1.The role of Zuckerkandl's tubercle in revealing recurrent laryngeal nerve

Objective To analyze the anatomical relationship between Zuckerkandl's tubercle (ZT) and the recurrent laryngeal nerve(RLN),to reduce the incidence of RLN injury risk.Methods 280 patients undergoing total thyroidectomy from Jan.2008 to Jan.2013 were included in our study.A meticulous technipue of excapsular dissection was used to dissect thyroid.ZT's size,classification,and its relationship with RLN were studied and classified.Results A total of 280 thyroid gland lobes were dissected.ZT identified in the left was 94.1%(128/136),right 96.4%(160/166),and bilateral 72.8%(220/302).ZT grades were as the following:Grade 0,left 11.8%(16/136) and right 11.4%(19/166);Grade I,left 30.1%(41/136) and right 25.3%(42/166);Grade II,left 44.1%(60/136) and right 43.4%(72/166);Grade III,left 8.1%(11/136) and right 16.3%(27/166).There was no significant difference on ZT classification between the left side and right side.For ZT with grade I or above,112 cases were at the left side,among which type A was 90.2%(101/112),type B 0.9%(1/112),type C 7.1%(8/112),and type D 1.8%(2/112),and 141 cases were at the right side,among which type A was 92.9 % (131/141),type B 0.7%(1/141),type C 5.0%(7/141),and type D 1.4%(2/141).There was no significant difference between the left side and the right side in terms of type.Type A of ZT was the most common type.Conclusion As an important anatomic landmark,ZT is essential for locating and dissecting RLN during thyroid surgery,however,due to its complex anatomical relationship with adjacent organs and the variability of RLN and its branches here,from which to reveal RLN has the possibility of increasing the injury risk.
2.Effect of Matteuccia struthiopteris polysaccharides on systemic lupus erythematosus-like syndrome induced by Campylobacter jejuni in BALB/c mice

Matteuccia struthiopteris is a nature plant, which contains a lot of potential active components. In the present study, we investigated the effect of polysaccharides extracted from Matteuccia struthiopteris on lupus-like syndrome induced by Campylobacter jejuni CJ-S131 in BALB/c mice. Mice were randomly divided into normal, model control, SLE model (vehicle treated), Matteuccia struthiopteris polysaccharides treated (30 and 15 mg x kg(-1)) groups and prednisone 5 mg x kg(-1) treated groups. The effect of Matteuccia struthiopteris polysaccharides (Ms) on weight and organ index of BALB/c mice was detected. Autoantibodies and total IgG production were measured by enzyme linked immunosorbent assay. Proteinuria was measured and kidneys were examined by light microscopy. Compared with SLE model group, treatment with Matteuccia struthiopteris polysaccharides 30 and 15 mg x kg(-1) reduced weight loss and Matteuccia struthiopteris polysaccharides 15 mg x kg(-1) reduced spleen swelling (P < 0.05). The increased production of autoantibodies and total immunoglobulin G (IgG) were also significantly inhibited. Matteuccia struthiopteris polysaccharides protected kidney against glomerular injury in BALB/c mice with reduced immunoglobulin deposition and lowered proteinuria (P < 0.01). Matteuccia struthiopteris polysaccharides had a protective effect on lupus-like syndrome induced by CJ-S131 in BALB/c mice.
3.Effect of surface treatment reagents and treatment time on bond strength of glass fiber posts to resin cement

BACKGROUND: At present, there is no uniform standard for the treatment time of chemical reagents for surface treatment of glass fiber posts. Therefore, studying the effect of treatment time of glass fiber post surface treatment reagents on the bond strength between fiber posts and resin cements is of great significance. OBJECTIVE: To evaluate the effect of two chemical agents on the bonding strength of glass fiber post and resin cement after surface pretreatment of glass-fiber posts at different times. METHODS: Forty-eight glass fiber posts were randomly divided into eight groups according to different surface treatment methods, six in each group. Group A received no special treatment; group B was treated with silanization for 1 minute; group C1 underwent a 30% hydrogen peroxide surface treatment for 5 minutes prior to 1-minute silanization; group C2underwent a 30% hydrogen peroxide surface treatment for 10minutes prior to 1-minutesilanization; group C3 underwent a 30% hydrogen peroxide surface treatment for 15 minutes prior to 1-minute silanization; group D1 underwent a 35% phosphoric acid surface treatment for 30 seconds prior to 1-minute silanization; group D2 underwent a 35% phosphoric acid surface treatment for 60 seconds prior to 1-minute silanization; group D3 underwent a 35% phosphoric acid surface treatment for 90 seconds prior to 1-minute silanization. The surface morphology of the treated glass fiber posts was observed under scanning electron microscope. The glass fiber post was bonded to the resin cement to form a cylindrical resin block and cut into a thin sample. The sheet was placed on a universal testing machine for micro-extrusion experiments. The failure mode of the specimens was observed under a stereomicroscope. RESULTS AND CONCLUSION: (1) Scanning electron microscope: the surface of the fiber post had different degrees of matrix dissolution and fiber bundle exposure after hydrogen peroxide and phosphoric acid treatment, but did not destroy the integrity of the fiber bundle. (2) Micro-extrusion experiments: the order of the bonding strength was as follows: group C3 > group C2 > group C1 > group D2 > groupD3 > groupD1> group B > group A, and there was significant difference between groups (P< 0.05) except for groups A and B, groups C2 andC3, and groups D2and D3. (3) Stereo microscope: the failure mode in the groups A and B was almost destruction in adhesion. The other six groups showed destruction in adhesion, but the cohesive failure and mixed failure were increased, and the failure mode changed from destruction in adhesion into cohesive failure and mixed failure. (4) These results indicate that the optimal treatment time of 30% hydrogen peroxide is 10 minutes, and the optimal treatment time of 35% phosphoric acid is 60 seconds. 30% hydrogen peroxide solution treatment of fiber post surface for 10 minutes has great clinical application value.
4.Clinical analysis of endometrial lesions in patients with breast cancer taking tamoxifen after surgery

