Objective To evaluate the feasibility of trans-umbilicus two-port laparoscopic procedures in pediatric surgery. Methods Sixty children (age, 3 months~14 years; mean, 3.5 years) with indirect inguinal hernia and 40 children (age, 2.5~14 years; mean, 6.3 years) with appendicitis underwent supra-high ligation and appendectomy, respectively, by using mini laparoscope from January 2003 to December 2003. During the supra-high ligation, the camera and forceps were respectively introduced via the left and right side of umbilical ring, and a hernial-ring needle with suture was brought in at the site of body surface projection of internal ring to apply external knotting. During the appendectomy, laparoscopic instruments were put through the site of right McBurney’s point, and explorations for contralateral hernia or other digestive tract malformations were applied simultaneously. Results All the operations were completed smoothly. The mean operating time was 8.5 min and 55.7 min in hernia repair (per side) and appendectomy, respectively. The mean postoperative hospital stay was 1~2 days in children with indirect inguinal hernia and 5~7 days in children with appendicitis. Follow-up for 3~12 months showed no recurrence of hernia except for 1 case of intraoperatively misdiagnosed latent hernia, which relapsed 2 months after the surgery. Postoperative abdominal cavity infection took place in 2 cases of perforated appendicitis. Conclusions Trans-umbilicus two-port laparoscopic technique is simple to perform, safe and minimally invasive. The technique has fewer complications and gives satisfactory cosmetic results, being highly recommended.

To discuss bacteria of urine infection reoccurrence , the routine culture and L-form bacteria culture of urine tract infection on 850 cases were adopted and clinica l watching and analysis of the bacteria plasmid were held .Result: of the l28 cases, re-infec tion rate l5.0%.,the positive rate of the routine bacteria culture for those initial diagnosis patie nts was 58.3% while that of the re-infection patients was 34.0% P<0.0l .The initia l diagnosis L-form inspected rate 5.6%yet that of re-infection was up to 32.0 % P<0.01 .The analysis of the pla smid bacteria of re- infection were of the same origins .Conclusion: the first infection dont nee d L-form culture but re- infection should have L-form culture .Analysis: plasmid positive bacteria of r e-infection was the result of the first infection not completely cured.

Objective To investigate the causes, diagnosis, treatment of the difficult postoperative decannulation resulting from tracheotomy between children and adults and the method of improving the rate of successful decannulation. Methods Clinical data of 56 cases with difficult decannulation after tracheotomy were analysed retrospectively, all the patients were admitted in our hospital between Jan 1990 and Oct 2006.All the patients were separated into two groups, children group including 24 cases of 14 months ~ 14 years old, adult group including 32 cases of age ranging 15 ~ 48 years old.The postoperative time after tracheotomy were 6 month ~ 1.5 year and 6 month ~ 2 year respectively. Results There were differences of the etiology, diagnostic choice and the clinical manifestation in the treatment of the diffficult decannulation after tracheotomy between children and adults.Children with difficult decannulation after tracheotomy occurred mainly in laryngitis and airway foreign body airway caused by obstruction retrieved after tracheotomy (14/24,58.33 %),followed by the emergency tracheotomy, such as head and neck trauma (7/24,29.17%).Adults with difficult decannulation after tracheotomy mainly occurred in the throat caused by traumatic lesions of the larynx (including malformations, subglottic stenosis of larynx or vocal cord paralysis) (23/32,71.88%).The average block time in children with anti-inflammatory treatment was significantly lower than that in without anti-inflammatory therapy (6.2 d vs 10.1 d,t =4.26,P < 0.01).However,there was no significant difference in the average block time in adults with anti-inflammatory and without anti-inflammatory treatment (t =1.63,P > 0.05).In the course of treatment, rates of children consciously breathing difficulties (21/21,100%) was also significantly higher than the adult group (13/28,46.43%).Conclusion Children less than 3 years old should avoid tracheotomy. There is no definite relation between the successful treatment in the difficult decannulation and the age of the patients over 3 years old.The first failure treatment in children may be related with the local inflammation. When in treatment, doctors should observe the characteristics of respiratory difficulties, take attention to the influence of children psychological factors to the treatment.Children should be treated in the non-traumatic examination methods.

Traditional Chinese Drug Research & Clinical Pharmacology 1993;0(03):-
Objective To establish a sensitive and specific liquid chromatography method for the determination of loganin and morroniside in rat serum containing Liuwei Dihuang Formula(LDF).Methods Loganin and morroniside were extracted from the rat serum using SPE column, then separated on a C_(18)column.A HPLC method for determining serum concentration of mor- roniside and loganin in rats before and after administration LDF was established. Results A good linear calibration curve of lo- ganin and morroniside was obtained. The recovery of the method was in the range of 95 %~105 %.The extracted recovery was more than 75 % ,and within-day and intra-day precisions were less than 12 %.Conclusion This method is sensitive, simple and rapid for monitoring of drug level in the study on pharmacokinetics and different combination of LDF.

Background:
Synaptic plasticity contributes to nociceptive signal transmission and modulation, with calcium/ calmodulin-dependent protein kinase II (CaMK II) playing a fundamental role in neural plasticity. This research was conducted to investigate the role of CaMK II in the transmission and regulation of nociceptive information within the nucleus accumbens (NAc) of naïve and morphine-tolerant rats.
Methods:
Randall Selitto and hot-plate tests were utilized to measure the hindpaw withdrawal latencies (HWLs) in response to noxious mechanical and thermal stimuli. To induce chronic morphine tolerance, rats received intraperitoneal morphine injection twice per day for seven days. CaMK II expression and activity were assessed using western blotting.
Results:
Intra-NAc microinjection of autocamtide-2-related inhibitory peptide (AIP) induced an increase in HWLs in naïve rats in response to noxious thermal and mechanical stimuli. Moreover, the expression of the phosphorylated CaMK II (p-CaMK II) was significantly decreased as determined by western blotting. Chronic intraperitoneal injection of morphine resulted in significant morphine tolerance in rats on Day 7, and an increase of p-CaMK II expression in NAc in morphine-tolerant rats was observed. Furthermore, intra-NAc administration of AIP elicited significant antinociceptive responses in morphine-tolerant rats. In addition, compared with naïve rats, AIP induced stronger thermal antinociceptive effects of the same dose in rats exhibiting morphine tolerance.
Conclusions
This study shows that CaMK II in the NAc is involved in the transmission and regulation of nociception in naïve and morphine-tolerant rats.
