1.miR-155 is conductive to chondrogenic differentiation of bone marrow mesenchymal stem cells

BACKGROUND:It is discovered recently that miRNA is a new regulator that is able to have an impact on gene expression and miRNA contributes to proliferation, differentiation and self-renewal of pluripotent stem cels.
OBJECTIVE:To investigate the mechanism by which miR-155 regulates chondrogenic differentiation of bone marrow mesenchymal stem cels.
METHODS: Sixty healthy Sprague-Dawley aged 12 weeks were randomized into study group and control group, with 30 in each group. Under anesthesia, rats were sacrificed to harvest bone marrow of the lower limbs. Then bone marrow mesenchymal stem cels were isolated, cultured, and transfected with miR-155 mimics in the study group and a negative control sequence in the control group. After chondrogenic induction, RT-PCR was used to detect the expressions of Sox9, Colagen II, Aggrecan and Colagen X gene, and western blot assay to detect the expression of Sox9 and Runx2 proteins.
RESULTS AND CONCLUSION:Compared with the control group, the mRNA expressions of Sox9, Colagen II and Aggrecan were higher, but the mRNA expression of Colagen X was lower in the study group (P < 0.05); the protein expression of Sox9 was higher, but the protein expression of Runx2 was lower in the study group (P < 0.05). These findings indicate that miR-155 promotes the chondrogenic differentiation of bone marrow mesenchymal stem cels and moreover, it can suppress the hypertrophy trend of bone marrow mesenchymal stem cels differentiating into chondrocytes.
2.Study on CXCL5 gene polymorphisms in obese patients with type 2 diabetes mellitus

The association between CXCL5 gene polymorphism and obese patients with type 2 diabetes mellitus (T2DM) was explored.The distribution of CXCL5 gene promoter region-156G/C polymorphism revealed no significant difference between normal control group and T2DM group (P>0.05).The frequency of C allele gene in obesity group was higher than that in non-obesity group(P<0.05).The results suggest that the CXCL5 promoter gene -156G/C polymorphsim has no relation with T2DM,but it is a risk factor for obesity.
3.Value of ultrasonography in the persistent sciatic artery

Objective To study the clinical value of color Doppler ultrasonography in the persistent sciatic artery.Methods The ultrasonographic images of eleven patients with persistent sciatic artery diagnosed by CT angiography or digital subtraction angiography were retrospective analyzed,the ultrasonographic features were summarized.Results The sciatic artery showed the enlarged internal iliac artery,which continued into the thigh in a posterior location,the sciatic artery described a tortuous course toward the knee,slowly filling normal-appearing popliteal artery in 8 cases,there were no connection with popliteal artery in 1 cases.The common femoral artery and superficial femoral artery was dysplasia in 7 patients,which was thinner than the popliteal artery.Conclusions The ultrasonography is the effective imaging method for diagnosis of the persistent sciatic artery.
4.Effect of sodium hyaluronate on malondialdehyde and superoxide dismutase levels in the synovial fluid of patients with knee osteoarthritis

BACKGROUND:Intra-articular injection of sodium hyaluronate is an effective method for the treatment of knee osteoarthritis, with significant effect and less adverse reactions, but the mechanism is unclear. OBJECTIVE:Through testing the malondialdehyde and superoxide dismutase levels in the synovial fluid of knee osteoarthritis before and after injection of sodium hyaluronate, to evaluate the clinical efficacy of sodium hyaluronate in the treatment of knee osteoarthritis. METHODS:Thirty-seven patients with knee osteoarthritis (40 knees) were enroled and divided into mild (n=10, 10 knees), moderate (n=17, 18 knees), and severe (n=10, 12 knees) groups according to the Japan's knee osteoarthritis indications. Patients were subjected to intra-articular injection of 25 mg sodium hyaluronate, once a week for 5 weeks. The levels of malondialdehyde and superoxide dismutase in the synovial fluid before and 4 weeks after treatment were detected, and then clinical effects were evaluated based on the clinical scores according to the Japan’s knee osteoarthritis indications. RESULTS AND CONCLUSION: The indication rating results of the mild and moderate groups were decreased significantly 4 weeks after injection (P < 0.05), but there were no significant difference in the severe group before and after treatment. The malondialdehyde level in the synovial fluid was decreased obviously in the three groups at 4 weeks after injection (P < 0.05), while the level of superoxide dismutase was increased remarkably (P < 0.05). These findings indicate that sodium hyaluronate can treat knee osteoarthritis by reducing the malondialdehyde level and increasing superoxide dismutase level in the synovial fluid, but this method is more suitable for treatment of mild to moderate knee osteoarthritis.
5.Application of 70 kV with 30 mL contrast medium by dual-source CT with Stellar photon detector in coronary angiography

