1.Autoantibodies and inflammatory cytokines in children with systemic-onset juvenile idiopathic arthritis

Objective To investigate the changes of autoantibodies and inflammatory cytokines in children with system‐onset ju‐venile idiopathic arthritis (So‐JIA) .Methods Serum concentrations of autoantibodies and inflammatory cytokines were measured in children with So‐JIA at active phase or remission phase by using indirect immunofluorescence method and enzyme‐linked immu‐nosorbent assay .Healthy children were enrolled as control group .Results The assay positivity of antikeratin antibody (AKA) and anti‐cyclic citrullinated peptide antibody (anti‐CCP) was 29 .4% and 47 .1% in active phase group ,and was 17 .2% and 27 .6% in remission group .The differences between the two groups were significant (P<0 .05) .The diagnostic sensitivity of AKA and anti‐CCP was 21 .1% and 41 .3% ,and the diagnostic specificity was 97 .2% and 91 .9% .The diagnostic sensitivity of anti‐CCP was higher than AKA .Serum levels of anti‐CCP ,interleukin‐6 (IL‐6) and interleukin‐17 (IL‐17) of active phase group were higher than remission phase group and control group (P<0 .05) ,and those of remission phase group were higher than control group ,but the differences were not significant (P>0 .05) .Conclusion AKA and anti‐CCP might be with high diagnostic specificity to So‐JIA ,but the sensitivity could be low ,and combined detection could increase the diagnostic positivity .Inflammatory cytokines ,such as IL‐6 and IL‐17 ,might be associated with the activity of So‐JIA ,which could be important biomarkers of So‐JIA .
2.Preparation of Sustained-Release Tetramethylpyrazine Phosphate Tablet and its Bioavailability on Rabbits

Purpose To prepare the sustained-release tablet of tetramethylpyrazine phosphate with hydroxypropylmethylcelluose(HPMC) as matrix material. MethodsThe paddle method and the HPLC method were erspectively used determined the cumulative drug released in vitro and the serum concentration in vivo.ResultsThe cumulative drug released in the first hour was about 20%, while in 12 hours it was above 85%. Drug release behavior can be best described by Higuchi equation, and the release rate decreased as the viscosity and/or the amount of HPMC increased. Compared with the market tablet on the rabbits, the sustained release tablet had the decreased peak concentration (P < 0.05 ); the prolonged peak time and mean residence time (P< 0.05).ConclusionsThe matrix tablet was a good sustained-release dosage form and it had a good in vitro-in vivo correlation.

4.Treating Displaced Patellar Fracture with Longitudinal Incision & Internal Fixation of the Sidelong Anterior Tension-band Wiring

Objective To explore the clinic effects of treating the displaced patellar fracture with a small longitudinal midline incision and internal fixation of the sidelong figure-of-eight anterior tension-band wiring. Methods 93 ea.ses of the displaced patellar fractures were internal fixated with the sidelong figure-of-eight anterior tension-band wiring(1.2 mm) simply, which made a longitudinal midline incision,it length can be made equal to the diam of patella approximately,that gives to enough expose the entire anterior surface of the patella and the patellar tendons. Results Follow-up for 6 to 30 months,93 cases were all obtained bony union. By Zhang Chun-cai score,there were 67 cases excellent,23 good and 3 fair,the excellent and good rate was 96.8%. Conclusion Treating patellar fracture with the fixation methods are very stabilization,and can functional exercise in early. It had similar results to reports using modified (with Kirschner wires) tension band techniques. The methods is simple and mini-trauma and invasive device minimally which results in fewer complications,especially as fractures traversed and comminuted with relative larger fragments are concerned.
5.Immunogenicity, safety and immune persistence of the sequential booster with the recombinant protein-based COVID-19 vaccine (CHO cell) in healthy people aged 18-84 years

