1.Influence of valsartan and amlodipine on the left ventricular diastolic function,endothelium function and serum insuline level in aged hyperpietic patients

Objective: To observe the effect of valsartan and amlodipine on the reduction of blood pressure in aged hyperpietic patients,as well as on the improvement of left ventricular diastolic function,endothelium function and insuline resistence. Methods: We divided 40 cases of aged hypertension patients(1 to 2 grade) into Valsartan and Amlodipine groups at random,giving valsartan 80-160 mg or amlodipine 5-10 mg,respectively,for 12 weeks as a treating period.We measured the blood pressure twice a week,observed the differences of blood pressure and heart rate before and after the treatment,as well as the changes of insuline sensitivity,left ventricular diastolic function and endothelium vascular disatolic function. Results and Conclusion: Both valsartan and amlodipine can effectivly reduce the blood pressure of aged hypertension patients(l to 2 grade).They can also improve the diastolic function,insuline resistance and endothelium dependent vascular diastolic function.
2.Effects of antagonistic peptide specifically binding to second extracellular loop of CCR5 on inflammatory cell infiltration and TNF-α expression in lung tissues of asthmatic mice induced by OVA

AIM: To investigate the effects of antagonistic peptide specifically binding to the second extracellular loop of CC chemokine receptor 5 (CCR5) on inflammatory cell infiltration and TNF-α expression in lung tissues of asthmatic mice.METHODS: The asthmatic model of BALB/c mice was induced by ovalbumin (OVA) and the optimal sensitization concentration of OVA was screened.After modeling, the mice were intervened by gradual concentrations of antagonistic peptide via tail-vein injection.The pathocytological analysis and grading were performed in the lung tissues with HE staining.The expression of TNF-α at mRNA and protein levels in the lung tissues was determined by real-time PCR and Western blot.RESULTS: The optimal concentration of OVA was 500 mg/L (0.1 mL) as this concentration of OVA stably induced moderate degree of inflammation in the BALB/c mice.Treatment with different concentrations (1.5 g/L, 2.5 g/L and 3.5 g/L) of antagonistic peptide at 0.2 mL through tail-vein injection inhibited the expression of TNF-α, and markedly reduced the degree of inflammation in the lung tissues.The optimal concentration of antagonistic peptide was 2.5 g/L as the lung inflammation degree in 2.5 g/L group alleviated by 2 grades, and the number of inflammatory cells was also significantly reduced.Moreover, the mRNA expression abundance of TNF-α was nearly decreased by 90%, and the protein expression of TNF-α was decreased by 70% compared with model group.Meanwhile, the use of antagonistic peptide at 2.5 g/L before OVA stimulation confirmed the preventive function to some degree.In this group, the lung inflammation degree alleviated by 1 grade, and the expression of TNF-α at both mRNA and protein levels decreased by nearly 50%.CONCLUSION: The antagonistic peptide of CCR5 effectively inhibits the expression of TNF-α and relieves the inflammation in the asthmatic mouse lung tissues in a concentration-dependent manner.
3.Anatomical measurements and clinical significance of anterior atlantoaxial transarticular screw fixation