Objective To study the effect of menopause status and the time of taking tamoxifen (TAM) on endometrial lesions after breast cancer surgery. Methods A total of 330 patients with postoperative vaginal irregular bleeding after breast cancer surgery or endometrial lesions after B ultrasonic from August 2007 to August 2017 in Northern Jiangsu People 's Hospital were retrospectively analyzed, including 180 cases of taking TAM treatment (medicine-taking group), and 150 cases of not taking TAM treatment (non medicine-taking group). The patients were also divided into the menopause group and the premenopausal group. According to the time of taking TAM, the patients were divided into < 2 years group, 2-5 years group and > 5 years group. Chi-square and Fisher test were used to compare the differences. Results The endometrial lesions incidence in the medicine-taking group was higher than that in the non medicine-taking group [84.44 % (152/180) vs. 56.00%(84/150);χ2=51.701, P=0.000]. The endometrial lesions rate in the menopause group was higher than that in the premenopause group [medicine-taking group: 69.70 % (46/66) vs. 92.98 % (106/114), χ2= 17.254, P= 0.000; non medicine-taking group: 46.15 % (35/65) vs. 63.53 %(54/85), χ2 = 4.513, P= 0.034]. For the patients in the menopause group and the premenopause group, the incidence of endometrial lesions for those who took medicine for>5 years [96.00%(48/50), 85.19%(23/27)] was higher than that in the<2 years group and 2-5 years group [78.26%(18/23), 42.86%(6/14);95.12%(39/41), 72.00%(18/25) respectively], and there were statistical differences (χ2=7.619, P=0.022;χ2= 8.070, P= 0.018). The menopause was not correlated with staging, muscular lawyer infiltration and lymph metastasis postoperative (P> 0.05), but with the type of endometrial cancer (P= 0.013); the length of taking medicine was related with the type of endometrial cancer and the lymph metastasis (P=0.027). With the prolonged time of medicine-taking for postmenopause patients, the incidence of type Ⅱendometrial cancer and positive rate of lymph metastasis were also increased. Conclusions Taking TAM after surgery for breast cancer patients increases the risk of endometrial lesions. The longer the patients take the medicine, the greater risk of the lesions take, and the worse the pathological, histological type and prognosis of endometrial carcinoma are, which is more obvious for postmenopausal women who take TAM for more than 5 years.
6.Recovery rule of atrial contractility after Maze Ⅳ in patients with valvular atrial fibrillation

Objective To analyze the recovery rule of atrial contractility (AC) function after Maze Ⅳ procedure of valvular atrial fibrillation (AF). Methods In our hospital from March 2016 to April 2018, 103 patients who underwent cryoablation Maze Ⅳ procedure due to mitral valve lesions associated with persistent or long-term persistent AF were enrolled. There were 42 males and 61 females, with an average age of 58.5±9.1 years. Electrocardiogram and echocardiography were followed up at discharge and 1, 3, 6, 12 months after procedure. A multivariate Cox analysis of predictive factors for AC recovery was applied. Results All the 103 patients were followed up for 1 year. The recovery rate of AC increased gradually after operation. It was not until 3 months after procedure that most of the right atrial contractility (RAC) was accompanied by synchronous recovery of the corresponding left atrial contractility (LAC, Kappa coefficient≥0.40, P<0.05). However, the coexistence of sinus rhythm (SR) and bilateral AC was not consistent well until 1 year after operation (Kappa coefficient≥0.40, P<0.05). One year after procedure, the recovery rates of SR and bilateral AC were 86.4% (89/103) and 66.0% (68/103) respectively. By Cox multivariate regression analysis, longer preoperative AF duration (P=0.040), larger preoperative left atrial diameter (LAD, P=0.003), and AC deletion 3 months after surgery (P=0.037) were predictive factors for AF recurrence in the middle and advanced stages (>3 months) after Maze surgery. At the same time, longer preoperative AF duration and larger preoperative LAD were also negative predictors of middle and late recovery of LAC and bilateral AC (All P<0.05). Receiver operating characteristic curve analysis showed that the best critical value of preoperative AF time and preoperative LAD for prediction of AC recovery was 37 months (sensitivity 99.6%, specificity 76.3%) and 60.5 mm (sensitivity 98.5%, specificity 78.9%), respectively. Conclusion The recovery of AC after Maze procedure is a dynamic improvement process. Early recovery of AC is beneficial to the stable maintenance of SR in the future. Prolonged duration of AF and enlarged LAD have adverse effects on the outcome of Maze Ⅳ procedure.

OBJECTIVE:
To evaluate the effect of rosmarinic acid (RA) on mitophagy and hypertrophy of cardiomyocytes exposed to high glucose (HG).
METHODS:
Rat cardiomyocytes (H9c2) exposed to HG (25 mmol/L) were treated with 50 μmol/L RA or with both RA treatment and Parkin siRNA transfection, with the cells cultured in normal glucose (5.5 mmol/L) and HG as the controls. The expressions of PINK1, Parkin and LC3II/LC3I in the cells were detected by Western blotting. The formation of mitochondrial autophagosomes was observed by transmission electron microscope. Flow cytometry was employed to detect the level of reactive oxygen species (ROS) and apoptotic rate of the cells.
