1.Diagnosis and treatment of chronic idiopathic intestinal pseudo-obstruction(CII P)

Objective To analyze the clinical characterist ic s and treatment of chronic idiopathic intestinal pseudo-obstruction(CIIP). Methods We retrospectively reviewed 8 C IIP patients admitted to our hospital from 1980 to 1999. There were 4 males and 4 females, with an average age of 29 years. Results The earliest onset of symptom was at an age of 2 years old. Re current abdominal distention is the most common symptom. Gastrointestinal imagin g and colonoscopy showed dilated loops of bowels with no evidence of mechanical obstruction. Delayed gastric emptying was presented by radionucleotide(~99m Tc-Sc) gastric emptying test. Gastrointestinal manometry showed low pressure in esophagus and colon. Biopsy taken in 2 patients showed chronic inflammation. On e patient underwent total colectomy for the treatment of lesions confined within the colon. Prokinetic agents have limited efficacy. Conclusions CIIP is a rare disease. Operation is not a prima ry method for treatment. The patients usually have poor prognosis.

We carried out a questionnaire survey on the career planning situation of pharmacy postgraduate students at Peking university, so as to collect information for career planning education of pharmacy postgraduate students, which is expected to promote their employment on completion of degree. by. The results revealed the status quo of career planning and the demand for guidance in this aspect. Recommendations for career planning education of pharmacy postgraduate students were made thereby.

BACKGROUND:Inflamed dental pulp stem cells are a new kind of dental pulp stem cells, and there is no systematic review on the cells by now. OBJECTIVE:To systematical y review the research progress in inflamed dental pulp stem cells. METHODS:A computer-based online search in PubMed, Web of Science, CNKI, WanFang and VIP databases was performed for related articles published from the establishment of the databases to February 2014. The keywords were“(pulptis or inflam*dental pulp*or human dental pulp with irreversible pulpitis) and stem cel*”in English and Chinese, respectively. Hand searching was also done to obtain further information or papers about the studies. The results were qualitatively analyzed to comprehensively summarize the progress in the research of inflamed dental pulp stem cells. RESULTS AND CONCLUSION:Total y 11 papers were involved in result analysis that comprehensively review the research progress in inflamed dental pulp stem cells at the fol owing aspects:the research of history, material origin, cellculture, cel-surface markers, proliferation ability, multi-directional differentiation potential, animal models and clinical use. Researches of inflamed dental pulp stem cells are stil in the initial stage, and cultivating conditions and the establishment of animal models are stil in the exploratory phase. Controversies stil exist in the capacity of proliferation and multi-directional differentiation of the inflamed dental pulp stem cells. And fewer studies have been done in the characteristics of immunity, subpopulation and clinical use of the inflamed dental pulp stem cells.
4.The value of serum bile acid in diagnosis of intrahepatic cholestasis of pregnancy

ObjectiveTo evaluate the application value of serum bile acid in diagnosing intrahepatic cholestasis of pregnancy (ICP).MethodsThe clinical data of 92 prepartal patients with ICP was analyzed retrospectively.The patients were divided into observation group ( > 40 μ mol/L ) and control group ( ≤40 μ mol/L) with 46 cases each according to serum bile acid level.The nccurrence time of skin pruritus,jaundice degree,serum alanine aminotransferase (ALT),aspartate aminotransferase (AST) and degree of polluted amniotic fluid of two groups were observed and compared.ResultsThe occurrence of skin pruritus in observation group was earlier than that in control group [(29.1 ± 3.4) weeks vs.(33.9 ± 3.2) weeks,P<0.05 ] ;the cases with severe jaundice,ALT > 100 U/L,AST > 100 U/L and amniotic fluid contamination Ⅲ degree in observation group were more than control group [27 cases vs.15 cases,28 cases vs.20 cases,28 cases vs.18 cases,23 cases vs.15 cases,P< 0.01 or < 0.05].ConclusionsSerum bile acid,as a sensitive indicator to assess TCP,has important value in determining the condition of pregnant women and prognosis analysis,and it can reflect the occurrence possibility of bad perinatal outcomes.
5.Evaluation of Comprehensive Benefit of Hospital Pharmacy Department with Comprehensive Index Method