BACKGROUND: Chinese anatomical parameters of antedor atlantoaxial transarticular screw fixation have been rarely reported although the technique is a novel method out of China for patients with C1-C2 instability. OBJECTIVE: To provide Chinese anatomical data for anterior C1-C2 transarticular screw fixation. DESIGN, TIME AND SETTING: A measurement experiment was performed at the Department of Anatomy, Southern Medical University and Department of Orthopedics, Wuhan General Hospital, Guangzhou Command of Chinese PLA between September 2006 and April 2008. MATERIALS: A total of 50 sets of dried Chinese adult human C1 and C2 specimens, without regard to gender and age, but no abnormality and breakage, were measured with an electronic digital caliper (precision 0.01 mm) and a goniometer(precision 0.5°) made in China. METHODS: One proper screw was drilled through the atlantoaxial joint respectively in the direction to middle part of laterosuperior angle in the posterior of C1 lateral mass, and the screw point should not break through the superior facet articularsurface of the C1. In the procedure, the screw drilling point was at the junction of the lateral border of C2 body to 4 mm above the inferior border of C2 anterior arch. border of transverse foramen of C2 body and the median line of C2 body, and the distance between the inserting point and the medial border of transverse foramen of C2 body. RESULTS: The data from all specimens were involved in the result analysis. In the sagittal plane, the minimum lateral angulation of the screw tract was (10.80±2.10)°(left) and (10.76±2.40)°(right) respectively, and the maximum lateral angulation was (25.13±3.12)°(left) and (25.12±2.86)°(right), respectively. In the coronal plane, the minimum posterior angulation was (8.85±2.12)° (left) and (9.28±2.65)° (right) respectively, and the maximum posterior angulation was (26.96±3.09)°(left) and (27.49±2.51)°(right), respectively. The left screw tract length was from (17.48±2.10) mm to (25.41±2.59) ram, and the right was from (17.49±2.23) mm to (25.58±2.42) mm. The left distance between the inserting point and the median line of C2 body was (9.84±0.69) mm, and the right was (9.81±0.66) mm. The left distance between the median line of C2 body and medial border of transverse foramen of C2 body was (14.12±1.28) mm, and the right was (14.60±1.36) mm. The left distance between the inserting point and medial border of transverse foramen of C2 body was (6.28±1.38) mm, and the right was (6.79±1.39) mm. CONCLUSION: It is optimal for the anterior C1-C2 transarticular screw fixation to place the antedor screw with a length of 17 to 25 mm in lateral angulation ranging from 10° to 25° and the posterior angulation ranging from 9° to 27°. During the procedure, the dissecting distance from the middle of C2 body to lateral should not exceed 14 mm.
4.Effects of antagonistic peptides binding specifically with first and second extracellular loops of CCR5 on colitis rats induced by TNBS

AIM:To study the effects of antagonistic peptides binding specifically with the first and second extracellular loops (ECL1 and ECL2) of C-C chemokine receptor 5 (CCR5) on the colitis rats induced by trinitrobenzenesulfonic acid (TNBS) and the mechanisms.METHODS:The colitis model of SD rats was induced by TNBS (100 mg/kg).The effects of 2 antagonistic peptides at different doses (ECL1:25, 35 and 45 mg/kg;ECL2:15, 25 and 35 mg/kg) on the model rats including the changes of disease activity index (DAI), colon macroscopic damage index (CMDI) and histological grading were observed.The mRNA and protein expression levels of TNF-α and COX-2 in the colonic mucosa were detected by real-time PCR and Western blot, respectively.RESULTS:Compared with model group, the changes of DAI, CMDI and histopathological injury of the rats treated with ECL2 antagonistic peptide HY at an appropriate dose were significantly reduced (P<0.05), and the protein and mRNA expression levels of TNF-α and COX-2 were significantly decreased (P<0.05).However, the effects of ECL1 antagonistic peptide GH on all scores and the expression levels of TNF-α and COX-2 were not obvious.CONCLUSION:ECL2 antagonistic peptide HY relieves TNBS-induced colitis in SD rats via down-regulating the expressions of TNF-α and COX-2 in the colonic mucosa, while the effect of ECL1 antagonist peptide GH was not obvious.
5.Clinical Value of 64-slice Spiral CT in the Diagnosis of Congenital Cardiopathy

Objective To explore the clinical value of 64-slice spiral CT in the diagnosis of congenital cardiopathy.Methods 64-slice spiral CT angiographic date of 20 cases with congenital cardiopathy were analyzed retrospectively in comparison with the ECG and surgical results.Results 4 cases of atrial septal defect,6 cases of ventricular septal defect,7 cases of tetralogy of Fallots,2 cases of patent ductus arteriosus,1 case of right cardiac ventricle double exit in company with ventricular septal defect,1 case of coarctation of aorta and 1 case of interruption of aortic arch were showed by 64-slice spiral CT angiography.Six cases were showed having abnormal coronary artery.Pulmonary arteries of 7 cases were showed straitness in some extent.One atrial septal defect(3 mm) and one ventricular septal defect(2 mm) were missed.The accuracy of diagnosis is 90.00%.Conclusion 64-slice spiral CT heart angiography is an important examination in diagnosing congenital cardiopathy.
6.Finite element modeling and comparison of fixation methods for acetabular fracture with involvement of the quadrilateral plate

