Objective To investigate the serum levels of IFN-? and IL-10 in RA patients before and after receiving total glucosides of paeony combined with vitamin D3 therapy respectively and to explore the immunology mechanism of treating RA. Objective To explore the expression and clinical significance of secretagogne (SCGN) in neuroendocrinal tumors.Methods Immunohistochemistry was applied for detecting expression of SCGN,chromogranin A (CgA),synaptophysin (Syn) and neuronspecific enolase (NSE) in 45 cases of neuroendocrinal tumors.The difference between the expression of SCGN and the remaining indicators were compared.And different expression profiles of SCGN between groups categorized by sex,age and degree of differentiation were also compared.Results The positive expression rate of SCGN,CgA,Syn and NSE in neuroendocrinal tumors was 95.56％ (43/45),100.00％ (45/45),46.67％ (21/45) and 88.89％ (40/45) respectively,and the differences among them were statistically significant(P ＜ 0.01).Expression of SCGN was not correlated with the patient's sex,age and degree of differentiation (P＞ 0.05).Conclusions Most cases of neuroendocrinal tumors are positive for SCGN and SCGN is more sensitive than CgA.Syn is the most sensitive one for diagnosis of neuroendocrinal tumors.SCGN can be seen as a novel neuroendocrinal marker in the clinical diagnosis of neuroendocrinal tumors.

BACKGROUND: The repair and functional reconstruction of peripheral traumatic nerves depend, on posttraumatic operation time and operative techniques which have certain referential value.OBJECTIVE: To explore the effects of related factors on the repair and functional reconstruction of common peroneal nerve (CPN) after injuries.DESIGN: A case analysis.SETTING: Department of Orthopaedics, Fourth Hospital Affiliated to China Medical University.PARTICIPANTS: Eighteen patients diagnosed with simple CPN injury received treatment in the Department of Orthopedic Surgery of the Fourth Affiliated Hospital, Chinese Medical University, between January 1991 and January 2001. There were 15 males and 3 females aged 12-61 years old with the mean age of 26.5 years; there were 9 cases of complete transection, 3 cases of incomplete transection, 1 case of compression, 4 cases of contusion, and 1 case of irreversible injury. Posttraumatic treatment at hospital was obtained 0.5 hour to 6 months after injury.METHODS: All patients were subjected to CPN explorative operation;neuroanastomosis was performed on 12 cases, neurolysis on 2 cases, tendon graft on 1 case and conservative treatment on 3 cases. Antibiotics were administered after operation together with blood activating medicine and neurotrophic medicine. Moreover, the patients were required to do passive functional exercise in order to avoid arkylosis and myoatrophy.MAIN OUTCOME MEASURES: Functional recovery of CPN was assessed at postoperative 6 months based on the myodynamia of the same side anterior tibia muscles (excellent: above grade 4; good: grade 3; passable: grade 2; poor: grade 1 or grade 0).RESULTS: All the 18 patients entered the results analysis; their outcomes were assessed as excellent in 10 cases, good in 6 cases, and passable in 2cases, with the excellent rate being 89% (16/18).CONCLUSION: Patients diagnosed with CPN injury should be treated with primary repair of traumatic nerves as early as possible, which will benefit the functional recovery of limbs.

Objective To study the benefits and drawbacks of endoscopic thyroidectomy via breast approach by using gasless anterior neck skin lifting method.Methods A total of 21 patients with thyroid diseases were enrolled in this study,including 16 cases of nodular goiter and 5 cases of primary hyperthyroidism.All the cases underwent endoscopic subtotal thyroidectomy through breast approach while the operational space was established by using an abdomen suspending device and 2 Kirschner pins to mechanically suspend and retract the anterior neck skin.Results The endoscopic subtotal thyroidectomy was completed successfully in all of the cases without conversion to open surgery.The mean operation time was 82.5 minutes(ranging from 57 to 125 minutes).Intraoperative blood loss was less than 20 ml.Post-operational recovery was satisfying in all the patients.Drainage volume averaged 80.6 ml(50 to 150 ml).The patients were followed up for 1 to 12 months after the operation,during which no complications occurred.Conclusions Endoscopic thyroidectomy via breast approach using gasless anterior neck skin lifting method is a safe and low-cost procedure.By using the method,CO2 insufflation-related complications can be avoided.Although the operational space established by this method is relatively small,the operation can be completed without much difficulies.

Objective This paper was conducted to study the preparation methods and the hypoglycemic effects of oral sodium alginate-insulin nanoparticles (INS-NPs) on the blood glucose level in Streptozotocin-induced diabetic Wistar rats. Methods The INS-NPs were prepared by an ionic gelation method. The changes of the morphology and size of the INS-NPs were examined by transmission electron microscope and Zetasizer 3000HS. The hypoglycemic effects of the INS-NPs were evaluated by monitoring the blood glucose levels in streptozotocin-induced diabetic Wistar rats. Results The INS-NPs were spherical or ellipsoidal in shape with a diameter of 236.4?19.3nm. The entrapment efficiency and load efficiency of INS-NPs were 78.5%?6.1% and 22.6%?4.4%, respectively. In vivo hypoglycemic study showed the levels of blood glucose of diabetic Wistar rats declined at 7h after oral administration of INS-NPs (26U/kg). Their hypoglycemic effects were maintained for 12h and the levels of blood glucose were kept with normal range for 6h (less than 7.0mmol/L). Conclusion The INS-NPs have the hypoglycemic effect on the blood glucose level of streptozotocin-induced diabetic rats.