Objective To investigate the value of prospectively electrocardiogram (ECG)-triggered high-pitch coronary computed tomography angiography at 70 kV and 30 mL contrast medium with Stellar detector dual-source CT.Methods 60 patients with the BMI<24.9 kg/m2 were randomized into two groups:Group A (30 cases)with the tube voltage of 70 kV and 30 mL contrast medi-um.Group B (30 cases)with the tube voltage of 100 kV and 50 mL contrast medium.All the patients were performed with a Stellar detector dual-source CT using a prospectively ECG triggered high-pitch spiral technique.The images of Group A were reconstructed with SAFIRE technique,while Group B were with FBP technique.The objective image quality includes CT attenuation,SNR, CNR.And radiation dose was also calculated.The subjective was evaluated by 4-scale point.Independent sample t test was used.Re-sults There was no difference for subjective image quality between the groups,A:(3.38±0.942),B:(3.50±0.682),(t=-0.562, P >0.05).The CT attenuation of the 70 kV group were higher than that of the 100 kV group for all the segments(P <0.01).The SNR and the CNR have no statistically significant difference between the groups (P >0.05).Compared with the 100 kV group,the radiation dose of the 70 kV group was reduced by 76.5% (A:0.19±0.023 mSv,B:0.81±0.101 mSv,P <0.01).Conclusion Using 70 kV with 30 mL contrast medium in Stellar detector dual-source CT coronary angiography for the patients with a normal BMI could obtain qualified diagnostic image with low radiation dose and contrast medium.
6.Design and application of Checklist for quality control in intensive care unit

Objective:To design a Checklist for quality control in intensive care unit and observe the effect of clinical application.Methods:By consulting guidelines and literature, such as Critical care medicine professional medical quality control index (2015 edition), the quality control Checklist of intensive care unit was designed. It included four parts: quality control data collection, medical record quality verification, special diagnosis and treatment, and hospital infection prevention and control supervision. Every month, a doctor with a senior professional title served as the quality control director, and was responsible for the quality control of the department's medical care, including collecting data of the past 24 hours during the morning handover, discussing and registering special diagnosis and treatment behaviors that would be performed on the day, and coordinating with the nursing team leader, controlling the quality of the whole department throughout the day, such as supervising each medical staff if they had unreasonable behaviors, checking the running and discharge medical records, and inspecting the status of the staff on duty. The data in 2018, 2019 (Checklist implemented) and 2017 (Checklist not implemented) were retrospectively analyzed, including the status of admitted patients, department management information, length of intensive care unit (ICU) stay, and the incidence of three-tube infection [ventilator-associated pneumonia (VAP), catheter-related bloodstream infection (CRBSI), catheter-associated urinary tract infection (CAUTI)], and standardized mortality, etc. Results:From 2017 to 2019, the number of patients admitted was 373, 446, and 480, with annual growth of 19.57% and 7.62% in 2018 and 2019, respectively, and an increase of 28.69% in 2019 compared with 2017. There was no statistically significant difference in the average age and acute physiology and chronic health evaluationⅡ (APACHEⅡ) of patients in the three years. Compared with 2017, the length of ICU stay of patients in 2018 and 2019 were significantly shortened (days: 8.99±6.12, 9.14±7.02 vs. 10.20±7.21), and the incidence of VAP, CRBSI and CAUTI were significantly reduced [VAP (cases/1 000 ventilation days): 12.97±3.60, 9.62±3.14 vs. 17.48±4.89, CRBSI (cases/1 000 catheter days): 3.75±2.19, 3.87±1.87 vs. 6.19±3.13, CAUTI (cases/1 000 catheter days): 3.29±2.18, 3.28±1.87 vs. 5.61±3.18]. The standardized mortality were also significantly reduced [(77.27±7.24)%, (70.61±7.49)% vs. (84.41±9.05)%], the number of non-compliance with hospital infection prevention per month decreased significantly (person times: 54.00±6.30, 41.08±10.76 vs. 72.08±19.68), and the number of special diagnosis and treatment per month increased significantly (person times: 1 056.67±235.27, 1 361.75±278.48 vs. 722.25±145.96), the rate of etiology submission before antimicrobial treatment [(93.21±3.68)%, (96.59±2.49)% vs. (87.86±5.28)%] and deep vein thrombosis (DVT) prevention rate [(91.13±6.36)%, (96.23±2.99)% vs. (85.58±7.68)%] were significantly improved, and all the differences were statistically significant (all P < 0.05). All medical records in the three years were Grade A, but the average scores in 2018 and 2019 were higher than those in 2017 (96.82±2.84, 96.73±2.94 vs. 93.70±3.33, both P < 0.01). Compared with 2018, the incidence of VAP, the rate of etiology submission before antimicrobial treatment, the DVT prevention rate, and the standardized mortality rate in 2019 were further improved, and the number of non-compliance with hospital infection prevention per month decreased and the number of special diagnosis and treatment per month increased, and the differences were statistically significant (all P < 0.05). Conclusion:The application of quality control Checklist in intensive care unit can build an effective quality control system, reduce the incidence of three-tube infection, standardized mortality and length of ICU stay, improve the quality control awareness and execution of medical staff, and promote the improvement of medical quality.
7.Expression and clinical significance of T cell immunoglobulin and mucin domain-3 and programmed death-1 on T lymphocytes of peripheral blood in breast invasive ductal carcinoma patients