Objective:To evaluate the immunogenicity, safety, and immune persistence of the sequential booster with the recombinant protein-based COVID-19 vaccine (CHO cell) in healthy people aged 18-84 years.Methods:An open-label, multi-center trial was conducted in October 2021. The eligible healthy individuals, aged 18-84 years who had completed primary immunization with the inactivated COVID-19 vaccine 3 to 9 months before, were recruited from Shangyu district of Shaoxing and Kaihua county of Quzhou, Zhejiang province. All participants were divided into three groups based on the differences in prime-boost intervals: Group A (3-4 months), Group B (5-6 months) and Group C (7-9 months), with 320 persons per group. All participants received the recombinant COVID-19 vaccine (CHO cell). Blood samples were collected before the vaccination and after receiving the booster at 14 days, 30 days, and 180 days for analysis of GMTs, antibody positivity rates, and seroconversion rates. All adverse events were collected within one month and serious adverse events were collected within six months. The incidences of adverse reactions were analyzed after the booster.Results:The age of 960 participants was (52.3±11.5) years old, and 47.4% were males (455). The GMTs of Groups B and C were 65.26 (54.51-78.12) and 60.97 (50.61-73.45) at 14 days after the booster, both higher than Group A′s 44.79 (36.94-54.30) ( P value<0.05). The GMTs of Groups B and C were 23.95 (20.18-28.42) and 27.98 (23.45-33.39) at 30 days after the booster, both higher than Group A′s 15.71 (13.24-18.63) ( P value <0.05). At 14 days after the booster, the antibody positivity rates in Groups A, B, and C were 91.69% (276/301), 94.38% (302/320), and 93.95% (295/314), respectively. The seroconversion rates in the three groups were 90.37% (272/301), 93.75% (300/320), and 93.31% (293/314), respectively. There was no significant difference among these rates in the three groups (all P values >0.05). At 30 days after the booster, antibody positivity rates in Groups A, B, and C were 79.60% (238/299), 87.74% (279/318), and 90.48% (285/315), respectively. The seroconversion rates in the three groups were 76.92% (230/299), 85.85% (273/318), and 88.25% (278/315), respectively. There was a significant difference among these rates in the three groups (all P values <0.001). During the sequential booster immunization, the incidence of adverse events in 960 participants was 15.31% (147/960), with rates of about 14.38% (46/320), 17.50% (56/320), and 14.06% (45/320) in Groups A, B, and C, respectively. The incidence of adverse reactions was 8.02% (77/960), with rates of about 7.50% (24/320), 6.88% (22/320), and 9.69% (31/320) in Groups A, B, and C, respectively. No serious adverse events related to the booster were reported. Conclusion:Healthy individuals aged 18-84 years, who had completed primary immunization with the inactivated COVID-19 vaccine 3 to 9 months before, have good immunogenicity and safety profiles following the sequential booster with the recombinant COVID-19 vaccine (CHO cell).
6.Qualitative Study on the Current Situation of Disease Communication between Cancer Patients and Their Families

To further explore the current situation of disease communication between cancer patients and their families in clinical practice, and provide decision-making reference for the construction of harmonious doctor-patient relationship. From September to November, 2021, 10 oncologists from a third-class A general hospital in X city were selected for semi-structured interviews by objective sampling method, and Colaizzi seven-step analysis was used to analyze, refine and summarize the interview data. The results showed that the current situation of disease communication between cancer patients and their families can be summarized into three themes: concealing information during the diagnosis period, embarrassing decision-making during the treatment period, and difficult in autonomy at the end stage of the disease. Based on this situation, this paper suggested that a clinical communication mode with medical staff as a bridge and patients and their families actively facing and participating together can be constructed from different dimensions such as disease notification, win-win cooperation and psychological care.
7.Study on the Mechanism of Wound Healing Promotion of Panax notoginseng-Bletilla striata Gum Sponge on Diabetic Foot Ulcer Model Rats Based on Wnt/ β-catenin Signal Pathway

OBJECTIVE: To study the mechanism of wound healing promotion of Panax notoginseng-Bletilla striata gum sponge on diabetic foot ulcer (DFU) model rats. METHODS: Healthy SD rats were selected and given high-lipid and high-glucose diet, intraperitoneal injection of streptozotocin once to establish diabetes model. Neodymium-iron-boron magnet was used to press the back of rats to make ulcer wound then established DFU model. Totally 60 DFU model rats were randomly divided into group A(blank group, i.e. normal saline gauze group), group B(vaseline gauze group),group C (gelatin sponge group) and group D (P. notoginseng-B. striata gum sponge group), with 15 rats in each group. The rats were given corresponding gauze/sponge to cover the wound for intervention treatment, changing dressing once every 1-2 days. On the 3rd and 7th day after intervention, the wound healing of rats in each group was observed with naked eyes, and the wound healing rate was calculated. The wound margin tissue was collected to obtain HE staining section, and histopathological observation was conducted under microscope. mRNA expression of β-catenin, GSK-3β and Rspo3 in wound tissue were determined by RT-PCR. RESULTS: On the 3rd and 7th day after intervention, compared with group A, B, C, healing rate of group D was increased significantly (P<0.05); inflammatory cell infiltration, collagen deposition, capillary and granulation tissue growth in wound tissue increased significantly. The mRNA expression levels of β-catenin and Rspo3 all increased, and those of GSK-3β all decreased; except for the difference of β-catenin at the 3rd day and GSK-3β at the 7th day after intervention between group D and group C were not significant, the difference of other indicators was statistically significant (P<0.05). CONCLUSIONS: P. notoginseng-B. striata gum sponge can effectively promote the wound healing in DFU model rats, the mechanism of which may be associated with up-regulating the expression of β-catenin and Rspo3 mRNA and down-regulating the expression of GSK-3β mRNA.
