory nerve active potential amplitude decreased, and F wave was not elicited; electromyography of upper limbs without symptoms showed that CMAP amplitude of median nerve decreased, and conduction velocity was normal. There are few reports of anti-Caspr1 positive CIDP in the world. The article summarized the characteristics of the patient and reviewed the relevant literature, in order to improve clinicians′ understanding and diagnosis and treatment ability of the disease.
8.Clinical Analysis of Cytomegalovirus Pneumonia in 24 Infants

Objective To explore the clinical characteristics and treatment of infants with cytomegalovirus(CMV)pneumonia.Methods The clinical and auxiliary examination data of 24 patients with CMV pneumonia,which were diagnosed by detection of serum CMV-IgM used enzyme immunodot technique.All patients were treated with ganciclovir (GCV).Results The most common clinical manifestations of CMV pneumonia were cough,breathlessness,fever,crackles and wheezing rale.Chest X-ray findings predominately revealed increased and thickened bilateral lung markings and patchy areas of increased opacity in both lungs.Sixteen cases were cured,8 cases improved.Conclusions Clinical manifestations of infant CMV pneumonia lack specificity.The presence of serum CMV-IgM antibody is a laboratory diagnostic evidence.GCV is the best choice of drug in treatment of infants with CMV pneumonia.

Objective To investigate the number and activity changes of endothelial progenitor cells(EPCs) in hemodialysis patients,and explore its correlation with the risk factors of coronary heart disease.Methords Total mononuclear ceils(MNCs) were isolated from peripheral blood of patients with chronic renal failure in long-term hemodialysis and from a normal control group by Ficoll density gradient centrifugation and then were plated on humanfibronectin-coated dishes.Mter 7 days of culture,EPCs were characterized as adherent cells by double staining with FITC-UEA-I and DI-LDL,and were further identified by the expression of CD34,CD133 and KDR with flow cytometry.EPCs migration was determined with modified Boyden chamber assay.EPCs adhesive assay was performed by replanting EPCs on humanfibronectin-coated plates and then counting the adherent cells.The relationship of the EPCs' number and activity with Framingham Risk Score of ten years was also be assessed.Results Number of EPCs and the migratory & adhesive capacity were significantly lower in patients than in the control(P