Objective To compared different fixation methods for acetabular fractures involving the quadrilateral plate using a finite element model of the acetabulum.Methods A model of acetabular fractures with quadrilateral plate involved was developed in the finite element software and processed in Hypermesh V10.0 to generate internal fixation with dual-column titanium plate (Group A),anterior special titanium moulding plate plus quadrilateral screws (Group B),and anterior special titanium moulding plate plus quadrilateral screws combined with posterior column screws (Group C).Pelvic stress in sitting and standing positions were simulated in sequence with constraint of tuber nodes and inferior femur.Maximum stress and displacement of the acetabulum and displacement of nodes on fracture lines were measured after a force of 600 N was applied to S1 verterbrae in line with the direction of gravity in sitting and standing positions.Results In sitting position,the maximum stress and displacement of the acetabulum exhibited a sequence of Group C (9.47,1.08) < Group B (19.84,1.11) < Group A (29.73,1.14).Moreover,the same result was found in standing position with Group C (9.62,1.09) < Group B (12.18,1.10) < Group A (13.28,1.13).Mean displacement of nodes on fracture lines ranked in order of Group C < Group B < Group A (P > 0.05).Conclusions The finite element model can reflect the distribution of pelvic stress effectively.Anterior special titanium moulding plate plus quadrilateral screws combined with posterior column screws provide favorable biomechanical stability in treatment of acetabular fractures involving the quadrilateral area.
7.Correlation of CCR5 expression with β-arrestin 2 expression in colonic mucosa of patients with inflammatory bowel disease

AIM:To analyze the expression of CCR5 and correlation with the expression ofβ-arrestin 2 in the intestinal mucosa of the patients with inflammatory bowel disease ( IBD) , so as to study the role of CCR5 andβ-arrestin 2 in the pathogenesis of IBD.METHODS:Paraffin sections of the colonic mucosa were prepared from 53 patients with active IBD, 26 patients with remissive IBD and 30 healthy people.Immunohistochemical EnVision two-step method was used to test the expression of CCR5 andβ-arrestin 2 in the biopsic intestinal mucosa.RESULTS:The positive rate, strongly posi-tive rate and immunohistochemical score of CCR5 expression in active IBD were significantly higher than those in normal controls or remissive IBD (P<0.05).No correlation of CCR5 expression with clinical severity, lesion distribution, and endoscopic grade in active IBD was observed.The expression ofβ-arrestin 2 was significantly lower in active IBD than that in the remissive IBD and normal controls, and there was a negative correlation ofβ-arrestin 2 expression with CCR5 expres-sion (P<0.05).CONCLUSION:The expression of CCR5 is higher, and expression ofβ-arrestin 2 is lower, and there is a negative correlation of expression of CCR5 with expression ofβ-arrestin 2 in intestinal mucosa of the active IBD.
8.AO mini plate combined with reconstruction plate in the internal fixation of acetabular posterior wall fractures