Objective To explore the expression of T cell immunoglobulin and mucin domain-3 (Tim-3)and programmed death-1(PD-1) on T lymphocytes in breast invasive ductal carcinoma patients and their clinical significance. Methods All peripheral blood of 40 breast cancer patients and 10 healthy people was obtained.The expressions of TIM-1 and PD-1 on T lymphocytes were analyzed by flow cytometry, and the relationship between them and clinical pathological features were analyzed. Results The frequency of TIM-3 cells among T lymphocytes in breast cancer patients peripheral blood was significantly higher than that in healthy people: (2.01 ± 0.62)% vs. (0.26 ± 0.08)%, P=0.03. The frequency of TIM-3 cells among T lymphocytes in peripheral blood of low differentiated breast cancer patients was also higher than that in middle- high differentiated patients: (4.45 ± 1.22)% vs. (1.02 ± 0.27)%,P=0.00.The number of PD-1 and Tim-3 cells showed a significant positive correlation (r=0.47,P=0.02). Conclusions TIM-3 cells may suppress immune to tumor cells and accelerate the development of breast cancer.
8.Exploration and practice of interdisciplinary collaboration between medicine and engineering in municipal hospitals: Take the First Hospital of Qinhuangdao as an example

Objective:The paper aims to analyze achievements and problems of interdisciplinary collaborative research between medicine and engineering carried out jointly by the First Hospital of Qinhuangdao and Yanshan University, to provide further reference for municipal hospitals.Methods:The data of scientific research of the medicine-engineering interdisciplinary collaboration in past 8 years were analyzed with specific focus on the characteristics, academic level and discipline distribution.Results:Academic level of the First Hospital of Qinhuangdao was significantly improved through such medicine-engineering interdisciplinary scientific research collaboration, especially in disciplines such as orthopaedics, rehabilitation medicine, medical imaging that are closely related to engineering disciplines.Conclusions:Deficiency of scientific research resources faced by municipal hospitals can be mitigated by medicine-engineering interdisciplinary collaboration with universities, and medicine-engineering interdisciplinary collaboration can be a possible way to improve academic level of municipal hospitals.
9.Analysis of scientific research system in a pharmaceutical department at hospital

Objective By researching the cognition and attitudes of the pharmacists of the Department of Pharmacy to further improve and perfect the scientific research policy of the department,and promote the construction of scientific research work.Methods The questionnaire used Microsoft Excel software for statistical analysis.Results 24.7% of the pharmacists engaged in scientific research to reflect the individual academic and technical ability level;only 14.7% of pharmacists are more willing to participate in the competition for scientific research positions;35.3% of the pharmacists are satisfied with the current policy;6.5% of the pharmacists are not satisfied with the rewards and penaltypolicy.Conclusions The Pharmaceutical Department of Central South University Xiangya Hospital should give proper guidance for pharmacist in research purpose and direction and make them clear that the scientific research purposes is to improve rational medication.Our scientific research system still exist some shortcomings.We need to increase investment in scientific research advertisement,optimize the reward system,set up research teams for employees for their development.
10.Changes in endothelial progenitor cell function in elderly patients with permanent atrial fibrillation and their clinical significance

Objective To investigate changes in the number and function of endothelial progenitor cells (EPCs) in elderly patients with permanent atrial fibrillation(AF).Methods This prospective study included 35 elderly patients in each the permanent atrial fibrillation group and the control group.The numbers of circulating CD34+/KDR+ cells in the two groups were determined by flow cytometry.After two sets of peripheral blood samples were taken,mononuclear cells were isolated through density gradient centrifugation and cultured in vitro.EPC colonies were identified by the methylthiazolyldipheny-tetrazolium(MTT) assay and adhesion assay.The proliferation,adhesion and vasculogenesis of EPC colonies were determined by Matrigel culture.Enzyme-linked immunosorbent assay and nitric acid reductase assay were used to measure the secretion of vascular endothelial growth factor (VEGF) and nitric oxide (NO) in EPCs.Results The numbers of CD34+/KDR+ cells were lower in the AF group than in the control group (20.0±12.7)/104 vs.(77.9±58.9)/104 (P<0.05).The number of EPC colonies in the atrial fibrillation group was significantly lower than that in the control group (1.8 ± 0.6) CFU vs.(3.5 ± 0.8) CFU (P < 0.01).The proliferation,adhesion and vasculogenesis of EPC colonies in the AF group decreased,compared with the control group (each P<0.01 or 0.05).Paracrine secretion of VEGF in the AF group (27.4±9.9)ng/L was lower than that in the control group (41.9±7.3)ng/L (P<0.01) and paracrine production of NO in the AF group also decreased (P<0.05).Conclusions EPCs In elderly patients with permanent atrial fibrillation show decreased numbers and reduced proliferation,adhesion and vasculogenesis.Paracrine VEGF and NO secretion is down as well.