OBJECTIVE: To discuss the evaluation of comprehensive benefit of hospital pharmacy department with comprehensive index method. METHODS: Suitable evaluation indexes on social effects and economic returns were respectively selected to be assigned with weight numbers by analytic hierarchy process, and the comprehensive benefits of pharmacy department in our hospital during the period from 2002 to 2004 were evaluated. RESULTS: The index values of social effects, economic returns and comprehensive benefits increased year by year, which was in line with the objective actuality. CONCLUSION: It is feasible to evaluate comprehensive benefit of hospital pharmacy department with comprehensive index method.
6.Clinical observation of retrobulbar injection of triamcinolone acetonide combined 577 nm laser macular grid photocoagulation for the treatment of cystoid macular edema

AlM:To evaluate the efficacy of retrobulbar injection of triamcinolone acetonide ( TA ) combined with 577nm laser macular grid photocoagulation for the treatment of cystoid macular edema.
METHODS: Fifty-eight cases ( 66 eyes ) with cystoid macular edema caused by different diseases were recruited in this study. The included patients were treated with both retrobulbar injection of triamcinolone acetonide and 577nm laser macular grid photocoagulation. The best corrected visual acuity, macular thickness, fundus and intraocular pressure were observed in the 1wk, 1 and 3mo after the treatment in all of the included cases.
RESULTS: After treatment, all of the 66 eyes showed cystoid macular edema partially or completely subsided according to optical coherence tomography and fluorescence fundus angiography; 54 eyes ( 82%) visual acuity improved, 12 vision remained the same.
CONCLUSlON: Retrobulbar injection of triamcinolone acetonide combined with 577nm laser macular grid photocoagulation has good curative effect, simple operation procedure and rare complications in the management of cystoid macular edema.
7.Expression of ESM-1 in hepatocellular carcinoma is associated with angiogenesis and tumor invasion.

OBJECTIVETo study the expression and significance of endothelial cell specific molecule-1 (ESM-1) in hepatocellular carcinoma (HCC) tissue and serum from HCC patients.
METHODSThe ESM-1 expression was evaluated by immunohistochemistry staining and quantitative real-time PCR in 30 tumor tissues from HCC patients, 15 surrounding non-cancerous hepatic parenchyma, and 15 normal controls. Serum levels of ESM-1 were also measured in the HCC patients and healthy controls by Enzyme-linked immunosorbent assay (ELISA).
RESULTSESM-1 was expressed in endothelium of 24 HCC tissues and 6 pericarcinomatous tissues, however, it was not expressed in normal liver tissues. The mRNA level of ESM-1 was 0.064 +/- 0.018, 0.383 +/- 0.103, 0.528 +/- 0.148 in the corresponding tissues, respectively. The mRNA level of ESM-1 in tissues was correlated with the TNM phase of HCC patients, tumor vascular invasion as well as metastasis. The serum ESM-1 was (12.643 +/- 2.280) ng/ml and (4.660 +/- 1.172) ng/ml in hepatic cell carcinoma and normal controls, respectively.
CONCLUSIONThe expression of ESM-1 is significantly up-regulated in HCC, suggesting that ESM-1 may play an important role in the pathoigenesis of HCC.