Objective To compare the effect of crystalloid and crystalloid plus albumin in the treatment of pediatric septic shock.Methods Data of 63 pediatric patients with septic shock admitted to Department of Critical Care Medicine of Hebei Provincial Children's Hospital were collected and retrospectively analyzed.The patients were divided into two groups according to whether they received albumin for volume resuscitation within 1 day after admission or not.The patients in observation group (crystalloid + albumin group,n =33) received normal saline (20 mL/kg) followed by 1 g/kg albumin 30 minutes after admission,and those in control group (crystalloid group,n =30) received only normal saline (20 mL/kg) 30 minutes after admission,and normal saline resuscitation was continued according to the effect of fluid therapy.Anti-infection and vasoactive drugs strategies were the same in both groups.The first-hour infusion volume,time showing stable hemodynamics,the incidence of pulmonary edema,and blood lactate levels at 0,6,12 hours after achieving the goals were compared,and blood lactate clearance rates were calculated.Results The first-hour infusion volume time in the observation group was lower than that in control group (mL:41.56 ± 10.50 vs.57.24 ± 7.54,t=4.596,P=0.000),and time showing stable hemodynamics was shorter than that in control group but without statistically significant difference (minutes:219.87 ±70.23 vs.287.10 ± 67.00,t=2.047,P=0.360).The incidence of pulmonary edema in observation group was slightly lower than that in control group [6.1％ (2/33) vs.10.0％ (3/30),x2±2.272,P=0.259].The lactic acid levels were decreased gradually along with rehabilitation time,while lactate clearance rate was increased in both groups.At 0 hour and 6 hours after resuscitation,the lactate level (mmol/L) in the observation group was significantly lower than that in control group [0 hour:3.65 ± 2.84 vs.5.72 ± 2.11,t＝ 1.940,P=0.046; 6 hours:2.12 ± 1.21 vs.4.09 ± 1.45,t=2.892,P=0.005],while the lactate clearance rate was significantly increased compared with control group [0 hour:(0.38 ± 0.15)％ vs.(0.18 ± 0.09)％,t=1.447,P=0.018; 6 hours:(0.62 ± 0.14)％ vs.(0.51 ± 0.11)％,t=1.920,P=0.047].However,at 12 hours after resuscitation,there were no statistically significant differences in the lactic acid level (mmol/L:1.46 ± 0.39 vs.1.54 ± 1.90,t=0.450,P=0.072) and the lactate clearance rate [(0.78 ± 0.19) ％ vs.(0.77 ± 0.18) ％,t =0.091,P=0.928] between observation group and control group.Conclusion Albumin resuscitation in children with septic shock can stabilize hemodynamics earlier,reduce the incidence of pulmonary edema,and improve the successful rescue rate of refractory septic shock.

Objective To investigate the methods of isolation, cultivation of adipose derived stem cells (ADSCs) from the rats'adipose tissue and the feasibility of inducting differentiation to chondrocyte.Methods The adipose tissue was obtained from the wistar's male rats, and it was isolated, cultivated, and subcultured. The expression of CD29 in the subcultured ADSCs was detected by immunofluorescence. The 3rd generation ADSCs was cultured in the medium contained TGF-β, and the presence was identified by type Ⅱ collagen immunohistochemistry. Results The character of ADSCs'morphology could be observed by microscope, and it can be identified by immunofluorescence. Through the course of inducting the ADSCs to chondrocyte, the feasibility of inducting ADSCs to chondrocyte showed that type Ⅱ collagen was expressed.Conclusion We demonstrated the presence of stem cells in the adipose tissue, and the stability of biological feature in vitro. The chondrocyte could be obtained from the adipose tissue through the committed differentiation, induction, cultivation by exogenous transforming growth factor (TGF-β). The ability of the adipose stem cell differentiating to chondrocyte was also proved.

Objective To explore the effectiveness and safety of high frequency oscillatory ventilation(HFOV) in children with measles complicated with severe pneumonia and the acute respiratory distress syndrome(ARDS).Methods A total of 63 children with measles complicated with severe pneumonia and the ARDS were divided into conventional mechanical ventilation(CMV) group and HFOV group.The PaO2/ FiO2,oxygenation index (OI),HR and mean arterial pressure (MAP) before treatment and 12 h,24 h,48 h after treatment were detected.The rate of air leak and the motality in two groups were compared.The efficacy and safety of HFOV treatment were evaluated in children with measles complicated with pneumonia and severe ARDS.Results In HFOV group,the PaO2/FiO2 ratio was elevated and OI was decreased significantly after 12 h and maintained for at least 48 h.Compared with CMV group,OI of HFOV group improved more significantly,and the difference was statistically significant.The ventilation time in HFOV group was shorten than that in CMV group[(7.97 ±3.06) d vs.(11.03 ±3.60) d],but there was no statistical difference between two groups (P ＞ 0.05).The heart rate after treatment 48 h was gradually returned to normal,and there was no statistical difference between the two groups.There were no significant changes in the MAP of two groups after treatment.There were no significant differences in the incidence of air leak between the CMV group and the HFOV group(24.2％ vs.16.7％).The mortality rate of CMV group and HFOV group was respectively 45.5 ％ and 33.3 ％,and there was no statistically significant difference.Conclusion HFOV was effective in oxygenation and seems to be safe for pediatric patients with measles complicated with severe pneumonia and the ARDS.Also it didn't influence the occurrence of complications.It has no adverse influence on hemodynamic parameters.Early intervention of HFOV is safe and effective for the children with measles complicated with severe pneumonia and ARDS.