Objective To evaluate the recent clinical outcomes of fixing posterior acetabular wall fractures combining the AO mini plate with reconstruction plate.Methods From Jan 2009 to Jul 2013,28 patients with posterior acetabular wall fracture were fixed by the AO mini-steel plate combined with reconstruction plate,including 17males and 11 females with the mean age of 45.8 years (range,18-65).After admission,reduce the dislocation of hip joint immediately,and take femoral condyle or tibial tubercle traction.Preoperative and postoperative anteroposterior and Judet oblique radiographs,acetabular CT scan and three-dimensional reconstruction imaging were routine taken.Using Kocher-Langenbeck approach,intraoperative,try to protect the soft tissue attached on the small fragments,and reduct the fracture.Cutting the mini plate into proper length with reconstruction plate fix to its surface crossover.The functional outcomes were evaluated by the modified Merle d'Aubigné and Postel clinical grading system.The radiographs were graded according to the Matta/Judet criteria.Results Twenty-three patients are followed up 13 months(82.2%),and 5 cases lost in 12 weeks (17.8%).By the Matta/Judet criteria,22 cases achieved excellent reduction,4 good reduction and two fair reduction,with a good and excellent rate of 92.8%.By the modified Merle d'Aubigné and Postel criteria,functional recovery of the affected hip six month post-operation was rated as excellent in 15 cases,good in 5 cases,fair in 2 cases and poor in one case.with a good and excellent rate of 87.9%.One case occurred femoral head necrosis.In 2 patients 2 years after surgery developed traumatic arthritis.Two patients had heterotopic ossification after six month operation.One patient with injury to the sciatic nerve recovered 3 months post-operation.Conclusion Using the AO mini plate combined with reconstruction plate can fix the acetabular posterior wall fractures stably.The technique meet the requirements of patients with early functional exercise,which provides an effective method of internal fixation.

The immune effect of two recombinant protein fragments of spike protein in severe acute respiratory syndrome coronavirus (SARS CoV) was investigated in Balb/c mice. Two partial spike gene fragments S1 (322 1464 bp) and S2 (2170 2814 bp) of SARS coronavirus were amplified by RT-PCR, and cloned into pET-23a prokaryotic expression vector, then transformed into competent Escherichia E. coli BL21 (DE3)(pLysS) respectively. Recombinant proteins were expressed and purified by Ni2+ immobilized metal ion affinity chromatography. The purified proteins mixed with complete Freund adjuvant were injected into Balb/c mice three times at a two-week interval. High titer antibody was detected in the serum of immunized Balb/c mice, and mice immunized with S1 protein produced high titer IgG1, IgG2a, IgG2b and IgG3, while those immunized with S2 protein produced high titer IgG1, IgG2a, but lower titer IgG2b and IgG3. Serum IFN-concentration was increased significantly but the concentrations of Il-2, IL-4 and IL-10 had no significant change. And a marked increase was observed in the number of spleen CD8+ T cells. The results showed that recombinant proteins of SARS coronavirus spike protein induced hormonal and cellular immune response in Balb/c mice.
10.The value of 3.0T DCE-MRI in the diagnosis of prostate cancer

Objective To analyze the characteristics of dynamic contrast enhanced MR imaging (DCE-MRI)in prostate cancer (PCa)at 3.0T,and to evaluate the diagnostic value of DCE-MRI.Methods 85 patients with suepected PCa received conventional MRI and DCE-MRI.The signal intense-time (SI-T)curve was analyzed.Then the time to maximum (Tmax),the maximum degree of enhancement (STmax%),and the rate of enhancement (Rmax)were calculated.The differences of styles of SI-T curve and the parameters between the positive and negative group were compared respectively.Results 59 cases of PCa were proved by biopsy,and there was no evidence of tumor in 26 cases.507 zones had histopathological results with 250 zones in positive group and 257 zones in negative group .The most common style of SI-T curve in positive group was rapidly ascending followed with descending curve,the most common style of SI-T curve in negative group was persistent ascending curve and plateau curve.The mean values of Tmax,SImax%,Rmax were (69.49±22.53)s,1.74±0.43,7.83±3.80 in positive group respectively,while (175.61±52.64)s,1.05±0.35,1.86±1.10 in negative group respectively,there were statistically significant differences between the two groups(t =-24.24,1 6.34,1 7.75,P <0.01)respectively. The mean values of Tmax,SImax% ,Rmax were (8 9 .1 9 ± 3 1 .7 2 )s,1 .5 8 ± 0 .4 6 ,5 .2 1 ± 3 .3 4 in the low-risk group (Gleason score 2 - 6 )respectively,while (64.25±14.68)s,1.76±0.43,8.25±3.70 in the high-risk group (Gleason score 7-10)respectively, there were statistically significant differences between them(t = 7.09,-8.74,- 7.83,P <0.01).Conclusion 3.0T DCE-MRI has great value in the diagnosis of PCa.