8.Impacts of short-term continuous subcutaneous insulin infusion on insulin secretion and sensitivity in newly-diagnosed type 2 diabetes

Objective To explore the impacts of short-term continuous subcutaneous insulin infusion (CSII) therapy on insulin secretion and sensitivity in newly-diagnosed type 2 diabetes. Methods Ten patients(7 males, 3 females) with newly-diagnosed type 2 diabetes accepted a 2-week course of intensive CSII therapy during June 2006 and February 2007. Hyperinsulinemia euglycemia clamp test and intravenous glucose tolerance test (IVGTT) were performed before and after CSII. Results (1) None of the patients had acute insulin response (AIR) before CSII treatment, and most of them had a partial restoration after 2-week intensive therapy. The AIR after CSII was significantly increased [(7.63±4.73 vs 0.83±1.96)mU/L, P<0.01]. The patients whose AIR restored better tend to be younger and obeser. (2) The glucose infusion rate (GIR) before CSII in the newly-diagnosed type 2 diabetic patients was significantly lower than that in healthy controls [(2.30±0.81 vs after 2-week CSII therapy (P<0.01). The patients whose GIR increased greatly had significantly lower waist circumferences and body mass index, and higher basal blood glucose levels. Conclusion Normalization of blood glucose levels by transient intensive CSII therapy improves pancreas β-cell function and insulin sensitivity, which seems to contribute to the long-term diabetic remission.
9.Application effect of contact team in international cooperation of Chinese registered nurse-helping smokers quit project (CRN-HSQ)

Objective To complete the large international cooperation nursing research project Adapting a Smoking Cessation Intervention Distance Learning Program to Educate Nurses in China(CRN-HSQ) cooperating with International Society of Nurses in Cancer Care (ISNCC).Methods The contact team was formed,convenience sampling method was used to choose 350 clinical nurses in our hospital.They were trained and investigated by web-based education mode.Results The contact team finished the project tasks smoothly.The satisfaction degree of research object with the contact team was high.Conclusions Establishment of contact team is good for shortening the information flow path,reducing the information transmission error,increasing the research objects' confidence and reducing the research objects turnover.It is very helpful for project with long period and large number of research objects.
10.Acute glucagon response associated with the long-term remission of newly-diagnosed type 2 diabetes

ObjectiveTo investigate the role played by acute glucagon response in the short-term intensive insulin therapy induced long-term remission of newly-diagnosed type 2 diabetes.MethodsTen newly-diagnosed type 2 diabetic patients( 7 males,3 females) received intensive continuous subcutaneous insulin infusion( CSII )therapy for 2 weeks.Intravenous glucose tolerance test (IVGTT) and hyperinsulinemia euglycemia clamp test were performed before and after CSII.Glucose infusion rate ( GIR ),acute insulin response ( AIR ) and acute glucagon response (AGR) were assessed.Long-term remission was defined as good glycaemic control without any hypoglycaemic agent (fasting plasma glucose < 7.0 mmol/L 及 2 h postprandial glucose < 10 mmol/L) one year after CSII.Relationship between AGR and long-term remission were analyzed by one way ANOVA and Spearman correlation analysis.Results ( 1 ) Five of the ten patients achieved long-term remission in this study.GIR and AIR in the remission group were increased significantly after CSII compared with those before CSII [ GIR ( 5.39 ± 1.76 vs 2.02 ± 0.83 )mg · kg-1 ·min-1,AIR ( mean 54.1 vs mean 3.39 ) mU · L-1 · min,P<0.01 respectively ],however both of them were not associated with the remission.(2) AGR in the remission group was significantly higher than that in the non-remission group before CSII ( 5.10±0.60 vs 2.85 ± 1.86,P<0.05 ) and was decreased significantly after CSII.The mean of AGR after CSII was apparently lower in the remission group than that in non-remission group (0 vs 3.04±2.00,P<0.01 ).(3)Spearman analysis showed that AGR before CSII and its range of reduction after CSII were correlated with remission ( r for both were 0.731,P=0.016).ConclusionHigher level of AGR before CSII and greater reduction after CSII in the subjects with newly-diagnosed type 2 diabetes were significantly associated with long-term remission,suggesting that pancreatic α cells may play a unique role in the induction of remission of type 2 diabetes.
